1.Effect of rhodiola sacra SH Fu on heart hemodynamics and myocardial contractility

BACKGROUND: Rhodiola rosco L acts on anti-aging,anti-fatigue and improving immunity in human body and improves angina pectoris.OBJECTIVE: To study the effect of rhodiola root on heart hemodynamics and myocardial contractility and probe into its mechanism.DESIGN: Observational and controlled based on the experimental animals.SETTING: Department of physiology in a medical university.MATERIALS: The experiment was performed in Laboratory of Cardiac Vessels of Department of Physiology of Guangxi Medical University,from January to April 2001.Thirty New Zealand white rabbits of either sex,weighted varied from 1.9 to 2.3 kg,were randomized into two groups,named as the control(physiological saline 4 mL/kg) and rhodiola sacra SH Fu (nuodikang) group(0.5 g/4 mL · kg-1),15 rabbits in each group. A hundred common wild frogs were employed,weighted varied from 120 to 160 g,randomized into 4 groups, named as the control group of physiological saline,nuodikang of small dosage group(blood concentration 209 mg/L),nuodikang of moderate dosage group(blood concentration 418 mg/L) and nuodikang of large dosage group(blood concentration 627 mg/L),25 frogs in each group.METHODS: ms2000 multiple medium biological signal analysis system was applied,by Straub method,frog heart perfusion specimen in vitro was prepared and the cardiac activity was assayed. The relevant indexes of ventricle activity in rabbits were determined with hemodynamics.MAIN OUTCOME MEASURES: Comparison of hemodynamics changes in rabbits of two groups and the comparison of effects of nuodikang at various dosages on frog heart work in vitro.RESULTS: Myocardial contractility in every nuodikang group was strengthened remarkably compared with the control(F = 8.939, P<0.01 ), the myocardial contractility in nuodikang groups at the dosages of 209 mg/L,418 mg/L and 627 mg/L was in the tendency of increasing,strengthened by 52. 3%,70. 1% and 86.4% respectively. But the significant difference was not apparent in the comparison among experimental groups(P>0.05). The significant difference in frog heart rates among groups was not apparent(P>0.05). Concerning to hemodynamics in rabbits,in nuodikang groups,the maximum increasing rate of pressure in the left ventricle was increased by 22.1% (F = 6.259,P<0.05),the mean pressure of artery was decreased by 8.2% (F = 5.688, P<0.05) . The significant difference in other indexes was not apparent in the comparison between the control and nuodikang groups.CONCLUSION: Nuodikang strengthens myocardial contractility and speeds up myocardial contraction, but does not present remarkable influence on myocardial dilation; simultaneously it reduces the mean pressure of artery.
3.Evaluation of operative risk and suvival of colorectal cancer patients 80 years of age and older

Objective To estimate the operative risk and outcomes of colorectal cancer patients 80 years of age and older. Methods Colorectal cancer resection was performed in 99 patients 80 years of age and older between Dec. 1987 and June 2005. The informations about clinical data, co-morbidity,complications, operative mortality and survival were retrospectively analyzed. The patients were followed-up for 45.12 months (range 1-136) months. Results Of 99 patients, co-morbidity was found in 43 patients (43.4% ) and complication in 10 patients (10.1%). No patient died of operation.Eighty patients completed the follow-up study. The overall 3-year survival rate and disease-free survival rate were 64.3% and 61.1%, respectively. Whereas the overall 5-year survival rate and disease-free survival rate were 52.8% and 52.1%, respectively. In univariate analysis, curative or palliative operation, tumor differentiation, cancer embolism in the vasculature, tumor staging and complications were proved to be significant prognostic factors. Multivariate survival analysis,however, showed that only the curative or palliative operation was independent factor for survival.Conclusions The high risk of co-morbidity for patients 80 years of age and older is not the obstacle to cancer resection.These patients will have satisfactory outcomes via optimal treatment and operation.
4.Pathological changes of the kidneys in mouse models of adriamycin-induced-nephrosis

Objective Our purpose was to observe the renal pathological changes in the mouse modells of adriamy -cin-induced nephropathy in different periods .Method 48 healthy male BALB/c mice were randomly divided into control group and model group .The model group received a disposable tail vein injection of adriamycin 10.5 mg/kg body weight , and the control group received the same amount of saline .24-hour urinary protein , serum biochemical indexes and kidney pathological changes were dynamically observed for 12 weeks.Results Proteinuria of model mice appeared in the 2th week after ADR injection, which lasted to the end of the 12-week experiment, At the 8th week, the amount of urine protein reached a peak (P<0.05);The serum albumin was decreased at the 4th week, cholesterol was increased at 8th week.At the end of experiment, serum creatinine was also increased (P<0.05).Minimal change nephrotic syndrome (MCNS) was observed in model mice at the 4th week;the lesions in renal tissues at 8th weeks were more serious than that at 4th weeks, but glomerular sclerosis was unconspicuous .Focal segmental glomerulonephritis ( FSGS) was seen at the 12th week.The GSI of the model mice was(2.81 ±0.84)%, significantly higher than that of the control mice ((0.33 ±0.21)%) at 12th week(P<0.01).Conclusions A mouse model with adriamycin-induced-nephrosis can be successfully established by a disposable tail vein injection of adriamycin in a dose of 10.5 mg/kg body weight .The early manifest ation of this model is MCNS, and at a late stage , it may be changed into FSGS .
5.Separation of salicylic acid drugs by aqueous and nonaqueous capillary electrophoresis with conductance detector.

AIMTo develop a method for separating salicylic acid drugs by aqueous and nonaqueous capillary electrophoresis with conductance detector.
METHODSFused-silica capillary (55 cm x 50 microns ID) was used. The effects of concentration and pH of the running buffer, running voltage and injection time were studied. Salicylic acid (SA), acetylsalicylic acid (ASA) and sulfosalicylic acid (SSA) can be separated in a 10 mmol.L-1 Tris-30 mmol.L-1 H3BO3 buffer (pH 8.0), the separation voltage and injection time were 24 kV and 10 s, respectively. But tailing peaks appeared. In order to improve the separation efficiency and the sensitivity, ethanol was used as nonaqueous solvent.
RESULTSHigh sensitivity and resolution for SA, ASA and SSA were obtained in ethanol media, and there was excellent linearity between peak area and concentration of the analytes in the concentration range of 0.05-100 mg.L-1, 5.0-250 mg.L-1 and 0.08-100 mg.L-1 for SA, ASA and SSA, respectively. All the correlation coefficients were over 0.995.
CONCLUSIONThe analysis of SA and ASA in aspirin tablets was tried and good results were obtained in ethanol media. There was higher sensitivity and separation efficiency than in aqueous media.

7.Gastric dynamics analysis of TEAS combined SNP induced general anesthesia when controlled hypotension dropped to 60% of the MAP baseline.

OBJECTIVETo observe the effect of gastric dynamics by transcutaneous electrical acupoint stimulation (TEAS) combined general anesthesia when controlled hypotension dropped to 60% of the mean arterial prenssure (MAP) baseline, and to provide experimental evidence for organ protection in clinical controlled hypotension.
METHODSEighteen male beagles were randomly divided into three groups, the general anesthesia group (blank), the general anesthesia induced controlled hypotension group (control), and the general anesthesia combined TEAS induced controlled hypotension group (experiment), 6 in each group. Controlled hypotension was performed in the latter two groups with isoflurane inhalation and intravenous injection of sodium nitroprusside (SNP). The mean arterial pressure (MAP) was lowered to 60% of the MAP baseline and kept for 60 min. Controlled hypotension was not performed in Beagles of the control group. For Beagles in the experiment group, TEAS [2/100 Hz, (4 ± 1) mA] was applied to bilateral Hegu (LI4), Quchi (LI11), Zusanli (ST36), and Sanyinjiao (SP6) from stable physiological conditions to the end of maintaining stages. Changes of EGG frequencies and EGG amplitudes were monitored. Serum levels of gastrin (GAS) and motilin (MTL) were also detected at corresponding time points during and after experiment.
RESULTSAs for the pressure control effect of TEAS combined general anesthesia in the controlled hypotension, during the process of controlled hypotension (T1-T4), MAP levels of two controlled pressure groups remained relatively stable, and were kept at 60% of the MAP baseline. When the blood pressure dropped to the target low MAP and maintained at 60 min (T1-T4), EGG amplitudes of Beagles in all the three groups showed decreasing tendency. But it was more obviously lower than its basic level in the control group (P <0.05), while it was not obviously decreased in the experiment group (P < 0.05). EGG frequencies of Beagles in all the three groups showed no obvious change during this stage. By the end of the MAP rising stage (T8), the EGG amplitude of the experimental group was significantly higher than that of the control group and the blank group (P < 0.05), while it didn' t show any obvious increase in the control group. During this period, EGG frequencies of the two controlled hypotension groups decreased more than those of the blank group. Two h after rising blood pressure (at T9), EGG amplitudes and frequencies in the two controlled hypotension groups basically restored to their respective baselines and levels of the blank group at T9. At 2 h (T9) after controlled hypotension, serum levels of GAS and MTL were lower than those of basic levels in the two controlled hypotension groups (P <0.05). However, serum levels of GAS and MTL had an increasing trend in the two controlled hypotension groups at 24-72 h (T10-T12). Besides, the increasing speed and amplitude was better in experiment group than in the control group at T10-T12. However, there was no statistical difference between the two groups (P > 0.05). At 72 h (T12) serum levels of GAS and MTL had basically restored to their basic levels in the two controlled hypotension groups and that of the blank control group.
CONCLUSIONEGG amplitudes could be effectively improved in TEAS combined general anesthesia for controlled hypotension at 60% of the MAP baseline, the recovery of the serum GAS level accelerated, gastric power improved and stomach protected.

8.Efficacy comparison between two kinds of vitrectomy in proliferative diabetic retinopathy

AIM:To compare the clinical effect of 23G and 25G+ vitrectomy for treatment of proliferative diabetic retinopathy (PDR).METHODS: A total of 128 PDR patients (195 eyes) requiring vitrectomy in our hospital from November 2013 to May 2016 were randomly divided into 25G+ group and 23G group, 64 cases (97 eyes) in 25G+ group and 64 cases (98 eyes) in 23G group.In 25G+ group, patients were treated by 25G+ vitrectomy.In 23G group, patients were treated by 23G vitrectomy.The visual acuity, as well as intraocular pressure (IOP), iatrogenic injury and complications in two groups were recorded before and 1d, 1wk, 1mo after treatment.The operation time was compared between two groups.RESULTS: The operation time in 25G+ group was lower than that in 23G group (P<0.05).The postoperative visual acuity at 1mo of two groups were improved compared with before surgery (P<0.01).However, visual acuity between two groups in the same period had no significant difference (P>0.05).IOP in 25G+ group before surgery had no significant difference compared with those after surgery at 1d,1wk, and 1mo(P>0.05), which it was the same in 23G group.IOP of two groups in the same period had no significant difference (P>0.05).The incidence rate of iatrogenic injury in 25G+ group was 4.1%, which was significant lower than that of 23G group (13.3%) (P<0.05).The incidence rate of complication in 25G+ group was 3.1%, which was significant lower than that of 23G group (11.2%) (P<0.05).CONCLUSION: Both 23G and 25G+ vitrectomy are safe and effective treatment for PDR.However, 25G+ vitrectomy is the better choice for PDR for the shorter operation time, lower incidence rate of iatrogenic injury and fewer surgical complications.
9.Endovascular recanalization for non-acute internal carotid artery occlusion using a new angiographic classification

Objective:To evaluate the safety and feasibility of endovascular recanalization for non-acute internal carotid artery occlusion (NA-ICAO), and to propose a new angiographic classification.Methods:From April 2015 to October 2019, 95 consecutive patients with symptomatic NA-ICAO who received endovascular recanalization were retrospectively analyzed in Beijing Tiantan Hospital, Capital Medical University. All the patients were divided into four groups according to DSA: type Ⅰ, petrous segments were distally reconstituted by collateral vessels; type Ⅱ, cavernous segments were distally reconstituted by collateral vessels; type Ⅲ, ophthalmic segments were distally reconstituted by collateral vessels; type Ⅳ, communicating segments were distally reconstituted by collateral vessels. Study data including clinical characteristics, surgical details, lesion classification, recanalization rate and perioperative complications. For the counting data, the χ 2 test was used to compare between groups. For the quantitative data, the ANOVA was used for the normal distribution data, otherwise the Kruskal-Wallis H test was used. The primary safety outcome was any stroke or death within 30 days. Results:Among the 95 patients, 67 (70.53%) had successful recanalization. The recanalization rates of type Ⅰ-Ⅳ were 92.31% (36/39), 81.82% (18/22), 47.83% (11/23) and 18.18% (2/11) respectively (χ2=29.557, P<0.001). And the complication rates of the four types were 5.13% (2/39), 13.64% (3/22), 21.74% (5/23) and 9.10% (1/11) respectively. The incidence of perioperative ischemic stroke was 2.11% (2/95). No other serious stroke and death occurred. Conclusions:Endovascular recanalization may be feasible and safe for carefully selected patients with NA-ICAO and therefore represents an alternative treatment. The patients with type Ⅰ and Ⅱ lesions had higher recanalization rates, while the patients with type Ⅳ lesions had significantly lower recalculation rate. The new angiographic classification is conducive to the selection of suitable patients and difficulty in grading.
10.The Audiological Characteristics of Neonate with Severe Hyperbilirubinemia Need to be Exchanged Transfusion

Objective To study the change of audiological characteristics of neonate with severe hyperbilirubi‐nemia need to be exchanged transfusion ,and to explore the correlation between the peak concentrations of serum to‐tal bilirubin and hearing loss .Methods A total of 130 cases of neonate diagnosed with severe hyperbilirubinemia need to be exchanged transfusion were included in this study .Hearing tests of auditory brainstem response (ABR) , distortion product otoacoustic emission (DPOAE) and acoustic immittance were administered to these neonates and they were followed up three months old .They were divided into the normal group ,the mild - moderate group and the severe - extreme group according to the hearing tests results of three months .The peak concentrations of ser‐um total bilirubin and hearing condition among the three groups were analyzed .Results There were 85 neonates with normal hearing (65 .38% ,85/130) ,45 with hearing loss (33 .85% ,45/130 .For 88 ears ,there were 2 cases of single ear and 43 cases of both ears) ,including 11 cases of mild - moderate group (22 ears ,24 .44% ,11/45) and 34 cases with severe - extreme group (66 ears ,75 .56% ,34/45) .There were 16 neonates (32 ears) met the crite‐rion of auditory neuropathy (12 .31% ,16/130) .The difference of the peak concentrations of serum total bilirubin a‐mong the three groups was statistically significant (F=16 .525 ,P=16 .525) .And positive correlation was observed between ABR threshold and peak serum total bilirubin concentration (r=0 .584 ,P<0 .001) .Conclusion The ma‐jor feature of the neonate with severe hyperbilirubinemia need to be exchanged transfusion was extremely severe sen‐sorineural hearing loss in both ears ;and the higher probability of serious hearing loss with the higher peak concen‐trations of serum total bilirubin .
