1.Correlative factors of the hydatidiform mole deterioration and clinical significance of matrix metalloproteinase 9 in gestational trophoblastic disease

Objective To discuss the possibility of prognosing the hydatidiform mole deterioration and metastasis by detecting the expression of matrix metalloproteinase 9(MMP-9)in different type of trophoblastic disease,and analyze the correlative factors of the hydatidiform mole deterioration,so as to improve the criterion of hydatidiform mole prognosis.Methods The expression of MMP-9 in 30 cases of normal human(Iess than 12 weeks)cytotrophoblast cells,32 cases of partial hydatidiform mole,93 cases of complete hydatidiform mole,30 cages of invasive mole and 5 cases of choriocarcinoma were detected by immunohistochemical streptavidin-pemxidase.The factors as the age of onset,uterine size,luteinizing cyst,β-human chorionic gonadotropin hCG of 7 cases of the hydatidiform mole deterioration were analyzed.Results The expression of MMP-9 was detected in all types of trophoblastic cells,with the development of the malignant transformation the stainning Was markedly increased in the invasion of the normal human cytotrophoblast cells(X2=23.49,P<0.05).Conclusion The difference of MMP-9 expressin gestational trophoblasts is helpful to predict the hydatidiform mole deterioration and metastagis,the hydatidiform mole deterioration correlates with the age of onset,β-hCG,uterine size,luteinizing cyst.
2.Effects of angiotensin Ⅱ receptor antagonist on expression of plasminogen activator inhibitor-1 in the renal biopsy of patients with chronic kidney disease

Objective To evaluate the effects of angiotensin Ⅱreceptor antagonist (ARB) on expression of plasminogen activator inhibitor(PAI)-1 in the human renal biopsy of patients with chronic kidney disease(CKD)and analyse its clinical significance.Methods PAI-1 expression of 63 specimens with CKD was detected by immunohistochemistry and quantitative analysis was achieved by image analysis system. meanwhile, the patients'clinical data were collected,and PAI-1 expression of 63 specimens with CKD was compared with that of normal renal tissue(19 cases).Results Compared with the normal renal tissue,the expression of PAI-1 in CKD 1,2 and 3 stage wag respectively drastically increased(P<0.05).The expression of PAI-1 in CKD 3 stage was higher than that in CKD 1 and 2 stage(P<0.05).The expression of PAI-1 in the patients treated by valsartan[renal corpuscle(4.73±1.18)%,renal tubule(37.16±6.81)%]was lower than that with no treatment[renal corpuscle(5.81±1.95)%,renal tubule(43.22 ±10.25)%](P<0.05).Conclusions It suggests that the abnormal expression of PAI-1 in CKD play an important role in the progression of glomerular and tubulointerstitiai sclerosis in patients with CKD.ARB may prevent the development of renal inflammation and sclerosis by inhibiting the production of PAI-1.ARB may contribute to the prevention and therapy of CKD.
3.Chimeric antigen receptor T-cell immunotherapy and its applications in hematological malignancies immunotherapy

Chimeric antigen receptor T-cell (CAR-T) immunotherapy is a new type of immunotherapy,which has been developed rapidly in recent years.By using gene recombination and transfection techniques,CAR-modified effector T cells that specially recognize tumor-associated antigen was produced,which show better properties of targeting,killing activity and durability than conventional T cells.With the development of translational medicine research,CAR-T technology has experienced four generations of optimization and innovation,and presented a promising clinical efficacy in the treatment of various cancers,especially hematological malignancies.However,there are also potential risks with clinical use of this new technology,such as the off-target effect and cytokine storm.In this review,the progress,side effects,coping strategy,and development prospects of CAR-T in hematological malignancies immunotherapy were discussed.
4.Improvement of the Method of Positive Control Test in Bacterial Endotoxin Test

OBJECTIVE:To improve the method of positive control test in bacterial endotoxin test.METHODS:A new method rather than a routine method was adopted for the bacterial endotoxin test in which the tachyplens amebocyte lysate(TAL)was dissolved directly by sample solutions or its diluted solution,which was then added with 2.0 ? endotoxins solution for positive control test for the common injections including metronidazole and glucose saline etc.RESULTS:The Et values of all the test samples detected by the improved method were similar to those by the routine method,which stood at 2.0 Es~0.5 Es,suggesting that the improved method has no interfere on the bacterial endotoxin test.CONCLUSION:The method is simple and time-saving.
5.Effects of tetramethylpyrazine on proliferation of rat glomerular mesangial cells induced by lipopolysaccharide and levels of intercellular adhesion molecule-1

AIM : To investigate the effects of tetramethylpyrazine (TMP) on the proliferation of rat glomerular mesangial cells (GMCs) induced by LPS and the levels of intercellular adhesion molecule-1 (I CAM-1). METHODS : Different concentrations of TMP were put in the cultured rat GMCs in vitro after induced by LPS. Cell proliferation was assessed by MTT colorimetric assay and cell cycle was analyzed by flow cytometry. The levels of ICAM-1 were detected by immunohistology. RESULTS : The proliferation of MC induced by LPS was inhibited significantly by TMP with a dose-dependant manner. In the cell cycle, TMP increased the cells in G 0/G 1 phase and decreased the cells in S phase, and the ICAM-1 expression level was decreased. CONCLUSION : LPS-induced proliferation of MC is inhabited significantly by TMP. It blocks G 0/G 1 phase into S phase and inhibited the ICAM-1 expression.
6.A Study on mandibular retrognathism treated by implantation of prosthesis under the periosteum via oral incision.

Ojective To evaluate the effect of implatation of prosthesis under the periosteum via oral incision on mentoplasty. Methods The buccal morphology and structure of cadaver was studied with topographic methods. 43 cases of mentoplasty was treated by implatation of prosthesis under the periosteum via oral incision. Results The mandibular shape of all cases treated by mentoplasty were improved with successful and satisfactory effects. No complication occurred. Conclusions It suggested that the implatation of prosthesis under the periosteum via oral incision was a safe, effective procedure for mentoplasty.
7.A review on the prevention and treatment pattern of primary angle closure glaucoma in China from points of evidence-based medicine and health economics

Glaucoma is the first leading cause of irreversible blindness globally. China is one of the major developing countries and has the biggest burden of primary angle closure glaucoma (PACG) in the world. As the aged population grows in China, the amount of patients of PACG and"oecludable angle"will be increased consequently. China is facing a number of barriers in the prevention of glaucomatous blindness such as unbalanced alloeation of heahhcare resources, laek of evidence-based guidelines of glaucoma in accor- dance with Chinese situation, and sequential severe over-treatment. In this article, we reviewed the current mode of the prevention of glaucomatous blindness in China from the point of view of evidenee-based medicine and health economics, and provided several sugges- tions in further researehes and clinical works.
8.Concerning about the influence of diabetes on the incidence of age-related macular degeneration

Age-related macular degeneration(AMD)is a kind of degenerative diseases with complex pathogenesis and stimulation of multiple factors.It has become one of the major diseases causing-blindness in the elderly population in China.Recently,some epidemiological findings demonstrated that diabetes mellitus possibly is one of risk factors for AMD,and this will offers a new approach for the prevention and treatment of AMD.Thus,it is very important for US to strengthen the research of mechanism of AMD impacted by diabetes mellitus,pay more attention to the progress of AMD,search for a scientific and rational therapeutic way of improving the prognosis and elevate the quality of life in the patients with diabetes mellitus.
9.Efficacy assessment and follow-up study with different small doses of adrenocorticotropic hormone in treating infantile spasms

Objective To investigate the efficacy and optimal dose of adrenocorticotropic hormone(ACTH) in treatment of infantile spasms (IS).Methods Sixty patients with IS were randomly divided into group A (ACTH 20 IU/d,n =20),group B (ACTH 40 IU/d,n =20) and group C (Depakine syrup,n =20) in a prospective randomized study.Patients in group A and group B were given ACTH in the dose of 20 IU/d and 40 IU/d for 2 weeks,respectively.After 2 weeks of treatment,ACTH administration was replaced by oral prednisone if the seizure frequency decreased by 50.0% or less.Patients showing incomplete control of epilepsy(seizure frequency reduction less than 50.0%) or failing to respond to treatment were continuously treated with the original amount of ACTH for another 2 weeks,then ACTH was replaced by oral prednisone.Patients in group C were treated with the combination regimen of oral clonazepam and Depakine syrup routinely.All participants were followed up by 0.5-3.0 years.The differences in clinical efficacy,changes in electroencephalogram(EEG) and adverse reactions in 3 groups were compared and analyzed.Results Mter 4 weeks of treatment,the total effective rates (TER) and EEG remission rate (ERR) in group A [TER:75.0% (15/20 cases),ERR:40.0% (8/20 cases)] and group B [TER:70.0% (14/20 cases),ARE:45.0% (9/20 cases)] were significantly higher than those in group C [TER:30.0% (6/20 cases),ERR:10.0% (2/20 cases)] (x2 =10.011,6.624,all P < 0.05),respectively,while no significant difference was found between group A and group B as for seizure control,hypsarrhythmia EEG resolution,and cognitive function restoration (x2 =0.125,0.000,t =1.95,all P > 0.05).During follow-up study,the TER and ERR in group A [TER:30.0% (16/20 cases),ERR:35.3 % (6/17 cases)] and group B [TER:30.0% (6/20 cases),ARE:37.5% (6/16 cases)] were also significantly higher than those in group C [TER:5.0% (1/20 cases),ARE:5.9% (1/17 cases)] (x2 =4.329,6.455,all P < 0.05),respectively.The incidence of adverse reactions in group A was lower than that in group B (x2 =3.956,P < 0.05).Conclusions ACTH proves to be an effective drug for the treatment of IS.Administration of ACTH in the dose of 20 IU/d for 2 weeks seems to be an optimal treatment recipe with remarkable efficacy with fewer adverse effects and may be worthy of clinical trials in a larger cohort.
10.Application of sub-Tenon's anesthesia for compound trabeculectomy with high intraocular pressure

AIM:To evaluate the efficacy and safety of Sub-tenon's anesthesia for compound trabeculectomy with high intraocular pressure.
METHODS: Forty - six eyes ( 46 cases ) of primary glaucoma received compound trabeculectomy under Sub-tenon's anesthesia, whose preoperative intraocular pressure were higher than normal after 24 to 48h of combined medication. Both efficacy and complication of the anesthesia were studied.
RESULTS: One minute after anesthetic injection, all cases were able to achieve the effect of analgesia and eye brake. During the operation, 0 level of anesthesia effect included 35 eyes ( 76%) , 1 level of anesthesia effect included 10 eyes ( 22%) , 2 level of anesthesia effect included 1 eye(2%). Only 1 case of these patients needed to add the surface anesthetic once, and other cases were successfully operatedunder Sub-tenon's anesthesia. The total effective rate was 98%. No anesthesia complications occurred in all cases.
CONCLUSION: Sub - Tenon's anesthesia is safe, effective, simple and quick for compound trabeculectomy with high intraocular pressure.
