2.Effects of Astragalus Granules on the Cav-3 and Smad3 Expression in the Myocardial Cells of Rats with Viral Myocarditis

OBJECTIVE:To study the effects of Astragalus granules on the expression of Caveolin-3(Cav-3)and Smad family member 3 (Smad3) in the myocardial cells of rats with viral myocarditis. METHODS:90 rats were randomly divided into a nor-mal group,a model group,a Shenmai injection group [positive drug,0.2 g/(kg·d)] and the groups of low,medium and high-dose Astragalus granules [0.42,0.84,1.68 g/(kg·d)],with 15 rats in each group. The rats in all groups except for the normal group were given CVB3 ip for the establishment of viral myocarditis model. Meanwhile,the rats in the drug administration groups were given corresponding drugs ig,while those in the normal group and the model group were given normal saline ig,for 15 consecu-tive days. 5 rats were selected from each group respectively on the 3rd,9th and 15th days of drug use to take an experiment. For the rats,the pathological change of the cardiac muscle tissue was observed and scored,and the mRNA and protein expression of Cav-3 and Smad3 in the myocardial cells were detected. RESULTS:After 15 days of drug use,compared to the normal group,the rats of the model group had hyperplasia of a large number of cardiac muscle fibers,obvious lesions at cardiac muscle fibers, and significantly higher pathological score and levels of the mRNA and protein expression of Cav-3 and Smad3 in the myocardial cells (P<0.05). Compared to the model group,the rats in the drug administration groups had cardiac muscle tissue lesions improved and had obviously lower pathological score and levels of the mRNA and protein expression of Cav-3 and Smad3 in the myocardial cells(P<0.05). CONCLUSIONS:Astragalus granules can markedly downregulate the gene expression of Cav-3 and Smad3 in the myocardial cells of rats with viral myocarditis,which is inferred as a prevention and treatment mechanism of viral myocarditis.
3.Imaging and clinical analysis of 12 cases with melioidosis

Objective Imaging and clinical manifestations of melioidosis were analyzed in order to improve our understanding of the disease and to reduce the rate of misdiagnosis.Methods From 2001 to 2006,12 melioidosis cases were confirmed by blood,pus and sputum culture.All cases were examined with radiography,and nine of them with CT and 2 with US.The imaging and clinical data were assessed retrospectively.Results Ten of 12 cases revealed lung abnormalities,the main organ involved by melioidosis in the body.The appearances were observed as follows:diffused sheets and sheets fused partly in bilateral fields of the lungs can be seen on chest radiograph;homogeneous dense shadow of the lobe or segment were observed on plain chest film and CT;blurred strips radiated from hilum were revealed on plain chest film;multitude nodules in 2 lungs showed on CT;cavity with air and liquid be seen on plain chest film and CT;patches and granules in superior and middle fields or(and)inferior fields be seen on plain chest film and CT;arc shadow of water attenuation in dorsal thorax showed on CT.Infections outside the lung can be observed in orbital,lumbar muscle,liver and spleen in 1 case,and thighbone osteomyelitis with multi abscesses in one case.Conclusion Alhough medioidosis has no characteristic imaging appearances,the disease's location,extent,severity and quantity objectively can be demonstrated by imaging,final confirmation is necessary using the culture of blood,pus and sputum.
4.Effect of oral mucosa transplantation in the treatment of severe contracted conjunctival sac after ocular prosthesis implantation

AlM: To observe the clinic effect of oral mucosa transplantation in the treatment of severe contracted conjunctival sac after ocular prosthesis implantation .
METHODS: Thirty-three cases ( 33 eyes ) with globe disorders and severe contracted conjunctval sac were operated ocular prosthesis implantation firstly, and conjunctival sac plasty using oral mucosa after 6mo.
RESULTS: Thirty - one cases were successful, no complications appeared. One case had primary ptosis and 1 case had recurrent conjunctival sac contracture.
CONCLUSlON:lt is recognised that the methods of oral mucosa transplantation in severe contracted conjunctival sac after ocular prosthesis implantation are effective on those cases.

6.Evaluation on dosage-based efficacy-toxicity correlation of Tripterygium wilfordii against immune inflammation in mice.

OBJECTIVETo study the anti-immune inflammation efficacy and toxicity of Tripterygium wilfordii decoction, in order to provide experimental basis for studies on its "efficacy-toxicity" correlation.
METHODThe delayed hypersensitivity model was established by dinitrofluorobenzene in mice. Different doses of T. wilfordii decoction was administered for 5 consecutive days. The ear swelling inhibition ratio and the toxic action were observed. After the final administration, the biochemical indexes of PGE2, TNF-α, IL-2, ALT, AST, PA, TBA, TBIL in serum were detected, and the visceral indexes of heart, liver, spleen and kidney were measured.
RESULTThe DNFB-induced ear swelling could be notably inhibited by multiple oral administration of T. wilfordii decoction, with the ED50 and its 95% confidence limit of 0.34 (0.21-0.42) g x kg(-1). The contents of PGE2, TNF-α, IL-2 in serum decreased in a dose-dependent manner. The activities of serum AST, ALT, TBA, TBIL and the PA content reduced.
CONCLUSIONT. wilfordii decoction shows a significant anti-immune inflammation efficacy within the dosage range between 0.59 and 2.34 g x kg(-1) in a dose-dependent manner. With a certain hepatotoxicity, high dose (2.34-4.68 g x kg(-1)) of T. wilfordii decoction can cause substantial liver injury, with a dose dependence in liver function index. Therefore, the efficacy and toxicity of T. wilfordii is dose dependent, which provides reference for preventing adverse drug reactions in clinic and developing early-warning schemes and ensure the clinical medication safety of T. wilfordii.

8.Analysis of clinical characteristics of 26 patients with melioiclosis spleen abscesses in Hainan province

Objective To analyze the clinical features of melioidosis spleen abscess.Methods A retrospective analysis of 26 cases with melioidosis spleen abscess at Hainan People′s Hospital between October 2002 and March 2015 was performed.Data with general conditions, clinical characteristics, imaging features, treatment and outcome were all extracted and analyzed.Results High fever (100.0%) and splenomegaly (73.1%) were the main clinical symptoms and signs.Laboratory examinations showed that white blood cell and neutrophil percentage increased, with a mean value of 12.6×10.9/L and 0.86, respectively.88.5% cases had diabetes mellitus which was the main underlying disease.Apart from spleen abscess, pulmonary infection (73.1%) and liver abscess (38.5%) were the majority.Imaging showed multiple small abscess (92.3%).All patients had at least one positive blood culture of Burkholderia pseudomalleiand were treated with imipenem, ceftazidime and sulfamethoxazole alone or in combination.Hospitalization days ranged between 8 to 65 days with the mean of 29.5 days.Twenty-three cases (88.5%) were cured, the remaining 3 cases (11.5%) abandoned treatment and were all lost for follow-up.Conclusions Melioidosis should be suspected in patients from epidemic areas, who have diabetes and imaging shows spleen abscess and other organ infections including pulmonary infection and liver abscess.Microbiological culture is needed immediately.

10.Wireless telemetry electrical activity of nucleus accumbens shell in morphine-induced CPP rats.

OBJECTIVETo analyse the relationship between the electrical activity changes of nucleus accumbens (NAc) shell and the drug-seeking behavior by recording NAc shell electrical activity in conditioned place preference (CPP) rats induced by morphine.
METHODSForty SD rats were randomly divided into operation-only control group and the morphine-induced CPP group after stereotaxic electrode was buried on rats NAc shell and the latter group was used to establish the morphine CPP model(n = 20). A CPP video system combining with the technique of electrical activity wireless telemetry was used in the study. The NAc electrical activity from each group of rats was recorded by wireless telemetry respectively, which included staying in black or white chamber of video box, shuttling between black-white chambers and between white-black chambers. The electrical activity differences were analyzed by the percentage of each wave.
RESULTSWhen the morphine-induced rats staying in black chamber, compared with the operation-only control group, the NAc shell electrical activity showed that the percentage of 0 - 10 Hz was increased(P < 0.05), meanwhile, those of 10 - 20 Hz and 30 - 40 Hz were reduced(P < 0.05, P < 0.01); when the morphine-induced rats staying in white chamber, the NAc shell electrical activity showed that the percentage of 0 - 10 Hz and 30 - 40 Hz were increased(P < 0.05 , P < 0.01) , that of 10 - 20 Hz was reduced(P < 0.05 , P < 0. 01); when the morphine-induced rats in black- white shuttling status, the NAc shell electrical activity showed that the percentage of 0 - 10 Hz was increased(P <0.05, P <0.01), that of 10- 30 Hz was reduced( P <0.05); and in the white-black shuttling status, the electrical activity showed that the percentage of 0 - 10 Hz was reduced(P <0.05), that of 10 - 30 Hz was increased(P < 0.05) ; the electrical activity was further compared between staying status and shuttling status in the morphine-induced CPP group. There was no significant difference of electrical activity between the rats in white-black shuttling status and staying in white chamber. However, when rats in black-white shuttling status, compared with staying in black chamber, the electrical activity showed that the percentage of 0 - 10 Hz and 40 - 50 Hz were increased(P < 0.05), meanwhile, those of 10 - 20 Hz and 30 - 40 Hz were reduced(P <0.05).
CONCLUSIONThe electrical activity changes of NAc shell in morphine-induced CPP rats were different from those of the operation-only control group, and these changes might be associated to the rat's drug-seeking behavior.
