1.The effect of exception / miracle questions in nursing on affects,confidence and actual solving problems

Objective To investigate the effect of exception questions/miracle questions in SolutionFocused Approach in nursing on affects,confidence to solve problem,the perception of problem's difficulty,and actual solving problems,to provide practical basis for practitioners of Solution-Focused Approach.Methods A total of 102 nurses were randomly divided into exception questions in Solution-Focused Approach group (exception group),miracle questions in Solution-Focused Approach group (miracle group)and cause questions in the traditional psychotherapy pattern group(cause group).The difference among three groups on the aspect of the change of affects,confidence to solve problem and the perception of problem's difficulty,and the help form the questions immediately and after 1 week (immediate help and 1 week after help) were studied.Results Eventually subjects effectively,34 nurses in exception group,31 nurses in miracle group,31 nurses in cause group.The difference among three groups in aspect of the change of affects,the change of confidence to solve problem,immediate help and 1 week after help were statistically significant (Z=9.68-18.58,P< 0.05).Furthermore,change in problem solving confidence on exception group was higher than cause group(51.9 vs.35.6,P < 0.05),change in affects on exception group and miracle group were higher than cause group (53.8,55.1 vs.36.0,P < 0.05),the immediate help on exception group and cause group were higher than miracle group (59.8,52.5 vs.32.0,P < 0.05),and 1 week after help on exception group was higher than miracle group and cause group(64.5 vs.42.3,37.0,P < 0.05).Conclusions Exception/miracle questions can both improve the emotional state,and exception questions can enhance the confidence to solve problems and provide more help for nurses.
2.Study of risk factors associated with hypospadias

Objective To study the risk factors which lead to increase in the incidence rate of hypospadias. Methods A case-control design was used.The mothers of 210 cases and 420 controls were interviewed by telephone or face-to-face,and the questionnaires were answered.Thirty probable risk factors were investigated.Statistical analysis was performed using univariate and multivariate unconditional Logistic regression analyses for evaluation of the data. Results All boys with hypospadias in the case group were of Han nationality.Only 5 cases had positive family history.Odds ratio (OR) of the factors investigated were:pregnant age (OR,38.63),threatened abortion (OR,4.71),progestin (OR,3.64),estrogen (OR,5.1),pesticide (OR,2.39),organic solvents (OR,2.12),season of pregnancy (OR,1.86) and detergent (OR, 1.87).But OR of frequency of abortion,weeks of gestation and birth weight were less than 1.The results of other 17 investigated factors were negative. Conclusions Threatened abortion,nonfull-term fetus,low birth weight and some environmental endocrine-disturbing chemicals ( pesticide,detergent,organic solvents,progestin and estrogen) may be risk factors associated with hypospadias.These environmental endocrine-disturbing chemicals may increase the incidence rate of hypospadias.
3.Treatment for pulmonary artery hypertension in liver transplantation

BACKGROUND: Serious lesion to liver function may cause the pulmonary artery hypertension and fluctuation of haemodynamics directly induces changes of pulmonary artery pressure during neo-hepatic stage in liver transplantation. OBJECTIVE: To observe the change of pulmonary artery pressure during liver transplantation and to explore treatment for pulmonary artery hypertension. DESIGN, TIME AND SETTING: Controlled observation was carried out among the liver transplant recipients who were enrolled from the 181 Hospital of Chinese PLA from June 2004 to June 2006. PARTICIPANTS: Totally 13 patients, all males, were reviewed. Their ages ranged from 19 to 53 years. They were selective for the liver transplantation. METHODS: Undergoing general anesthesia, mean arterial blood pressure, central venous pressure, pulmonary arterial pressure and pulmonary arterial wedge pressure were continuously monitored by Swan-Ganz catheter method and reversible Fick’s law continuous monitoring. Available respiration administration, low dose of glyceryltrinitrate (0.1-5.0 ?g/kg/min) and Alprostadil (1.0-2.0 ?g) were administered to diminish elevated pulmonary arterial pressure and pulmonary arterial wedge pressure during the period after unclamping portal vein in liver transplantation. MAIN OUTCOME MEASURES: Changes of mean arterial blood pressure and pulmonary arterial pressure. RESULTS: The mean arterial blood pressure deceased at the time of unclamping portal vein in all 13 patients, pulmonary arterial pressure and pulmonary arterial wedge pressure both increased significantly at 5 and 15 minutes within neo-hepatic stage, which were more obvious than those during anhepatic stage in 13 patients (P

CONCLUSION: Reasonable application of glyceryltrinitrate and Alprostadil can obviously release pulmonary arterial pressure during neo-hepatic stage in liver transplantation.
5.CONSTRUCTION OF YARROWIA LIPOLYTICA tsr1 MUTANTS BY GENE DELETION

This TSR1 gene associated with the secretion of Proteins of the yeast Yarrowia lipolytica was deleted. Transformation of a Y. lipolytica strain.with the disrupted tsr1 fragment resulted in the substitution of partially deleted TSR1 gene for the wild-type TSR1 gene on the chromosome. The result of southern hybridization show that the tsr1 mutants have been constructed. The isolation of these mutants would be useful for studing the function of TSR1 gene in yeast Y. lipolytica.
6.Nursing rare of 12 newborns with thyroglossal cyst

This paper summarize the perioperative nursing of 12 newborns with thyroglossal cyst. The preoperative nursing measures focused on prone position,gastric tube feeding,close monitoring of vital signs and tracheal intubation as needed. If the TeSO2 was normal,oxygen therapy was not recommended. After the operation,continuously monitoring vital signs,respiratory care and use of sedatives as needed were carried out. As a result,all of the 12 infants recovered and discharged.
7.Prevention of Post-operative Adhesion with Early Electro-acupuncture Treatment

ObjectiveTo investigate the effect of early electro-acupuncture treatment on prevention of post-operative adhesions.Methods40 rabbits were randomly divided into 5 groups: normal control group, model group 1, model group 2, early electro-acupuncture group and later electro-acupuncture group with 8 animals in each group. Muscle adhesion model was established by operation. The early electro-acupuncture group was received the treatment from the 11th to 20th day after the operation; later electro-acupuncture group was from the 21st to 30th day after the operation. The weight, gait and range of motion (ROM) of animals were observed and analyzed.ResultsThe standing, walking and ROM of animals of the early electro-acupuncture group improved; but only few gait indexes improved and ROM not changed in the later electro-acupuncture group.ConclusionThe early electro-acupuncture treatment can prevent the post-operative adhesion of muscles, but not having effect on formed adhesion.

9.Discussion on circulating fluid replacement in the polyuria stage after simultaneous islet cell-renal transplantation

Objective To discuss and improve the scheme of circulating fluid replacement in the polyuria stage after simultaneous islet cell-renal transplantation.Methods Nursing data of 26 patients after simultaneous islet cell-renal transplantation during the polyuria stage were analyzed retrospectively,the impact on spend-and-tax circulating fluid replacement on the outcome after surgery was observed.Results In 26 patients,3 case suffered graft function recovery delay,23 patients' renal function recovered smoothly without water-electrolyte and acid base disorders in period of diuresis,compared with the 16 cases with traditional fluid replacement in the same time,serum creatinine (SCr),blood urea nitrogen (BUN) and blood glucose (BG) decreased significantly in 72 h.Conclusions Using spend-and-tax fluid replacement can reduce postoperative complications such as water-electrolyte and acid-base disorders in simultaneous islet cell-renal transplantation patients,and promote the functional recovery of transplanted islet cells and kidney.
