Journal of Clinical Hepatology 2014;31(5):417-420
Objective To investigate the clinical value of serum soluble intercellular adhesion molecule-1 (sICAM-1 )in patients with primary biliary cirrhosis (PBC)and autoimmune hepatitis (AIH).Methods Sixty-one PBC patients and 59 AIH patients, who were hos-pitalized or visited the outpatient department from June 2012 to September 2013,as well as 50 healthy controls, were included in the study. The PBC patients included 29 incipient cases,21 cases in remission, and 11 recurrent cases;the AIH patients included 26 incipient cases, 20 cases in remission, and 13 recurrent cases.Serum sICAM-1 level was measured by double-antibody sandwich enzyme-linked immu-nosorbent assay, and serum levels of alanine aminotransferase (ALT)and total bilirubin (TBil)were determined by biochemical enzyme as-say.Comparison between groups was made by analysis of variance;Pearson correlation analysis was performed. Results Among PBC pa-tients,the incipient group and recurrent group had significantly higher serum sICAM-1 levels than the remission group and control group (P=0.000 for all);there was no significant difference in serum sICAM-1 level between the incipient group and recurrent group (P=0.484);the remission group had a significantly higher serum sICAM-1 level than the control group (P=0.000).Among AIH patients, the incipient group and recurrent group had significantly higher serum sICAM-1 levels than the remission group and control group (P=0.000 for all);there was no significant difference in serum sICAM-1 level between the incipient group and recurrent group (P=0.802);no significant difference in serum sICAM-1 level was seen between the remission group and control group (P=0.281).For patients with PBC and AIH,serum sICAM-1 level was positively correlated with serum levels of ALT (r=0.664,P=0.000;r=0.784,P=0.000) and TBil (r=0.715,P=0.000;r=0.580,P=0.000).Conclusion Serum sICAM-1 may be involved in the immunologic injury in PBC and AIH.In patients with PBC and AIH,the elevation of serum sICAM-1 level is closely correlated with the severity of liver damage. Clinical monitoring of serum sICAM-1 level may play an important role in severity assessment, prognostic evaluation, and therapy guidance among patients with autoimmune liver diseases.

Objective To investigate the effect of activation of peroxisome proliferator-activated receptor β/δ ( PPARβ/δ)on protein synthesis and expression of angiotensin (Ang)Ⅱ-induced hypertrophic myocytes(MC) in vitro. Methods Hypertrophy in neonatal rat cardiac MC culture was established with AngⅡ, then the effect of GW0742 on hypertrophy was detected. The synthetic rate of protein in MC was detected by 3 H-leucine incorporation.mRNA and protein expression of atrial natriuretic IL-1βwas measured by reverse transcription-polymerase chain reaction ( RT-PCR) and Western-blotting. Results and Conclusion GW0742 could reduce the synthetic rate of protein in hypertrophic MC while down-regulating the mRNA and protein expression of IL-1β, but no changes were observed after treatment with DMSO.The result demonstrated that activation of PPAR beta/delta inhibited cardiac hypertrophy in vitro and this effect might be related to inflammatory factors.

Objective To propose a new observation procedure of blood pressure after the surgery of fiducial implantation for cyber knife, using the statistical analysis of the measurements of blood pressure at different time after the surgery. Methods By taking 200 patients after the surgery of fiducial implantation, the usual blood pressures and the blood pressures at different time after the surgery were compared statistically. Results Based on the statistical analysis, the usual blood pressures were statistically different from the blood pressure immediately after surgery, as well as the blood pressure at 0.5 hours after surgery [ systolic pressure (131.60 ±16.86), (129.08 ±14.77) mmHg (1 mmHg=0.133 kPa) vs. (123.00 ±11.80) mmHg, t=2.897 and 2.308, P<0.01 or 0.05;diastolic pressure (83.49±10.01), (81.75±8.99) mmHg vs. (78.22±7.59) mmHg, t=2.901 and 2.066, P<0.01 or 0.05]. The blood pressures measured at the other time after surgeries were not statistically different from the usual blood pressures, P>0.05. Conclusions Measuring the blood pressures at the time of 0, 0.5, 3.5 hours after the surgery can not only assure the complete recovery of the patients after the surgery, but also greatly reduce the work of nurses.

Objective:To study the predictive value of combining alpha-feto protein (AFP) with contrast-enhanced MRI imaging features in predicting incidence of microvascular invasion (MVI) in patients with hepatocellular carcinoma. Methods:The data of 206 patients with hepatocellular carcinoma treated at Tianjin Medical University Cancer Institute and Hospital from January 2017 to April 2019 were retrospectively analyzed. There were 179 males and 27 females, with an average age of 58.7 years. The roles of preoperative MRI imaging features and clinical data on predicting the incidence of MVI in patients with hepatocellular carcinoma were evaluated by univariate and multivariate logistic regression analyses. Multivariable regression analysis was then used to plot a nomogram. Results:There were 86 patients (41.7%) with MVI positivity and 120 patients (58.3%) with MVI negativity. Multivariate logistic regression analysis showed that AFP >400 μg/L ( OR=3.318, 95% CI: 1.243-8.855, P=0.017), two-trait predictor of venous invasion (TTPVI) ( OR=13.111, 95% CI: 6.797-28.119, P<0.001), diffusion weighted imaging/T 2 weighted imaging (DWI/T 2WI) mismatch ( OR=17.233, 95% CI: 4.731-44.490, P<0.001), and rim enhancement( OR=5.665, 95% CI: 2.579-18.152, P=0.013) predicted increased risks of MVI in patients with hepatocellular carcinoma. The constructed nomogram directly predicted the risk of MVI in these patients. Conclusions:AFP>400 μg/L, TTPVI, DWI/T 2WI mismatch and rim enhancement were independent risk factors in predicting MVI in patients with hepatocellular carcinoma. This predictive model of MVI which was based on multivariate logistic regression analysis was helpful to clinicians in making individualized treatment plans for patients with hepatocellular carcinoma.
