AIM: To discuss the effect of Smart Plug canalicular plug on aqueous-deficient dryeye.
METHODS:Forty-eight cases of aqueous-deficient dry eye patients in our hospital from May 2012 to April 2013 were selected for the study. After treated by Smart Plug canalicular plug, postoperative clinical efficacy, foundation Schirmer I test ( SIt) , tear film break-up time ( BUT ) , corneal fluorescein staining ( FL ) changes were observed.
RESULTS: Forty-eight patients were cured, 31 cases were markedly effective(65%), effective 14 cases (29%), invalid in 3 cases (6%), the total effective rate was 94%. Before treatment, SⅠt, BUT, and FL was ( 3. 49±1. 24 ) mm/5min, ( 3. 15±1. 07 ) s, and ( 2. 52±0. 11 ) points, respectively. After treatment, SIt, BUT, FL were significantly improved compared with before treatment, the difference was statistically significant (P<0. 05). One patient had postoperative foreign body sensation, 8h after shedding emboli; One patient granulation tissue after surgery 8mo, canalicular plug off. The remaining cases had no inferior lacrimal duct infection or granuloma.
CONCLUSION: Smart Plug canalicular plug is an effective treatment for aqueous-deficient dry eye, can effectively relieve symptoms, worthy of promotion.

To establish a GC method to determine the related substances in pharmaceutical excipient benzyl alcohol. Methods:A GC method was used with an Agilent DB-wax eapillary column(0. 32 mm × 30 m,1. 8 μm)and programming temperature. The initial temperature was 50℃, and then raised to 220℃ with a rate of 5℃·min-1 and maintained for 35min. The detector was FID. The temperature of the injection port was 200℃,and the detector temperature was 310℃. The results were confirmed by GC-MS. Results:Within a certain range,the peak area and concentration of every impurity had a good linear relationship (r≥0. 999 9). The recovery was between 96. 1% and 102. 7%. The quantitative limit was between 1. 37-3. 63 ng. Toluene, benzyl chloride, benzalde-hyde and benzyl ether were found out in the samples. Conclusion:The method is accurate and convenient, and suitable for the quanti-tative determination of related substances in pharmaceutical excipient benzyl alcohol.

Objective:To establish a method for the determination of benzene, chlorine alcohol and pyridine residues in piperazine ferulate. Methods:GC was used with a DB-624 (30 m × 0. 53 mm, 1. 0 μm) elastic quartz capillary column. The flame ionization detector was used with nitrogen as the carrier gas. The initial temperature was 50℃, maintaining for 5 min, and raised to 80℃ at the rate of 10℃·min-1 , and then raised to 200℃ at the rate of 50℃·min-1 , and maintaining for 4 minutes. The inlet temperature was 200℃, and the detector temperature was 220℃. The split ratio was 1 ∶1 and the injection volume was 1μl. The flow rate was 3 ml· min-1. Results:The linear range of benzene was 0.16-0.96 μg·ml-1(r=0.999 5), the average recovery was 95.7% (RSD =2.1, n=9), and the detection limit was 0.16 ng. The linear range of chlorine alcohol was 16.11-96.65 μg·ml-1(r=0.999 7), the average recovery was 97. 8% (RSD=2. 1, n=9), and the detection limit was 0. 62 ng. The linear range of pyridine was 15. 87-95. 23 μg·ml-1(r=0. 999 8), the average recovery was 99. 2% (RSD=1. 3, n=9), and the detection limit was 0. 15 ng. Con-clusion:The method is reliable, simple, accurate and stable, and suitable for the determination of benzene, chlorine alcohol and pyri-dine residues in piperazine ferulate.

Objective To investigate the relationship between carotid intima-media thickness (IMT) and coronary atherosclerosis plaque through coronary CT angiography (CTA) in asymptomatic individuals.Methods Two hundred and fifty cases with CTA screened were selected in asymptomatic individuals,and according to the inspection result they were divided into no plaque group (detection of no coronary atherosclerosis plaque,180 cases) and plaque group (detection of coronary atherosclerosis plaque,70 cases).All cases were checked carotid IMT with B-ultrasonic.Results ANOVAs analysis showed that age and body mass index (BMI) had significant correlation with coronary atherosclerosis plaque (P ＜ 0.05).x2 test was used to show the smoking and family history of coronary heart disease had significant correlation with coronary atherosclerosis plaque (P ＜ 0.05).Univariate Logistic regression analysis showed that carotid IMT,systolic pressure,diastolic pressure,low density lipoprotein cholesterol,fasting serum glucose and creatinine had significant correlation with coronary atherosclerosis plaque (P ＜ 0.05).Multifactor Logistic regression analysis showed that carotid IMT had correlation with coronary atherosclerosis plaque (P ＜ 0.05).Conclusion There is significant relationship between carotid IMT and coronary atherosclerosis plaque in asymptomatic individuals.

The management model “primary care in community,team service,first contact physician responsibility and two-way referral” was implemented for migrant workers with hypertension in the community.The hypertensive patients under the management for over six months were assigned as management group; the patients who were registered but not under management for over six months served as the control group.The number of new cases detected in hypertension screening increased from 32 in 2008 to 701cases in 2010; and number of hypertensive patients under management increased from 7 cases to 348 accordingly.A questionnaire survey was conducted in 464 hypertensive patients,including 196 cases in management group and 268 cases in control group with a response rate of 94.9％ (186/196) and 92.5％ (248/268) respectively.The results showed that drug compliance rate in management group (168/186,90.3％ ) was significantly higher than that in the control group ( 26.6％,66/248,P =0.00 ).The rate of blood pressure control in management group (87.1％,162/186 ) was significantly higher than that in the control group (25.8％,64/248,P =0.00).The overall satisfaction (98.9％) in management group was significantly higher than that in the control group (88.7％,P =0.00 ).The results indicate that implementation of the hypertension management in migrant workers improves the health seeking behavior and blood pressure control of patients.

Objective:To establish an HPLC method for the determination of related substances in piperazine ferulate. Methods:An HPLC method was used to determine the related substances in piperazine ferulate. The separation was performed on an Xtimate C18 column (250 mm × 4. 6 mm, 5 μm). The mobile phase was 0. 5% acetic acid-methanol-acetonitrile with gradient elution. The flow rate was 1. 0 ml·min-1 and the column temperature was 30℃. The detection wavelength was 286 nm and the injection volume was 20μl. Results:Ferulic acid had a good linear relationship within the range of 5-30 μg·ml-1(r=1.0000). The detection limit was 0. 02 ng. Conclusion:The method is reliable, simple, accurate, stable and durable, and suitable for the determination of related sub-stances in piperazine ferulate.

AIM: Recent studies have identified the importance of inflammation in the development and progression of heart failure.Chemokines are essential factors in the recruitment and activation of leukocytes.They are closely related with inflammation.So the relation between chemokines expression and lymphocytes recruitment and cardiac function after acute myocardial infarction(AMI) was studied.METHODS: Ligating left interventricular branch induced AMI.Experimental rats were divided into three groups: heart failure group(MI-HF),non-heart failure group(MI-NF) and sham group(sham).Sham group was made by the same procedure only without ligation.The blood dynamics of rats was examined after 3 days,1 week and 2 weeks following ligation.Rats,which had a left ventricular end-diastolic pressure(LVEDP) above 15 mmHg,were considered to be in heart failure.Reverse transcription polymerase chain reaction(RT-PCR) was used to analyze the mRNA expression of chemokines,including monokine induced by IFN-?(MIG),macrophage inflammatory protein-1 alpha(MIP-1?) and regulated upon activation,normal T cell expressed and secreted(RANTES),in the infarcted region(circumjacent region included).The numbers of lymphocytes infiltrated in the infarcted region were also counted.RESULTS: MIP-1?,RANTES and MIG mRNA increased at 3 days and reached the peak at 1 week after AMI,significantly higher in MI-HF group than those in MI-NF group(RANTES,0.83?0.05 vs 0.51?0.19,P

OBJECTIVE:To establish a method to determine and compare the contents of sodium benzoate in medicinal(phar-maceutical excipients and active pharmaceutical ingredients) and non-medicinal (chemical reagents and food additives) grade. METHODS:HPLC was conducted for content determination,SPSS 18.0 software was adopted to compare the results. The column was Purospher STAR LP RP-18 endcapped with mobile phase of acetotrile-0.02% formic acid(adjusted pH to 4.0 with aqua ammo-nia)(30∶70,V/V)at a flow rate was 1.0 ml/min,the detection wavelength was 230 nm,column temperature was 35 ℃,and vol-ume injection was 20 μl. RESULTS:The linear range of sodium benzoate was 10.5-525.3 μg/ml(r=0.999 9);RSDs of precision, stability,reproducibility and durability tests were lower than 0.5%;recovery was 99.38%-101.26%(RSD=0.56%,n=9). The av-erage contents of sodium benzoate in medicinal and non-medicinal grade were between 99.400%-99.875%,but the average content of non-medicinal grade is lower than the medical grade. CONCLUSIONS:The method is accurate and simple with high specificity and good reproducibility,and can be used to determine and compare the content of sodium benzoate in medicinal and non-medici-nal grade.

BackgroundBiological behavior of choroidal melanoma is closely related with angiogenesis.The microcirculation pattern in which tumor cells may be involved is different from neovascularization.ObjectiveThis study was to observe the microcirculation pattern of the choroidal melanoma tissue and analyze its relationship with the clinical pathology factor and the degree of cellular proliferation.Methods Forty-eight specimen of choroidal melanoma tissue were collected at Beijing Tongren Eye Centre from November 1997 through March 2006.Periodate acid Schiff staining was used to determine the microcirculation pattern of choroidal melanoma.The morphology of the tumor cells and the distribution of microcirculation pattern in the tumor were observed under a fluorescence microscopy with 544 nm wavelength,and the Ki-67 expression in the tumor was detected by immunochemistry.The relationships between the choroidal melanoma clinical pathology factor with Ki-67 expression and microcirculation pattern were evaluated by multiple stepwise regression model.ResultsA total of 9 kinds of microcirculation patterns were found by periodate acid Schiff staining.The occurring rate of loop- or network-like vascular pattern showed a significant elevation in the tumor tissue with epithelial cells in comparison with the tumor with spindle-like cells (66.7％ vs.30.6％ ) ( P=0.027 ).The expression rates of Ki-67 were 18.961 ± 10.995 and 19.481 ± 12.167 in the tumor tissue with loop- or network-like microvascular pattern,and those in the tumor tissue without loop- or network-like microvascular pattern were 10.261 ±5.669 and 12.021 ± 6.802,presenting significant differences between them ( P =0.000,P =0.010).Loop-like microvascular pattern was determined as the risk factor of the proliferation and metastasis of choroidal melanoma by multiple stepwise regression analysis (P=0.002).ConclusionsAmong the nine microcirculation patterns in choroidal melanoma,networks pattern is the most fashion,and Ki-67 expression is more strong in the tumor with epitheliod cells,suggesting that this microcirculation pattern is associated to the malignant degree and extent of proliferation in choroidal melanoma.

Objective To review the clinical and histopathological features,management and outcomes in 67 patients with seba- ceous carcinoma of the eyelid.Design Retrospective case series.Participants Sixty-seven cases of the sebaceous gland carcinoma were retrieved from the ophthalmology archives of Tongren Hospital between 1996 and 2005.Methods Clinical data of all patients were re- viewed from the charts.Pathology specimens were reviewed in all cases.Main Outcome Measure Presenting features,sites of origin, location,differentiation,management,histopathologic findings,and incidence of recurrence in 67 patients.Results A total of 36 female and 31 male patients were identified.The mean age at diagnosis was 59 years (range from 35 to 84).Initial clinical diagnoses elsewhere were sebaceous carcinoma (43.28%),chalazion (25.37%),basal cell carcinoma(7.46%).Initial histopathologic diagnoses were sebaceous carcinoma(79.10%),basal cell carcinoma(5.97%),squamous cell carcinoma(1.49%) and other or not available(4.48%).The initial treat- ment involved excisional techniques with reconstruction in 42 cases,anti-inflammatory in 12 cases,without any treatment in 10 cases. 61.19% cases presented the pagetoid invasion.Local recurrence developed in 58.21% and two died from metastatic sebaceous carcino- ma (8.70%).Regional lymph node excision was performed in 13.04% because of local metastases.Five year survival rate was 94.87%. Conclusions About 1/3 cases had not receive the right treatment at initial diagnosis eyelid.The recurrence rate of patients was still high.
