Hilar cholangiocarcinoma always presents with a mass, thickness of the bile duct wall or nodules in the bile duct. Computed tomography (CT) can demonstrate the above findings with high partial resolution as well as dilation of the intrahepatic bile duct. CT can show the field and the involvement of adjacent vessels on 3D reconstruction images for hilar cholangiocarcinoma spreading along the lumen of the bile duct. CT images can also demonstrate vessel involvement when evaluating the resectability. When combined with 3D reconstruc-tion, the sensitivity and specificity of CT imaging can reach 90.5% and 91.7% , respectively, for detecting vessel involve-ment.

Objective Investigation of satisfaction of clinical nurses and analysis of the influence factors.Methods The ERG theory of Clayton Alderfer is used as the framework to design the questionnaires,for questionnaire investigation of 249 nurses in the hospital in a stratified sampling,Results The score of general satisfaction of nurses for their work is 3.40±0.58;the satisfaction of nurses for the management of their leaders and teamwork is high,as the satisfaction scores are 4.01±0.76 and 3.60±0.71 respectively;their satisfaction for salary,welfare and social identity is low,as the scores are 2.46±0.86 and 2.50±1.02 respectively.Work satisfaction varies with departments,academic levels,monthly income,work shifts,age,seniority,and objectives for their choice of nursing as profession.Conclusion Managers are prompted to enhance their work in view of the satisfaction scores on survival,mutual relations and growth,as well as factors affecting satisfaction,and take various incentives to improve overall satisfaction of nurses.

Objective To investigate the distribution of syndromes in hospitalized hypertensive patients with maintenance hemodialysis. Methods A investigation-table of Chinese medicine was established and used in 105 eases for survey in clinic to discover the regularity. Results The occurrence rate from high to low of asthenia syndrome symptoms was: deficiency syndrome of both yin and yang, yin asthenia, deficiency of both vital energy and yin, yang asthenia, and the deficiency of qi. Objective To investigate the epidemiological characteristics of hospitalized hypertension patients with different metabolic abnormalities associated with cardiovascular disease.Methods 1 843 patients with diagnosis of hypertension were selected,including 327 patients with metabolic syndrome as the research object.Metabolic syndrome of different age,gender,abnormal components epidemic status were analyzed.Results Positive rates of overweight,dyslipidemia,hyperglycemia,cardiovascular disease in 327 patients were 42.20％,37.92％,35.17％,71.25％,the highest positive rate for cardiovascular disease was significantly higher than the other three indexs(x2 =56.22,73.28,85.51,all P ＜ 0.05).There was no significant differences in the rates of overweight between men and women (x2 =0.42,P ＞ 0.05).Dyslipidemia and hyperglycemia incidence of women were significantly more than those of men,there was statistically significant difference (x2 =5.73,6.47,all P ＜ 0.05).The incidence of cardiovascular disease in males was high than that in females (x2 =24.66,P ＜ 0.05).Conclusion Preventive different metabolic abnormalities need to take different measures according to age and gender.Weight control,blood lipids and blood sugar are the key to the prevention of cardiovascular disease.

AIM: By introducing the thinking and experience of professor Deng Tietao to treat myasthenia gravis, to improve the therapeutic effect and prognosis.benefiting vital qi had been the main line throughout the whole process of phasized that while we grasp spleen-asthenia, we must give consideration to other organs at the same time, regulate and treat the patient with multistrengthening muscle and increasing strength (radix astragali, radix codonopsis pilosulae, rhizoma atractylodis macrocephalae, rhizoma cimicifugae, pericarpium citri reticulatae, radix fici simplicissimae, etc.), the disease had been treated with different dosage of drugs according to the py for one or two years, even though the symptoms had disappeared completely.RESULTS: Because of grasping the root cause, therapy with multi-means and modified prescription according to the varied symptoms, it had a satisfactory effect. An excellent therapeutic effect had been obtained with the decoction for strengthening muscle and increasing strength, which is created by Deng himself.CONCLUSION: Deng's theory holds that myasthenia gravis is mainly due to spleen-asthenia, and related to other four organs (with a predominance of kidney), and the therapy should be strengthening the spleen-qi, so the prescription was selected for strengthening muscle and increasing strength (which is created by Deng himself), with radix astragali being in a lager quantity. Moreover, he paid more attention to psychological therapy and dietary regulation. The Deng's theory is of considerable clinical significance in guiding treatment of myasthenia gravis.

This report presented a case of 75-year-old woman who had received drug treatments two months earlier for nephrotic syndrome and was admitted to our hospital for inferior wall myocardial infarction with elevated creatinine and anemia.Kidney pathology after myocardial infarction showed allergic acute interstitial nephritis which induced acute renal failure.We stopped tripterygium glycosides and used cortical hormone,consequently.Thereafter,the symptoms of renal failure and anemia were improved and we considered tripterygium glycosides resulted in above allergic acute interstitial nephritis and anemia.Therefore,we had to carry out renal needle biopsy in the patient with the elderly nephrotic syndrome before confirmatory treatment to avoid blind use of tripterygium glycosides.

In order to explore the relationship between the expression of TGF ? 1,its receptor and the wound age during the healing process of rats skin incision wound,a preliminary study were performed on the vital skin wound 0 5～168h after injury by using the immunohistochemical and the molecular biological methods.The results were then compared with those of the postmortem skin injury.The results revealed that the expression of the cytokine TGF ? 1 in the epithelial cells were enhanced 0 5h after the antemortem skin incision and the strongest reactions were seen at 24～96h post injury.The expression of TGF ? 1 was also found in the macrophages and the fibroblasts in the granulation tissues.Analysis of the immunoblotting(Western blot)results showed that the peak value of the TGF ? 1 protein was appeared at 168h after the antemortem skin incision and peaked at 96h.TGF ? 1 was detected 0 5～3h after postmorten skin injury by using the immunohistochemical method.No expression of mRNA was found.It is suggested that some regular and characteristic expression of TGF ? 1 in the incised skin wounds were related with the wound age.It might be used for timing of skin wound on rat.

Objective To observe the rotenone induced changes of nitric oxide (NO) concentration and nitric oxide synthase (NOS) activity in rat caudate nuclei and blood plasma. Methods Wistar rats were daily administrated with rotenone at the doses of 0.335, 0.670 and 1.005 mg/kg(sc). Animals were sacrificed on days 30, 60 and 90. The NO concentration and the NOS activity in the caudate nuclei and blood plasma were determined by biochemical methods. Results At each time point of days 30, 60 and 90, rotenone induced significant increases in NO concentration and NOS activity in both the caudate nuclei and blood plasma in dose and time dependent manners. Conclusion Long term exposure to rotenone leads to the increase of nitric oxide and nitric oxide synthase in brain tissues and blood plasma in rat.
