1.On patient-entered design of hospital environment

It elaborates the importance of designing a patient-centered hospital environment, and presents the concept of such a design. The authors hold that such a concept is mostly composed of systematic, people-oriented, informationized and variability designs, and describe the design trends in contemporary hospitals. It is also pointed out that patient-centered principle should be dominant in building a contemporary hospital, as the guideline and development trend, as a satisfactory hospital environment makes a hospital more competitive.

4.Effects of temperature and time on the quality of cryoprecipitate

Objective To investigate the effects of temperature and time on the quality of cryoprecipitate.Methods Fresh frozen plasmas were divided into the three groups,and the cryoprecipitate were prepared from fresh frozen plasma by siphonage method thawing at 2℃,4℃ or 6℃ water bat.Thawing time,FⅧ and fibfinogen (Fg) were compared among the three groups.Results The thawing time at 2℃,4℃ or 6℃ was (96.3 ± 5.9) min,(89.6 ± 4.3) min,(81.2 ± 2.7) min.The activity of F Ⅷ was (95.36 ± 4.8) IU,(91.21 ± 3.3) IU,(89.8 ±3.1)IU,respectively.The activity of FⅧ was the highest at 2℃ condition and there was significantly different compared with other two groups (t =6.94,P =0.009,t =8.96,P =0.004).The activity of FⅧ in 4℃ group and 6℃ group were comparable (t =0.669,P =0.19).The concentration of Fg at three groups were similar (258.6 ±12.6) mg,(253.3 ±8.2)mg,(255.9 ±9.7) mg (t =0.321,P =0.37;t =0.270,P =0.56;t =0.424,P =0.29).Conclusion The activity of FⅧ at 2℃ condition were the highest but the thawing time was the longest.The activity of FⅧ at 6℃ condition were lower but met quality requirements and the thawing time was very short.There was no significantly different at Fg concentration among three groups.Thus 6℃ is more suitable for blood center to prepare cryoprecipitate from fresh frozen plasmas.
5.Clinical Study of Huangqi Danshen Drop Pill for Angina Pectoris Due to Coronary Heart Disease

Objective To evaluate the therapeutic effect and safety of Huangqi Danshen Drop Pill (HDP) for angina pectoris (AP) due to coronary heart disease (CHD) classified as qi- deficiency and blood- stasis syndrome. Methods A multi- center, randomized, double- blind, double- simulation, positive- controlled, parallel group trial was adopted. 209 cases of AP due to CHD classified as qi- deficiency and blood- stasis syndrome were enrolled in this study: 104 in HDP group and 105 in Yangxinshi Tablets group. Results The markedly effective rate was 43.27 % , total effective rate 86.53 % , and total effective rate in improving TCM symptoms 92.31 % in HDP group, which were better than those in Yangxinshi Tablets group(P
6.Hand hygiene status of residents living in Hangzhou City of Zhejiang Province

Objective To investigate the hand hygiene status of urban and rural residents in Hangzhou City of Zhejiang Province to provide scientific base for health promotion.Methods Urban residents,rural residents,personnel in government institutions,urban students,rural students and enterprise workers were enrolled in this investigation.By using constant volume sampling method,fifty individuals in each group were recruited from 13 districts of Hangzhou City,and a total of 3 337 people filled in the questionnaire,which included demographic characteristics,knowledge of hand hygiene and hand washing.Logistic regression analysis and x2 test were used for data analysis.Results Correct hand-washing was found in 35.84% urban and rural residents (urban vs.rural:41.45% vs.27.06%).Awareness rate of hand hygiene was 35.06%.The influencing factors of proper hand washing were location,gender,age,educational level and understanding of hand hygiene.The influencing factors of awareness of hand hygiene were location,gender,age and educational level.Conclusion Hangzhou citizens had a relatively lower rate of proper hand washing,and significant difference was found between urban and rural residents; the study suggested that para-urben residents need to improve their knowledge and behavior of hand hygiene.Meanwhile,the students urgenty need to improve their hand hygiene
7.Synovial sarcoma of the head and neck--a retrospective study of 39 cases.

OBJECTIVE:
Synovial sarcoma (SS) of the head and neck is rare in comparison with those took place in the extremities. This study was planned to investigate the relationship between pathological diagnosis, tumor location and clinical outcome of SS of the head and neck.
METHOD:
Thirty-nine cases of SS in head and neck hospitalized in West China Hospital from 1966 to 2011 was retrospectively studied by reviewing the medical record data, the pathological slices of the operative specimen and followed-up from 1 to 192 months with the mean time of 43.2 months postoperatively. The parameters of clinical outcome were focused on the time to first recurrence after primary surgery and follow-up time. The reviewed results were statistically processed.
RESULT:
The age of the patients ranged from 8 to 66 years old with the median age of 35, among them 27 are males. Pathologically, 18 cases are biphasic, 17 cases are monophasic and 3 cases are low-differentiated SS. 4 cases were proved by cytogenetic methods of either fluorescence in situ hybridization(FISH) or RT-PCR. 23 cases experienced repeated recurrence with the most up to 4 times operations after sole surgical approach. Only one lymphatic metastasis was suspected in all. 16 patients got adjuvant radiotherapy or chemotherapy. 4 patients died but only one death was associated directly with SS recurrence. There was no significant relationship between pathological subtype and recurrence (Fisher's Exact Test P-value > 0.05), no significant relationship between tumor location and recurrence (Fisher's Exact Test P-value > 0.05).
CONCLUSION
SS of head and neck is a special entity that has potential of easy recurrence but good prognosis. Surgery should still be the primary treatment approach. Cytogenetic methods are recommended to as certain the diagnosis in order to choose reasonable treatment protocols.

8.Effect of Digital Auditory Activation and Touching Intervention on Infants with Cerebral Dysfunction

ObjectiveTo observe the effect of digital auditory activation and touching intervention on infants with cerebral dysfunction.Methods388 infants with perinatal high-risk factors and abnormal result of Denver Development Screening Test (DDST) were randomly divided into group A (n=135), group B (n=128) and group C (n=125), and treated with digital auditory activation combined with touching (group A), simple touching (group B) and simple drug (group C) with 10 days as a course. All infants were tested with DDST after one therapeutic course, and tested again with DDST after the infants of group B and group C treated continuously for six therapeutic courses; and all infants were assessed with the Gesell development quotient (DQ) after six months.ResultsAfter one therapeutic course, normal rate of DDST was 71.11% in group A, 26.69% in group B and 20.00% in group C. After six therapeutic courses, that was 90.37 % in group A, 62.50 in group B and 40.00% in group C. After six months, the children with the Gesell DQ over 86 scores was 125 (92.60%) in group A, 90 (70.31%) in group B and 62 (49.60%) in group C. There were significant differences among three groups ( P<0.01).ConclusionThe digital auditory activation combined with touching has short therapeutic course and high efficacy, so it is a good therapeutic method for infants with cerebral dysfunction.
9.Mitral annulus velocities and time intervals for evaluation of global left ventricular diastolic function in patients with coronary artery disease

AIM:To detect and compare the longitudinal mitral annulus diastolic velocity and time interval changes by pulsed Doppler tissue imaging(DTI)in patients with angina pectoris(AP)and myocardial infarction(MI),and to explore the value of mitral annulus diastolic velocities and time intervals for evaluation of global left ventricular diastolic dysfunction.METHODS:Fifty patients with established coronary artery disease were divided into AP group(16 cases)and MI group(34 cases).Sixteen age-matched healthy individuals served as the control group.The septum,lateral,anterior and inferior walls of the mitral annulus were displayed,and selected for DTI spectrum sampling.Peak early and late diastolic velocities and their ratio,time to the onset and peak of the early diastolic wave,and regional isovolumic relaxation time were measured,and the average values of the four mitral annular sites were calculated and presented as Em,Am,Em/Am,QEm,TEm and IVRTm,respectively.RESULTS:Compared with the control group,Em and Em/Am were significantly lower in both the AP and the MI groups(P<0.01).Em was even lower in the MI group than that in the AP group(P<0.01).QEm,TEm and IVRTm were significantly longer in the AP and the MI groups than those in control group(P<0.01 or P<0.05).IVRTm was even longer in the MI group than that in AP group(P<0.01).IVRTm had significantly negative correlation with Em(r=-0.64,P<0.01).CONCLUSION:Em,Em/Am,QEm,TEm and IVRTm as measured by pulsed DTI may be promising indexes for quantitative assessment of global left ventricular diastolic dysfunction in patients with coronary artery disease.Em and IVRTm may indicate the severity of ischemic myocardial damage.
10.Correlation between genital tract Ureaplasma urealyticum infection in pregnant women and fetal malformation

BACKGROUND: Ureaplasma urealyticum may cause female genital tracts infection and lead sterility, infertility and spontaneous abortion. But it still is not clear whether Ureaplasma urealyticum is one of the factors responsible for fetal malformation.OBJECTIVE: To investigate whether Ureaplasma urealyticum is one of the key factor responsible for fetal malformation.DESIGN: Non-randomized observation of compromised children and their parents with concurrent control.SETTING: The department of immunology and microbiology of a medical college.PARTICIPANTS: Totally 16 malformed fetuses(3 female and 13 male cases) and their parents were chosen from the family seeking pregnancy care in the Department of Gynecology and Obstetrics of the First Affiliated Hospital of Luzhou Medical College and Luzhou First People's Hospital from 1995 to 1997.METHODS: Ureaplasma urealyticum detection was performed in 16 samples of the umbilic blood of the malformed fetuses, sperms of the father, and cervical mucus of the mother who conceived the malformed fetus. The 11 couples positive for Ureaplasma urealyticum were treated with macrolides and aminoglycoside antibiotics and followed up for the outcome of the following pregnancy.MAIN OUTCOME MEASURES: The relation between Ureaplasma urealyticum infection and fetal malformation was examined.RESULTS: Totally 11 of the 16 umbilical blood samples(69% ) were positive for Ureaplasma urealyticum, and serological tests of parents in the 11 positive cases detected rubella virus, cytomegalovirus or Toxoplasma gondii antibodies, nor was abnormality found in chromosomal examination. The following pregnancies and delivery were followed up in 4 women, and the babies showed no functional or organic abnormities with good development till the age of 2 years.CONCLUSION: Female genital tract Ureaplasma urealyticum infection is related to fetal malformation, which provides evidence to support interventions of the occurrence of fetal malformation.
