2.Efficacy of Medical Therapy after Conservative Operation for Endometriosis

Objective To investigate the efficacy of medical therapy after conservative operation for endometriosis.(Methods)Endometriosis was confirmed in 89 patients by laparoscopic inspection,and conservative operations were performed to remove the adhesion,excise or destroy all the endometriotic tissues,including single excision of endo-(metriotic) masses or plus unilateral salpingoophorectomy,and to restore pelvic anatomy to the best possible condition.All the patients were divided into two groups randomly: treatment group(n=44),three months drug therapy of go-(nadotropin-)releasing hormone analogs(GnRHa) after operation;control group(n=45),no drug therapy after operation.Clinical symptoms,concentrations of CA-125,manual and ultrasonic pelvic check-up,as well as pregnancy rate to those who had a desire for future fertility(n=28)were followed up. Results All the patients experienced alleviation of clinical symptoms after the operation.The recurring rates: 0% in treatment group vs 4% in control group six months later;2% in treatment group vs 4% in control group 12 months later(P
3.Baboon syndrome induced by mercury:a case report

A case of mercury-induced Baboon syndrome is reported.A 31-year-old female presented with a 2-3 day history of pruritic symmetric erythematous papules on both axillas,popliteal fossa,buttocks and groins.The patient was exposed to a broken mercury thermometer two days before the onset of the eruption.Patch testing showed that the patient was sensitive to ammoniated mercury,mercury,mercuric chloride and mercurochrome.The mercury levels in serum and full blood reached the up limit of normal value three days after the onset,and gradually decreased over time.The patient was diagnosed with Baboon syn- drome caused by mercury according to the clinical manifestation and results from laboratory studies.
4.Protective effect of vitamin D3 on ocular structure in diabetic rat

BackgroundResearch demonstrated that vitamin D3 mediated by its receptor has the potent nonclassical effects,including immunomodulatory,antiinflammatory,and neuroprotective properties,and it can enhance the secretion and sensitivity of insulin and therefore down-regulate hyperglycemia and attenuate the corneal edema.ObjectiveThe present study was to investigate the protective effect of vitamin D3on ocular structure in experimental diabetic rat.Methods Twenty-two healthy SPF C57BL/6 rats were randomly divided into vitamine D3 group (8 rabbits),diabetic control group ( 11 rabbits) and normal control group ( 3 rabbits).2% streptozotocin ( STZ,175 mg/kg)was intraperitoneally injected to create the diabetic models in the rats of the vitamine D3 group and diabetic control group.Blood glucose was examined for 3 times in the third day after STZ injection,and the rats with the blood glucose concentration >16.7 mmol/L was identified as the successful diabetic models.After modeling,the rat tail blood was collected for the monitoring of blood glucose.Two weeks after modeling,vitamine D3 was intraperitoneally injected in each week for 5 times.The fundus was examined using direct ophtalmoscope,and the eyeballs were obtained under the excessive anesthesia for the measurement of thickness of the central cornea,retina and choroids by histopathological examination once a week for 7 weeks after administration of vitamin D3.The administration of the animals complied with the Statement of ARVO.ResultsThe corneal edema appeared with the corneal thickness of (339.14± 11.13) μm in the first week and gradually attenuated with time elapse after modeling in the diabetic group ( F =382.446,P =0.000).The corneal thickness values were significantly decreased from the second week through the seventh week in the vitamin D3 group compared with diabetic control group(P<0.05).The atrophy of the corneal epithelium was found from the fifth week to the seventh week in diabetic control group,but that in vitamin D3 group was slight (P<0.05).The gradually thinning of the choroids was seen from the first week to the seventh week in the diabetic control group ( F =437.411,P =0.000 ),however,the thickness values in the vitamin D3 group were significantly increased in comparison with the diabetic control group in various time points (P<0.05).The retina thickness was gradually reduced during the seven-week duration in the diabetic control group (F =91.859,P =0.000),but no significant change was identified in retina thickness in the vitamin D3 group(P>0.05).ConclusionsVitamin D3 has prevent and therapeutic effects on experimental diabetic oculopathy.

This article mainly summarise the results of the chemical compositions and their pharmacological activities of Arnebiae Radix since 1966. The chemistry components isolated from Arnebiae Radix are mainly naphthoquinone, monoterpene phenol and quinone, phenolic acids and their salts, alkaloids, aliphatic and esters. Pharmacological results showed that the chemical compositions and the extracts of Arnebiae Radix have antibacterial, anti-inflammatory, anti-viral, hepatoprotection, antioxidant, anti-tumor and immune function and other activities. This article hopefully to provide a reference for further research, development and utilization of Arnebiae Radix.

6.Does Hospital Ownership Influence Hand Hygiene Compliance?

The issue as to whether hospital ownership has an impact on the quality of care has long been a serious concern.Hand hygiene (HH) compliance is regarded as an important indicator of the quality of care in the control of hospital-acquired infections.However,little information is available on whether hospital ownership influences HH compliance.In this study,of 229 hospitals selected from Hubei province in China,152 were public and 77 were private hospitals.A total of 23 652 healthcare workers (HCWs) were surveyed,using a convenience sampling.HH compliance,the WHO's "My Five Moments for hand hygiene" (5MHH),among HCWs,together with the factors of hospital ownership,training frequency,bed occupancy rates,etc.were collected.Univariate analysis and ordinal logistic regression analysis were used to analyze factors affecting HH compliance.Overall,HH compliance rates were 67% and 79% for public and private hospitals,respectively.The HH compliance rates of HCWs and 5MHH were between 55% and 95%,and influenced by hospital ownership (P<0.05),excluding compliance rate at the moment after body fluid exposure,and other influence factors included training frequency and bed occupancy rate (P<0.05).HH compliance is better in private than in public hospitals.Hospital ownership is a significant factor affecting HH compliance,in addition to training frequency and bed occupancy rate.
7.An analysis on influencing factors of job burnout for nurses from infectious disease hospital

Objective To learn the current status of job burnout and influencing factors for nurses from infectious disease hospital.Methods The Chinese version MBI -HSS was used to survey 218 nurses from infectious disease hospital,and linear regression was used to analyze the influencing facters of job burnout.Results A total of 210 questionairs were cocleted.The incidence of severe occupational job burnout was 22.38%,and the score of emotional exhaustion (EE), depersonalization (DP),personal accomplishment (PA)was(22.82 ±9.98),(6.48 ±5.20),and (35.20 ±8.82), respectively.Regression analysis demonstrated that the main influencing factors were entering the isolation ward, opportunity for infectious diseases ,disinfection damage,fear of occupational exposure and concern on family infection(P <0.05).Conclusion The status of job burnout of nurses in infectious disease hospital is not optimistic.There is a positive relationship with the working environment,occupational exposure.Managers need to explore an effective way to ease the job burnout of nurses,and to stabilize the nursing team.
8.A novel HIF-1 inhibitor--manassantin A derivative LXY6099 inhibits tumor growth.

Hypoxia-inducible factor-1 (HIF-1) is a key transcription factor on hypoxia responses in mammalian tissues. HIF-1 plays as a positive factor in solid tumor and leads to hypoxia-driven responses that enhance its downstream gene expression for tumor growth and survival. LXY6099 was obtained by the structural modification and optimization of manassantin A (MA) as a high potent HIF-1 inhibitor. Antitumor activity of LXY6099 was observed in this study. LXY6099 with an IC50 value of 2.46 x 10(-10) mol x L(-1) showed more sensitive inhibition activity to HIF-1 than that of MA detected by reporter gene assay (> 100 folds). It showed strong inhibition on the growth of human solid tumor cell lines. Furthermore, LXY6099 exhibited significant antitumor activity against established human tumor xenografts in nu/nu mice with treatment of MX-1 breast cancer. Thus, LXY6099 as a novel HIF-1 inhibitor could be further developed into anti-cancer agents.

9.A novel microtiter plate radioimmunoassay of insulin autoantibody

Objective Insulin autoantibody (IAA) is known to exist in sera of type 1 diabetes mellitus (T1DM) patients and pre-T1DM individuals. The aim of this study was to establish a novel microtiter plate radioimmunoassay (RIA) for IAA and evaluate its clinical value. Methods Diluted 125Ⅰ-insulin was mixed with 5 ul serum samples in a 96-well microtiter plate and then incubated for 72 h on an orbital plate shaker (4℃). The immunocomplexes were transferred to another protein a coated Millipore plate, and then the plate was washed with Tri-Buffered Saline Tween-20 (TBT) buffer. Counts per minute (CPM) was measured with liquid scintillation and luminescence counter. The positive cut-off point of IAA index was defined as ≥0.06 based on the 99-percentile of the distribution in 317 healthy individuals. The specificity and sensitivity of the assay were calculated from the samples provided by the fourth Diabetes Autoantibodies Standardization Program (DASP 2005). The IAA levels were determined in 71 T1 DM and 551 newly diagnosed type 2 diabetes (T2DM) patients, and 317 healthy controls. The t test, non-parametric test, x2 test and linear correlation analysis were performed on the data using SPSS 11.5 software. The concordance rate was estimated with Kappa value. Results (1) The optimized testing condition was described as 2×104 CPM of 125Ⅰ-insulin, 5 ul serum sample and slowly horizontal shaking for 72 h. (2) The intra-assay CV was 4.8%-8.9% and inter-assay CV was 6.4%-10.5%. Based on DASP 2005 samples, the specificity and sensitivity of the assay were 97% (97/100) and 50% (25/50), respectively. Ninety-six serum samples with different IAA levels were selected and tested to compare between our new method and a domestic IAA RIA kit. The results showed that the IAA indices from the two methods were positively correlated (r= 0.678, P<0.001). The concordance rate was 72.9 %(Kappa value=0.402). There were 25 samples with discordant results, which were positive for IAA titer using the corresponding microtiter plate RIA but negative using the novel RIA kit. (3) In TIDM group the positive rate of IAA was 19.7% (16/71), higher than the healthy controls (0.9%, x2=54.36, P<0.001). The subgroup of T1DM children (with 0-9 years) showed the highest IAA positive rate (55.6% ,x2=4.85, P<0.05). In T2DM group the frequency of IAA was 1.5% (8/551), which had no significant difference comparing with that of healthy controls (x2= 0.95, P >0.05). Conclusions Our proposed microtiter plate RIA method for IAA is highly sensitive and specific, likely to be feasible for clinical application. The frequency of IAA is high in children with T1DM.
10.Subclavicular angiography with brachial-axillary artery externally and temporally occluded in interventional treatment of hemoptysis

Objective To evaluate subclavicular angiography with brachial-axillary artery externally and temporally occluded in depicting hemoptysis-related ectopic arteries originating from shoulder, neck and chest wall in the interventional treatment of hemoptysis. Methods A retrospective analysis of the medical records and angiograms of 103 patients who underwent transarterial procedures for hemoptysis was carried out. The arteries originating from the subclavicular arteries supplying the shoulder, the neck and the chest wall, ectopic bronchial arteries (EBA) as well as non bronchial systemic arteries (NBSA), were depicted by subclavicular angiography with brachial-axillary artery externally and temporally occluded. The number and distribution of the responsible aberrant arteries were investigated. Results Of the hemoptysis-related arteries(n=372) depicted and embolized in the studies, 51.9% was normally-originated bronchial arteries(n=193),6.7% was ectopic bronchial arteries (n=25),41.4% was NBSA(n=154).Both anomalous and NBSA arising from subclavicular-axillary area(n=80) were accounted for 21.5% (80/372) of all aberrant arteries, and 44.7%(80/179) of all anomalous and NBSA (n=25+154=179).Of those arising from subclavicular-axillary arteries,56.3%(45/80) were found to be originated from the internal thoracic artery (n=45),98.2%(112/114)of responsible internal thoracic arteries and their distal branches were well depicted by subclavicular angiography with brachial-axillary artery externally and temporally occluded. Conclusion Subclavicular-axillary artery is a very important vessel where EBA and NBSA originate. Subclavicular angiography with brachial-axillary artery externally and temporally occluded could help to determine the abnormal arteries in the region quickly. The method should be taken as a routine procedure for interventional treatment of hemoptysis.
