1.Effects of repetitive transcranial magnetic stimulation on post-stroke depression, sleep quality and activities of daily living

Objective To explore the effects of repetitive transcraninal magnetic stimulation (rTMS) on depression,sleep disorder and activities of daily life (ADL) in post-stroke depression (PSD) patients.Methods A total of 90 PSD patients were divided into a control group,a treatment group and a combined treatment group according to a random number table,30 cases in each group.All patients were given basic treatment and comprehensive rehabilitation training.Moreover,the control group was additionally received escitalopram treatment,the treatment group rTMS treatment,the combined treatment group the above both,lasting 8 weeks.Before and after the treatment,the Hamilton depression (HAMD) scale,the Pittsburgh sleep quality index (PQSI),the Chinese stroke scale (CSS) and the modified Barthel index (MBI) were employed to evaluate depression,quality of sleep,neurological impairment and the activities of daily living (ADL) of patients in three groups.Results Before treatment,there was no significant difference in the HAMD,CSS,PSQI and MBI score among the 3 groups.After 8 weeks' treatment,the HAMD,PSQI and CSS scores of three groups had significantly decreased,but their MBI scores had significantly increased.Compared with the control group,no significant differences were observed in HAMD,CSS,PSQI and MBI scores of the treatment group and the combined treatment group (P > 0.05).After treatment,there was no significant difference in the above four values between the combined treatment group,and the treatment group.Conclusion The rTMS can significantly improve PSD patient's depressive symptoms,relieve sleep disorders and neurological deficit situation,and effectively improve their ADL.
2.Comparison of expression of renal transforming growth factor-?_1 and its receptors in rats treated by(cyclosporine) A and tacrolimus

Objective To compare the expression of renal transforming growth factor-?_1(TGF-?_1),its receptors type Ⅰ and Ⅱ(T?RⅠ and T?R Ⅱ) in two rat models of chronic nephropathy induced by CsA and FK506 respectively.Methods Rat models of chronic CsA-/FK506-induced nephropathy were established by administering sandimun Neoral and Prograf separately.Then their kidneys were dissected and the protein expression of TGF-?_1,T?RⅠand T?RⅡ,the mRNA levels of T?RⅠ and T?RⅡ was detected by using immunohistochemistry(IHC) and in situ hybridization(ISH) respectively.Results The IHC revealed that the integrated optical densities(IODs) of renal TGF-?_1,T?RⅠ and T?RⅡ were(605.24)?(140.24),(876.28)?(208.73) and(981.59)?(258.65) respectively in CsA-treated rats,(488.37)?(101.76),(586.63)?(131.21) and(543.34)?(105.26) respectively in FK506-treated ones.The ISH indicated that the IODs of renal T?RⅠ and T?RⅡ mRNA were(905.08)?(158.75) and(1090.92)?(242.73) respectively in CsA-treated rats,(661.37)?(205.65) and(716.27)?(195.55) respectively in FK506-treated ones.The difference of the above-mentioned five factors between the two groups was significant(P

4.Extracorporeal membrane oxygenation support for inter-hospital transport of potential cardiac death donors

Objective To discuss Extracorporeal Membrane Oxygenation(ECMO) management method and effect during inter-hospital transport of potential cardiac death donors after cardiac death (DCD).Methods 8 potential donors after cardiac death with brain injury were supported by ECMO for inter-hospital transport.All donors were inserted Medtronic overall cannula into one side femoral artery and venous.The position of catheters were guided by ultrasound.The front-end of venous catheter located in the junction of atrium and inferior vena cava,meanwhile the front-end of artery catheter was below renal artery.100 IU/kg heparin was injected before inserting cannulas.Flow of ECMO maintained at 2.0~3.0 L/min,and oxygen flow was 2~3 L/min during ECMO supporting.When hemodynamics of potential donors were stable,patients were moved into ambulance with ECMO for inter-hospital transport.Results A total of 8 ECMO transports were performed for central circulatory collapse caused brain injury.Patients were previously cannulated and on ECMO prior to transport and transported a distance of more than 100 kilometer from our institution by ambulance.ECMO running times were 120 min,and operation process circulatory stable.Conclusion ECMO can ensure inter-hospital transport of potential donors after cardiac death safety.
5.Influence of proteolytic cleavage blockage and N-linked oligosaccharide inhibition on the membrane targeting of rodent Muc3 C-terminal domain

Objective To explore the correlation between membrane targeting of rodent Muc3 C-terminal domain and proteolytic cleavage blockage within its SEA module and N-linked oligosaccharides inhibition.Methods COS-1 cells were transfected with three different expression vectors containing rodent Muc3 C-terminal domain,namely p20,p20t and p20s/a by lipofectAMINE reagent.Inhibition of N-glycosylation of the expressed protein was performed by using tunicamycin.The transfected COS-1 cells(fixed or unfixed) were detected by immunolocalization experiments(anti-V5 and anti-Myc antibody) for the protein expression.Results In fixed COS-1 cells,the expressed product of p20 transfectant detected using both anti-Myc and anti-V5 antibodies was found to localize in perinuclear position and on the plasma membrane.While in the unfixed cells,immunostaining was only confined on cell surface using anti-V5 antibody.The expressed product of p20t transfectant was detected by anti-V5 antibody to localize only in perinuclear region,as observed in a few fixed cells.The distribution of p20s/a fluorescence resembled that of p20 transfectant.Plasma membrane targeting of the non-glycosylated products due to tunicamycin treatment still occurred in transfected COS-1 cells and resembled the glycosylated products.Conclusions The blockage of proteolytic cleavage within C-terminal domain of rodent Muc3 and its inhibition of N-linked oligosaccharides in SEA module cannot affect its membrane targeting.The only apparent requirement for membrane targeting is the transmembrane and/or cytoplasmic tail segments which exist in the C-terminal domains of rMuc3.
6.Endemic diversity and interventions on schistosomiasis after reserving plain for flooding in Jicheng and Qingshanhu

Objective To understand endemic diversity on schistosomiasis transmission after reserving plain for flooding. Methods In two study pilots, Jicheng and Qingshanhu, epidemiological factors were investigated longitudinally, and the effectiveness of the interventions was evaluated. Results The infection rates of mobile people who engaged in activities in the discarded plain were increased year by year. The density of infected snails was high. The snail habitats increased significantly in Jicheng, but decreased in Qingshanhu. The infection rate and number of livestock pastured in the discarded plain increased. In the discarded plain, most of the mobile people came from the local areas, and main activities for water contact were fishing and pasturing. About 90% of local residents migrated into endemic areas, and the others into non-endemic areas. Conclusions The discarded plains were evolving to a serious transmission zone of schistosomiasis. Interventions combined with agriculture and fisher productions can decrease snail-spreading. Schistosomiasis examination and chemotherapy for the migrants to non-endemic areas are vital.
7.The effection of environmental and health message forecasting service offer to the allergic rhinitis in children.

OBJECTIVE:
To investigate the impact of environment and message information for the treatment efficacy of children with allergic rhinitis (AR).
METHOD:
To choose 100 cases of children diagnosed with AR. The parents of children record daily symptom scores of AR of children everyday. While the doctors send the short message to the parents by using meteorological environment warning forecast technology to tell the parents the risk of AR attacks. We observe the morbidity of AR of the children and the treatment efficacy of children for 1 year. The 100 cases control group children diagnosed AR, their parents do not record daily symptom scores of AR and the doctors do not give the short message. We also observe the morbidity of AR of the control group children and the treatment efficacy of the children for 1 year.
RESULT:
The 100 cases intervention group children with AR, there are 11 cases refused to participate the group. We included a total 89 cases, 74 cases of children did a full year of intervention, 15 cases of children gave up. The control group 100 cases, nine cases refused to participate, 91 cases were included. The average episodes in the intervention group was 4. 67 times, the control group was 8. 12 times,--there were significant differences between the groups statistically. The execution rate on the prescribed clinic date in the intervention group visits was 91. 5%, while in the control. group, was only 67%. The compliance of parents of children to complete course of medication in the intervention group was 95. 6%, while the control group was 74.1%. Both sets of data are statistically significant differences. The children with sinusitis in intervention group was 26.97%, significantly lower than the 64.04% in the control group. The incidence in the intervention group and the control group of secretory otitis media was 8.99% and 6.60%, there was no statistically significant differences.
CONCLUSION
It can significantly increase the degree of attention of parents of children with AR that the parents of children record daily symptom scores of AR of children everyday, while the doctors send the short message to the parents by using meteorological environment warning forecast technology to tell the parents the risk of AR attacks. It can also improve the execution rate on the prescribed clinic date and improve compliance of parents of children to complete course of medication. It can significantly reduce seizure frequency and severity of episodes of AR, thereby improving the quality of life of children with AR, reduce the economic burden on families and society.

8.Changes in pentane concentration in expired air induced by hepatic ischemia-reperfusion injury in rabbits

Objective To investigate the changes in pentane (marker of lipid pemxidation) concentuation in the expired air induced by hepatic ischemia-reperfusion(IR)injury in rabbits.Methods Thirty healthy male Japanese white rabbits weighing 2.4-3.0 kg Were randomly divided into 2 groups(n=15 each):group Ⅰ sham operation and group Ⅱ hepatic IR.Anesthesia was induced with 3% pentobarbital 30mg/kg iv and maintained with The animals were tracheostomized and mechanically ventilated.Femoral artery and vein were cannulated for BP monitoring and blood sampling.Hepatic blood flow was occluded for 25 min followed by 180 min reperfusion.The expired air was collected before(baseline,T0)and at 1,10 and 25 min of ischemia and at 1,10,25,60,120 and 180 rain of reperfusion (T1-9) for determination of pentane concentration which was extracted by solid phase micro-extraction (SPME) and detected by gas chromatography-mass spectrography(GC-MS).Blood samples were also taken at the above time points for measurement of serm ALT,AST and SOD activities and MDA concentration.The animals were killed at the end of 180 min reperfusion.Liver specimens were obtained for histology.Results There was no significant difference in age,body weight,MAP and HR between the two groups.There was no significant change in pentane concentration in the expired air and the ALT,AST and SOD activities and MDA concentration at all time points as compared with the baseline in sham operation group.In IR group the pentane concentration in the expired air was significantly increased at T4,5 and the ALT and AST activities and MDA concentration were significantly increased and SOD activity was significantly decreased at T7-9 as compared with the basehne and the sham operation group.Conclusion The pentane concentration in the expired air can reflect the occurrence of lipid peroxidation during hepatic IR injury.
9.Rigid choledochoscopy via biliary fistula tracts to remove bile duct stones

Objective To study the indications,feasibility and efficacy of rigid choledochoscopy via biliary fistula tracts to remove bile duct stones.Methods A retrospective analysis was performed on the clinical data of 86 patients with bile duct stones treated with rigid choledochoscopy via biliary fistula tracts at our hospital between November 2011 and July 2016.Patients with bile duct stones were divided into the percutaneous transhepatic cholangio drainage (PTCD) group and the T tube tract group.There were 40 patients who underwent lithotomy using rigid choledochoscopy via the PTCD tract and 46 patients who underwent choledocholithotomy using rigid choledochoscopy via the T-tube tract.A comparison was conducted to compare the duration of the procedures,the amount of perioperative bleeding,the postoperative complication rates and residual stone rates between the two groups.Results In the PTCD group,the average operation time was (77.0 ± 36.5) min,the amount of perioperative bleeding was (26.5 ± 54.1) ml,and the postoperative complication rate was 37.5 % (15/40).Complete lithotomy in one-stage was successful in 33 patients,and in two-stages in 1 patient.The residual stone rate was 15.0% (6/40).In the T tube tract group,the average operation time was (82.5 ± 44.1) min,the amount of perioperative bleeding was (14.8 ± 21.0) ml,and the postoperative complication rate was 32.6% (15/46).Complete lithotomy in one-stage was successful in 34 patients,and two-stages in 2 patients.The residual stone rate was 21.7% (10/46).There were no significant differences in the residual stone rates,complication rates and operation time between the two groups (P > 0.05).The amount of operative bleeding was significantly better in the T tube tract group than the PTCD group,(P < 0.05).Conclusions There was no significant differences in the clinical efficacy in the treatment of bile duct stones using choledochoscopy either via the PTCD tract or the T tube tract group.Both approaches can be used for bile duct stones.
10.Determination of hesperidine inErchen mixture by HPLC

Objective To establish a method for the determination of hesperidine inErchen mixture. Methods HPLC method was used with Fortis C18(250 mm×4.6 mm,5μm)column and the mobile phase consisted of acetonitrile-acetic acid-water(22∶4∶74). The flow rate was 1.0 ml/min,the column temperature was 30℃, and the UV detector was set at 283 nm.Results The linear ranges of hesperidine was 2.86-57.20μg(r=0.999 8). The average recovery of Aloin was 99.4%.Conclusion The method is rapid, reliable, simple, repeatable and with high specificity. It can be an important supplement for Erchen mixture elevated standard.
