1.Etiological factor and treatment of abnormal blinking in children

Abnormal eye blinking in children is a disease of frequent eye blinking in children, more than 15 times per minute, without facial spasms and neurological diseases, with relatively independent clinical symptoms. If combining with organic or neurological disease, we consider it as children eye blinking syndrome. It's a common and frequently encountered disease in pediatric ophthalmology. The etiologies include body and psychological disorders, such as refractive errors, ocular surface and ocular adnexal disease, bad habits, lead pollution, tic disorders and so on. We think that most of abnormal eye blinking in children firstly caused by oculopathy, neurological disorders and psychological illness are the important factors for increasing or making it repeatedly happen. Recognizing and identifying the etiologies of abnormal eye blinking in children, so as to take a targeted theray and avoid misdiagnosis and delayed treatment.
2.Effect of regulation in human umbilical vein endothelia cells treated by microRNA-132

AIM:To evaluate the regulatory effect of microRNA-132 ( miR - 132 ) in human umbilical vein endothelial cell ( HUVEC) .
●METHODS: ln vitro cultured human umbilical vein endothelia cells in hypoxic environment for 6h, then maintained under normal oxygen condition for 3h, 6h, 12h, 24h. miR-132 and peroxisome-proliferator-activated receptor-γ coactivator-1α ( PGC-1α) expression was detected by quantitative Real - time polymerase chain reaction and Western blot analysis. Human umbilical vein endothelial cells transfected miR-132 mimic and miR-132 inhibitor( anti-miR-132 ) were measured by quantitative Real-time polymerase chain reaction and Western blot.
●RESULTS: miR - 132 and PGC - 1α expression was significantly ( P < 0. 01 ) upregulated in the hypoxic environment of cells at 3h compared with the normal oxygen condition. After cells transfection, the hypoxic environment the miR-132 and PGC-1α expression were markedly increased compared with the normal oxygen condition. The cells transfected miR- 132- mimic, the expression of the miR-132 and PGC-1α were higher than that of transfected anti-miR-132 and contrast group ( P<0. 01).
●CONCLUSION:miR-132 level is highly expressed in the HUVEC under hypoxia and may be an effect of regulation for PGC-1α.
3.Significance of hypoxia induced alveolar epithelial cell apoptosis and hypoxia inducible factor-1alpha expression

Objective To investigate the effects of cobalt chloride(COCl2)induced rat alveolar epithelial cell apoptosis and the significance of related apoptosis protein hypoxia inducible factor-lalpha(HIF-1 a)expression. Methods Alveolar epithelial cells were cultured in vitro and divided into four groups with 6 samples in each.Hypoxia 4 h group:added 500μmol/L COCl2 for 4 hours inducing alveolar epithelial cell hypoxia;hypoxia 24 h group:co-cultured with COCl2 for 24 hours;hypoxia 48 h group:co-cultured with CoCl2 for 48 hours;control group:no CoCl2 added.The apoptosis rates were detected by fluorescent microscope and flow cytometry.The apoptosis morphological changes of the cell were observed by transmission electron microscopy.The protein expression of HIF-la was detected bv Western Blot. Results (1)Fluorescent microscope results showed that apoptosis rate in hypoxia 4 h、24 h、48 h group[(5.83±0.76)%,(15.00±3.28)%and(51.50±3.00)%],were higher than control group[(1.50±0.50)%](P<0.05).(2)Flow cytometry analysis reached the same results as with fluorescent microscope.(3)Typical morphological changes of apoptosis were observed in hypoxia 24 h group.(4)HIF-1α expression in hypoxia 4 h、24 h and 48 h groups[0.69±0.035,1.02±0.044 and 0.71±0.046],were higher than control group(0.21±0.026)(P<0.01).(5)Positive correlations were found between relative amount of HIF-1αprotein and apoptosis rate by fluorescent microscope and flow cytometry analysis(r=0.484,P=0.016;r=0.713,P=0.009).Conclusions Hypoxia could induce apoptosis of alveolar epithelial cells,which may participate in the pathogenesis of hypoxia-induced lung injury.
4.Pulmonary protective effects of dexmedetomidine in septic rats induced by LPS

Objective To assess the role of intervention of dexmedetomidine in lung tissue and plasma of septic rats induced by lipolysaccharide.Methods Forty SD rats weighing 250-300 g were divided into 4 groups:control group (n =10)with saline (1 ml·kg-1·h-1 )infused through the tail veins of rats for 6 h;dexmedetomidine (DEX)group (n =10)with dexmedetomidine,injected 6.5 μg ·kg-1·h-1 for 10 min followed by 5 μg·kg-1·h-1 ,continuously infused through tail veins for 6 h;li-polysaccharide (LPS)group (n = 10 )with lipolysaccharide (7.5 mg/kg)infused through the tail veins of rats;LPS+DEX group (n =10),after infusion of LPS,rats were treated with dexmedeto-midine (bolus 6.5 μg·kg-1·h-1 ,10 min;5 μg·kg-1·h-1 ,continuous infusion)through the tail veins for 6 h.ELISA and Western blot were performed to assess the protein expression of IL-1,IL-6, TNF-αand TLR4;the wet/dry ratio (W/D)of lung tissue in each group was measured and the injury scores were recorded through the Murakami method.Results There was no statistical significance between the control group and DEX group in plasma level of inflammatory factors,pulmonary TLR4 expression,wet/dry ratio and injury score of lung tissues,however,compared to the control group, there was a significant elevation in the above-mentioned biomarkers,W/D and injury score of lung tis-sue (P <0.01).Compared to the control group,there was no statistical significance in the LPS+DEX group.Compared to the LPS+DEX group,the biomarkers mentioned above,W/D and injury score of lung tissues were significantly elevated in the LPS group (P <0.01).Conclusion The irrita-tion of LPS can trigger the elevation of inflammatory factors in plasma,TLR4 expression in lung tis-sues in rats,while the intervention of dexmedetomidine can significantly alleviate this tendency. Dexmedetomidine can suppress the inflammatory reaction and pulmonary edema in septic rats.
5.Preliminary study of the acoustic radiation force impulse imaging in diagnosis and treatment of IgA nephropathy

Objective To investigate the value of acoustic radiation force impulse (ARFI) on evaluating the renal tubular atrophy and interstitial fibrosis of IgA nephropathy(IgAN).Methods 117 patients diagnosed with IgAN,and 80 normal cases were examined with conventional ultrasonography and ARFI,recording the shear wave velocities(SWV),the hemodynamics and the renal biopsy results to quantify the elastic changes of renal cortex,with the aim of assessing AFRI's advantages on early diagnosing the impair of kidney.Results The SWV value of renal cortex,renal medulla of IgAN group were smaller than that of normal group (P <0.05),while the collective system SWV value had no difference(P >0.05).According to the ROC curve,the cut-off value of SWV in renal cortex was 2.7 cm/s.The accuracy of the diagnosis was 71 %,which was better than RI (61.9%).The SWV value of the renal cortex decreased with the different stage of 1,2,3,4,but stage 5 was larger than stage 4.The degree of renal tubular atrophy and interstitial fibrosis(T) can be classified by the SWV of renal cortex,and the SWV of T2 was larger than T1 (P <0.05).Conclusions By quantifying the elastic change of kidney,ARFI technology is able to distinguish the degree of renal tubular atrophy and interstitial fibrosis,diagnose renal impairment early and judge the clinical stages.Furthermore the sensitivity,specificity,accuracy of it are superior to hemodynamic.
6.Melanoma immunotherapy: difficulties and strategies

Melanoma is a highly malignant immunogenic tumor. Although immunotherapy represented by immune checkpoint inhibitors can markedly improve the survival rate of patients with metastatic melanoma, nearly half of patients are still tolerant or resistant to immunotherapy, with high incidence of immune-related adverse reactions. This review focuses on the key difficulties in tolerance, resistance and immune-related adverse reactions to current melanoma immunotherapy, and summarizes corresponding strategies and research advances. By intervening in the immunosuppressive microenvironment in melanoma, screening precise biomarkers, and optimizing immunotherapy-based combination strategies, the problem of tolerance and resistance to immunotherapy can be solved. Moreover, the combination of traditional immunotherapy and nanotechnology can also greatly reduce the occurrence of immune-related adverse reactions. In the future, more extensive and in-depth research on the tumor immune microenvironment will help to explore the best immunotherapy regimens for melanoma.
7.Apoptotic mechanism of neuron-like cells differentiated from adult rat mesenchymal stem cells in vitro

AIM:To investigate the mechanism of apoptosis during the process of adult rat mesenchymal stem cells(MSCs)differentiating into neuron-like cells in vitro.METHODS:The MSCs were isolated primarily from adult rats bone marrow by density gradient and then expanded in medium as undifferentiated cells for 3-5 generations.The MSCs were divided into three groups at random.The control group was induced with ?-mercaptoethanol(?-ME).Meanwhile,the U0126 group was given ?-ME and U0126(10 ?mol/L)added at the beginning of induction.The PMA groups were treated with ?-ME and different concentrations of PMA since pre-induction.The effects of U0126 and PMA on the activity of neuron-like cells were observed by MTT.The effects of U0126 and PMA on the expression of neuron specific marker nestin and expression of apoptosis-related protein caspase,Bcl-2,Bax in neuron-like cells were detected by using immunocytochemistry method.TUNEL technique was used to detect apoptosis index.RESULTS:Compared to control group,neuron viability,nestin and Bcl-2 increased and neuron apoptosis decreased in U0126 group(P

Notoginseng Radix et Rhizoma (Sanqi), the underground part of Panax notoginseng (Burk.) F. H. Chen (Araliaceae) is commonly used in Chinese medicine for treatment of haemorrhage, haemostasis, swelling, etc. The aerial part including leaves, flowers and fruits are also applied for similar functions. Triterpenoid saponins are considered to be responsible for the biological activities of Sanqi. Up to date, more than 100 saponins have been isolated from theroots, rhizomes, leaves, flowers and fruits of P. notoginseng. The reported saponins can be classified into protopanaxadiol (PPD), protopanaxatriol (PPT), C17 side-chain varied and other types, according to the skeletons of the aglycons. The present review summarizes the saponins isolated from P. notoginseng and their distribution in different medicinal organs, as well as the pharmacological actions on cardiovascular system.

9.Nursing care of a patient with hemophilic arthritis and hepatitis C undergoing unilateral hip and knee replacement simultaneously

Objective:To summarize the perioperative nursing experience of a patient with hemophilia arthritis complicated with hepatitis C undergoing unilateral hip and knee arthroplasty at the same time.Methods:In August 2018, a patient with hemophilic arthritis complicated with hepatitis C was admitted to the first hospital of Lanzhou University. Preparations for health education, functional exercises, examination and evaluation before operation was made. Unilateral hip and knee arthroplasty were performed at the same time, strengthen the management of incisions and catheters after the operation, actively prevent complications, and guide patients to perform functional rehabilitation exercises under the conditions of good pain management and safe coagulation factor activity; pay attention to the nursing of coagulation factor replacement treatment in the whole perioperative period.Results:After careful nursing, no serious complications occurred during the patient's hospitalization, and the position and force line of the prosthesis were satisfactory by imaging examination before discharge.Conclusion:The perioperative care of this patient proved that the above-mentioned nursing measures are effective for hemophilic arthritis patients with hepatitis C undergoing unilateral hip and knee arthroplasty at the same time.
