1.Application of zebra fishes in studies on traditional Chinese medicines.

The zebra fish model, as an integral animal model, features small volume, high throughput, low cost, short cycle and reliable experimental results, thus has been widely used in medical studies. Traditional Chinese medicines (TCM) constitute a complex system, their active ingredients and action mechanisms are among study hotspots in during the development of modern TCMs. Along with the constant improvement of advanced technologies and methods, zebra fishes have been increasingly applied in studies on TCMs and shown advantages in active screening, and toxicity and metabolism studies. In this paper, TCM studies by using zebra fishes in recent years are summarized to provide new ideas and methods for basic studies on TCMs.

2.Experimental study and clinical application of rehabilitation therapy for short bowel syndrome

ObjectiveTo observe the efficacy of rehabilitation therapy for patients and experimental animals with short bowel syndrome.MethodsThirty SD rats were randomly divided into control group,short bowel group and growth hormone (GH) treatment group.Short bowel was produced by resecting 80% of the rats′ small intestine, growth hormone at a dosage of 1?U/kg?d was delivered to the rats from POD1 to POD28.Nine patients with short bowel syndrome were enrolled in the study,the average length of the small intestine was (44?24)?cm, 3 cases had also suffered a short colon.ResultsIncreased ratio of S phase cells, proliferation index and expression of proliferative cell nucleus antigen (PCNA) were observed, indicating the putative effects of exogenous GH on residual small intestinal mucosa. The mechanism of action was probably through the alteration of protooncogene C-jun. The nutritional state of the patients and the absorptive capacity of the residual intestine improved markedly. Follow up were made to 8 patients,75% of the patients have weaned TPN,25% of them need intermittent TPN supplementation.ConclusionsRehabilitation therapy provides a new and effective alternative for short bowel patients.
3.Silencing effect of lentivirus-mediated RNA interference on metastasis-associated gene 1(MTA1) in human nasopharyngeal carcinoma cell line

Objective To investigate the difference between two substrains,5-8F and 6-10B,of nasopharyngeal carcinoma cell line SUNE1on expressing metastasis-associated gene 1 (MTA1),and evaluate the silencing effect of lentivirus-mediated RNA interference (RNAi) on MTA1 in human nasopharyngeal carcinoma cell line.Methods The differential expression of MTA1 mRNA and MTA1 protein in 5-8F and 6-10B cell lines were detected by real time PCR and Western blotting respectively.Short interference RNA (siRNA) fragment targeting MTA1 gene was designed using online databases and software.A specific RNAi lentiviral vector targeting human MTA1 gene was constructed and transfected into nasopharyngeal carcinoma cell line 5-8F.Real time PCR and Western blotting were performed to detect the expressive changes in MTA1 mRNA and MTA1 protein in the transfected 5-8F cells respectively.Results Compared with cell line 6-10B,the expressions of MTA1 mRNA and MTA1 protein were higher in 5-8F cell line.Sequence analysis validated the correct insert of siRNA targeting MTA1 gene into the lentiviral vector.Real time PCR and Western blotting results showed that the expressions of MTA1 mRNA and MTA1 protein in 5-8F cell lines were down-regulated significantly after siRNA transfection.Conclusions MTA1 may promote the malignant transformation and enhance the metastatic potential of nasopharyngeal carcinoma cell lines.The expression of MTA1 in 5-8F cells can be inhibited effectively by MTA1-specific siRNA expression lentiviral vector,which provides a valuable tool for investigating the role of MTA1 gene in the carcinogenesis and progression of nasopharyngeal carcinoma.

5.Clinical Observation of Telbivudine Combined with Adefovir Dipivoxil in Long-term Treatment of Active Hep-atitis B Cirrhosis

OBJECTIVE:To observe therapeutic efficacy and safety of telbivudine combined with adefovir dipivoxil in long-term treatment of active hepatitis B cirrhosis. METHODS:76 patients with active hepatitis B cirrhosis in our hospital were ran-domly divided into observation group and control group,with 38 cases in each group. Control group was treated with adefovir dip-ivoxil 10 mg,qd;observation group was additionally treated with telbivudine 600 mg,qd,on the basis of control group. Both groups received 2-year consecutive treatment. TBIL,ALB,ALT,AST,rate of HBV-DNA negative conversion and serum virologi-cal index were compared between 2 groups after treatment,and clinical efficacy and ADR were also compared between 2 groups. RESULTS:After treatment,the levels of AST,ALB,ALT and TBIL in observation group were significantly better than the control group,with statistical significance(P<0.05);the rate of HBV-DNA negative conversion,the decreased of serum virus in observa-tion group were significantly higher than control group after 1 and 2 years treatment,with statistical significance(P<0.05). The to-tal effective rate of observation group was significantly higher than that of control group,with statistically significance(P<0.05). No obvious ADR was found in 2 groups. CONCLUSIONS:Telbivudine combined with adefovir dipivoxil can significantly improve the liver function of patients with hepatitis B cirrhosis and have good long-term therapeutic efficacy with high safety.
6.CO2 laser assisted posterior cordotomy for bilateral vocal cord paralysis.

OBJECTIVE:
To investigate the feasibility and efficacy of CO2 laser assisted posterior cordotomy for patients with bilateral paralysis of the vocal cord.
METHOD:
Twenty-one patients with bilateral paralysis of the vocal cord underwent CO2 laser assisted posterior cordotomy in our hospital from Jul. 2009 to Jun. 2015. The causes of the bilateral paralysis of the vocal cord were thyroidectomy in 15 cases, cervical trauma in 2 cases, resection of pituitary tumor in 1 case, inflammation in 1 case and without cause in 2 cases. Ten patients underwent tracheotomy before the operation; 9 patients underwent tracheotomy after the operation; 2 cases without tracheotomy.
RESULT:
The tracheotomy tube was plugged next day after posterior cordotomy. Lots of patients were breathing well without any effort. A few patients were breathing hard, so the tracheotomy tube was plugged discontinuously. Seventeen patients were decannulated 1 year after operation. Two patients were failed to decannulated. The patients were followed up for 3 months 6 years after operation, and all of them were breathing well.
CONCLUSION
Our limited experience showed that CO2 laser assisted posterior cordotomyis an effective and reliable surgical procedure for patients with bilateral paralysis of the vocal cord.

7.Investigation of hand hygiene compliance after patient contact

Objective To evaluate hand hygiene compliance (HHC) after patient contact and the influence of different factors on HHC in China. Methods HHC by different levels of medical workers from different departments were observed on 8 tertiary hospitals in Beijing, Shanghai and Guangzhou. Results The average HHC was 56. 5%( 61. 0% by the doctors and 53. 8% by the nurses). HHC was 52. 8% in the presence of gloves and 82. 0% in the absence of gloves; the difference was statistically significant. 8.An analysis on current situation and development tendency of hospital infection management in China

Objective To understand the current situation of hospital infection management in some hospitals in China and explore its development tendency. Methods Surveys were made by means of questionnaires on hospital infection management in 16 hospitals of various tiers in Beijing, Shanghai and Guangzhou and the results were compared with those of the two nation-wide surveys conducted respectively in 1994 and 1999. At the same time a dynamic assessment was made of hand hygiene implementation. Results The infection management systems in large and medium-sized hospitals in China were becoming increasingly perfect and a fairly high level was reached in function construction, team structure and expertise of hospital infection management departments. However, there were still many problems. 60% of medical staff washed their hands after patient-contact while only 35% did so before patient-contact and after item contact. Conclusion Hospital infection management in China has reached an important stage of accelerated development with substantial accumulation and more solid foundation. However, as far as development tendency is concerned, we should introduce technical indexes for quality control, put more emphasis on process-evaluation rather than result-evaluation in quality control management, stress the guiding role of evidence-based medicine in hospital management and promote a benign interaction between the hospital infection management and medical treatment process.
9.Influence of triamcinolone acetonide on peribulbus tissues via periocular injection

Background Triamcinolone acetonide (TA) is often used in the treatment of uveitis and fundus disease,but whether it has toxic effect to normal tissue around eyes is unclear.Objective The aim of this study was to explore the adverse and toxic effects of TA on the periorbitally normal tissue following the periocular injection.Methods Twenty-seven New Zealand albino rabbits were randomly divided into experimental group,normal saline solution group and blank control group.TA was periocularly injected twice (20 mg for each) at 1-month interval in the rabbits of the experimental group,and 0.5 ml normal saline solution was used in the same way in the rabbits of the normal saline solution group.Not any drug was used in the blank group.The rabbits were sacrificed 1 month and 2 months,3 months after the secondary injection.For the preparation of the specimens of extraocular muscles,peribulbus adipose tissue,lacrimal gland and optic nerve.Hematoxylin and eosin staining of the specimens was performed to examine the the pathological change under the optical microscope,and the ultrastructural of peribulbus tissues was observed under the transmission electron microscope.Results No statistically significant change was found in IOP among various time points and groups (Fgroup =0.952,P =0.881 ; Ftime =7.297,P =0.411).The hyphological structures and ultrastructure of extraocular muscles,peribulbus adipose tissue,lacrimal gland and optic nerve were normal in the rabbits of the normal saline solution group and the blank control group.However,the optical microscopy showed varying degrees of atrophy or dissolving in the extraocular muscle fibers,the increase of glandular epithelial cells and enlargement of nuclei in the lacrimal gland as well as visible disorder arrangement of nerve fibers,vacuoles degeneration and the decrease of glial cells in optial nerve tissue in the rabbits of the experimental group from 1 month though 3 months after the secondary injection.Under the transmission electron microscope,the muscle stripes,bright band and dark band were incompletely disappeared,and expansionary sarcoplasmic reticulum and oval nucleus at the inferior to sarolemma were seen in the extraocular muscles.In addition,swelling lacrimal gland epithelial cells,thickening nuclear membrane and more organelles were exhibited in the lacrimal gland.In the optic never tissue,uneven myelin,space between axons and myelin,lamellar separation and degeneration also were revealed in the rabbits of the experimental group after injection of TA.Peribulbus adipose tissue was near normal both by optical microscope and transmission electron microscope in the experimental group.Conclusions The periocular injection of TA dose not elevated the IOP.However,it will result in the histogical and ultrastructural damages of extraocular muscles,lacrimal gland and optic nerve.
