Chinese Journal of Rheumatology 2009;13(5):320-323
Objective To investigate the relationship between serum surfactant protein-A (SP-A) and -D (SP-D) in patients with systemic lupus erythematosus (SLE) complicated with interstitial lung disease.(SLE-ILD) and its clinical significance. Methods Serum SP-A and SP-D levels of SLE patients and controis were assessed using a sensitive enzyme-linked immunosorbent assay (ELISA).The relationship between SP-A and SP-D and SLE-ILD,the correlation between SP-A and SP-D with age, disease activity index, pulmonary function test and high-resolution computed tomography (HRCT) score were analyzed. Results The level of serum SP-A and SP-D in patients with SLE is higher than that in healthy controls(P<0.05).The level of serum SP-A and SP-D in patients with SLE complicated with ILD was higher than that in patients without ILD and healthy controls (P<0.05).The level of serum SP-D in patients with SLE-ILD was correlated positively with HRCT score (r=0.508,P=0.004) and interstitial score (r=0.468,P=0.009),and it correlated inversely with vital capacity (%VC) (r=-0.590,P=0.001 ) and diffusing capacity of carbon monoxide (%DLCO)(r=-0.588,P=0.001 ).There was no correlation between the level of serum SP-A with SLE-ILD and HRCT score,%VC and %DLCO.The level of serum SP-D in patients with SLE-ILD correlated positively with serum IgG (r=0.376,P=0.040),and the level of serum SP-A in patients with SLE-ILD correlated positively with serum C reactive protein (CRP)(r=0.403,P=0.027).There was a positive correlation between the level of serum SP-D in patients with SLE and age (r=0.352,P=O.001 ).Conclusion The levels of serum SP-A and SP-D may be useful markers for ILD in patients with SLE.There is correlation between the level of serum SPD and HRCT score, pulmonary function test, and it is positively correlated with age and disease activity index.

Objective To analyze the principal components and trends of total health expenditure from 1995 to 2011 in Xinjiang,in order to put forward policy recommendations to improve health financing. Methods Calculating the total health expenditure in Xinjiang using the financing source method, and analyzing the calculation results. Results Total health expenditure increase in Xinjiang averaged 14.75% from 1995 to 2011.The total health expenditure in Xinjiang and per capita health expenses maintained a growing trend, and the level of financing was higher than regional economic development, but the health consumption elasticity coefficient was unstable; Residents' individual health burden was gradually decreasing, the health expenses of financing structure were relatively reasonable; Urban and rural residents health care spending grew faster than the per capita income, and the proportion in people's living consumption was found rising. Conclusion It is recommended to continue with the health reform, make full use the policy of strategic health support to Xinjiang, promote primary health service equity, develop economy and improve the urban and rural income, strengthen social health care security systems, tighten macro-control and curb the fast rising trend of health expenses.

Bibenzyl is a type of active compounds abundant in Dendrobium. In the present study, we investigated the inhibitory effects of six bibenzyls isolated from Dendrobium species on vascular endothelial growth factor (VEGF)-induced tube formation in human umbilical vascular endothelial cells (HUVECs). All those bibenzyls inhibited VEGF-induced tube formation at 10 micromol x L(-1) except tristin, and of which moscatilin was found to have the strongest activity at the same concentration. The lowest effective concentration of moscatilin was 1 micromol x L(-1). Further results showed that moscatilin inhibited VEGF-induced capillary-like tube formation on HUVECs in a concentration-dependent manner. Western blotting results showed that moscatilin also inhibited VEGF-induced phosphorylation of VEGFR2 (Flk-1/KDR) and extracellular signal-regulated kinase 1/2 (ERK1/2). Further results showed that moscatilin inhibited VEGF-induced activation of c-Raf and MEK1/2, which are both upstream signals of ERK1/2. Taken together, results presented here demonstrated that moscatilin inhibited angiogenesis via blocking the activation of VEGFR2 (Flk-1/KDR) and c-Raf-MEK1/2-ERK1/2 signals.

AIM: To investigate the effect of soybean isoflavones (SI) on ventricular remodeling induced by myocardial infarction (MI). METHODS: MI was induced by permanent ligation of the left anterior descending coronary artery (LAD) in male Sprague-Dawley (SD)rats. Rats were randomly divided into six groups: shamThree hours after the operation, the drugs or solvent were administrated ig qd for 35 d. Twenty-four hours after the last administration, heart rate (HR), left ventricular end-diastolic pressure (LVEDP), peak systolic left ventricular pressure (Peak), maximal rate of left ventricular pressure rising (+dp/dtmax), maximal rate of left ventricular pressure declining (-dp/dtmax) and myocardial maximal shortening velocity (Vmax) were measured. After above hemodynamic parameters were measured, hearts were extracted. The ratio of total ventricle weight to body weight(TVW/BW) was calculated. Myocardial collagen was shown with the collagen-specific picrosirius red stain, myocardial interstitial collagen volume fraction(ICVF), perivascular collagen volume fraction(PCVF), infarct size, septal thickness (ST) and left ventricular diameter (LVD) were measured by image analysis system. RESULTS:(31 ± 5) %, respectively, all of which were significantly smaller than that in MI group (( 38.9 ± 2.9) %, P < 0.01 ).In MI group, TVW/BW, LVD, ICVF and PCVF were remarkably increased compared with those in sham group (P < 0.01 ). The increased TVW/BW, LVD, ICVF and PCVF were significantly reduced by treatment with eaprats in MI group was significantly decreased than that in sham group (P < 0.01 ). The reduced ST could be in-dp/dtmax and Vmax were decreased (P < 0.01 ), while LVEDP was increased (P < 0.01 ), significantly. The reSION: SI can improve cardiac function and ventricular remodeling induced by myocardial infarction in the rat.

Objective To summarize the measures for nursing mechanical ventilated patients who suffered from respiratory failure caused by acute severe organophosphorus pesticide poisoning (ASOPP). Method Retrospectively, the nursing clinical data of 18 mechanically ventilated patients who suffered from ASOPP-induced respiratory failure in ICU from January 2013 to December 2014 were analyzed for summarizing the nursing measures concerning preventive extubation, airway humidification and sputum sucking, prevention of ventilator-associated pneumonia and final extubation. Result Sixteen ASOPP patients successfully recovered after hospital discharge and another two died from multiple organ function failure because of excessive pesticides. Conclusions Effective observation and nursing care of airways based on their characteristics in ASOPP patients including prevention of unexpected shedding , appropriate humidity of airways and prevention of infection of ventilator-associated pneumonia(VAP). All of these measures can reduce complications and contribute to the early recovery of patients.

To evaluate the practical photodynamic therapy (PTD) efficiency on tumor of en-dogenous ALA-induced PP Ⅸ. Methods: Morphoraglcal alterations were observed after PDT 0f protopor-phyrin Ⅸ (PP Ⅸ) by application of endogenous 6-aminolaevulinic acid(ALA) in Kunming mice bearing H22hepatic carcinoma. Results: After PDT, the tumor cells showed degeneration, deformation and nuclearnecrosis and the tumor tissues were defused with hemorrhage and necrosis. The ultrastructural pathologyindicated that mitochondria in tumor cells were injured and destroyed. C0nclusi0n: The pathological alter-ations suggest that PDT of ALA on H22 hepatic carcinoma was conducted by the destruction of mitochon-dria.
