1.The application of telescopes in the restoration of alveolar process cleft and the defects of front maxillae

Objective: To explore the possibility to restore alveolar process cleft and the defects of front maxillae with telescope techniques. Methods: 4-5 telescopes were used as retainers of a prosthesis for cleft alveloar process or the defects of front maxillae; metal framework was used as connector of abutments and obturator in soft-tissue defect. Porcelain or resinmetal crowns were applied to restore defects in dentition. Acrylic resin was applied to repairs the deftcts of alveolar process and front maxillae or defect of palate. By above ways, prostheses were fabricated, the patients were followed up. Results:The following up for 26-28 months in 6 cases showed that the prosthesis retained with telescopes had follow advantages: good retention, convenient application, high masticatary function and satisfactory esthetics effects. Conclusion:The prosthesis with telescopes is feasible for the restoration of cleft alveolar process and the defects of front maxillae.
2.Clinical Study on Morphological Characteristics of Placenta in Severe Pregnancy Induced Hypertension

Objective To study morphological characteristics of placenta in severe pregnancy induced hypertension (SPIH) and its relationship to pregnant outcome. Methods Morphological changes were observed by light microscopy; Blood biochemical analyses were used to predispose pregnant outcome. Results Difference in pathological changes of placental bed between normal term pregnancy (NTP) and SPIH groups was significant: the placental weight, proliferation of cytotrophoblasts, numbers of the placental villi with syncytial knots, thickness of basal lamina, fibrinoid necrosis and deposition of matrix, stromal edema and fibrosis of villi, the vascular numbers of villi, stasis; lack of physiologic changes in decidual spiral arteries. Clinical examination showed that the rate of anemia, thrombocytopenia, blood concentration, hypoproteinemia, ascites, eye ground artery spasm in SPIH were higher in NIT. Conclusions Pathological changes of placenta play important roles in development of SPIH, and the pathological changes are paralleled with the severity of the disease.
4.Gene mutation screening and the genotype-phenotype correlation of hereditary multiple exostoses

Objective To establish the method of gene mutation screening for HME and investigate the relationship between genotype and clinical phenotype in HME patients. Methods Fifteen cases of HME probands were divided into the following four subgroups: mild (M) and severe ( Ⅰ S, Ⅱ S, Ⅲ S) according to the clinical diagnosis. DNA samples were obtained from the probands and family members. All of the EXT1 and EXT2 gene exons and their boundary sequences were amplified by PCR, and sequenced by directsequencing. Then the relationship between the genotypes and clinical phenotype was analyzed. Results Among the fifteen cases of HME probands, nine harbored EXT1 gene mutation, while the other 6 were positive for EXT2 gene mutation. Moreover, six novel mutations in EXT1 gene, including I8 + 2T > G, c. 1182delG,c. 1108G >T(p. E370X) ,c. 335delA,c. 361C >T(p. Q121X) and c. 1879_1881delCAC were identified. In 9 patients with EXT1 gene mutation, 2 (22. 2% ) were M-type, 2 (22. 2% ) were Ⅰ S -type, 4 (44. 4% )were Ⅱ S-type,and 1 ( 11.1% ) was ⅢS-type. Whereas, 5 cases (83.3%) were M-type and only one case was Ⅱ S-type( 16. 7% ) in 6 patients with EXT2 gene mutation. Conclusions An accurate and simple gene diagnostic method for HME was established. Six novel EXT1 gene mutations, including I8 + 2T > G,c. 1182delG, c. 1108G >T(p. E370X), c. 335delA, c. 361C >T(p. Q121X)and c. 1879_1881delCAC were identified as well. The clinical phenotype of the patients with EXT1 gene mutation was more severe compared to those with EXT2 gene mutations.
5.Preparation and Quality Evaluation of Xiaochuan cataplasm

Objective To study the influence of the preparation and quality control of Xiaochuan cataplasm on its drug action. Methods Xiaochuan cataplasm was prepared with hydrosoluble polymers matrix. A sequence of tests such as adhesive force test, recontour test, paste-content test, cold-resistance test, heat-resistance test, toxicity and pharmacodynamic action tests were performed on this drug. Results Xiaochuan cataplasm contains a sound volume of medicine, and shows good adhesive property, no sensibilization and stimulation, and notable drug action. Conclusion The results demonstrate that the preparation craft of Xiao-chuan cataplasm is reasonable and practicable., the quality is controllable and the method is a safe and effective transdermal drug delivery system.
6.Analysis of circulating Th17 cells and regulatory T cells in mynsthenia gravis

Objective To investigate the percentage of Th17 cells and regulatory T cells in patients with myasthenia gravis (MG) and observe the effects of methylprednisolone infusion on these cells.Methods The circulating Th17 cells and CD4+ CD25highT cells of 66 MG patients and 35 healthy controls were detected using four colour cytometry. The relationship between these cells and MGFA score in 18 MG patients and in 8 MG patients after methylprednisolone was infusion were also analyzed in this study. Results There was significant difference in the percentage of circulating Th17 cells between MG patients (2. 61% ±0. 28% ) and the healthy controls (0. 94% ± 0. 12%, Z = 4. 059, P = 0. 0001 ). Methylprednisolone therapy significantly reduced the percentage of circulating Th17 cells from 4. 72% ± 1.21% to 1.81% ± 0. 69%(Z = 1. 995,P = 0. 0460). In addition, the percentage of Th17 cells showed a positive correlation with MGFA score(r =0. 5359, P =0. 0219). Conclusions Circulating Th17 cells are elevated in MG patients.Methylprednisolone therapy can reduce such elevation, and this may be important in mediating symptomatic improvement in MG patients.
7.Distribution and effects of gene expression of four efflux pumps in Pseudomonas aeruginosa with multi-drug resistance in Hunan province

Objective To investigate the effects and distribution of 4 efflux pumps and correlated mutation of regulatory gene in multi-drug resistance Pseudomonas aeruginosa (Pa) in Hunan province. Methods Forty non-duplicated clinical strains of multiple-drug-resistant Pseudomonas aeruginosa were collected in Hunan in 2008, then the phenotype was screened with efflux pumps inhibitor phenylalanine-L-β-naphthylamide and 4 antimicrobial susceptibility test discs. Genes of the efflux pump membrane fusion protein were amplified by PCR, and correlated efflux pump regulatory genes were amplified and sequenced to analyze the role of efflux pump gene expression in multi-drug resistance compared to that of 18 strains with non-multi-drug resistance. Re-suits The positivity rates of MexAB-OprM, MexCD-OprJ, MexEF-OprN, MexXY-OprM were 45.0% (18/40), 30.0% (12/40), 42.5% (17/40) and 12.5% (5/40) respectively with phenotype screening in multi-drug resistance group. The positivity rates of mexA, mexC, mexE and mexX were 100% (58/58), 22.5% (9/40), 45.0% (18/40) and 22.5% (9/40) with RT-PCR. The overexpressed positivity rates of mexA and mexX were 55.0% (22/40) and 22.5% (9/40) respectively by semi-quantitative analysis with real-time PCR. However, no over-expression of mexA and mexX in non-multi-drug resistance group with real-time PCR. The positive expression rates of mexC and mexE with RT-PCR were 11.1% (2/18), 38.9% (7/18) in non-multi-drug resistance group. The difference of overexpression of mexA and mexX was significant(P<0.001, P=0.045) between two groups. The strain Pa20 with mexA overexpressian displayed gene mutations in mexR(164GTC→GAG) and amino acid substitution(126Val→Glu), the strain Pa34 with overexpression of mexX had 164GCG→GAG,55Ala→GIu. Conclusion Most of Pa with multi-drug resistance contained the resistant mechanism of efflux pumps and most-ly due to the overexpression of MexAB-OprM and MexXY-OprM in Hunan. There are mostly regulatory genes mutation in the strains with overexpression of MexAB-OprM and MexXY-OprM.
8.The establishment of pediatric reference intervals for creatine kinase-MB and troponin Ⅰ

Objective To establish the pediatric reference intervals for Troponin Ⅰ and Creatine kinase-MB (CK-MB).Methods Healthy children (223 boys and 162 girls) aged from 7 months to 12 years old were studied.Blood specimens were collected and centrifuged,all serum samples were kept refrigerated below-70 ℃.Troponin Ⅰ and CK-MB were measured on the UniCel DxI 800 immunoassay system (Beckman Coulter) on the same day.Nonparametric statistics was used to estimate the 99th percentile reference intervals.Results The children were divided into three groups (group A 128 cases; group B 128 cases;group C 129 cases) according to age.The upper reference limits for CK-MB were dependent of age,and the group A (7 months-1 years) was shown to be 13.41 μg/L,the group B (2-3 years) was shown to be 9.35 μg/L,the group C (4-12 years) was shown to be 5.48 μg/L.The upper reference limit for Troponin Ⅰ was independent of age or gender and shown to be 0.01 μg/L among 385 children.Conclusions Pediatric reference intervals for Troponin Ⅰ and CK-MB have been defined in healthy children at a relevant age group (7 months-12 years).It is effective for clinical diagnosis and treatment of myocardial damage for children by determining reference intervals for Troponin Ⅰ and CK-MB.
9.A tumor transplant experiment: Theanines function to fight A549 with T lymph cells through stimulating dendritic cells (DC)

Objective To reveal the impact of anti-lung cancer A549 by theanine, which may regulate T lymphocytes through dendritic cells (DCs), and to observe the immune protection of theanine on SCID mice transplanted tumors. Methods SCID mice were reconstructed after immunization, then A549 cells were inoculated. The immune protection and immune treatment of theanine stimulated DCs were observed. Results In the theanine-stimulated DC group, as well as DC group, the time needed to form tumor were significantly prolonged (F = 29. 5, P < 0. 01). And the transplanted tumors were smallest (F =69. 51, P <0. 01) and lightest (F = 190. 9, P <0. 01) in the theanine-stimulated DC group. 50 days after tumor-bearing, the transplanted tumors in the theanine-stimulated DC group were smaller than in the control group, and the surface were smoother with no adhesions. Under light microscope, a large thin slice necrosis was showed in the theanine-stimulated DC group, and the VEGF expression was also reduced (F = 31.4, P < 0. 01). Conclusions The anti-tumor immune protection and treatment mechanism of theanine-stimulated DCs were possibly through regulation of T cells, as well as the VEGF expression reducing,thereby inhibited tumor blood vessel formation.
10.Treatment for severe abdominal infection caused by acute obstructive suppurative cholangitis combined with diabetes mellitus

Acute obstructive suppurative cholangitis (AOSC) is one of the most serious acute abdomen.The incidence of abdominal infection is significantly improved when patients were complicated with AOSC and diabetes mellitus,and then bile leakage,intestinal fistula and even death were induced by abdominal infection.The management of abdominal infection is very important for the prognosis of patients.One patient with AOSC and diabetes mellitus was admitted to the No.425 Hospital of PLA from May to July in 2013.Recurrent abdominal infection and abscess occurred after the operation due to his family members' refusal of surgery at early period and lax control of blood glucose before and after surgery,and then bile leakage and intestinal fistula were induced.The patient underwent operation for 3 times,and was cured after receiving fasting,gastrointestinal decompression,parental nutrition,somatostatin administration and antibiotic treatment.
