2.Analysis of soluble expression,purification and antigenic characterization of P protein of GⅡ.2[P16]norovirus

ObjectiveTo obtain the soluble expression of P protein of norovirus(NV)G Ⅱ. 2[P16]and analyze its antigenic characteristics.MethodsThe recombinant plasmid GⅡ.2-NV-pET-28a was constructed by optimizing the NV GⅡ.2[P16]P protein gene,synthesizing and cloning it into pET-28a(+)vector. The recombinant plasmid was transformed into DH5α competent cells,and the expression strain was optimized to make the P protein express in soluble form. The expressed P protein was purified by Ni-NTA affinity chromatography,of which the antigenicity was detected by Western blot,the purity was detected by size exclusion high performance liquid chromatography(SEC-HPLC),and the antigenantibody binding ability was identified by ELISA.ResultsThe target protein of recombinant BL21 Star(DE3)pLySs was highly expressed in soluble form. After purification by affinity chromatography,the purity of GⅡ.2 P protein was 95. 53%,and the antigenicity was good.ConclusionSoluble NV P protein was successfully prepared by prokaryotic expression strain,and the antigenicity of P protein was confirmed,which laid a foundation of the development of NV G Ⅱ.2[P16]detection kits and the recombinant subunit vaccines.
3.Effect of articular step-off on knee joint contact pressures in fractures of the medial tibiul plateau

Objective To study the effect of articular step-off on knee joint contact pressures in fractures of the medial tibial plateau. Methods Eight specimens of embalmed adult knee joint were used to establish models of articular step-off of the medial tibial plateau by 0,1,2,3 mm respectively (Schatzker Ⅳ fracture) .The technique of pressure sensitive film was used to measure the contact pressures on the weight-bearing area of the knee joint in the 3 models. Results When the step-offs of medial tibial plateau were 0,1,2,3 mm,the average contact pressures on the lateral compartment were respectively 3.34 ± 0.06 MPa,3.48 ± 0.06 MPa,4.19 ± 0.05 MPa,4.19 ± 0.04 MPa,while the average contact pressures on the medial coMPartment were 4.04±0.17 MPa,4.57±0.20 MPa,5.64±0.20 MPa,5.64±0.19 MPa.When the step-off of medial tibial plateau was 2 mm,the average contact pressures on the medial and lateral plateaus increased by 39.6% and 25.4% compared with the 0 mm step-off,with statistically significant differences (P <0.05). Conclusions If the step-off of the medial tibial plateau reaches ≥ 2mm,open reduction and internal fixation should be indicated to prevent secondary traumatic arthritis.When the step-off of the medial tibial plateau is < 2 mm,conservative treatment can be a proper choice.
4.Most common illnesses in outpatient clinic of one primary health center in Shanghai

There were 461939 outpatient visits in Shanghai Tianping Primary Health Center from January to December 2010,the data were retrospectively analyzed based on the electronic outpatient record.The 20 most common illnesses accounted for 73.67% of total outpatient consultations (340310/461 939),hypertension ranked the first position of all illnesses ( 21.06% ) and followed by coronary heart disease (8.36%).Patients aged over 65y accounted for 72.35% of all patients,the frequency of consultations was positively correlated with age of patients(P <0.01 ).The frequency of consultations varies according to the gender,age of the patients as well as the organ system involved (P < 0.01 ).It can be concluded that the most frequently encountered illnesses in primary health center are common illnesses and chronic diseases.There are different needs for primary health service between genders and different age groups.
6.The stradegies on protecting the resource of Yi medicine

The medicine of Yi ethnic plays an important role of the traditional medicine of China. Recently, the facility of Yi ethnic medicine has made great progress on its development process. Now many medicine industries develop the production of Yi ethnic medicine. Thus, this demands a great quantity of Yi ethnic medicine, but it also leads rapid decline with overuse of the medical resources. Here we discussed a series of problems, and put forward the corresponding stradegies of protection of Yi medicine.
7.Study on correlation between serum Omentin-1 and gestational diabetes mellitus

Objective To observe the change of serum Omentin-1 in gestational diabetes mellitus(GDM)and to investigate its clinical significance.Methods Serum Omentin-1 level was detected in 85 cases of GDM and 85 matched cases of normal glucose tol-erance(NGT)by ELISA,the glycolipids biochemical indexes,inflammation indexes and fasting insulin (FINS)level were simultane-ous detected,then the insulin resistance index (HOMA-IR)was calculated in both groups.Results The levels of prenatal prepreg-nant BMI,FPG,hs-CRP,blood lipids,blood glucose,FINS and HOMA-IR in the GDM group were significantly higher than those in the NGT group,while serum Omentin-1 level was obviously lower than that in the NGT group,difference was statistically signifi-cant (P<0.05);when prenatal obesity and/or HOMA-IR≥2,the serum Omentin-1 levels was significantly decreased.Serum Omentin-1 level had significant positive correlation with HDL,and negative correlation with prepregnant BMI,prenatal BMI,FPG, FINS and HOMA-IR.The multiple stepwise regression analysis showed that prepregnant BMI,TG,FPG and FINS were independ-ent influencing factors of serum Omentin-1 in GDM.Conclusion Serum Omentin-1 is associated with GDM closely,which can re-flect the degrees of sugar,lipid metabolic disorder and insulin resistance in pregnant women,and may be involved in the occurrence and development of GDM.
8.Clinical Observation of Modified Hezhong Powder for Treatment of Children Mesenteric Lymphadenitis

Objective To observe the clinical effect of modified Hezhong Powder for the treatment of children mesentetic lymphadenitis.Methods Sixty children with mesentetic lymphadenitis were randomly divided into treatment group and control group,30 cases in each group.The control group was treated routine anti-inflammatory drug Cefaclor for Oral Suspension,and was given symptomatic treatment when necessary.The treatment group was given modified Hezhong Powder on the basis of treatment for the control group.The treatment course lasted for two weeks.After treatment,clinical efficacy and safety,and effects on relieving primary and secondary symptoms and on shortening the swollen lymph nodes were observed.Results (1) The total effective rate of the treatment group was 96.7%,and that of the control group was 83.3%,the difference being significant (P < 0.05).(2) The treatment group had better effect on relieving the primary symptom of abdominal pain,and secondary symptoms of poor appetite,vomiting,abdominal distention and abnormal stool than the control group.The differences of time for the symptoms disappearing were significant (P < 0.05).(3) After treatment,the swollen.lymph nodes of the two groups were shrunk(P < 0.05),and the shrinkage in the treatment group was superior to that of the control group (P <0.05).(4) During the treatment,no obvious adverse reaction occurred in the two groups.Conclusion Modified Hezhong Powder combined with western medicine is effective and safe for the treatment of children mesentetic lymphadenitis,and its efficacy is superior to that of the western medicine alone.
9.Analysis and Safety Evaluation of the Clinical Efficacy of Laparoscopic Appendectomy for Acute Appendicitis

Objective To investigate the clinical efficacy and safety of laparoscopic appendectomy for acute appendicitis.Methods Nighty-two cases of acute appendicitis were included in this study,randomly divided into observation group (n =46) and control group (n =46) according to the random number table method.The observation group was treated with laparoscopic appendectomy,and control group with the traditional open appendectomy treatment.We compared the two groups of cases in postoperative conditions and the level of WBC changes and 1d and 3d after operation.Results The operation time of the observation group was shorter than that of the control group (t=4.049,P<0.05);there was no significant difference in lymph nodes (t=0.570,P>0.05);the amount of bleeding was less than that of the control group (t=20.748,P<0.05).The get-out-of-bed activity time in the observation group was faster than the control group (t=9.221,P<0.05);eating time was faster than the control group (t=8.794,P<0.05);the anus recovery exhaust time was faster than the control group (t=17.423,P<0.05);hospital stay was shorter than the control group (t=13.479,P<0.05);the complication rate was lower than that of the control group (t=13.883,P<0.05).The levels of 1d and 3d WBC were lower in the observation group than that in the preoperative group (t=7.277,26.911,P<0.05);the levels of 1d and 3d WBC in the observation group were lower than those in the control group at the same time (t=3.6189,17.827,P<0.05);the 3d WBC level of the observation group was lower than that of 1d (t=20.732,P<0.05).Conclusion Laparoscopic appendectomy is a safe and effective treatment for patients with acute appendicitis,and it is worthy of further clinical research.
10.Exploration of a new evaluation framework for talented professionals in science and technology——a case study on an institution of military medical sciences

Objective To explore a new evaluation framework for talented professionals in science and technology, and design an appropriate methodology and management mechanism.Methods Key evaluation indicators were set according to the organization′s strategic objectives and innovation elements.Following the introduction to new concepts of evaluation, methods and steps were designed that could analyze evaluation results using the radar chart to achieve full-dimensional identification of different indicators, scopes or levels.Results and Conclusion A pluggablehierarchical management mechanism was established, and an organic architecture that involved data, evaluation logics and operation management was constructed.A pilot study was carried out in an institution of military medical sciences to verify the applicability of this framework.
