1.Laparoscopic radical gastrectomy in treatment of advanced gastric cancer: current status and clinical trials

Different from Japan and Korea,the gastric cancer cases in China are mostly advanced gastric cancer cases.Surgical radical gastrectomy,as the primary method in treating gastric cancer patients has achieved enormous development in recent years,especially laparoscopic radical gastrectomy.Whereas,laparoscopic radical gastrectomy in treatment of advanced gastric cancer remains controversial.The heated debates are mainly focusing on the indications,choosing of surgical approaches and reconstruction methods,completion of laparoscopic D2 lymphadenectomy,short-term postoperative outcomes and long-term oncologiceffectiveness.In this article,current status of laparoscopic radical gastrectomy in treatment of advanced gastric cancer and results of relevant clinical trials were reviewed.
2. Protective effect of uridine 5′-triphosphate on brain tissue after cerebral ischemia-reperfusion injury in rats

Objective: To use MRI method for continuous observation of the protective effect of uridine 5′-triphosphate (UTP) against cerebral ischemia-reperfusion (IR) injury in rats. Methods: The cerebral ischemia was induced by middle cerebral artery occlusion (MCAO) in rats. UTP solution was delivered through an indwelling tail venous catheter via microinfusion pump 30 min after MCAO at a rate of 5 ml·kg-1·min-1. Neurological deficit score (NDS) and brain water content were determined 24 h after reperfusion. The infarction volume was determined by 2,3,5-triphenyl- tetrazolium chloride (TTC) staining and MRI at 6, 30, and 54 h after reperfusion. Results: The NDS scores in 10, 30, and 90 μg/kg UTP groups were significantly lower than that in the saline control group (P<0.01); the water contents in the right cerebral hemisphere of 10, 30, and 90 μg/kg UTP groups were significantly less than that in the normal saline group (P<0.01). The cerebral infarction volume was gradually decreased with the increase of UTP dose, and the proportions of infarction volumes in 10, 30, and 90 μg/kg UTP groups were significantly decreased compared with the saline control group (P<0.01). The infarction volumes at 6, 30, and 54 h after reperfusion in 90 μg/kg UTP group were significantly lower than those in the saline control group (P<0.01). Conclusion: UTP has a protective effect on focal cerebral ischemia-reperfusion injury in rats; it can greatly reduce the volume of cerebral infarction.
3.Metronomic chemotherapy and breast cancer

Angiogenesis plays a very important role in the development and metastasis of breast cancer. Low-dose metronomic chemotherapy based on the traditional chemotherapy drugs has antiangiogenic effect. As far as its mechanism, low toxicity or low incidence of drug resistance is concerned, it is feasible for breast cancer therapy. It displayed the validity and feasibility in the pre-clinical studies and clinical trials, which provided a new treatment strategy for the breast cancer.
4.Clinical Study on Acupuncture at the Trigger Points on the Back for Cardioneurosis

Objective To observe the clinical efficacy of acupuncture at the trigger points on the back in treating cardioneurosis. Methods Fifty patients with cardioneurosis were selected to receive syndrome differentiation of traditional Chinese medicine, and then were evaluated for the improvement of symptoms respectively after first acupuncture treatment at the trigger points on the back and after 3~5 acupuncture treatments. Results Of the 50 cardioneurosis patients, 30 showed marked effect, 14 showed effective, 6 failed in the treatment, and the total effective rate was 88.0%. The total effective rate was 96.2%in patients of deficient heart yang, and was 75.0%in those of excessive fire due to yin deficiency, 83.3%in those of qi stagnation due to liver depression, and 83.3%in those of deficient heart yin. The improvement rate of subjective symptoms after 3~5 treatments was significantly different from that after the first treatment (P<0.05). Conclusions Acupuncture at the trigger points on the back is an effective approach in treating cardioneurosis, and it can achieve a marked efficacy with successive 3~5 sessions.
5.Investigation and analysis for impact factors of distress in patients with first diag-nosed lung can-cer

Objective:To study the impact factors of psychological distress in patients with first diag-nosed lung cancer.Methods:The cross-sectional study was applied to newly diagnosed lung cancer pa-tients who received treatments in Cancer Center of West China Hospital of Sichuan University in Chengdu from June 201 3 to March 201 5 by distributed questionnaires.The general information of the patients,the hospital anxiety and depression scale (HADS)and distress management screening measure (DMSM) were included in the questionnaire to evaluate the states of distress,pain and the factors related to the distress of the patients.Results:The survey investigated a total of 390 patients with first diagnosed lung cancer,including 291 male patients and 99 female patients.The proportion of the patients with positive anxiety symptom was 26.7% (1 04 /390),with positive depression symptom was 27.7% (1 08 /390), and with positive distress symptom was 30.0% (1 1 7 /390).On the top five problem list of DMSMwere worry,disease treatment,breathing,pain and sleep.The Spearman correlation analysis showed that an-xiety score,depression score,and the pain intensity were positively correlated with the distress.Multiple linear regression analysis showed that gender (β=-0.209,P =0.003),age (β=-0.098,P =0.042),chronic disease (β=0.378,P <0.001 ),and pain score (β=0.1 00,P =0.029)could affect the distress of the patients.Smoking (β=0.1 1 1 ,P =0.041 )could affect the anxiety of newly diagnosed lung cancer patients.Conclusion:The gender,age,chronic disease and pain score of the patients are the independent factors of the psychological distress of first diagnosed lung cancer patients.Smoking is the predictive factors of the patients with anxiety.Controlling the pain intensity and the clinical process of chronic disease of the patients actively,solving the sleep and breathing problem and helping the patients to quit smoking progressively could alleviate the psychological distress of the patients.
6.Effect of surgical resection on the prognosis if children patients with advanced neuroblastoma

Objective To investigate the effect of the extent of surgical resection on the prognosis of children patients with advanced neuroblastoma. Methods Thirty-seven children patients with advanced neuroblastoma were selected as our subjects,who underwent surgical treatment in the People's Hospital Binzhou from Jan. 2001to Dec. 2006. All the patients were treated with surgical operation or surgical operation combined with chemotherapy,among them 24 cases were completely removal,and 13 cases were partially removal. All patients were followed-up for more than 5 years. Log rank test and multivariate COX regression analysis were used to explore the effect of surgery resection on prognosis. Results The influencing factors of the complete resection group included lymphatic metastasis,clinical stage,combined with chemotherapy,tumor markers neuronspecific enolase(P = 0. 002,0. 000,0. 019,0. 015 respectively). COX regression analysis showed that the independent factors included chemotherapy( OR = 1. 952,95% CI = 1. 258 - 2. 759,P = 0. 000),lymph node metastasis(OR = 2. 856,95% CI = 1. 356 - 3. 859,P = 0. 019),and extent of surgical resection(OR = 3. 069, 95% CI = 1. 585 - 4. 685,P = 0. 005). The shortest survival period of patients in this study was a month,and the median survival period was 33. 96 months. One year survival rate was 62. 21% and 2-year survival rate was 41. 09% as well as 3-year survival rate was 23. 59% . The average survival period between total and partial of surgical resection was significant((56. 96 ± 13. 22)months,(19. 63 ± 10. 20)months;P = 0. 019). Conclusion Surgical resection is an important factor of the prognosis of patients with NB. If the patient's own conditions permit,it can be appropriate to expand the scope of surgical resection in order to reduce the risk of tumor recurrence.
7.Analysis of 23 G and 25 G+vitrectomy for retinal detachment

?AIM: To compare the clinical effect of 23G and 25G+vitrectomy for retinal detachment.?METHODS:Forty seven patients with retinal detachment were treated with 23G vitrectomy (27 eyes in 27 cases as group A) and 25G+ vitrectomy (20 eyes in 20 cases as group B ) . The operation time and the incidence of intraoperative complications were recorded. The occurrence of retinal reposition, visual acuity, intraocular pressure ( IOP ) and complications were observed. Postoperative follow-up time of the two groups were 3d, 1wk, 3mo. The relevant records were statistically analyzed and compared.?RESULTS: The operation time of 23G group and 25G+group were 50. 21+4. 52min, 49. 15+5. 14min,respectively and there was no significant difference between the two groups (P>0. 05). The main complications were retinal hemorrhage and iatrogenic retinal hole. There were 3 eyes with retinal hemorrhage, 2 eyes with iatrogenic retinal hole in 23G group, and 1 eye with retinal hemorrhage, 1 eye with iatrogenic retinal hole in the 25G+group, and the difference was statistically significant ( P<0. 05). The postoperative visual acuity of 23G group and 25G + group were significantly improved, and the differences between the two groups were not statistically significant at different time points after operation ( P>0. 05). The number of eyes with hypotonia in 23G and 25G+group were 3 and 1 eyes respectively, the difference was statistically significant ( P < 0. 05 ). But there were no significant differences between the two groups on IOP at 1wk and 3mo after surgery (P>0. 05). At the last follow-up, the results showed that 26 eyes ( 96%) with retinal reposition in 23G group, 19 eyes (95%) in 25G+ group, the difference was not statistically significant (P>0. 05).?CONCLUSION: The clinical effect of 23G and 25G+vitrectomy for retinal detachment is similar, but 25G+vitrectomy can reduce incidence of complications and early postoperative low IOP.
8.Mood congruency of involuntary autobiographical memories in college students with different moods

Objective:To explore whether there is mood congruency effect of involuntary autobiographical memories among different mood undergraduates.Methods:Totally 60 college students were induced to generate positive,negative or neutral mood by emotional pictures and music.Vigilance task paradigm was used to induce involuntary autobiographical memory in the present experiment.All participants were asked to rate the pleasantness of involuntary autobiographical memories,then the memory proportion of different valence was calculated.Two raters who had not taken part in the experiment were instructed to divide the objective memory contents into positive,negative and neutral three types.Furthermore,the number of involuntary autobiographical memories recalled was evaluated according to the valence of objective memory contents.Besides,all participants were asked to rate the mood state at the end of the experiment.Results:There was no significant difference in pleasantness ratings among different mood college students (P > 0.05).There was a significant interaction between mood state and the valence of objective memory contents (P < 0.01).Further examination of the interaction suggested the number of positive memory contents recalled was greater in positive state compared with neutral state [(6.3 ±5.5) vs.(2.8 ±2.2),P <0.01].However,there was no difference in the number of negative memory contents and neutral memory contents among different mood states (Ps > 0.05).Additionally,no matter the pleasantness ratings was positive or negative,there was no significant correlation between the proportion of memories and mood state rated at the end of the experiment (r =0.29,-0.13,Ps > 0.05).Conclusion:It suggests that there does not exist mood congruency effect of involuntary autobiographical memories among college students in different mood states.Even in negative mood state,their involuntary autobiographical memories would still display a kind of optimistic bias.
9.Studies on anti-inflammatory and antiallergic effects of CH_2Cl_2 extract of Flos Magnoliae

Object To study the pharmacological effects of CH 2Cl 2 extract of Flos Magnoliae.Methods Anti inflammatory and antiallergic effects of CH 2Cl 2 extract of Flos Magnoliae were studied on various experimental models. Results The extract remarkably inhibited the contraction caused by histamine and acetylcholine, as well as the ovalbumin sensitized allergic constriction of isolated ileum smooth muscle of guinea pigs. It could inhibit the increase of capillary permeability induced by acetic acid in mice, inhibit the mice paw edema produced by carrageenin and reduce the production of PGE 2 in inflammatory tissues. Conclusion CH 2Cl 2 extract of Flos Magnoliae showed remarkable anti inflammatory and antiallergic effects.
10.Change of Lipid,Esterase and Lipase in Mosquito Larvae Infected with Lagenidium giganteum

Objective To explore possible physiological and biochemical mechanisms of Lagenidium giganteum infection in killing mosquito larvae. Methods The content of lipid and the activities of esterase and lipase between the normal and infected mosquito larvae were observed with histochemical method. The results were photomicrographed and analysed by image analysis using computer. Results In 24 hrs after infection, the content of lipid in the infected Culex quinquefasciatus larvae was lower while the activities of esterase and lipase in the larvae were higher than the control. In 48 hrs and 78 hrs after infection, the content of lipid in the infected larvae of C. quinquefasciatus and Ae. albopictus were significantly lower while the activities of esterase and lipase were significantly higher than those of control. Conclusion The content of lipid in the mosquito larvae decreased while the activities of esterase and lipase increased after the infection of Lagenidium giganteum . The disorder of lipid metabolism might be part of the killing mechanisms to C. quinquefasciatus and Ae. albopictus larvae.
