1.The effect of non-directed musical imagery therapy on the automatic thoughts and therapeutic effect of depression

Objective To explore the effect of non-directed musical therapy on the automatic thoughts and therapeutic effect of depression.Methods 60 people outpatient and inpatient who with depression disorder participated in this research.According to the medical order,we choose the people who with Cardinality order as the group of the musical therapy (study group) and the people with the even order as the group of drug therapy (control group).all the patients was test with the Hamilton's Depression Scale (HAMD-24) and Automatic Thoughts Questionnaire (ATQ) before and after 2,4,8 weeks treatment,and comparison of these questionnaires scores of the two groups.Results After the 2,4,8 weeks treatment,the HAMD scores of the study group was (15.70±2.63),(10.03±2.28),(6.00± 1.98) and the scores of control group was (17.43±2.62),(12.47±2.75),(7.50±2.03).The result of HAMD scores had significant difference(F=11.754,P=0.001).ATQ scores of the study group was (73.27± 17.60),(58.33± 13.66),(47.43± 11.94) and the scores of control group was(84.20±22.30),(72.53±20.86),(58.90± 18.61).The result of ATQ scores had significant difference(F=4.136,P=0.047).Conclusion Non-directive musical imagery could be an effective way to improve the automatic thoughts of people who with depression disorder and improve the effect of treatment.
2.Protective effect of miR-221 onβcells in mice with diabetic nephropathy and its mechanism

Objective:To study the influence of miR-221 in theβcells of mice with diabetic nephropathy (DN), and to clarify the protective effect of miR-221 on DN.Methods:Twenty wild type C57BL/6J mice aged 8 weeks were divided into control group and DN group (n=10).The DN mice models were constructed with 14 weeks of high fat diet,and 100 mg·kg-1 streptozotocin (STZ)was used to induce the partial insulin deficiency.10 weeks later the blood glucose,serum creatinine,blood urea nitrogen content and 24h-urinary albumin excretion rate were detected.Theβcells of islet were isolated from the DN mice.The expression level of SOCS3 mRNA inβ cells of islet was valued by Real-time PCR.The proliferation of pancreaticβcells of islet was examined by MTT assay.The contents of insulin and insulin release levels in pancreaticβ cells of islet were detected by ELISA assay.Luciferase reporter gene assay was used to detect the luciferase reporter gene activity of SOCS3.Results:The DN mouse models were constructed successfully.The blood glucose,serum creatinine,blood urea nitrogen and 24h-urinary albumin excretion rate of the mice in DNA group were higher than those in control group (P < 0.05 ).After overexpression of miR-221,the expression level of SOCS3 mRNA in pancreaticβcells of islet and the proliferation ability ofβcells of islet of the mice in DN group were lower than those of normal islet cells (P < 0.05).Then luciferase reporter gene method found SOCS3 as one of the target genes of miR-221.After overexpression of miR-221,the insulin release level and insulin content in pancreaticβcells of islet were higher than those of normal islet cells (P <0.05).Conclusion:miR-221 can promopt the synthesis and secretion ofβ cells of islet and inhibit the dysfunction ofβcells of islet in the DN mice by down-regulating the SOCS3 level.
3.The high risk factors and clinical analysis of respiratory distress syndrome in neonates at different gestational age

Objective To compare the high risk factors, complications, treatment and prognosis of respiratory distress syndrome (NRDS) in neonates at different gestational age (GA). Methods Between August 2012 and July 2013, 156 neonates with RDS were selected and distributed into 3 groups, 42 early preterm (GA<34weeks), 52 late preterm (GA 35 to 36 weeks), and 62 in term group (GA≥37 weeks). Retrospectively analysis was performed for high risk factors, complications, treatment and prognosis of the three groups. Results In 156 neonates with RDS, the male and female proportion was 2.25:1. All groups had more males, but the gender difference has no statistical signiifcance in three groups (P=0.923). The onset time of RDS and the hospitalization time both show an increasing trend of statistical signiifcance (P<0.05). Comparing the difference of high risk factors for RDS of the 3 groups, birth asphyxia, placental abnormalities, multiple pregnancy, premature rupture of membranes was most common in early preterm group, and followed by late preterm group, and C-section was most common in term group and unexplained preterm was more common in early preterm group than that in late preterm group (all P<0.05). Among the three groups, the ratio of pulmonary surfactant application was the lowest in the term group, the ratio of X-ray grade over II was high-est in early preterm group, oxygen and hospitalization time were the longest in early preterm group (P<0.05). The risks of com-plicated with pulmonary infection, intracranial hemorrhages and bronchopulmonary dysplasia were the highest in early preterm group and the risk of complicated pneumothorax was the highest in term group. Among three groups, the recovery rate was the lowest in the early preterm group (P<0.01). Conclusion The clinical characteristics, high risk factors, complications and treat-ment responses of RDS in neonates with different GA were different, so GA should be considered for diagnose and treatment. For the term infants, the elective caesarean section should be strictly controlled, in order to reduce the incidence of RDS.
4.Analysis on risk factors of thyroid nodule among elderly patients with type 2 diabetes mellitus in Xinjiang region

Objective To explore the prevalence and related risk factors of thyroid nodule in elderly patients(age ≥ 60 years old) with type 2 diabetes mellitus(T2DM)) in Xinjiang region.Methods Two hundreds and two elderly patients with T2DM were selected,including 91 cases of thyroid nodule(thyroid nodule group) and111 cases of non-thyroid nodule (control group).The disease history,body mass index(BMI),fasting blood-glucose (FBG),2-h postprandial blood glucose(2 h PBG),glycosylated hemoglobin(HbA1c),triglyceride(TG),cholesterol(TC),high-density lipoprotein(HDL-C) and low density lipoprotein(LDL-C) were collected for statistical analysis.Results The age,gender,BMI,TC and HbA1c had statistical difference between the two groups (P<0.05);the disease course,pulse pressure,systolic pressure,abdominal perimeter,smoking history,HDL,TG,FBG and 2 h PBG had no statistical differences netween the thyroid nodule group and non-thyroid nodule group (P>0.05).The multivariable Logistic regression analysis showed that the age,gender,TC,HbA1c and BMI were the risk factors of thyroid nodule occurrence.Conclusion Conducting the thyroid nodule related imaging examination has an important clinical significance for diabetic elderly women patients with obesity and cholesterol increase.
5.Short-term effects on reconstruction of Paprosky type III severe acetabulum bone defect with tantalum metal block.

OBJECTIVETo investigate methods and short-term effects on tantalum metal block for reconstruction of Paprosky type III severe acetabulum bone defect.
METHODSTwelve patients (12 hips) with Paprosky type III severe acetabulum bone defect,treated with tantalum metal block in revision of total hip arthroplasty, were retrospectively analyzed from March 2009 to December 2012. Among the patients, there were 5 males and 7 females aged from 45 to 78 years old with an average of 64 years old; the courses of disease ranged from 3 to 32 months with an average of 16 months. According to classification of Paprosky, all patients were Paprosky type III acetabulum bone defect. All patients were used tantalum metal block in revision of total hip arthroplasty, and large diameter biological acetabulum cups were used in combination. Operative time,blood loss and postoperative complications were recorded and compared, Harris score were used to evaluate clinical effects.
RESULTSEleven patients ( 11 hips) were followed up from 9 to 36 months with an average of 25 months, 1 case was lost to follow-up for dying in car accident. The incisions were healed at stage I . Operative time ranged from 2 to 6 h with an average of 3 h; blood loss was 300 to 1 600 ml with an average of 900 ml. No deep venous thrombosis, periprosthetic femoral fracture and infection, sciatic nerve injury occurred after operation. Harris score were improved from (46.25 ± 8.99) before operation to (83.75 ± 6.76) after operation. There was significant difference in Harris score before and after operation (t = 3.14, P = 0.00 < 0.05), and 7 cases got excellent results, 2 good and 2 moderate. CONCLUCSION: Tantalum metal block is suitable for revision of Paprosky III acetabulum bone defect, could repair different kinds of acetabulum cup defect, could fill up inclusive defect of inner wall combined with particulate bone graft, induce bone growth pressured between metal block and metal cup, provide a new further choose for the treatment of Paprosky type III severe acetabulum bone defect.

6.Factors influencing the efficacy of endoscopic mucosal resection for early colorectal cancer

Objective To investigate the clinical risk factors ot endoscopic mucosal resection (EMR) for early colorectal cancer.Methods The clinical data of 140 patients with early colorectal cancer who received EMR at the Changzheng Hospital of Second Military Medical University from January 2005 to June 2012 were retrospectively analyzed.EMR was applied for patients with early colorectal cancer with diameter lesser than 20 mm (protrusive lesions) or 5 mm (depressed lesions).If the diameter of the early colorectal cancer was above 3 cm and can not be resected at one time,endoscopic piecemeal mucosal resection was adopted.Clinicopathological factors including gender,age,tumor location,number,diameter,shape,resection type,depth of tumor invasion,tissue differentiation,lymphatic or vascular invasion,excision degree and curability were retrospectively analyzed.All the patients were divided into the complete cure group and the incomplete cure group.Univariate and multivariate analysis were done using the chi-square test and Logistic regression model,respectively.Results Of the 143 lesions,en-bloc resection was done in 125 lesions,and piecemeal resection in 18 lesions.There were 90 intramucosal carcinomas and 53 submucosal carcinomas.The numbers of high,moderate and low differentiated colorectal cancer were 96,37 and 10.Lymphatic or vascular invasion was found in 8 lesions.The numbers of R0,R1,R2 and Rx resection were 118,15,8 and 2.Two patients with Rx resection were excluded from this study.There were 98 patients (101 lesions) in the complete cure group,and 40 patients (40 lesions) in the incomplete cure group.The results of univariate analysis showed that tumor location,shape and resection type were the risk factors influencing the efficacy of EMR for early colorectal cancer (x2=7.523,16.280,4.156,P <0.05).The results of multivariate analysis showed that tumor located at the proximal colon,superficial protrusive and depressed type and piecemeal resection were the independently risk factors influencing the efficacy of EMR (OR =7.219,6.115,5.211,P <0.05).Conclusions EMR is not easy to achieve complete cure for early colorectal cancer with superficial protrusive and depressed type or located at the proximal colon.Endoscopic piecemeal mucosal resection should try to avoid.
7.Comparison of oral and gastrointestinal opportunistic pathogen contamination between tooth mugs placed up-ward and downward

The oral and gastrointestinal opportunistic pathogen contamination was compared between tooth mugs placed upward and down-ward(n=9)for 1 4 days.Selective cultivation of the pathogens was uesd to measure the extent of contamination.The colony forming units (CFU)of colibacillus in group up and group down were 4.25 ±0.71 and 2.84 ±1 .40(P=0.046),S.mutans 89 ±0.31 and 2.84 ±1 .40 (P<0.001 ),Candida 2.28 ±1 .36 and 2.53 ±1 .92(P=0.002),fungus 2.44 ±0.99 and 0,respecitvely.Thus,tooth mug placed open-ing down is superior for health.
8.Prediction of pregnancy complication occurrence using fetal cardiac output assessments made by ultrasonography at 20 to 24 weeks of gestation.

OBJECTIVE: To evaluate the importance of assessment of fetal cardiac output (CO) for the prediction of complications of pregnancy. METHODS: We evaluated 65 fetuses and all of them had a fetal cardiac scan at 20 to 24 weeks of pregnancy. To measure CO, diameters (d) of the left right ventricle outflow tract were measured just above the valves. Each left CO (LCO) and right CO (RCO) was derived using the following equation: CO = velocity time integral ×π× d²/4 × heart rate. Pregnancy complications included gestational hypertensive disorders, fetal growth restriction (FGR) and preterm birth (PTB) caused from preterm labor or preterm premature rupture of membrane (PPROM). RESULTS: There were 23 cases with one more pregnancy complication (FGR, 9; gestational hypertensive disorders, 8; PTB caused from PTB or PPROM, 12). The LCO was lower in complication group than in normal group (88±53 vs. 117±48 mL/min, P=0.028). The RCO to the LCO ratio (RCO/LCO) was higher in complication group (2.43±1.69 vs. 1.48±0.81, P=0.001). Regression analysis demonstrated that RCO/LCO was a significant predictor of pregnancy complication; Odds ratio was 7.76 (95% CI, 1.15 to 52.21; P=0.029). The area under the receiver-operating characteristic curve for prediction of pregnancy complications from LCO was 0.71. The diagnostic cut-off value of LCO was 80 mL/min. The area under the receiver-operating characteristic curve from RCO/LCO was 0.68 and cut-off value was 1.41. CONCLUSION: This study demonstrated that pregnancy complications can be suspected based on fetal CO assessments at a GA of 20 to 24 weeks.

9.Rapid culture and identification of human parainfluenza viruses

Objective Parainfluenza virus is an important pathogen of lower respiratory tract infections in infants and young children.This study was to search for a method for rapid culture and identification of human parainfluenza viruses from nasal swabs. Methods Nasal swab specimens were collected from 0-5 years old children with acute respiratory tract infection.The specimens were inoculated onto 96 plates with prefabricated LLC-MK2 cells and then centrifuged for 1 hour at 3000 r/min and also inoculated using the traditional culture method, followed by addition of virus mainte-nance medium containing 4 μg/mL TPCK trypsin.The cytopathic effect was observed daily, and hemagglutination and blood absorption tests were done at 2, 5, and 8 days after inoculation.In case of posi-tive result of either test, the specimen was subjected to immunofluo-rescence staining. Results Six strains of parainfluenza virus were isolated from the 83 nasal swab specimens, with a positive rate of 7.2%.There was a significant difference in the rate of separation be-tween the rapid and traditional culture methods after 2 days of culturing (7.2%vs 0%, P<0.05).The infected cells produced a cy-topathic effect that characterized by syncytium and crush formation.Hemagglutination and blood adsorption tests were positive at 4℃and negative at the room temperature.Immunofluorescence staining exhibited specific apple green fluorescence. Conclusion The method for rapid culture and identification of human parainfluenza viruses in nasal swab specimens was successfully established, which can be used to obtain and identify parainfluenza viruses with virulence and biological activity in 2 days.
10.Salmeterol/fluticasone combined with N-acetylcysteine in treatment of airway inflammation of stable chronic obstructive pulmonary disease

Objective To evaluate salmeterol/fluficasone combined with N-acetylcysteine in treatment of stable chronic obstructive pulmonary disease (COPD). Methods Sixty patients with stable COPD were randomized into treatment group(n = 30) and control group(n = 30). Patients in control group were given salmeterol/fluticasone twice per day ; while patients in treatment group in addition to salmeterol/ fluticasone, also took N-acetylcysteine 0.6 g three times per day. The course of treatment lasted for four weeks. Pulmonary function was measured in all patients; interleukin-8 (IL-8), tumor necrosis factor α (TNF-α), superoxide dismntase (SOD) and malondialdehyde (MDA) in blood serum and induced sputum were determined. The white cell count and classification in sputum smear was examined and the quality of life (QOL) of patients was evaluated. Results FEV1, FEV1 %, QOL evaluation, IL-8, TNF-α, white cell count and the percentage of neutrophil granulocytes in induced sputum after treatment were significantly improved compared with those before treatment in control group (P <0.05 or P <0.01). FEV1, FEV1%, QOL evaluation, SOD and MDA in blood serum, IL-8, TNF-α, SOD and MDA, total white cell count, the percentage of neutrophil granulocytes and macrophage in induced sputum after treatment were significantly improved in treatment group (P <0. 05 or P <0.01). The differences in SOD and MDA in blood serum and the percentage of neutrophil granulocytes in induced sputum smear between treatment group and control group were staffsticaUy significant (93 ± 8) × 10-6 U/L, (4. 0 ± 1.0) × 10-3 mmol/L and 0. 5 ± 0. 3 vs (85 ± 10) ×10-6U/L,(4.2±1.1) ×10-3mmol/Land0.6±0.2; allP<0.05. Conclusion Combination of salmeterol/fluticasone and N-acetylcysteine has better therapeutic results in treatment of airway inflammation of stable COPD.
