1.Behavioral characteristics of accident proneness and its influencing factors among children and juveniles

Objective To investigate the behavior characteristics of children or juveniles with accident proneness and to explore the influencing factors of children and juveniles accident proneness in behavior.Methods The study was carried out bv case-control method,and 1 601 children and juveniles from a rural area of hunan province were investigated with homemade general injury questionnaire.Accident proneness children or juveniles screened out from cross-sectional studv were selected into research group,while uninjured children or juveniles whose gender,age and grade were the same with the research group were included into control group with 1:1 matched.Achenbach Child Behavior Checklist was used to evaluate and compare these two groups of children and juveniles' behavior.Results 147 accident proneness children and juveniles were screened out from the study and the accident proneness incidence of children and juveniles in rural areas was 9.42%.The total score and each factor score of behavior problem and behavioral problems detection rate iu research group were higher than that of the control group and there were statistically significant (P<0.01).Multivariate analysis showed that attention problem (OR=1.110,95% CI =1.041-1.185) and aggressive behavior (OR=1.123,95% CI =1.068-1.181) were risk factors of accident proneness.Conclusion Accident proneness children and juveniles' behavior is different from the uninjured,which is a risk factors of accident proneness.
2.Relationship between mental health status and coping styles among village poverty alleviation cadres in Leshan City

ObjectiveTo analyze the relationship between mental health status and coping styles among village poverty alleviation cadres in Leshan City, and to provide references for the improvement of mental health and coping styles of them. MethodsA cluster random sampling method was used to select 6 of the 11 districts and counties in Leshan City, and all the village poverty alleviation cadres in the selected areas were evaluated through the psychological cloud CT system. A total of 700 cadres participated in the questionnaire evaluation, and were assessed using Symptom Checklist 90 (SCL-90) and Coping Style Questionnaire (CSQ). Results①A total of 207 (35.9%) village poverty alleviation cadres factor scored above 2, and 92 (15.9%) cadres scored above 200. ②There was a statistically significant difference in SCL-90 obsessive-compulsive symptom factor score and other factor score between different poverty alleviation time groups (P<0.05 or 0.01), and the factor score of SCL-90 obsessive-compulsive symptom showed significant difference among village poverty-alleviation cadres of different genders (P<0.05). ③The scores of problem-solving factor in CSQ showed significant differences among village poverty alleviation cadres of different genders and different ages (P<0.01). ④Correlation analysis showed that the scores of each dimension and total score of SCL-90 of village poverty alleviation cadres in Leshan City were positively correlated with the score of self-blame factor in CSQ (r=0.423~0.521, P<0.01). ConclusionThe village poverty alleviation cadres of female and with a working length of less than 90 days or more than 360 days are at high risk of obsessive-compulsive symptoms, while the cadres of male and aged over 41 years adopt mature coping styles. Furthermore, the immature coping style of cadres will lead to serious mental health problems.
3.Influence of different kinds of valve types on clinical benefits of elderly patients with valvular heart disease

Objective 聽 聽To investigate the influence of mechanical and biological valves on clinical benefits of elderly patients with valvular heart disease. Methods 聽 聽We retrospectively analyzed the clinical data of 280 elderly patients with valvular heart disease treated by valve replacement between 2008 and 2014 year. The patients were divided into two groups by tendency score matching including a group A with biological valves and a group B with mechanical valves. Finally, there were 96 patients in each group. There were 43 males and 53 females at age of 64.41卤6.52 years in the group A, 44 males and 52 females at age of 64.07卤6.20 years in the group B. Results 聽 聽The bleeding rate of skin and mucosa of the group B was significantly higher than that of the group A (P<0.05). There was no statistical difference in mortality within 30 days after operation, all-cause mortality, re-hospitalization rate, re-valve replacement rate, combined atrial flutter/atrial fibrillation ratio, drug use, incidence of cerebral infarction, cerebral hemorrhage, new peripheral vascular embolism and visceral hemorrhage, heart function (NYHA) classification, the cumulative survival rate of all the patients during follow-up (P=0.63), or the cumulative survival rate of the patients with no thrombus/hemorrhage (P=0.75) between the two groups (P>0.05). Conclusion 聽 聽Mechanical valve replacement and bioprosthetic valve replacement in the treatment of valvular heart disease in the elderly can achieve similar clinical benefits and both have clinical application value.
4.Clone of Full-length cDNA Sequence of Actin Gene of Eleutherococcus Senticosus and Bioinformatics Analysis

This study was aimed to clone the full-length cDNA sequence of actin gene of Eleutherococcus sentico-sus. And bioinformatics analysis was used. The total RNA was isolated from leaves of E. senticosus , and cDNA was synthesized by reverse transcription of total RNA. Primers were designed according to the conserved se-quence that had been cloned of Actin of E. senticosus . Then, the 3'and 5' cDNA fragments were cloned by nested PCR . The full-length gene was obtained by gene splicing method . Sequencing results were compared and treated with similarity analysis by blast analysis in the GenBank. Protein secondary structure and tertiary struc-ture of Actin of E. senticosus was predicted by online software. The results showed that the full-length cDNA of Actin of E. senticosus is 1507 bp, which named EsActin1, GenBank accession No. KC469585. The conserved sequence, which contained a 1134 bp open reading frame that encoding a 377 amino acid residues, a 5'-UTR of 140 bp and a 3'-UTR of 233 bp. Homologous alignment showed that it shared over 75% nucleotide se-quence similarity and over 94% amino acid sequences similarity with Actins in other plants. It was concluded that this study first isolated and reported the full-length cDNA sequence of actin gene of E. senticosus , and laid a foundation for the molecular biology research of E. senticosus .
5.Research progress between childhood obesity and breastfeeding

Obesity as a global issue is a serious threat to children's health.To control childhood obesity must begin with effective preventions.Most studies have found that breast milk can protect against childhood obesity.It is recommended that children should be exclusively breastfed for at least 4-6 months.The duration of breastfeeding has some negative correlation with the prevalence of childhood obesity.Increasing the duration of breastfeeding can properly decrease the risk of childhood obesity.
6.Efficacy of transthoracic device closure versus surgical closure on ventricular septal defects: A systematic review and meta-analysis

Objective To compare the effects of transthoracic device closure and surgical closure on ventricular septal defect systemically. Methods A systematic literature search was conducted using the PubMed, EMbase, The Cochrane Library, VIP, CNKI, CBM, Chinese Clinical Trial Register, ClinicalTrials. gov and Wanfang Database up to July 31, 2016. Quality was assessed and data of included articles were extracted. The meta-analysis was conducted using RevMan 5.0 and Stata 14.0 software. Results Eleven studies were identified, including 5 RCTs and 6 cohort studies involving 2 504 patients. For success rate, there was no statistical difference between the transthoracic closure group and the surgical closure group in RCT (RR=0.99, 95%CI 0.96 to 1.03, P=0.70); the success rate in the transthoracic closure group was lower than that in the surgical closure group in the cohort study (OR=0.21, 95%CI 0.08 to 0.55, P=0.002). Both results of RCTs and cohort studies showed that compared with surgical closure, transthoracic device closure reduced duration of the operation (RCT MD=–79.38, 95%CI –95.00 to –63.76, P<0.000 01; cohort study MD=–66.26, 95%CI –71.20 to –61.31, P<0.000 01) and hospital stay (RCT MD=–2.10, 95%CI –2.65 to –1.55, P<0.000 01; cohort study MD=–3.99, 95%CI –6.03 to –1.94, P=0.000 1), and the patients with blood transfusion (RCT RR= 0.04, 95%CI 0.01 to 0.11, P<0.000 01; cohort study OR=0.01, 95%CI 0.00 to 0.13, P=0.001). In the transthoracic closure group the risk of postoperative arrhythmia reduced (RCT RR=0.20, 95%CI 0.13 to 0.32, P<0.000 01; cohort study OR=0.46, 95%CI 0.31 to 0.67, P<0.000 1). In the transthoracic closure group a higher postoperative valvular regurgitation risk in RCT induced (RR=1.45, 95%CI 1.07 to 1.96, P=0.02) and the rate of postoperative valvular regurgitation in cohort study reduced (OR=0.43, 95%CI 0.20 to 0.92, P=0.03). However, there was no statistical difference in postoperative residual shunt (RCT RR=0.96, 95%CI 0.57 to 1.62, P=0.89; cohort study OR=0.52, 95%CI 0.12 to 2.25, P=0.38). Conclusion Transthoracic device closure can shorten duration of the operation, hospital stay and reduce the patients with blood transfusion and post- and intraoperative arrhythmia risk. Therefore, transthoracic device closure may be a better approach for some ventricular septal defect patients.
7.Effects of the escharectomy during burn shock stage on expression of glucose translator-4 mRNA in skeletal muscle and adipose tissue.

OBJECTIVETo investigate the effect of the escharectomy during burn shock stage on expression of glucose translator-4 (GLUT4) mRNA in skeletal muscle and adipose tissue.
METHODS30% TBSA scalded rats were employed. Escharectomy were conducted at 8 h, 24 h, 168 h after burns respectively. Insulin, glucagon, cortisol and glucose levels in serum were analyzed. RT-PCR were employed to analyze GLUT4 mRNA expression in skeletal muscle and adipose tissue.
RESULTSGlucagon, cortisol and glucose levels in serum were declined in groups which escharectomy were conducted during burn shock stage. GLUT4 mRNA expression in both skeletal muscle and adipose tissue were downregulated after burns and escharectomy conducted during burn shock stage made it restored to near normal.
CONCLUSIONGLUT4 mRNA expression will declined after major burns in skeletal muscle and adipose tissue. Escharectomy during shock stage could make it upregulated, which will be helpful to improve glucose metabolism and hypermetabolism after major burns.

8.Analysis of Risk Factors of SkinAdverse Reactions by Intravenous Injection of Iodine ContrastAgent

OBJECTIVE:To investiga te main risk factors for adverse drug reactions (ADR)of skin by intravenous injection of iodine contrast agent. METHODS :From Jan. 2009 to Apr. 2020,the patients suffering from skin ADR after enhanced CT with iodine contrast agent were collected from our hospital. The basic information ,laboratory test results before using iodine contrast agent and ADR related information were collected through hospital information system (HIS). The use of iodine contrast agent ,main manifestations of skin ADR and drug combination were analyzed statistically. Taking the sex ,age,body mass index (BMI),the dosage of iodine contrast agent ,length of stay ,laboratory examination ,tumor history ,basic disease ,allergy history ,drinking history as independent variables ,the incidence of skin ADR related to iodine contrast agent was analyzed by single factor analysis ,and the variables with statistically significant were selected for multivariate Logistic stepwise regression analysis. RESULTS :There were 157 cases of skin ADR ,involving 79 males(50.3%)and 78 females(49.7%). The age ranged from 19 to 86 years old ,being(52.68± 18.73)years old in average. BMI was 14.6-40.7 kg/m2,being(22.5±3.7) kg/m2. 67 cases(42.68%)were treated with iprodione ,34 cases(21.66%)with iodixanol ,31 cases(19.74%)with iohexol and 25 cases(15.92%)with iopamidol ;the dose of iodine contrast agent were 50-100 mL,being(73.06±13.29)mL in average. There was no significant difference among different dosage of 4 kinds of iodine contrast agents (P≤0.05). Among 4 kinds of iodine contrast agents ,the incidence of skin ADR induced by iopromide was the highest(0.197%). The skin ADR related to iodine contrast agent was mainly acute (89.2%),the severity was mild (75.2%),and urticaria(38.9%)was the most common. After symptomatic treatment ,135 cases were cured ,13 cases were improved and 9 cases were not improved. Among the patients with iodine contrast agent related skin ADR ,the incidence of ADR induced by combined use of anti infective drugs was the highest (33.1%);however,the combined use of anti-tumor drugs was the main cause of severe skin ADR. The length of stay {11~20 d[OR=1.21,95%CI(1.07,1.20),P=0.042]、21~30 d[OR=1.39,95%CI(1.12,1.52),P=0.035]、31~40 d[OR=1.15,95%CI(1.03,1.37),P=0.008]、>40 d[OR=1.33,95%CI(1.28,1.53),P=0.003]},respitatory and circulatory system tumor history[OR =1.51,95%CI(1.35,1.61),P=0.037],injection allergy history[OR =1.50,95%CI(1.37,1.59),P=0.005] can significantly increase the incidence of iodine contrast agent related skin ADR. CONCLUSIONS :The main manifestation of skin ADR related to iodine contrast agent was urticaria. The main risk factors of skin ADR related to iodine contrast agent were length of stay (> 10 d),respiratory and circulatory system tumor history and injection allergy history.

With the deepening of microbial research in central nervous system diseases, the role of gut microbiota in spinal cord injury has also received extensive attention. At present, it is mainly focused on exploring the changes of gut microbiota after spinal cord injury and the potential effects of aryl hydrocarbon receptor agonists, short-chain fatty acids and lipopolysaccharides produced by bacterial metabolism on secondary inflammation after injury, and it is recognized that gut microbiota or supplemental flora metabolites may be a potential pathway for the treatment of spinal cord injury.
10.Transluminal repair of iatrogenic bladder fistula: a report of 7 cases

【Objective】 To explore the technical methods and clinical efficacy of transvaginal or transrectal repair in the treatment of iatrogenic bladder fistula. 【Methods】 The clinical data of 7 cases of iatrogenic bladder fistula patients treated during 2016 and 2019 were retrospectively analyzed, including 6 cases of vesicovaginal fistula (VVF) and 1 case of vesicorectal fistula (VRF). The operation was conducted 3 to 10 months after the diagnosis of urinary fistula, and the vagina or rectum was fully cleaned before operation. Modified Latzko technique was employed to separate the gap between the bladder wall and vaginal or rectal wall along the fistula, the fistula scar was sharply removed, and the fistula, bladder wall, vaginal or intestinal wall, and vaginal or intestinal mucosa were sutured in layers. The urinary catheter was indwelled for 4 weeks. 【Results】 All 7 cases were successfully repaired at one procedure. No urine leakage was found after the urinary catheter was removed. There was no recurrence after 6 to 12 months of follow-up. 【Conclusion】 Selective application of the modified Latzko technique to repair iatrogenic urinary fistula through the natural lumen is an advantageous treatment scheme, which simplifies the operation and reduces trauma.
