Attention deficit hyperactivity disorder(ADHD) and developmental coordination disorder(DCD) are both childhood disorders identified in the DSM-Ⅳ.Studies investigating ADHD have found that around half of ADHD cases also have motor problems severe enough to be diagnosed as DCD.Further, children initially diagnosed as DCD have also been found to meet moderate to severe diagnosis for ADHD.Both disorders have been linked to a lot of psychosocial problems.Furthermore, when ADHD and DCD are co-morbid,the outcome tends to be more severe than when either disorder occurs alone. In the past decade, many studies identified shown that ADHD children experienced difficulties with both fine motor movements and gross motor movements.Notably, most of these foregoing studies found balance functions of ADHD were impaired. In order to maintain balance under a verity of environmental conditions, sensory information from somatosensory, visual, and vestibular origins must be integrated by the central nervous system. But there are rich evidences suggesting that children with ADHD can not organize the motor information effectively. The maintenance and control of posture and balance, whether in static or dynamic conditions, are essential requirements for daily activity. The balance function has been closely associated not only with both gross motor movements, such as sitting, standing, walking and fine motor movements, but also with human behaviors. There are increasing evidences suggesting that balance deficit correlates with symptoms of ADHD.Therefore, motor coordination difficulties co-occurring with ADHD should be given emphasis in clinical practice;the training of balance may be one of the possible methods to improve the motor coordination function in ADHD children.

Objective To evaluate the efficacy and clinical value of percutaneous transluminal angioplasty(PTA)with stenting for the treatment of superior vana cava syndrome(SVCS)with malignant causes. Methods A total of 13 patients with SVCS with malignant causes were enrolled in this study.Among the patients,2 had mediastinal cancer, and 11 had metastatic mediastinal tumor. The malignancy of the primary tumors of the 11 patients, including pulmonary cancer in 8,esophageal cancer in 2,and breast cancer in 1,were all confirmed by pathological examination.Via the right femoral vein,a pigtail catheter was introduced percutaneously into the proximate or remote end of the stenotic segment for the visualization of the SVC.After the location, length, and gravity of the stenosis were determined,Wallstent(Boston Scientific,USA)was placed into the SCV,if the patients had no local thrombosis in the stenotic segment. In one patient,a Z-shape stent(COOK,USA)was inserted simultaneously. For the cases complicated with thrombosis, the Wallstent was inserted after local thrombolysis. Results The procedure was completed in all the 13 patients with a success rate of 100%.The average length of the stenotic segment was 4.3 cm(3-6 cm).In one patient, two stents were used, while in the others only one stent was inserted.Thrombolysis was carried out before stenting in 6 patients. The intravenous pressure at the proximate end of the stenotic segment was determined with the patients supine before and after stenting. The pressure decreased from(26.2?1.6)cm H2O to(4.3?0.8)cm H2O after the operation.Postoperative angiography showed no collateral vein in the patients. The SCVS disappeared 0 to 3 days after the surgery. The patients were followed up for 8-26 months with a median of 13.During this period,8 patients died of multiple organ failure caused by multiple metastasis of the primary tumor in 4 to 10 months;the other 5 patients survived(3 of them received further therapies)without recurrence of SCVS.Conclusion PTA with stenting combined with local thrombosis is an effective and invasive treatment for patients with SCVS with malignant causes.

Objective To review the results of limb lengthening over an intramedullary nail in management of femoral defect and length discrepancy. Methods Femoral lengthening over an antegrade intramedullary nail was applied to treat length discrepancy of 4 to 7 cm for three cases, and femoral defects of 8 and 11 cm respectively for two cases from January 2001 to December 2003.The nail and an external fixator(Ilizarov external fixator for bone defect and Orthofix limb-reconstruction system for discrepancy)were applied concomitantly at the time of femoral osteotomy. After a mean fixation time of 16.2 days(13 to 24 days),gradual distraction began at a rate of 0.25 millimeter four times a day. When the managed femur reached the expected length, the nail was locked and the fixator was removed in patients with length discrepancy. The procedure was not finished in patients with bone defects until bony consolidation was partially seen. Results All the patients were followed up for a mean time of 36.4 months(range,22 to 47 months). The mean gain in length was 7.2 cm(range,4 to 10 cm)with good consoli- dation, the mean duration of external fixation was 5 months(range,3 to 8 months),and the mean external fixation index was 0.75 month per centimeter(range,0.57 to 1.00 month per centimeter).Neither axial deviation nor re-fracture occurred. At the last follow-up,the mean range of movement of knee was 107?(80?to 135?).Con- clusions In management of defect and length discrepancy of limbs, lengthening over an intramedullary nail has the advantages of short duration of external fixation, good protection against refracture, and earlier rehabilitation of knee joint,as well as disadvantages of long operation time, massive intraoperative bleeding, high cost and much post- operative scarring.
