1.The effects of Shenfu injection on brain injury in patients undergoing valve replacement

Objective:To investigate the effects of Shenfu injection on cerebral oxygen balance and cerebral ischemia-reperfusion injury in patients undergoing valve replacement during cardiopulmonary bypass,in order to Explore its efficacy and find reasonable route of administration. Methods: 32 patients undergoing valve replacement,aged 25~54,with the heart function of 2~3 level,were randomly divided into four groups with eight in each. Patients in group E1 received SFI 1.5 ml/kg before blocking of the aorta;Patients in group E2 received SFI 1.5 ml/kg after opening of the aorta;Patients in group E3received SFI 0.75 ml/kg before blocking and after opening the aorta,too. Patients in group C received 0.9% NaCl. Blood samples were taken from internal jugular vein and arteria radialis after trachea cannula(T1),10 min after CPB(T2),cooling stability stage(T3),recovery temperature to 33℃(T4),the end of CPB(T5) and 1 hour after the end of CPB(T6)for blood gas analysis.Then,Plasma S100? protein concentration、MDA、Glu and the activity of SOD in jugular vein bulb were determined. 2.Prevention and Control of Hospital Infection During Urethral Catheterization and Indwelling Urethral Catheterization

OBJECTIVE To discuss the prevention and control of the urinary tract infection related to the catheterization.METHODS According to recent research on the control of the urinary tract infection related to the catheterization in our country,we investigated and assessed the problems existing in urethral catheterization and indwelling urethral catheterization in our hospital via a questionnaire.RESULTS There were lots of pitfalls in catheterization.By organizing training,carefully examining,and adopting the examination of the quality of health care,our staff had significantly improved their levels in the field of necessity of catheterization,choice of catheters,asepsis,maintenance of closing cycle of drainage system and others.CONCLUSIONS Enhancing the prevention and control of the urinary tract infection related to the catheterization is very meaningful to reduc of the rate of hospital infections.

To observe the effect of hypotension induced by metoprolol and nitroglycerin (NTG) on the blood gas and hemodynamics. Method: 36 patients were randomly assigned to two groups. ObjectiveTo analyze the withdrawal condition of Chinese patent medicine and the problems encountered; To provide references for reducing unreasonable withdrawal medicine.MethodsNumber, money and reasons of medicine withdrawal of Beijing hospital during 2013-2014 were collected, and several cases and problems were analyzed.ResultsThe causes of medicine withdrawal ranked in descending order in terms of their proportions were changes in illness state (26.82%), adverse reactions (19.53%) and patient refusal to take (14.58%), etc. ConclusionTo reduce the incidence of medicine withdrawal, it is important to improve the expertise of medical personnel, enhance their sense of responsibility, carry out and improve pharmaceutical services and establish a sound medicine withdrawal system.

Case based teaching was applied in order to enhance the teaching efficacy and clinical safety for resident doctors in anesthesia training centers and to arouse their learning incentives and improve their clinical performance.Case based discussions on basic cardiopulmonary resuscitation and multiple traumas were conducted.Process consciousness was enhanced and theoretical knowledge analysis was combined with practical clinical skill training for residents in department of anesthesiology to improve the quality of training.
9.The protective effect of erythropoietin on myocardial ischemia-reperfusion injury in rats and its mechanism

Objective To investigate the protective effect and possible mechanisms of erythropoietin (EPO) on myocardial ischemia-reperfusion injury(MIRI) in rats.Methods Ninety male SD rats were randomly divided into three groups,the sham operation group (n =20),I/RI group (n =20),and EPO treatment combined I/RI group (n =20).The left anterior descending coronary artery was ligated for 30min utes and then loosed for 150min utes to establish the rat model of MIRI.Rats of EPO + I/RI group were injected with EPO(5000u/kg) in abdominal cavity,the others were injected with nomal sodium.The serum MDA content was determined in a method of the thiobarbituric (TBA),the cardiac injury markers(cTn Ⅰ,CK,CK-MB) contents were determined by ELISA.Results The serum MDA and MPO levels of the I/RI group were significantly higher than the sham group,difference was significant(t =10.445,9.848,all P < 0.05) ; Intervention by EPO,the serum MDA and MPO levels of the EPO + I/RI group were significantly lower than the I/RI group,the difference was significant (t =5.087,6.683,all P < 0.05).The serum cTn Ⅰ,CK and CK-MB levels of the I/RI group were significantly higher than the sham group,the difference was significant (t =8.153,5.411,3.729,P < 0.05) ; Intervention by EPO,the serum cTn Ⅰ,CK and CK-MB levels of the EPO+ I/RI group were significantly lower than the I/RI group,the difference was significant(t =4.808,4.089,3.002,all P <0.05).Conclusion EPO has protective effect on MIRI,the effect may be through EPO's antioxidant,antiinflammatory and protective effect on myocardial cells in the MIRI rats.
10.The effects of hyperbaric oxygen preconditioning on aged rats after acute cerebral ischemia

Objective To investigate the effects of hyperbaric oxygen preconditioning on cognitive function in aged rats after acute cerebral ischemia, and to analyze any changes in the cerebral cortex by magnetic resonance imaging (MRI). Methods Twenty-four aged male SD rats were randomly divided into a normal control group, a pure hyperbaric oxygen group, an ischemia group and an ischemia group preconditioned with hyperbaric oxygen. There were six rats in each group. A model of ischemia was induced in the ischemic group and the preconditioned ischemia group using a modified version of Pulsinelli's four-vessel occlusion method. The preconditioned ischemia group (before setting up the acute cerebral ischemia model) and the pure hyperbaric oxygen group were given hyperbaric oxygen treatment once a day for 5 days. Each group received axial line and coronal MRI scans in T1WI and T2WI. The rats' learning and memory abilities were evaluated with a Morris water maze, including escape latency and the percentage of swimming time in the platform quadrant. Results There was no obvious evidence of ischemic brain infarction in the normal control group or the pure hyperbaric oxygen group. There were clear arc-shaped bilateral cortex ischemic infarct areas in the ischemic group. The average ischemic infarct area in the preconditioned group was smaller than that in the simply ischemic group. Escape latency in the ischemia group was significantly longer than in the preconditioned group, and latency in the preconditioned group was significantly longer than that in the normal control and hyperbaric oxygen groups. The percentage of swimming time in the platform quadrant in the ischemic group was shorter than that in the preconditioned group, and that in the preconditioned group was shorter than those of the normal control group or the hyperbaric oxygen group. There was no significant difference between the normal control group and the hyperbaric oxygen group. Conclusions Continuous hyperbaric oxygen preconditioning can reduce the ischemic infarct area in aged rats after acute global cerebral ischemia and improve cognitive function.
