1.Intraperitoneal chemotherapy combined with intravenous chemotherapy after operation in the ovarian epithelial cancer

Purpose:To study the intraperitonal chemotherapy combined with intravenous chemotherapy after surgery for the treatment of ovarian cancer. Methods:The 88 patients of ovarian cancer after cytoreductive surgery were divided into two groups A and B. A group(45 cases) received intraperitonal chemotherapy three times once a week with DDP and 5-FU plus either the MMC or AT-1258, then intravenous chemotherapy, stages Ⅰ/Ⅱ 21 cases(23.86%),stages Ⅲ/Ⅳ 24 cases(27.27%). B groups were given intravenous chemotherapy with CAP protocol once every three weeks,stage Ⅰ/Ⅱ 24 cases(27.27%),stages Ⅲ/Ⅳ 19 cases(21.59%).Results:After three or six courses of chemotherapy there was significant decrease in CA125 both Ⅰ/Ⅱ and Ⅲ/Ⅳ in A and B groups . After three courses of chemotherapy there were 34 cases((38.64%))of A groups and 19 cases(21.59%)of the B groups with CA12535 u/ml(35 cases) who were given three courses of chemotherapy after entire treatment. A and B groups in the Ⅰ/Ⅱ stages showed average survival time (35.05?0.54),(32.38?1.19) months,and in the Ⅲ/Ⅳ stages showed average survival time (31.33?1.41),(28.26?1.88) months.There was no significance (P=0.156) with three year survival rate between A and B groups in the Ⅰ/Ⅱ stages with Log Rank test. There was no significance (P=0.08) in three-year survival rate between A and B groups in the Ⅲ/Ⅳ stages with Log Rank test. There was significance in three-year survival rate between A and B with intraperitonal fluid.Conclusions:The intraperitonal chemotherapy combined with intravenous chemotherapy after surgery for the treatment of ovarian cancer could marketly decrease CA125 than intravenous chemotherapy after three or six courses. Three-year survival ratewas significantly different between CA125 35 u/ml(35 cases) when they were given three courses of chemotherapy after entire treatment. Although the intraperitonal chemotherapy after surgery for the treatment of ovarian cancer had no significant difference between A and B groups in Ⅰ/Ⅱ and Ⅲ/Ⅳ stages with three-year survival rate , it appears longer survival time . There is significant difference with three-year survival between A and B with intraperitonal fluid. The intraperitional chemotherapy combined with intravenous chemotherapy after surgery can be used for the ovarian cancer with intraperitonal fluid and metastases.
2.The characteristics of functional impairment in acute stroke patients in the early stage and their correlation with efficacy of rehabilitationintervention

Objective To explore the functional impairment characteristics of acute stroke patients and cor- relate them to the functional independence after rehabilitation therapy. Methods Seventy-nine inpatients with stroke were divided into hemorrhage group and infarct group according to their condition.They are assessed by using measures of ESS,MBI,MMSE,Sheikh,Fugl-Meyer Balance and Fugl-Meyer motor assessment before rehabilitation treatment.After 1 month of treatment,all the patients were evaluated again using FIM.Correlation analysis was con- ducted with all the parameters. BACKGROUND: The patients with hemiplegia of stroke commonly appear balance disturbance and the mean of assessment on it is applied in quality commonly. But the implementation of assessment on rehabilitation schedule and therapeutic results requires much more quantitative standards on balance function.OBJECTIVE: To probe into interrelationship between every parameter of stabilometer test and variables in other function evaluations in the patients with hemiplegia of stroke as well as the value of evaluation on functional recovery in the patients with hemiplegia of stroke.DESIGN: Quantitative assessment based on the qualitative assessment.SETTING: Department of Otolaryngology and Department of Rehabilitation Medicine in General Hospital of Chinese PLA.PARTICIPANTS: Nineteen first-attack cases of hemiplegia of stroke received rehabilitation treatment in Department of Rehabilitation Medicine of General Hospital of Chinese PLA from September 2002 to April 2003, diagnosed as disorder of cerebral hemisphere on lateral internal carotid artery. Of those, 14 cases were males, 5 cases females; aged varied from 33 to 55 years and the course of sickness was at the range from 0.5 to 4 months.METHODS: Nineteen patients with hemiplegia of stroke received functional independence measures(FIM), timed up-go test(TUGT), Berg balance scale (BBS) and evaluation on balance measure with stabilometer. Of those, stabilometer is a kind of quantitative analysis method, including the parameters of total length of locus, speed, peripheral area, average central changing site of X wavering direction, average central changing site of Y wavering direction, etc.MAIN OUTCOME MEASURES: Pearson correlation analysis was used to determine the interrelationship between every parameter measured with stabilometer and other measured functional variables.RESULTS: By intention-to-treat management analysis, 19 cases of hemiplegia of stroke all entered analysis on results. Among the parameters of stabilometer test, the moderate negative correlation presented in the comparison of total length of locus, speed and peripheral area with BBS during eyes opened and closed ( r = - 0. 475 to - 0. 705, P < 0.05), of which, the correlation between peripheral area and BBS score during eyes closed was better(r= -0.475, P < 0. 05) . Correlation did not present in the comparison of average central changing site of X wavering direction and average central changing site of Y wavering direction with BBS during eyes opened and closed( r = - 0. 346 to 0. 234, P > 0.05) . Moderate positive correlation presented in the comparison of TUGT result with total length of locus, speed and peripheral area during eyes closed( r = 0. 508 to 0. 583, P < 0.05). The correlation did not present in the comparison of the parameters determined with stabilometer and FIM ( r = - 0. 048 to - 0. 296, P > 0.05).CONCLUSION: Stabilometer test can determine balance function in quantity in patients with hemiplegia of stroke, and reflect the severity of balance disturbance, of which, total length of locus speed and peripheral area during eyes opened and closed can judge balance function of patients sensibly and reliably.
4.The effect of preoperative chemotherapy in stage Ⅰ_(b2) cervical cancer

Background and purpose:The greastest diameter of cervical cancer with stage Ⅰ_(b2) disease was more than 4 cm in diameter. surgery as the fi rst priority was diffi cult in these patients, bleeding was the most frequent adverse effect. This article studied the effect of the cervical cancer with stage Ⅰ_(b2) disease underwent neoadjuvant chemotherapy with gemcitabin plus cisplatinum(DDP). Methods:23 cases (A groups) stage Ⅰ_(b2) cervical cancer were treated with gemcitabin 1.5 g/m2 iv infusion at d1, plus cisplatinum(DDP) 20 mg/m2 iv infusion at d1-3. The interval between the two cycles was two weeks.19 cases (B groups) were treated with cisplatinum(DDP) 20 mg/m2 iv infusion at d1-3,plus VCR 1.5 g/m2 iv infusion at d1, and BLM 10 mg/m2 im at d1-3, The interval between the two cycles was three weeks. The assessment for clinical effect and side effect were conducted for the patients with completion of at least two cycles of chemotherapy. Results:42 cases were enrolled in this trial. 6.Dynamic changes of cytokine in aqueous humor of rabbits after traumatic cataract

BACKGROUND: Cytokine is a poly-functional and effective regulator factor to regulate growth of multiple cells. Researches suggest that, as an inflammatory medium, cytokines play a key role in inflammatory reaction of eye; however, there are rare studies on dynamic changes after traumatic cataract.OBJECTIVE: To investigate the relationship among inflammatory reaction and dynamic changes of interleukin 1 (IL-1),IL-6 and tumor necrosis factor alpha (TNF-α) in aqueous humor of rabbits after extracapsular cataract extraction.DESIGN: Randomized controlled animal study.SETTING: Shenzhen Municipal Ophthalmology Hospital, Shenzhen Municipal Ophthalmology Center Affiliated to Medical College of Jinan University.MATERIALS: The experiment was carried out in Institute of Shenzhen Municipal Ophthalmology Center Affiliated to Medical College of Jinan University from March 2004 to March 2006. Fifteen healthy adult New Zealand rabbits of 30 eyes,general grade, weighting 2.5-3.0 kg, of either gender, were provided by Animal Center of the Second Clinical Medical College of Jinan University [certification: SYXK (yue) 2005-006]. Eyes of rabbits were normal before experiment. All rabbits were randomly divided into normal control group, traumatic control group and operative group with 5 in each group and in total of 10 eyes in each group. IL-1, IL-6 and TNF-α kits were provided by Shenzhen Yawei Biotechnology Company Limited.METHODS: Rabbits in traumatic control group and operative group were totally anesthetized with intravenous injection of 10% 1 mi/kg urethan, and then, 5# needle was punctured from corneal limbus to anterior chamber to scarify anterior membrane of lens about 5 mm to establish animal models of traumatic cataract of oculus uterque. Rabbits in normal control group were fed normally. After successful modeling, common antibiotic eyedrops was used to clean conjunctival sac of rabbits in traumatic control group and operative group 3 times a day. On the 3rd day of successful modeling, rabbits in operative group were totally anesthetized with intravenous injection of 10% 1 mL/kg urethan, and then, they undertook extracapsular cataract extraction of oculus uterque: horizontally intercepting bladder or waterly separating with breakage of anterior bladder membrane, expulsing nucleus of lens, washing lens cortex and suturing incisions. On the operative day and on the 1st, 3rd, 7th and 14th days after operation, inflammatory reaction of anterior chamber in traumatic control group and operative group was measured and 0.2 mL aqueous humor was extracted from rabbits in three groups to count and classify cells; meanwhile, expressed level and dynamic changes of IL-1, IL-6 and TNF-α in cytokines of aqueous humor were measured with double-antibodies ELASA technique.MAIN OUTCOME MEASURES: Total numbors of leucocytes and contents of IL-1, IL-6 and TNF-α in aqueous humor of rabbits in three groups after operation.RESULTS: ① On the 1st, 3rd, 7th and 14th days after operation, numbers of leucocytes were (2.4±0.7)×106/L, (2,2±0.5)×106/L, (2.8±0.8)×106/L and (2.0±0.5)×106/L in aqueous humor; (19.7±7.3)×106/L, (28.1±9.6)×106/L, (14.2±5.6)×106/L and(8.4±3.8)×106/L in traumatic control group; (65.3±14.5)×106/L, (79.8±12.7)×106/L, (21.7±8.2)×106/L and (12.4±4.1)×106/L in operative group. In addition, numbers of leucocytes were more in traumatic control group and operative group than those in normal control group (F =22.5, 27.9, 11.6, 8.4;P<0.05). ② Within 1-14 days after operation, contents of IL-1, IL-6and TNF-α in aqueous humo were higher in traumatic control group and operative group than those in normal control group (P<0.05), and there was a significant difference between traumatic control group and operative group (P<0.05);however, there was no significant difference among three groups on the operative day (P>0.05). ③ Contents of cytokines reached peak on the 7th day after operation, decreased gradually, and reached the lowest value on the 14th day.Contents in traumatic control group and operative group were higher than those in normal control group (P<0.05).CONCLUSION: The intraocular inflammation after lens extraction is closely related to the dynamic changes of IL-1, IL-6and TNF-α levels in aqueous humor. Cytokine may be one of crucially inflammatory agents in the eyes after traumatic cataract.
7.Effect of VEGF/bFGF complex peptide vaccine on toxicity and reproduc-tivity in female-mice

[ ABSTRACT ] AIM: To investigate the toxicity and reproductive effect of vascular endothelial growth factor ( VEGF)/basic fibroblast growth factor ( bFGF) complex peptide vaccine ( VBP3) on the female-mice.METHODS:The VBP3 was purified with Ni-NTA affinity chromatography.The female BALB/c mice were immunized with the purified VBP3.The antibody titer in the serum was detected by ELISA.The data of the body weight and the organ weight of the par-ent mice were gathered and analyzed, and the pathological changes of the organs were observed with HE staining to investi-gate the toxicity of VBP3.To investigate the toxicity of VBP3 in the F1 mice, the parent immunized female mice were ma-ted with the parent non-immunized male mice.After the F1 mice were born, the survival rate was calculated, the change of the body weight and the organ weight of the F1 mice were also determined.The pathological changes of the organs in F1 mice were also observed with HE staining.RESULTS: In the parent mice, high titers of the antibodies were detected against VEGF and bFGF, and no difference of the body weight, the organ weight and the pathological change between the immunized mice and control mice was found.In the F1 mice, a low titer of anti-bFGF antibody was detected compared with blank group.The survival rate in control group was higher than that in immunized group.In the 2 groups of the F1 mice, no obvious difference of the weight of the spleen, kidney, lung and heart was observed, and there was some difference in the weight of liver between the 2 groups.The results of the HE staining in the F1 mice showed some difference in the liver between the 2 groups.CONCLUSION:VEGF/bFGF complex peptide vaccine has no obvious organ toxicity in the parent female mice, but has some side effects on the reproductive and the healthy processes of F1 mice.
8.Significance of triggering receptor expressed on myeloid cells-1 of cord blood leukocytes in neonates

Objective To investigate the triggering receptor expressed on myeloid cells-1(TREM-1) of cord blood leukocytes in neonates and the transcription level of mRNA, and analyze its promoting function of inflammatory cytokine secretion. Methods During the period from September 2013 to March 2014, cord blood was collected from 20 term neonates at the time of birth, and peripheral blood was collected from 20 healthy adults. The expression of TREM-1 and TREM-1 mRNA on leukocytes was observed using flow cytometry and real-time reverse transeription-polymerase chain reaction, respectively. After the whole cord blood was stimulated by lipopolysaccharide (LPS) or LP17 plus LPS, the contents of interleukin (IL)-6, tumor necrosis factor-α (TNF-α), IL-8 and soluble TREM-1 (sTREM-1) in the supernatant were analyzed by enzyme linked immunosorbent assay. The statistical significance was determined using the one-way ANOVA test, t test, q test and Pearson correlation coefficient. Results The mean fluorescence intensity of TREM-1 on leukocytes of newborns was not different compared with healthy adults (P>0.05), while the percentage of TREM-1 positive on polymorphonuclear cells was lower than that of healthy adults [(82.3±7.1)% vs (98.6±4.8)%, P<0.05]. The level of TREM-1 mRNA in newborns was lower than in healthy adults (1.16±0.13 vs 1.63±0.24, t=7.714, P<0.01). The LPS treatment significantly increased sTREM-1 in newborn whole blood compared with the control treatment [(156.7±36.3) vs (34.6±6.1) pg/ml, t=13.623, P<0.01]. The concentration of IL-6, TNF-αand IL-8 decreased significantly when TREM-1 was blocked by LP17. In addition, the concentration of sTREM-1 showed a positive correlation with the levels of TNF-α(r=0.519, P<0.05), IL-6 (r=0.507, P<0.05) and IL-8 (r=0.538, P<0.05). Conclusions Healthy newborns exhibit expression of TREM-1 on monocytes similar to healthy adults, and most PMNs express TREM-1 at the newborn stage. Blocking the TREM-1 signal transduction pathway may reduce inflammatory responses of neonate leukocytes.
9.Treatment of 32 patients with severe thoracic and abdominal injury accompanied with acute respiratory distress syndrome

Objective To study an effective treatment of severe thoracic and abdominal injury accompanied with acute respiratory distress syndromes (ARDS).Methods Emergency treatments of 32 patients with severe thoracic and abdominal trauma accompanied with ARDS were retrospectively analyzed.Results All of the 32 patients had severe thoracic and abdominal injury,ribs fracture or pulmonary contuson.Anti-shock treatment,reasonable supplemental blood volume,rational mechanical ventilation and emergency operation were performed.Twenty-six patients were cured,and 6 died,with mortality 18.75%.Conclusions Early diagnosis,timely anti-shock treatment,early treatment for thoracic and abdominal injury and correct mechanical ventilation are essential for treating thoracic and abdominal trauma accompanied by ARDS,and is also an effective method for reducing mortality.
10.Effect of blocking Wnt-?-catenin signaling pathway on activation of hepatic stellate cells

Objective To investigate whether functional Wnt-?-catenin signaling is present in activated hepatic stellate cells (HSC),and the effect of blocking this signaling on activation of HSC.
