1.Clinical practice of low-and mid-mixtures insulin analogues in patients with type 2 diabetes mellitus

Premixed insulin is currently the most popular insulin starter in China.According to the different ratios of basal and prandial components,premixed insulin analogues could be classified into low-and midmixtures.The former is now mainly used as insulin starter in the patients with type 2 diabetes mellitus after the failure of monotherapy or combination with oral antidiabetic drugs,while the later is usually used as simplified intensive insulin treatment in patients with type 2 diabetes mellitus inadequately controlled on insulin once-or twice-daily.
2.Research on effect of esomeprazole in treating elderly patients with NSAIDs correlated peptic ulcer bleeding

Objective: To study the effect of esomeprazole in treating elderly patients with NSAIDs correlated peptic ulcer bleeding. Methods: Patients with NSAIDs correlated peptic ulcer bleeding in our hospital from January, 2010-December, 2012 were enrolled and randomly divided into observation group given esomeprazole and control group given omeprazole. Then the baseline data were surveyed and effect of ulcer bleeding, condition of abdominal pain alleviation and ulcer ulcer, adverse reaction cases were observed. Results: (1)there were no significant difference between the baseline data of two group;(P>0.05);(2)in observation group, excellence rate was 42/70(60%), effective rate was 24/70(34.3%), the overall treatment effect was better than that of control group, the difference had statistical significant(P<0.05);(3)there were no significant difference between two group of abdominal pain and ulcer healing;(4) during treatment, there were no serious adverse reactions in the two groups. Conclusion:Compared with omeprazole, esomeprazole has equal effect on abdominal pain relief and ulcer healing, and it is much more effective in treating elderly patients with NSAIDs correlated peptic ulcer bleeding.
3.Observations on the Efficacy of Warm Needling plus Chinese Herb Fumigation in Treating Plantar Fasciitis

ObjectiveTo investigate the clinicalefficacy of warm needling plus Chinese herb fumigation in treating plantar fasciitis.MethodSixty-one patients with plantar fasciitis were randomly allocated to a treatment group of 31 cases and a control group of 30 cases. The treatment group receivedwarm needling plus Chinese herb fumigation and the control group, warm needling alone.ResultThe total efficacy rate and the cure rate were 100.0% and 61.3%, respectively, in the treatment group and 96.7%and 40.0%, respectively, in the control group. There was no statistically significant difference in the total efficacy rate between the two groups (P>0.05), but the cure rate was significantly higher in the treatment group than in the control group and there was a statistically significant difference between the two groups (P<0.05). There was a statistically significant pre-/post-treatment difference in the VAS score in the two groups (P<0.01), indicating that both the groups had a good analgesic effect. There was a statistically significant post-treatment difference in the VAS score between the treatment and control groups (P<0.01), indicating that theanalgesic effect was better in the treatment group than in the control group.ConclusionThe efficacy of warm needling plus Chinese herb fumigation is better than that of warm needling alone in treating plantar fasciitis.
4.Comprehensive utilization of compressed air in hyperbaric oxygen chamber

Objective To realize centralized supply of hospital compressed air to reduce repeate investment.Methods Air turned into compressed one after passing through air pressure pump, which undergoes preliminary cooling and drying by refrigerated drier, then the compressed air went into air tank for further cooling and storage, and finally was transmitted into hyperbaric oxygen chamber, ICU and operating room as desired.Results Centralized supply was realized for hospital compressed air with little technical modification and investment.Conclusion The compressed air of hyperbaric oxygen chamber is introduced into ICU and operating room and gains economic and social benefits, and thus is worth popularizing practically.
5.Research progresses of the effective mechanism of pyroptosis and its related diseases

Pyroptosis is a newly discovered form of programmed cell death, which has a close relationship with infectious diseases and spontaneous inflammatory disease and autoimmune disease.Gasdermin-D is the effector of pyroptosis.Inflammatory caspases cleave Gasdermin-D, break the structural autoinhibition and active the pore-forming activity of Gasdermin-D.Gasdermin-D-N domain is the active form of Gasdermin-D.Oligomerized Gasdermin-D-N domain forms membrane pores.Both pyroptosis and necroptosis can form pores.In this article, the progress of the research on pyroptosis, the involvement of Gasdermin-D in the molecular mechanism of pyroptosis, the distinguishment of pyroptosis from necroptosis and pyroptosis-related diseases are reviewed.
6.Intervenient study of Shengjiang San on Th1/Th2 imbalances and related regulatory factors in sepsis pa- tients

Objective To investigate the intervention of Chinese herb Shengjiang San(SJS)for the imbal-ances of serum Th1/Th2 cells and related regulatory factors in sepsis patients. Methods Fifty-five sepsis patients were randomly divided into two groups:conventional treatment group of 27 cases and SJS group of 28 cases. Nine cases of healthy volunteers were enrolled as control group in the study. Cases of conventional group were treated with western medicine only and cases of SJS group were treated with both western medicine and Chinese herb SJS (100 mL twice one day). The therapy course of both groups was 3 days. Score of Chinese medical syndromes ,se-rum level of leukocyte count,C reactive protein(CRP)and procalcitonin(PCT),serum T-bet,GATA-3,Th1&Th2 cells in the proportion of whole CD4+Th cells and Th1/Th2 ratio were compared respectively in each group be-fore and after treatment. Results There were significant differences between SJS group and conventional group in score of Chinese medical syndromes,serum level of leukocyte count,T-bet,GATA-3,Th1&Th2 cells in the pro-portion of whole CD4+Th cells and Th1/Th2 ratio(P<0.05 or P<0.01). There were no significant differences be-tween the two groups in serum level of CRP,PCT and GATA3. Compared with control group,there were signifi-cant differences in all indicators except GATA3 of the two groups(P<0.01). Conclusion Chinese herb Shengji-ang San has an effective benefits to Chinese medical syndromes,inflammatory reaction,the imbalance of serum Th1/T2 and related regulatory factors(T-bet&GATA-3)in sepsis patients.
7.EXPERIMENTAL STUDY OF ACTIN AND MYOSIN IN THE SCAR TISSUE

Objective To investigate the different expression of actin, myosin Ⅱ in hypertrophic scars, keloids and normal skins, and to understand the relationship of actin, myosin Ⅱ and the scar contracture. Methods Fifteen cases with hypertrophic scars, 10 cases with keloids and 15 cases with normal skins were chosen randomly. The expression of actin and myosin Ⅱ were detected by immunohistochemical method in the hypertrophic scars, keloids and normal skins. The fibroblasts isolated from three types of tissue were cultured in vitro, then actin and myosin Ⅱ in three different fibroblasts were measured using flow cytometry. Results The immunohistochemical staining of myosin Ⅱ in hypertrophic scars was positive, while the staining in keloids and normal skins were negative. The positive rate of myosin Ⅱ expression in hypertrophic scars, keloids and normal skins were (95.11±2.78)%, (16.86±7.11)%, and (5.31±1.79)% respectively. There were significant difference between keloids and the two others(P<0.01). The actin expression in three difference tissues were positive, there were no significant difference in hypertrophic scars, keloids and normal skins(P>0.05). The positive rate of actin expression in hypertrophic scars, keoids and normal skins were(77.77±15.43)%, (88.89±10.29)%, and (82.92±13.48)% respectively, and there were no significant difference(P>0.05). Conclusion Myosin Ⅱ may play an important role in the scar contracture. Actin is the contractile protein of cell, it plays important role in cellular movement. Actin is necessary protein in the cell.
8.Effect and it's mechanism of microgravity on biological characteristics of hematopoietic stem/progenitor cells

Hematopoietic stem/progenitor cells have the potential of self-renewal,proliferation and multidirectional differentiation,which maintain the normal haematopoiesis of bnmml body.Alterations in the number and function of mature blood cells in the peripheral blood commonly observed in humans exposed to microgravity (μ-g)are results of many reasons.The change of biological characteristic of hematopoietic stem/progenitor cells is one of the important ressons.Migration,proliferation,and differentiation of hematopoietic stem/progenitor cells under microgravity are summarized in this article.
9.Expression of the hBD-2 in human airway epithelium induced by IL-1? in vitro and its molecular mechanism

Objective To investigate induction of the expression of human ?-defensin-2 (hBD-2) mRNA and the changes in activation of I-?B? and NF-?B by IL-1? in the human airway epithelium. Methods Primary human respiratory tract epithelium were stimulated with IL-1? and and/or PDTC. The expression of hBD-2 mRNA was detected by reverse transcription polymerase chain reaction (RT-PCR). The I-?B? protein level in the cytoplasm was determined by Western blot, and the nuclear factor-kappaB (NF-?B) binding activity was analyzed by electrophoretic mobility shift assays. Results The hBD-2 mRNA expression could be detected after 1.5 hours of IL-1? stimulation at concentration of 1?g/L, and the expression of HBD-2 was in a dose dependent manner. Stimulation with IL-1? resulted in a reduction of I-?B? protein levels. The supershifts assays indicated that the p65-p50 heterodimer complexes of NF-?B were involved in the activation of NF-?B. Conclusions I-?B? can induce the expression of HBD-2 mRNA in a dose dependent manner; the p65-p50 heterodimer complexes of NF-?B play an important role in the regulation of hBD-2 gene expression in response to IL-1?.
10.Epidemiological study and key-point analysis of severe heatstroke patients

Objective To investigate and analyze the cause of the illness, types and conditions combined with dysfunction of important organs and death of severe heatstroke patients in military hospitals in southern China. Methods 117 cases of heatstroke patients from Jan 1988 to Mar 2005 admitted to 8 military hospitals were retrospectively analyzed. Results 94.02% of the cases occurred during military drill; 10% of patients complained of slight discomfort before or during the drill; the incidence of thermoplegia, heat cramp and heat exhaustion were 43.26%, 42.55% and 13.86%, respectively, in severe heatstroke, and the respective death rate was 9.76%, 5% and 23%. The incidence of severe heatstroke combined with liver dysfunction was 31.9%, with central nervous impairment was 14.9%, and with MOF was 21.3%. All patients survived when no organ dysfunction occurred; the death rate in those with single organ dysfunction was 3.6%, while it was 35% in MOF patients. Conclusion An attempt to set up a predicting index of heatstroke might be beneficial for scientifically supervising the military drill for the servicemen. Reduction of the core temperature is the“the first critical point”in the treatment. Other “critical point” in the treatment may also be emphasized for those with MOF. The present therapeutic system for heat stroke as executed in the military needs to be perfected.
