Objective:To discuss and set up indicator system of medical equipment procurement bidding, propose the evaluation methods which based on Fuzzy synthesis and confirm the most appropriate bidder for improving the management level of hospital in medical equipment purchase. Methods: From the bidder's ability to compete and innovation ability and financial standing and development potential and corporate credibility, five aspects to determine the evaluation index. Establish evaluation index system, the application of fuzzy comprehensive method to analyze the bid scheme offered by the tender.Results: According to the maximum subordinate principle, the evaluation value of the bidder is 0.28, the evaluation value of the B tender is 0.32, the value of the bid quotient is 0.23. Comparison of the comprehensive evaluation of the bid evaluation of the tender score of three, B the highest bid quotient, choose B tender bid.Conclusion:Facing different bidders, in the case of each of the advantages and making program quantification, conducting analysis of fuzzy synthetic method can realize the reasonable choice for medical equipment suppliers.

Objective:To investigate the status of vitamin D deficiency in middle aged and elderly patients at an orthopedic department.Methods:The data of 25-hydroxy vitamin D[25(OH)D]in patients aged over 55 years at an orthopedic department in Beijing Hospital from November 2014 to January 2020 were collected.The differences in serum 25(OH)D status were compared in different gender and age.Results:A total of 3004 subjects were included with 725 males and 2279 females, ranged from 55 to 102 years old with an average of(71.1±9.7)years.The average serum 25(OH)D level was(17.77±9.92)ng/ml, which was diagnosed as vitamin D deficiency status.Serum 25(OH)D levels were(19.66±9.77)μg/L, (18.34±9.78)μg/L, (16.19±9.51)μg/L and (14.74±10.49)μg/L, at different age groups of 55-64, 65-74, 75-84, 85 years and over, respectively, with statistically significant differences( F=29.357, P<0.05). Serum(OH)D levels of patients were severely deficient, deficient, insufficient and sufficient, with their proportion of 21.3%, 44.6%, 23.3% and 10.8%, respectively.The proportion of patients achieving from vitamin D levels severe deficiency to deficiency to insufficient to sufficient were 18.2%, 44.8%, 25.1% and 11.9% in men, and 22.2%, 44.6%, 22.7% and 10.5% in women, respectively. Conclusions:The deficiency of vitamin D is prevalent in elderly patients visiting at orthopedic department.Serum 25-hydroxy vitamin D has a decreased trend along with age increasing, showing an increased trend of vitamin D deficiency.Patients with severe vitamin D deficiency are the focus for the intervention.
5.Impact of pamidronate disodium on the calcification of vascular smooth muscle cells induced by hyperphosphate

Objective To investigate the protective effect and mechanism of pamidronate disodium on calcification in rat vascular smooth muscle cells (RVSMCs) induced by hyperphosphate.Methods RVSMCs were placed in various culture mediam, including normal phosphate medium (Pi 1.4 mmol/L), high phosphate medium (Pi 4.5 mmol/L), different pamidronate disodium concentrations medium (Pi 4.5 mmol/L+pamidronate disodium 10-5, 10-6, 10-7 mmol/L). Calcium content and cell protein content were quantified by the O-cresolphthalein complexone method and BCA protein assay respectively. Calcification was visualized by yon Kossa staining, and cbfα-1,osteocalcin were quantified by Western blotting. Results After culture for 3 days, calcium content in high phosphate group were much higher than that in control group, and pamidronate disodium groups had a lower calcium content compared with high phosphate group (all P<0.05).Calcium deposit in RVSMCs was greater in high phosphate group, while pamidronate disodium groups revealed obviously decreased deposit (all P<0.05). Protein expression of cbfα-1 and osteocalcin in pamidronate disodium groups was much lower than that in high phosphate group.Conclusion Pamidronate disodium can protect RVSMCs from phosphate-induced calcification in vitro, which may be associated with the blockage of transformation of RVSMCs into osteoblast.
6.Effect of Chinese Yam Polysaccharide on Expression of HIF-1α and VEGF in Rats with Renal Ischemia Reperfusion Injury

Objective To observe the effect of Chinese yam polysaccharide on the expression of hypoxia inducible factor-1α (HIF-1α) and vascular endothelial growth factor (VEGF) in rats with renal ischemia reperfusion injury. Methods SD rats were randomly divided into sham operation group, model group and Chinese yam polysaccharide group. Renal ischemia reperfusion injury model in rats was prepared. Seven days before operation, Chinese yam polysaccharide group was gastrically administrated with yam polysaccharide (200 mg/kg) daily, sham operation group and model group with same volume of saline respectively. BUN and SCr were detected postoperative 6 h, the protein expression of HIF-1αin renal tissue was detected by Western Blot, and HIF-1αand VEGF mRNA expression in renal tissue were detected by RT-PCR. Results Compared with sham operation group, serum levels of BUN and SCr of model group and Chinese yam polysaccharides group increased (P<0.01), and the mRNA and protein expression of HIF-1α, mRNA expression of VEGF in renal tissue increased (P<0.01). Compared with model group, serum levels of BUN and SCr of Chinese yam polysaccharide group decreased (P<0.01), and HIF-1α mRNA and protein expression, VEGF mRNA expression in renal tissue increased (P<0.01). Conclusion Chinese yam polysaccharide has obvious protective effect on renal ischemia reperfusion injury, which may be related with up-regulation of HIF-1αexpression and induction of VEGF expression.
7.Effects of Liuwei Dihuang Decoction on Expressions of HIF-1α, CollⅠ, and CollⅢin Renal Interstitial Fibrosis Rats

Objective To discuss the mechanism of Liuwei Dihuang Decoction against renal interstitial fibrosis. Methods The rat models of renal interstitial fibrosis were induced by 5/6 nephrectomy, and were randomly divided into sham-operation group, model group, Liuwei Dihuang Decoction group and enalapril group. Liuwei Dihuang Decoction group was given 6.25 g/(kg?d) of Liuwei Dihuang Decoction for gavage, enalapril group was given 10 mg/(kg?d) enalapril for gavage, sham-operation group and model group were given 10 mL/(kg?d) of distilled water for gavage, 1 time per day for 12 weeks. Expressions of HIF-1α, CollI, and CollIII in kidney tissue were detected by immunohistochemistry. Results Compared with the sham-operation group, the expressions of HIF-1α, CollI, and CollIII of renal tissue in the model group significantly increased (P<0.05). Compared with the model group, Liuwei Dihuang Decoction group and enalapril group can inhibit the expressions of HIF-1α, CollI, and CollIII (P<0.05), and Liuwei Dihuang Decoction group was better than enalapril group (P<0.05). Conclusion The mechanism of Liuwei Dihuang Decoction against renal interstitial fibrosis may improve the renal tissue of chronic hypoxia to play the role of preventing renal interstitial fibrosis by decreasing the expressions of HIF-1α, CollI, and CollIII.
8.Diagnosis and treatment of 1 110 cases of premature children

ObjectiveTo aralyze The prevalence of premature children, mortality and complications to related factors, discussed measures to reduce the premature children mortality. MethodsFrom January 2003 to December 2006 and January 2007 January 2010 12 after 8 years in our hospital obstetric preterm children born in 1110 into the first 4 years of group and group 4 years after the two groups. Each group of children on the incidence of premature causes of death, complications were retrospectively analyzed. Results Preterm birth rate of first 4 years 2.85% to 3.67% after 4 years,the difference was statistically significant( P <0.05 ). Between the two groups the proportion of different age groups and the main types of diseases,no significant difference(P> 0.05). By comparison,the mortality of premature children dropped from the 12.06% to 2.36%, a significant difference before and after ( P < 0.01 ).ConclusionThe prevention and treatment of premature children was still very trduous task, particularly in preventing complications of preterm children was very important to prevent the occurrence of lung disease.
9.Therapeutic effect and its mechanism of antioxidant taurine plus metoprolol on elderly spontaneously hypertensive rats

Objective To investigate the protective effects of antioxidant taurine plus metoprolol on the blood pressure(BP)and blood vessel function of elderly spontaneously hypertensive rats(SHR).Methods SHRs were divided into three groups (6 rats,each):metoprolol group (100 mg · kg-1 · d-1,intragastric administration);taurine group (200 mg · kg-1 · d-1,intraperitoneal injection);taurine plus metoprolol group(metoprolol 100 mg· kg-1 · d-1,intragastric administration ± taurine 200 mg· kg-1 · d-1,intraperitoneal injection).The control group[6 Wistar-Kyoto(WKY) rats]was treated with the same volume of sterile normal saline on the same schedule in intragastric administration and intraperitoneal injection.Blood pressure variability(BPV),diurnal variation of BP,the level of glutathion peroxidase (GSH-Px),malonaldehyde (MDA) and the aorta GSTM1 enzyme expression were evaluated before and 14 days after treatment.Results The serum GSH-Px activities were higher in metoprolol group[(2 759.8 ± 117.6) kU/L],taurine group [(2 848.0 ± 280.2) kU/L] and taurine plus metoprolol group[(3 052.8±283.7)kU/L]than in control group[(2 368.0± 60.4) kU/L] (all P<0.05).Fourteen days after treatment,MDA level was significantly lower in taurine group[(9.5±0.7)ng/L,P<0.01]than in control group[(13.7±1.5)ng/L].However,there was no statistical difference in MDA level between metoprolol group/taurine plus metoprolol group and control group.Forty weeks after treatment with taurine,GSTM1 protein expression was increased,but it had no statistical difference.Conclusions The combined antioxidant taurine and metoprolol can effectively decrease BPV of elderly SHRs,in which the antioxidant effect might be associated with elevated GSTM1 protein expression.
10.Method for calculating the blood concentration of isoflurane with the inspired and expired concentrations during laparoscopic surgery

Objective: To estimate the blood concentration of isoflurane with the inspired andexpired concentrations during laparoscopic surgery. Methods: 2 sections were divided in this experiment: section 1:23 adult patients (ASA I~II)were received abdominal surgeries,of whom,12 patients for non-laparoscopic surgeries and the others for laparoscopic surgeries. Central venous blood samples were collected for gas chromatography determination 20, 40, 60, 80 min after inhalating isoflurane . We could receive two different F value (the fraction of uptake of isoflurane)through the formula. Section 2: 27 patients were recruited for abdominal surgeries, 13 for laparoscopic surgeries and the others for non-laparoscopic surgeries. All of the processes were the same as the former. Then we confirm the results obtained from section1. Results: The F was 0.52 in the laparoscopic surgeries and 0.44 in the nonlaparoscopic surgeries. In the laparoscopic surgeries, the bias (MDPE) was 17% , accuracy (MDAPE)was 22.88% and the wobble was 11.7% .The correlation coefficient (r)was 0.83. In the nonlaparoscopic surgeries, the MDPE was 5.37% , the MDAPE was 16.02% , the wobble was 15.86% .The correlation coefficient was 0.90. Conclusion: The formula could be used in abdominal surgeries to evaluate the concentration of isoflurane according to the clinical standard of MDPE
