Objective To investigate the effect of dexmedetomidine sedation on perioperative hemodynamics and heart rate variability (HRV) in the aged patients with cataract operation.Methods Fifty aged patients with cataract who accepted phacoemulsification operation in local anesthesia,were divided into experiment group(dexmedetomidine group) and control group (0.9％ sodium chloride group) by random digits table method with 25 cases each.The experiment group was given local anesthesia,after dexmedetomidine load at 0.5 μg/kg intravenous infusion,dexmedetomidine continuous pumping [0.2-0.3 μg/ (kg ·h)] to maintain the depth of anesthesia.The control group was given the same dose 0.9％ sodium chloride intravenous infusion.Local anesthesia method:2％ lidocaine and 0.75％ bupivacaine 3 ml mixing for retrobulbar anesthesia.The changes of HRV and hemodynamics in the 2 groups before operation (T0),beginning operation (T1),perioperative10min (T2),perioperative30min (T3),postoperative (T4) were recorded,and hemodynamies was analyzed.Results The normalized units of low frequency (LFnu) and low frequency (LF)/high frequency (HF) at T1,T2,T3 in control group were significantly higher than those at T0 [(73.9 ± 9.2)％,(74.5 ± 9.0)％,(75.8 ± 8.5)％ vs.(61.3 ± 7.5)％ and 2.4 ± 1.1,2.6 ± 1.3,2.7 ± 0.9 vs.1.6 ± 0.5],total frequency power (TP) was significantly lower than that at T0 [(1372 ± 419),(915 ± 243),(1389 ± 142) Hz vs.(2351 ± 608) Hz],there were statistical differences (P ＜ 0.05).The LFnu and LF/HF at T1,T2,T3 in experiment group were higher than those at T0,but there were no statistical differences (P ＞0.05).The TP at T1,T2,T3 in experiment group was significantly lower than that at T0 [(1458 ± 325),(983 ±272),(1497 ± 210) Hz vs.(2235 ± 513) Hz],there was statistical difference (P ＜ 0.05).There was no statistical difference in normalized units of HF at all time points between the 2 groups (P ＞ 0.05).The LFnu,LF/HF and TP at T1,T2,T3 in experiment group were better than those in control group,there were statistical differences (P ＜ 0.05).The fluctuation of hemodynamics in experiment group was significantly lower than that in control group,there was statistical difference (P ＜ 0.05).Conclusion Dexmedetomidine sedation little effects on HRV during perioperative period in aged patients with cataract operation,can significantly reduce the influence of operation stimulation on autonomic nervous system and hemodynamics.

Objective To investigate the effects of pediatric subclinical hypothyroidism on bone age and height and their treatment outcome.Methods Ninety-seven children with subclinical hypothyroidism were served as this study group and 80 children with normal thyroid function as control group.Bone age and height were detected.Patients in study group were treated with thyroid hormone replacement therapy.The efficacy and lipid changes were evaluated before and after treatment.Results The height and bone age in patients of study group were (1.31 ± 0.06) m and (8.29 ± 0.35) years,significantly lower than the control group ((1.41± 0.09) m,(10.02 ± 1.45) years; t =13.532,P ＜ 0.001 and t =14.590,P ＜ 0.001).Thyroid-stimulating hormone showed a significant negative correlation with bone age (r =-0.783,P ＜ 0.001).In a total of 97 children with subclinical hypothyroidism,71 cases were cured,24 cases with improvement and 2 cases with ineffective,and the total effective rate was 97.9％.Before treatment total cholesterol ((3.45 ±0.42) mmol/L),low density lipoprotein cholesterol((2.50 ± 0.35) mtmol/L) in the study group were significantly higher than those of control group((3.33 ± 0.26) mmol/L,(2.24 ±0.22) mmol/L;t =4.378,5.103 ;P =0.036,0.002).After treatment,the level of low density lipoprotein cholesterol in children with subclinical hypothyroidism was significantly lower than before treatment((2.34 ±0.36) mmol/L vs (2.50 ±0.35) mmol/L,t =4.389,P =0.033),but there was no significant difference in terms of other parameters.Contusion Children with subclinical hypothyroidism can lead to lower bone age and shorter height and it may also lead to changes in lipid levels.More attention should be paid on this disease and early intervention should be provided.

Considering the World Health Organization's classification of Helicobacter pylori as a definite (class I ) carci- nogen, the relationship between oral microbial community and tumors is gaining increased interest. This review focused on three relationships between oral microbiota and tumors, i.e., between oral Helicobacter pylori infection and gastric tumors, between oral microbiota and oral squamous cell carcinoma, and between human immunodeficiency virus and tumors. The aims were to realize the early diagnosis of tumors with oral microbiota and support studies on treatment development.

Objective To evaluate the incidence of contrast-induced nephropathy(CIN)in non-selected patients undergoing coronary angiography and intervention,and to investigate,the clinic risk factors of CIN as well as the renal safety of isosmolar iodixanol.Methods A total of 532 patients undergoing diagnostic coronary angiography or percutaneous coronary intervention were enrolled.Serum creatinine(SCr)was measured at any time within a week before the procedure and 2 days(48 hours)post procedure.The definition of CIN is the increase of serum creatinine of 44.2 ?mol/L(0.5 mg/dL)or 25% of the baseline at 48 hours after the use of contrast media.The effects of contrast media on renal function in patients undergoing coronary intervention were investigated and predictors of CIN were determined using logistic regression test.Results(1)Among the 532 patients,CIN occurred in 29 patients and the incidence of CIN in non-selected patients was 5.5%.(2)The incidence of CIN was significantly higher in patients with baseline eGFR

Objective To study the protective effects of total flavones of Herba Epimedii(TFE)on focal cerebral ischemia of rats.Methods Rat model of middle cerebral artery occlusion was established.With Jinnaduo as a positive control drug,the effect of TFE on the neurological behavior of cerebral ischemia rat,the cerebral water volume,the cerebral infarction area,the cerebral biochemistry and the pathologic change of brain were evaluated.Results TFE improved rats neurological behavior,decreased the cerebral water volume,reduced cerebral infarction area,increased ATPase and SOD activity,decreased MDA content,and also improved the pathological changes of ischemic brain.Conclusion TFE have the protective effect on cerebral ischemia in rats,its mechanism may be concerned about improving energy metabolism and anti-oxidation.

Objective To explore the clinical and electroencephalography features of children's frontal lobe epilepsy(FLE)and improve the diagnosis and therapy.Methods Medical records were reviewed and neuropsychological evaluations of patients with FLE diagnosed from 2000 to 2006.Children were examined by 24 h EEG and imaging examinations.Their charts for family medical history,seizure,aura,simultaneous phenomenon and anti-epilepic drug therapeutic effects were reviewed.Results Seizures were clonus in 24 cases,tonic in 4 cases,adversive,atonic,vomit in 2 cases,respectively,paresthesia,cephalic,visul,sensory in 1 case,Imaging finding were in 9 of 40.All the 40 cases were bought into 1 to 6 years' follow-up.Long-term control was achieved in 35 of 40.Partial control was achieved in 3 of 40.The left 2 cases had no response to the anti-epileptic drugs.Conclusions The common features of FLE included high seizure frequency,short duration and nocturnal preponderance.There is high positive rate of active EEG in detecting epileptic discharges,and active EEG shoud be necessary.Carbamazepine is recommended to be the first choice to FLE.
