Chinese Journal of Physical Medicine and Rehabilitation 2008;30(4):248-251
Objective To study the effects of electroacupuncture(EA)on gastrointestinal motility and nitric oxide synthase(NOS)positive nerves in myenteric plexus in acute exhaustive exercise rats. Methods A total of 54 male Sprague-Dawley rats were randomly divided into an acute exhaustive exercise(AEE)group,an AEE plus EA group and a control(C)group. The gastric empty rate and intestinal motility rate were measured, and histological changes of NOS-positive nerves in ileum myenteric plexus were observed with enzymatic histochemical method and image analysis technique. Results In the rats of AEE group, the gastric empty rate and intestinal motility rate were delayed but the numbers of NOS-positive neurons and expression levels of NOS in the ileum myenteric plexus significantly increased comparing with those of the C group(P<0.01).Compared with AEE group, the gastric empty rate and intestinal motility rate were significantly increased but the number of NOS-positive neurons and expression level of NOS in the ileum myenteric plexus significantly decreased in AEE plus EA group(P<0.01). Conclusion The findings of this study suggest that increase of NOS-positive neurons and expression levels of NOS in the myenteric plexus is one of the mechanisms of slowing down of gastrointestinal movement after acute exhaustive exercise.Electroacupuncture at Zusanli acupoint may decrease the number of NOS-positive neurons and expression levels of NOS in the myenteric plexus, and improve gastrointestinal movement in acute exhaustive exercise rats.

ObjectiveTo investigate the etiology, exacerbating factors and outcome of geriatric patients with acute renal failure (ARF) in emergency and critical internal medicine. MethodsClinical features of elderly inpatients with ARF who were diagnosed in internal medicine of our hospital during the past 10 years were retrospectively analyzed. Patients were divided into community-acquired ARF (CA-AFR) group and hospital-acquired ARF (HA-ARF) group. Comparisons between CA-ARF group and HA-ARF group, and between elderly patients and non-elderly patients with ARF in internal medicine were performed. ResultsAmong 381 elderly ARF patients in internal medicine, there were 218 (57.2%) CA-ARF patients and 163 (42.8%) HA-ARF patients. Most of the HA-ARF patients (153 cases) were distributed in medical intensive care. Compared with CA-ARF group, patients in HA-ARF group were older, with more underlying diseases, a higher ratio of infections and/or heart failure and more severe acute renal failure. Infection and heart failure/severe myocardial isehemia were the major exacerbating factors for HA-ARF. Of the 163 patients with HA-ARF, 146 patients died and 17 patients survived. Patients in the death group had more underlying diseases, a higher ratio of severe infections and heart failure/severe myocardial ischemia, a higher ratio of cases with multiple organ dysfunction syndrome(MODS) and a higher score of APACHE Ⅱ.When compared with non-elderly patients with ARF, the elderly patients had a higher proportion ofHA-ARF, a higher ratio of MODS, a higher score of APACHEⅡ and a higher mortality of ARF.ConclusionsThe elderly critical patients are susceptible to HA-ARF with more chronic underlying diseases. Infection and heart failure/severe myocardial ischemia are the major exacerbating factors. The prognosis depends on the treatment of predisposing diseases and removal of exacerbating factors.

Objective To construct and express fusion toxin human IL15 mutant/TNF? mutant (hIL15M/TNF?M) in order to develop a targeted toxin capable of eliminating abnormal cells expressing high levels of hIL15 receptors. Methods DNA fragments of truncated hIL15M and TNF?M coding gene were joined in frame and cloned into T7 expression vector pET16b. The expressed fusion protein was purified from E. coli . Cytotoxicity of hIL15M/TNF?M was measured by MTT assay. Results E. coli transformed with the recombinant plasmid expressed an extra band with the expected size of 30?10 3. MTT assay indicated that PHA activated T cells were 13 times more sensitive to the cytotoxic effect of hIL15M/TNF?M than to the resting T cells. Conclusion hIL15M/TNF?M has high toxic effect against cells expressing high levels of IL15 receptors, suggesting that hIL15M/TNF?M has potential therapeutic effect on T cell leukemia and other diseases related with improperly activated T cells.
