1.Treatment of 97 Cases of Prolapse of Lumbar Intervertebral Disc by Manipulations of Bone Setting plus Acupuncture

Improved manipulation of oblique-pulling (Ban) and fixed-position rotating pulling were applied to the patient with the prolapse of lumbar intervebral disc for reduction. Then acupuncture therapy, which used Shuigou (GV 26) and Weizhong (BL 40) as the main acupoints, was used. Totally 97 cases were treated, 58 cases were cured, 35 cases were improved, and 4 cases had no effect. The total effective rate was 95.5%.

3.Methylation status of promoter of mismatch repair genes hMLH1 and hMSH2 in epithelial ovarian cancer

Objective To explore the methylation status of hMLH1 and hMSH2 promoter region in the epithelial ovarian cancer and its role in oncogenesis.Methods Methylation status of hMLH1 and hMSH2 promoter region was assayed in 20 normal ovarian tissues,25 benign epithelial tumor,56 malignant epithelial tumor and cell lines SKOV3,3AO by methylation-specific PCR (MSP).SKOV3 and 3AO were analyzed before and after 5-aza-2′-deoxycytidine (5-Aza-CdR) treatment.In addition,an alterations of mRNA expression of hMLH1 and hMSH2 was observed by reverse transcription polymerase chain reaction (PT-PCR).Results No methylation of hMLH1 and hMSH2 promoter was found in normal ovarian tissues. 4.Preoperative and intraoperative adjunctive intravitreal conbercept in 23G vitrectomy for proliferative diabetic retinopathy

Objective To discuss the effects and influence of preoperative and postoperative adjunctive intravitreal conbercept in 23G vitrectomy for proliferative diabetic retinopathy (PDR).Methods A retrospective research was performed on 42 PDR eyes from January 2015 to February 2016 in the Second Affiliated Hospital of Nanchang University,who received either intravitreal 0.05 mL conbercept injection 7 days before 23G vitrectomy (group A,n =22) or intravitreal 0.05 mL conbercept injection at the end of 23 G vitrectomy (group B,n =20).The operative time,postoperative vitreous hemorrhage (VH),intraoperative and postoperative other differences of clinical indicators and postoperative best-corrected visual acuity (BCVA) between the two groups were compared.Results The average operation time,intraoperative electric coagulation hemostasis rate,iatrogenic hiatal incidence and intraoperative hemorrhage rate of group A were lower than those of group B (all P < 0.05).BCVA at 6 months after surgery did not differ significantly between two groups (P > 0.05),but the difference was statistically significant between pre-operation and post-operation (P < 0.05).The incidences of early (≤ 1 month) postoperative VH were 18.2% (4 eyes) and 15.0% (3 eyes) in group A and B,respectively (P > 0.05).The incidences of later (> 1 month) postoperative VH were 27.3% (6 eyes) and 0 in group A and B,respectively,the difference was statistically significant (P <0.05).The percentages of reoperation were 13.6% (3eyes with postoperative VH) and 10.0% (2 eyes with traction retinal detachment) respectively in group A and B.The average times of supplementary laser photocoagulation were (2.3 ± 1.0) times and (1.4 ±0.6) times in group A and B,respectively in follow-up period (P < 0.05).Conclusion The adjunctive use of intraoperative intravitreal injection of conbercept can prevent effectively postoperative VH and decrease conveniently the time of supplementary laser photocoagulation in 23G vitrectomy for PDR,as well as the preoperative adjunctive use can decrease the operation time,intraoperative complications and incidences of early postoperative VH.
5.Diagnostic value of thyroglobulin measurement in fine-needle aspiration biopsy for detecting lymph nodes metastasis in patients with papillary thyroid cancer

Objective To investigate the diagnostic value of thyroglobulin measurement in fine-needle aspiration biopsy(FNA-Tg).Methods This study included 63 patients with thyroid cancer or with enlarged lymph nodes during the follow-up from Aug.2010 to Jun.2012.Ultrasound-guided fine-needle aspiration was done in the 63 patients with 79 suspicious enlarged lymph nodes.Then thyroglobulin(Tg) in FNA washout fluid and serum Tg were assayed with electrochemiluminescenceimmunoassay(ECLIA).Diagnostic sensitivity,specificity and area under the ROC curve were evaluated according to 5 predetermined threshold levels:FNA-Tg/serum-Tg > 1,mean + 2SD of node-negative patients,10 ng/ml,optimal operating point of ROC curve and 1000 ng/ml.Results The sensitivity value was 90.70%,90.70%,95.35%,90.70% and 65.12% respectively on the 5 threshold level,significantly lower than those on the threshold level of 1000 ng/ml(P <0.05).The specificity value was 97.22%,91.67%,72.22%,97.22% and 100% (P >0.05) on those threshold levels,significantly lower than those on the threshold level of 10 ng/ml (P <0.05).The area under the ROC curve was 0.940,0.912,0.838,0.940 and 0.826 respectively.There was no statistical difference(P >0.05).Conclusion Selecting FNA-Tg/serum-Tg > 1 as a positive threshold value can be a secondary diagnosis of lymph nodes metastasis in patients with papillary thyroid cancer.
6.Clinicopathological features of six cases of calcifying aponeurotic fibroma

Purpose To investigate the clinicopathologic features of calcifying aponeurotic fibroma (CAF). Methods The clinical and pathologic profiles of 6 cases of CAF were retrospectively analyzed. Results CAF appeared a firm, painless and slowly growing mass with a predilection for the distal portions of the extremities in young children and adolescents. Microscopically, the tumor consis-ted mainly of benign appearing spindle cells, chondroid cells and calcific debris. Immunohistochemically, the tumor cells were positive for vimentin, HHF-35, S-100, Calponin and CD68. Conclusion CAF is a rare, locally aggressive fibroblastic lesion. It can make an accurate diagnosis by integrating the clinical location with histopathological features. Repeated minor trauma to the extremities and some embryologic cell nests maybe its pathogenesis.

OBJECTIVE:To establish the quality standards of Pseudostellaria heterophylla.METHODS:P.cyclopeptides was identified by thin layer chromatography protosite reaction with ninhydrin reagent.The contents of moisture,total ash,acid-insoluble ash,water-soluble extract and alcohol-soluble extract were detected according to the standards stated in Chinese Pharmacopoeia.The content of pseudostellarin B was determined by HPLC.RESULTS:TLC identification of P.cyclopeptides and HPLC determination of pseudostellarin B were established preliminarily.Limitation of moisture content,ash content,extract and pseudostellarin B were defined.CONCLUSION:The qualitative method and quantitative method and physicochemical index can be used as standards for the quality control of P.heterophylla.
8.Clinicopathologic features of peripheral neuroblastic tumors.

OBJECTIVETo study the clinicopathologic characteristics of peripheral neuroblastic tumors and to evaluate the prognostic significance of these features.
METHODSThe clinical and pathologic findings were retrospectively reviewed in 121 cases of peripheral neuroblastic tumor. The clinical outcomes of patients were evaluated. The three-year event-free survival rate was analyzed, with respect to age of patients, Evan's staging, International Neuroblastoma Pathology Classification and mitosis-karyorrhexis index.
RESULTSThe median age at diagnosis was 2.7 years; and 96 cases (79.3%) occurred in patients younger than 5 years old. The number of cases in Evan's staging I, II, III, IV and IVs was 24, 39, 24, 29 and 5, respectively. There were 82 cases of neuroblastoma (NB) (including 2 cases of undifferentiated NB, 52 cases of poorly differentiated NB and 28 cases of differentiating NB), 9 cases of ganglioneuroblastoma, intermixed type (GNBi), 19 cases of ganglioneuroma, maturing type (GN) and 11 cases of ganglioneuroblastoma, nodular type (GNBn). Forty-nine cases were in the favorable histology subgroup and 72 cases in the unfavorable histology subgroup. The overall three-year event-free survival rate of the 121 cases was 73.0% ± 4.3%. The three-year event-free survival rates were associated with age (P = 0.002), Evan's staging (P = 0.000), histologic category (P = 0.000), mitosis-karyorrhexis index (P = 0.043), prognostic subgroup (P = 0.000).
CONCLUSIONSMost of the peripheral neuroblastic tumors occur in the children younger than 5 years old. It is composed of NB, GNBi, GN and GNBn. The three-year event-free survival rate is approximately 70%. Significant prognostic parameters include age of patients, Evan's staging, International Neuroblastoma Pathology Classification and mitosis-karyorrhexis index.

10.Dual roles of Nrf2 in tumorigenesis

Activation of the transcription factor NF-E2-related factor 2 (Nrf2)-signaling pathway is an adaptive response to environmental and endogenous stresses and serves to render animals resistant to chemical carcinogenesis and other forms of toxicity. However, overexpression of Nrf2 has been recognized to promote oncogenesis and resistance to chemotherapy. The application of Nrf2 in tumor chemoprevention and treatment has also become a hot topic. This review focuses on the dual roles of Nrf2 in tumorigenesis and the application of Nrf2 in tumor chemoprevention and treatment.
