1.Study on the Effects of Total Cost Prospective Payment System on the Consumption of Drugs

OBJECTIVE:To investigate the consumption of drugs after the inplementation of total cost prospective payment sys-tem(PPS),and to provide reference for hospital drug cost control. METHODS:The data of prescriptions collected from 9 hospital during 2007-2014 were divided into group A,B and C according to PPS,and then summarized statistically in respects of increase rate of total consumption sum,consumption sum ratio of major category;the drug cost per time of outpatient department,emergen-cy department and inpatient department were calculated as well as consumption sum ratio of self-paid drugs;the dosage per time of drugs in blood pressure and diabetes prescription were also calculated. RESULTS:The total consumption sum of drug increased slowly after the implementation of total cost PPS;the consumption sum ratio of major category kept stable,while that of anti-infec-tive agent decreased;the emergency drug cost per time achieved a negative growth,and outpatient and inpatient drug cost per time increased slightly. The proportion of self-paid drugs was relatively stable. The dosage per time of drugs in hypertension and diabetes prescriptions was stable,too. CONCLUSIONS:Total cost PPS is useful in controlling the rapid growth of drug costs,and promote the reasonable drug use. The consumption sum of self-paid drugs are well controlled. The increase of drug cost per time in outpa-tient and inpatient should arouse the attention of the relevant departments. In addition,it has no effect on drug dosage for the pa-tients with hypertension and diabetes.
2.Surface Electromyography of Lower Limbs during Activities of Daily Living for Normal Adults

Objective To survey the characteristics of surface electromyography (sEMG) of lower extremity during the activities of daily living (ADL). Methods 30 normal adults performed 7 basic activities, while their average electromyography (AEMG) of sEMG of rectus femoris, vastus medialis, vastus lateralis, gastrocnemius, anterior tibialis, and soleus was determined. Results and Conclusion The AEMG of lower limbs during the ADL was different in left or right, male or female, and different muscles.
3.Analysis of microbial diversity in the midgut of Haemaphysalis longicornis infected with severe fever with thrombocytopenia syndrome virus

Abstract: Objective To investigate the composition and diversity of midgut microbial community of Haemaphysalis longicornis infected with severe fever with thrombocytopenia syndrome virus (SFTSV). Methods The midgut DNA of three group Haemaphysalis longicornis infected with SFTSV was extracted, and the 16S rDNA gene of the sample was sequenced by HiSeq platform. The composition and diversity of endosymbiotic microbial community were clarified by OTU cluster analysis and alpha diversity analysis. Results The midgut microbial clusters of the three groups infected with SFTSV were 143, 113, 163 OTUs respectively; the sparsity curve and abundance grade curve showed that the data had sufficient sequencing depth, and the midgut of Haemaphysalis longicornis infected with SFTSV was rich in microbial composition, but the species distribution was uneven. The analysis of microbial community composition showed that Proteobacteria, Firmicutes and Actinobacteria were the main dominant bacteria at the phyla level. At the class level, Gammaproteobacteria, Bacilli, Betaproteobacteria and Actinomycetia were the main dominant bacteria. At the order level, Legionellales, Bacillales, Burkholderiales and Actinomycetales were the main dominant orders. At the family level, Coxiellaceae, Bacillaceae, Moraxellaceae and Rhodococcaceae were the main dominant families. At the genus level, the relative abundance of Coxiella was the highest, followed by Aeribaillus and Azonexus. Alpha diversity analysis showed that the average Shannon index was 139.67, the average Simpson index was 0.48, the average Chao index was 145.06, and the average ACE index was 147.11. Conclusions The species diversity of intestinal microorganisms in Haemaphysalis longicornis infected with SFTSV is rich. The results provide a basis for further exploring the interaction between intestinal microbes of Haemaphysalis longicornis and SFTSV and developing new ideas for the prevention and control of ticks and tick-borne diseases.
4.Effects of Compound Catechu Anti-Diarrhea Ointment on Expression of Aquaporin3 in Colonic Tissues of Diarrhea Model Rats

Objective To investigate the effect of compound catechu anti-diarrhea ointment on the expression of aquaporin3 (AQP3) in the colonic tissues of diarrhea rat model, and the mechanism thereof. Methods Forty SD rats were randomly divided into four groups:blank control group, model control group, positive control group and compound catechu anti-diarrhea ointment group. Rats were given 6 g blank matrix cream in model control group, 2 mL suspension of berberine in positive control group and compound catechu anti-diarrhea ointment in compound catechu anti-diarrhea ointment group, two times/d for 7 d. The rat model of diarrhea was established by using senna intragastric administration. The water content of feces was measured. The expression of AQP3 in colonic tissues was detected by immunohistochemistry assay. Results The water contents of feces were significantly higher in model control group (64.09±0.41)%than those of other three groups (F=53.879,P<0.05). There was no significant difference in the water content of feces between compound catechu anti-diar-rhea ointment group (48.83 ± 1.08)%and positive control group (46.87 ± 2.19)%. The AQP3-positive cells were mainly ex-pressed in the intestinal mucosa. The dyeing index was significantly lower in model control group (0.85±0.18) than that of other three groups (F=14.971,P<0.05). There was no significant difference in the dyeing index between compound catechu anti-diarrhea ointment group (1.30±0.18) and positive control group (1.37±0.14). Conclusion Compound catechu anti-di-arrhea ointment can significantly reduce the water contents of feces, which may be related to the increased AQP 3 expression in colonic tissues.
5.Effect of platelet-rich plasma on wnt3 gene and klotho gene of adipose-derived stem cells of rabbit

Objective To investigate the effect of platelet-rich plasma (PRP) on starting of wnt3 gene and klotho gene of adipose-derived stem cells (ADSCs) of rabbit.Methods Epididymal adipose tissue stem cells were obtained from New Zealand white rabbits,and the cells identified by morphology and inducing differentiation,and the cells were cultured to the fourth generation,PRP and PPP (platelet-poor plasma) were prepared by traditional centrifugal method from abdominal aortic of rabbit; ADSCs were cultured in culture medium containing PRP (experimental group),PPP (control group) and all medium (blank group) for each 5% for 24h,48h and 72h.Cells of each group were dissociated and total RNA extracted.Effects of the starting of wnt3 gene and klotho gene were detected by RT-PCR.Results Primary ADSCs of rabbit grew in the way of long spindle swirly.The results of oil red O and alizarin red staining of the ADSCs were positive.Expression of wnt3 gene and klotho gene in the experimental group significantly increased from the results of RT-PCR (P<0.05).Conclusions PRP can promote proliferation of the ADSCs of rabbit and increase the expression of wnt3 gene and klotho gene significantly.
6.Preparation and biocompatibility of panax notoginseng saponins-hydroxyapatite/chitosan scaffold

BACKGROUND:Panax notoginseng saponins promotes bone repair by improving vascular proliferation. Therefore, the scaffolds carrying panax notoginseng saponins were supposed to be used to improve bone repair at the bone defect region. However, the biocompatibility of scaffolds remains unclear.
OBJECTIVE:To evaluate the biocompatibility of panax notoginseng saponins-hydroxyapatite/chitosan scaffolds.
METHODS:A new bone repair scaffold has been generated by thoroughly mixtures of 0, 0.1, 1, 10 mg panax notoginseng saponins and chitosan/hydroxyapatite using in-situ composite technique and freeze-drying technique. Morphology and mechanical property of the scaffold were observed under a scanning electron microscope. (1) Cel proliferation test:rabbit bone marrow mesenchymal stem cel s of passage 3 were cultured in four kinds of drug loaded hydroxyapatite/chitosan composite material leaching liquor. Cel s normal y cultured were considered as controls. 3-(4, 5-Dimethylthiazol-2-yl) 2, 5-diphenyl tetrazolium bromide was used to measure absorbance value of cel s in each group. (2) Hemolysis test:Rabbit anticoagulated blood was added with four kinds of drug loaded hydroxyapatite/chitosan composite material leaching liquor. Absorbance values were measured using a microplate reader in each group. (3) Pyrogen test:The four kinds of drug loaded hydroxyapatite/chitosan composite material leaching liquor and saline were respectively injected into ear vein of rabbits, and the increase of rabbit body temperature was detected.
RESULTS AND CONCLUSION:Drug loaded hydroxyapatite/chitosan composite material contained three dimensional porous structure of 110μm in diameter. Drug loading process of panax notoginseng saponins did not significantly affect the porosity, pore size and density of the composite material, but decreased its breaking strength and elastic modulus. The larger amount of drug loading showed a more obvious effect. Simple hydroxyapatite/chitosan composite material had good cel ular compatibility. The composite material after drug loading obviously suppressed cel proliferation, and the larger amount of drug loading showed a more obvious effect. The composite material had good blood compatibility before and after drug loading. The composite material had good pyrogen effects before and after drug loading, but accorded with acceptable quality level of pyrogen test.
7.The mechanism underlying the analgesic effect of the iontophoresis of aconitine on the adjuvant arthritis in rat

Objective To study the mechanism of analgesic effects of the iontophoresis of aconitine on the rat with adjuvant arthritis. Methods Forty-five Wistar rats were randomly divided into a normal control group, a model group and a therapy group, with 15 rats in each group. The model of rat adjuvant arthritis(AA) was induced by injection of complete Freud′s adjuvant(0.1 ml) into the right ankle joint of the rats. The brain stem, hypothalami and local inflammation tissue of rats in the 3 groups were obtained after 1, 5 and 10 days of treatment, respectively. The concentrations of 5-HT, NE, ?-EP in brain stem, hypothalami and local inflammation tissue were detected with high performance liquid chromatography (HPLC) and radioimmunoassay (RIA). Results It was shown that the 5-HT content in the inflammatory tissues was not significantly different from that in the model group after 1 day of treatment, but significantly lower than that in the model group after 5 to 10 days of treatment (P0.05). 8.Effects of PPAR-α silence on ET-1 induced myocardial hypertrophy and PI3K/Akt/GSK3β-NFATc4 signal pathway

AIM: To investigate the role and signal mechanism of PPAR-α in the pathogenesis of cardiac hypertrophy. METHODS: Small interfering RNA (siRNA) was applied to efficiently silence the gene expression of PPAR-α in cardiac myocytes. [~3H] leucine incorporation assay was performed to measure protein synthesis. Reverse transcription-polymerase chain reaction (RT-PCR) was used to analyze the mRNA level of atrial natriuretic factor (ANF) and PPAR-α. Western blotting analysis was performed to investigate the levels of phosphorylation of protein kinase B (PKB/Akt) and glycogen synthase kinase 3β (GSK3β). Immunofluorescence analysis was used to examine the cellular localization of NFATc4. RESULTS: (1)RSS304168 was the most efficient stealth RNAi duplex to specifically inhibit PPAR-α expression. (2)RSS304168 significantly potentiated the ET-1-induced cardiomyocyte hypertrophy and enhanced ET-1-induced protein synthesis and ANF mRNA expression in cardiomyocytes. Moreover, RSS304168 completely reversed the inhibitory effects of fenofibrate on ET-1-induced protein synthesis and ANF mRNA expression. (3)RSS304168 enhanced ET-1-induced phosphorylation of Akt at Ser473 and GSK3β at Ser9. The effects of ET-1 or ET-1 combined with RSS304168 on phosphorylation of Akt/GSK3β were completely blocked by LY294002, a PI3K specific inhibitor. Fenofibrate markedly inhibited ET-1-induced phosphorylation of Akt/GSK3β while RSS304168 abolished these effects of fenofibrate. (4)Fenofibrate prevented the nuclear translocation of NFATc4 induced by ET-1 while RSS304168 abolished this effect of fenofibrate. CONCLUSION: Activation of PPAR-α inhibits ET-1-induced cardiomyocyte hypertrophy through blocking Akt/GSK3β-NFATc4 signaling pathways.
9.Clinical application of vertebroplasty: a report of 1080 cases

Objective To analyze the efficacy of vertebroplasty using the patient data in our hospital so as to better serve those clinical patients.Methods A retrospectively review was performed on 1 080 with data integrity out of 1 084 cases undergone vertebroplasty from January 2003 to March 2012.Basic information measured was age,gender,entity,number of operated vertebral bodies,distribution of operated vertebral bodies,postoperative X-ray films,bone cement volume,bone cement leakage into the vertebral canal,cases with balloon kyphoplasty,cases with bilateral or bilateral vertebroplasty,and intraoperative vital signs.Clinical manifestations and quality of life were observed after operation.Results In all,306 cases occupying 28.33% presented bone cement leaks based on postoperative X-ray films.Volume of bone cement infused in each vertebral body varied from 1.5 ml to 7.5 ml.No bone cement leaked into the vertebral canal during operation.There were 706 cases occupying 65.37% managed by balloon kyphoplasty and 984 cases occupying 91.11% by unilateral vertebroplasty.Within 3-5 days after surgery,associated symptoms were found in 23 cases,among which 3 had neurologic symptoms and 20 composing 5 men and 15 women had symptoms related to pulmonary embolism including one severely affected was transferred to the department of pneumology.Among the 23 cases,8 were managed by unilateral vertebroplasty and 15 by bilateral vertebroplasty,including 17 involving thoracic surgery.Conclusion Although various complications relate to the procedure,vertebroplasty may be widely used on condition that techniques and indications iu surgery are correctly mastered.
10.Analysis of the characteristics of minimal fat renal angiomyolipoma and clear cell renal carcinoma in high resolution multi-slice spiral CT

Objective To investigate the characteristics of minimal fat renal angiomyolipoma (MFAML)and clear cell renal cell carcinoma(CCRCC)in high resolution multi-slice spiral CT(MSCT)and to improve the diagnosis accuracy for the renal tumors.Methods A retrospective analysis was performed on 24 MFAML patients(16 females,8 males)with mean age of 43(19-74)years and 24 CCRCC patients(16 females,8 males)with mean age of 44(21-76)years.All patients had undergone MSCT and proved histopathologically after surgery.The characteristics included tumor location,tumor attenuation on unenhanced CT,enhancement characteristics(degree of tumor enhancement in the early corticomedullary phase,homogeneity of enhancement,amount of enhancement,enhancement pattern over time),tumor margin,intratumoral calcification,and perinephric changes.The predictive value of each CT characteristic was determined by using multivariate logistic regression analysis.Results The tumor location in the kidney (upper pole:MFAML,6 cases,CCRCC,6 cases;middle:MFAML,7 cases,CCRCC,9 cases;lower pole:MFAML,11 cases,CCRCC,9 cases)and smooth tumor margin(MFAML,n=21;CCRCC,n=19)were not significantly different between MFAML patients and those with CCRCC,P>0.05.Twenty-one cases of both MFAMLs and CCRCCs had the significant enhancement in the early corticomedullary phase,which were hypovascular tumors,whereas the mean amount of tumor enhancement was greater in CCRCC than in MFAML in both the early corticomedullary and the corticomedullary phases(CCRCC:175 HU,196 HU;MFAML:125 HU,145 HU;P<0.05.MFAML usually showed homogeneous enhancement(n=15)rather than heterogeneous enhancement(n =9),whereas most CCRCC showed heterogeneous enhancement(n =17)rather than homogeneous enhancement(n =7),P<0.05).Enhancement pattern was not a significant predictor.Within the 13 MFAML cases,8 cases had sufficient blood supply(6 cases showed obvious wash-in-and-wash-out,2 cases were with prolonged enhancement),5 cases with hypovascular showed a pattern of prolonged or gradual enhancement,while 21 CCRCC cases had sufficient blood supply and 71% of them showed obvious wash-in-and-wash-out.High tumor attenuation on unenhanced scans(MFAML:17 patients (75%);CCRCC:2 patients(8%),P=0.002,OR=0.010)and threshold enhancement values of 129.5 HU in the corticomedullary phase(MFAML:5 patients(20%);CCRCC:20 patients(83%),P =0.004,OR =0.057)were valuable predictors for differentiating MFAML from CCRCC at multivariate logistic regression analysis.Conclusions MSCT is useful in differentiating MFAML from CCRCC,with high tumor attenuation on unenhanced scans and threshold enhancement values of 129.5 HU in the corticomedullary phase being the most valuable CT findings.75% of MFAMLs with sufficient blood supply also show a pattern of wash-in-and-wash-out,which can easily misdiagnosed as a renal cancer.
