3.Advances in Study on Effects of Helicobacter pylori Eradication Therapy on Intestinal Microecology
Chinese Journal of Gastroenterology 2017;22(4):241-244
It is documented that nearly half of the global population have been infected with Helicobacter pylori (Hp).In Chinese consensus report on management of Hp infection, bismuth quadruple therapy composed of proton pump inhibitor, bismuth and two antibiotics is recommended as the first-line Hp eradication regimen. However, large doses of oral antibiotics and PPI often disrupt the well-balanced host-microbial symbiotic state, and impair the protective effect of biological barrier.Furthermore, the alteration of microbial composition secondary to eradication therapy might be associated with the increased adverse effects of Hp eradication therapy and re-infection.This paper summarized the advances in study on effects of Hp eradication therapy on intestinal microecology and the advantages of probiotics supplementation in Hp eradication therapy.

Objective Imaging and clinical manifestations of melioidosis were analyzed in order to improve our understanding of the disease and to reduce the rate of misdiagnosis. Methods From 2001 to 2006,12 melioidosis cases were confirmed by blood, pus and sputum culture. All cases were examined with radiography, and nine of them with CT and 2 with US.The imaging and clinical data were assessed retrospectively. Results Ten of 12 cases revealed lung abnormalities, the main organ involved by melioidosis in the body. The appearances were observed as follows:diffused sheets and sheets fused partly in bilateral fields of the lungs can be seen on chest radiograph;homogeneous dense shadow of the lobe or segment were observed on plain chest film and CT;blurred strips radiated from hilum were revealed on plain chest film;multitude nodules in 2 lungs showed on CT;cavity with air and liquid be seen on plain chest film and CT;patches and granules in superior and middle fields or(and)inferior fields be seen on plain chest film and CT;arc shadow of water attenuation in dorsal thorax showed on CT.Infections outside the lung can be observed in orbital, lumbar muscle, liver and spleen in 1 case, and thighbone osteomyelitis with multi abscesses in one case. Conclusion Alhough medioidosis has no characteristic imaging appearances, the disease's location, extent, severity and quantity objectively can be demonstrated by imaging, final confirmation is necessary using the culture of blood, pus and sputum.

Purpose:To investigate the therapeutic effect a nd toxicity of radiotherapy plus ultrasound hyperthermia for treatment of malign ant tumors. Methods:41 cases of locally advanced, recurrent or radioresista nt cancer treated from January 1997 to January 2000 were given radiotherapy plus Sonotherm 1000 ultrasound hyperthermia. The mean radiation dose was 61?12 Gy;h yperthermia was administered within 30 minutes following radiotherapy with mean heating time of (52.4?6) minutes at 1-2 times/week. 3-year local control and survival were estimated by Kaplan-Meier method and Log rank test, factors affec ting prognosis were analyzed by Cox regression. The significance of differences between temperatures was tested by one-way ANOVA. Results:There was significant difference between Tmin and Tmax on tumor local response. Tumor complete response was 58.5%, the 3-year local co ntrol and survival were 13.6%% and 32.69% respectively. There is significant cor relation between radiation dose, tumor volume, Tmin and 3-year local control ( P

Objective To evaluate the sensitivity and specificity of a nested polymersase chain reaction (nested PCR) for detection of the Mycobacterium tuberculosis IS6110 in paraffin embedded tissues. Methods 31 samples from tuberculosis patients and 5 biopsy specimens from patients with hepatitis were subjected to detection of Mycobacterium tuberculosis IS6110 by nested PCR. PCR products of two randomly selected samples were cloned and sequenced. Results Mycobacterium tuberculosis IS6110 was positive in 28 of 31 samples of tuberculosis. The sensitivity and specificity of nested PCR for detection of IS6110 were 90.3% and 100%, respectively. The predictive value of nested PCR was 100%. The sequences of two samples were compared with known sequence of H37Rv isolate (reported by Thierry D) and with nucleotide homology of 97% and 95.3%, respectively. Conclusions Nested PCR is sensitive and specific in the diagnosis of Mycobacterium tuberculosis infection in tissues of routinely paraffin embedded. We propose the diagnostic application of nested PCR for the identification of Mycobacterium tuberculosis infection, especially in the cases which can not be distinguished with certainty from other diseases by histopathology and Ziehl Neelsen's staining.
