1.Laser in situ keratomileusis (LASIK) for high myopia

EXCIMER laser vision correction in the form of LASIK and PRK/LASEK has been proved to be highly effective and safe in the treatment of low-to moderate myopia (less than -6 diopters [D]) and astigmatism. It is the most common refractive-surgery procedure done worldwide. However, the outcome of laser vision correction among those with high myopia (greater than -6D) may not be the same as in those with low to moderate myopia CLINICAL SCENARIO: A 26-year-old, female, myopic patient unhappy with spectacle correction and contact lenses heard about LASIK and sought opinion regarding the probability of her achieving 20/20 vision. Her last refraction was -7.00 sphere -1.00 cylinder x 100 in the right eye (OD) and -9.00 sphere -0.50 cylinder x 90 in the left eye (OS). Best-corrected visual acuity (BCVA) was 20/20 OD and 20/20 OS. Her refraction has been stable for 5 years CLINICAL QUESTION: The patient has high myopia and is concerned about her chances of seeing 20/20 after undergoing laser vision correction. Among patients with high myopia, how effective is LASIK in achieving 20/20 vision? SEARCH METHOD: An electronic literature search was performed using MEDLINE (PubMed). The following search terms were used: "Myopia," "LASIK," "laser in situ keratomileusis," "technology assessment." The search was further limited to the English language and human studies published from 1968 to April 2005. 2.Primarily evaluation of efficacy of the laser surgery CO2 in combination with injection of triamcinolone after for treatment of keloid and hypertropic scars

An evaluation of combination of scar removal surgery by laser CO2 with steroid injection triamcinolone was carried out in 10 patients with the keloid and hypertrophy scars according to the criteria of Cosman 1996, Peacok 1970 and Cohen 1976. The results have shown that the combination was a simple method with easy implementation and convenience for patients. There were no complications or accidents during treatment period.

3.Laser application for plastic surgery

LASER is an abbreviation of English sentence: "Light Amplification by Stimulated Emission of Radiation". The first Ruby laser was been constructed successfully in July 1960 by American physicist Theodore Maiman. Just after this event laser has been applied in medicine (1962) initially in ophthalmology and dermatology. Up to now many kinds of laser have been constructed with technological excellence to meet requirements of physiotherapy and surgery. In plastic surgery, the laser application is slowed respect to in the other surgical branches. In this paper the author try to describe the interaction mechanism of laser radiation on the living creatures as well as the popular laser types used in treatment. In Vietnam, high power lasers have been applied since 1992, mostly CO¬2 lasers. Some Nd-YAG and argon lasers are used in several hospitals and clinics. In Center for Bio-medical Physics CO¬¬2¬¬ and Nd: YAG lasers have been used in general and plastic surgery since 1992. 4182 cases were treated mainly for aesthetic purposes, particularly: epidermal and subdermal vascular lesions, dermal pigment lesions, dermal tumors. ¬

4.Laser CO2 in the surgery of the facio- maxillary pathology.

Laser surgery and medicine were considered as one of the topic of mordern medical technologies and has been widely applied in the various medical fields. We have been using CO2 laser in maxillofacial surgery for 506 patients at Khanh Hoa provincial general hospital, since Feb 1994. Result: good: 93.08%, medium: 5.33%. Poor: 1.58%.

The aticle briefly introduced the working principle and clinical applicability of Coblation. The application of Coblation promoted the improvement of traditional surgery and the generation of new operation, it is the most important to hold the indication of the operation strictly. This review summarized that and discussed the advantages and the notes of Coblation in operations of the pharynx and larynx.

6.Haemorrhoidectomy by laser CO2

A retrospective study was carried out on 2509 patients with heamorrhoid in which heamorrhoid grade 2, 3 and 4 were 26.8%; 67.5% and 5.7%, respectively. The rate of patients with the circumferential haemorrhoid was 23.2%. All patients received haemorrhoidectomy by laser CO2 and monitored within 1-6 years by direct consultation and survey paper. The results have shown that the technique had a little intraoperative bleeding, little complication, complete outcome and less relapese. The study concluded that the haemorrhoidectomy by laser CO2 was effective and safe method.

7.Electric and laser energy for endoscopic surg.

Improvements in surgery have focused on refinements in the ability to dissect and limit blood loss. The electrosurgery unit (ESU) and laser (light amplification by stimulated emission of radiation) have been widely used in recent endoscopic surgery. ESU is a form of surgery in which high-frequency (100 kHz-5 MHz) electric currents are used to incise, destroy, and remove tissue and to seal blood vessels in order to maintain hemostasis. The use of lasers in surgery has added a new dimension to operative technique through increased precision, improved hemostasis, and less tissue manipulation. This paper aims to explain the basic principles and applications of electrosurgery and lasers.
