1.Effect of three therapies on shoulder pains after gynecological laparoscopies

Objective To study the effects of three therapies on shoulder pains after gynecological laparoscopies.Methods After the gynecological laparoscopies,180 patients were randomly assigned to hot compress group(n=57),oxygen therapy group (n=65)and routine care group as control group(n=58).All the patients in the three groups were given oxygen inhalation for three hours.Besides,those controls were given mental care,comfort support and massage,while those in the two experiment groups were managed with hot compress on the shoulder and oxygen inhalation via nasal tube at 2 L/min each for 2 hours at 8 am and 4 pm on the first day.The three groups were compared in terms of shoulder pains,at different time points on the first day(at 8 am,4 pm and 8 pm). Results After intervention,there were significant differences between the three groups in terms of shoulder pains(P<0?05).In both experiment groups,the scores on the shoulder pains after intervention were significantly lower than those before intervention(P<0?001). Conclusions Hot compress and oxygen treatment are both effective in relief of the shoulder pain after gynecological laparoscopy. Therefore,the therapy can be decided on economic conditions and postoperative rehabilitations.
2.The effect of insulin aspart on postprandial glucose and the excursion of serum glucose level in type 2 diabetic patients managed with delivered in insulin pump therapy

Objective To compare the efficacy of insulin aspart and human soluble insulin on postprandial glucose control and blood glucose excursion in type 2 diabetic patients managed with insulin pump therapy. Methods All of 345 hospitalized type 2 diabetic patients were randomized divided into two groups. One group underwent insulin pump therapy with insulin aspart (aspart group, 173 cases),another group with human soluble insulin (humulin R group, 172 cases). Capillary glucose concentrations were measured at 9 time points,including preprandial,2 hours postprandial,bedtime (22:00),midnight(0:00) and 3:00 every day during the treatment. The change of blood glucose at each time point and the variation of postprandial blood glucose excursion was compared between the two groups. The frequency of hypoglycemia was also evaluated. Results After treatment, fasting blood glucose and post breakfast and post dinner blood glucose levels were decreased more significantly in the aspart group than those in the humulin R group. And a significantly smaller postprandial blood glucose excursion was shown in the aspart group compared with that in the humulin R group (P< 0.05). The time to achieve good glycemic control in the aspart group was (4.40 ± 2.16) d, significantly shorter than that in the humulin R group[(5.68 ± 2.29) d](P< 0.05). The incidence of hypoglycemia was significantly lower in the aspart group (P <0.05). Conclusion Insulin aspart results in better control of blood glucose and less glycemic variability compare with human soluble insulin in type 2 diabetic patients during delivery by continuous subcutaneous insulin infusion.
3. The correlation between lung ultrasound score and neonatal critical illness score and its clinical predictive value

Objective:
To explore whether the lung ultrasound(LUS) score can be used to assess and predict the criticality of neonates with pulmonary disease at an early stage.
Methods:
The newborns born in the obstetrics department of Affiliated Hospital of Jining Medical University from April to October 2018 were transferred to the neonatal intensive care unit due to respiratory distress. The children underwent LUS examination and scoring at 2 hours after birth. The correlation analysis were performed between LUS score and neonatal critical illness score (NCIS ), NCIS+ single index, respectively. And the ROC curve was used to analyze the value of LUS score in predicting neonatal criticality.

4.Establishment and management of endoscopic ̄guided enteral nutrition pathway

Nutritional therapy is very important for critically ill children. Most children need to put nasogastric tube for enteral nutrition. Severe esophageal stricture, gastric compression, gastric retention and gastric outlet obstruction may require endoscopic catheterization; dysphagia caused by various reasons, or children who need long ̄term catheterization for enteral nutrition,need percutaneous endoscopic gastrointesti ̄nal stoma catheterization. Gastroscopy ̄guided intubation is divided into nasal intubation of gastric tube,nasal intubation of jejunum tube and nasal intubation of three ̄chamber gastrointestinal tube. Percutaneous endoscopic gastrointestinal stoma catheterization can be performed through gastrostomy,jejunostomy and duodenostomy. Choosing suitable catheterization method can help all kinds of critically ill children to establish enteral nutri ̄tion pathway as soon as possible and promote recovery of illness.
5.Construction and identification of the replication-deficient recombinant vaccinia virus co-expressing HPV type 16 L1 and L2 proteins.

OBJECTIVETo generate an HPV16 prophylactic vaccine candidate for cervical cancer.
METHODSHPV16 major capsid protein L1 gene and minor capsid protein L2 gene were amplified using PCR. These genes were mutated by PCR site-directed mutagenesis for removal of sequence motifs (TTTTTNT) which would cause transcription termination when expressed from a vaccinia virus early promoter, then inserted into a vaccinia virus expression vector. A strain replication-deficient recombinant vaccinia virus containing the mutant sequences was obtained through a homologous recombination and identified.
RESULTSThe nucleotide sequence remained the correct amino acid sequence of the L1 and L2 proteins after mutated. Full-length L1 and L2 proteins were generated in cells infected with the recombinant virus. The virus strain propagated at very low titer or could not reproduce in some kinds of cell derived from different human tissues.
CONCLUSIONSThe authors have generated a strain replication-deficient recombinant vaccinia virus expressing HPV16 L1 plus L2 proteins as an HPV16 prophylactic vaccine candidate for cervical cancer.

T he correlation coefficient between the LUS score and the NCIS + single index was -0 .44 ( P=0 .005) . ③T he area under the ROC curve of LUS score predicting neonatal criticality was 0 .88 ( 95%CI :0 .725-0 .965 , P <0 .000 1) ,the optimal diagnostic threshold was 6 points with sensitivity of 80% and specificity of 100% . Conclusions The LUS score at a postnatal age of 2 hours after birth can early assess and predict the criticality of neonates with pulmonary disease . And the LUS score greater than 6 has the highest diagnostic value .
7.Rapid health technology assessment of dienogest in the treatment of endometriosis

AIM:To rapidly evaluate the effective-ness,safety and economy of dienogest in the treat-ment of endometriosis so as to provide evidence-based reference for clinical drug selection and deci-sion.METHODS:Retrieved from PubMed,Embase,Cochrane Library,CNKI,SinoMed,Wanfang and health technology assessment(HTA)organization websites,HTA report,systematic review/Meta-anal-ysis and pharmacoeconomic study were included during the inception to Sep 2023.Data extraction and quality evaluation were carried out for the lit-erature that met the inclusion and exclusion crite-ria,and the research results were summarized and analyzed qualitatively.RESULTS:Nine systematic re-view/Meta-analyses,and 2 pharmacoeconomic studies were included.The quality of Meta-analysis literatures was low,and the quality of economic re-search was good.In terms of effectiveness,com-pared with no treatment,dienogest can significant-ly reduce postoperative recurrence rate,VAS score,and improve pregnancy rate and effective rate;Di-enogest has similar therapeutic effects in terms of recurrence rate as GnRHa and COC;With regard to recurrence rate and pregnancy rate,dienogest is superior to danazol,gestrinone,and mifepristone.In terms of safety,compared with placebo,the inci-dence of vaginal bleeding and headache was signifi-cantly increased with dienogest,and there was no difference in bone loss;The incidence of vaginal bleeding caused by dienogest was significantly high-er than that of GnRHa,but the incidence of hot flashes and bone loss was lower.In terms of econo-my,dienogest has a more cost-effective advantage comparing with GnRHa,but does not have econom-ic benefits comparing with COC.CONCLUSION:Di-enogest has good effectiveness and safety in the treatment of endometriosis,and economically su-perior to GnRHa,but inferior to COC.
8.Effects of personalized nutritional intervention on the nutritional risks and quality of life for patients with prostatic neoplasms and endocrine therapy

Objective To investigate the effect of personalized nutritional intervention on the nutritional risks and quality of life for patients with prostatic neoplasms and endocrine therapy.Methods Totally 136 patients with prostatic neoplasms who were admitted and treated in the day care ward of Ningbo First Hospital between August and October 2016 were selected, and equally and randomly divided into an observation group and a control group. Patients in the control group received conventional diet nursing, while patients in the observation group were screened for nutritional risks, and received step-by-step and personalized nutritional intervention and followed up by nurses based on the nutritional protocols made by both nurses and nutritionists. Patients in the two groups were then compared in the incidence of nutritional risks, laboratory nutrition indicators and life quality at admission and 8 weeks later.Results There was no statistically significant differences between the incidences of nutritional risks before and after the intervention (χ2=0.052,1.952;P>0.05), the observation group's prealbumin (PA), serum albumin (ALB) and hemoglobin (Hb) were (29.01± 6.37)g/L, (1.20±3.41)g/L and (12.36±1.78)g/dL, respectively, higher than those of the control group (P<0.05); and there were statistically significant differences between the patients in the two groups in their overall health condition, somatic function, role function, cognitive function, emotional function, fatigue,ache, diarrhea and financial difficulty (P<0.05).Conclusions Personalized nutritional intervention helps to improve the nutritional status and life quality of patients with prostatic neoplasms and endocrine therapy.
9.Efficacy of percutaneous endoscopic gastrostomy in pediatric patients.

OBJECTIVETo analyze the efficacy of percutaneous endoscopic gastrostomy (PEG) in pediatric patients.
METHODFrom October 2011 to October 2014, children in the gastrointestinal ward of Guangzhou Women and Children's Medical Center received PEG or jejunal tube PEG(JET-PEG). The success rate, operation time were recorded. The changes of their weight, enteral nutrition calories and the incidence of pneumonia before and after the first 6 months of operation were compared. Follow-up was conducted until October 2014, the recent and long term complications, the length of indwelling time, the replacement or removal of the tube were recorded, the patients swallowing function or the primary disease's outcomes were observed.
RESULTOf the 13 cases, 10 were male, 3 were female, their average age was 2 years (range 1.8 months-9 years). We performed PEG for 12 of the patients who had congenital craniofacial problems that led to feeding difficulties or recurrent cough and pneumonia (6/12), or neurological disorders (6/12) with inability to swallow, and in one case JET-PEG was performed, this child suffered from chronic intestinal pseudo-obstruction with vomiting and abdominal distension. The gastrostomy was successful in all the patients through one operation, the average operation time of PEG was (25 ± 3) minutes, JET-PEG was 60 minutes. One local skin infection was noted, no long-term complication occurred. In the first 6 months after operation, all the patients gained weight((5.5-30.5) kg postoperation vs. (3.0-30.0) kg preoperation), and 12 cases' enteral nutrition calories increased (from (209-502) to(272-543) kJ/(kg·d)), the incidence of pneumonia decreased in the children who had recurrent pneumonia before the operation (from (0-1.5) to (0-0.16) per month). Until October 2014, their average length of gastric tube indwelling time was 17.8 months (range 4-36 months). In 4 cases PEG tube was removed when they could eat completely independently, the other 9 needed enteral vein nutrition via PEG tube or jejunal tube, in 3 of them balloon type gastric fistula tube was applied. Two of the 13 cases who had cleft palate received stomatological operations when their weight grew to meet the standard.
CONCLUSIONPEG and JET-PEG are safe and effective method for enteric nutrition feeding in pediatrics, the technique causes minimal trauma and has rapid postoperative recovery, few complications, good aesthetic appearances and simple nursing, it can significantly improve their nutritional status and quality of life.

10.Safety risk assessment of in vitro heart in antitumor drug development

BACKGROUND:Tyrosine kinase inhibitors,as well as other types of small-molecule cancer drugs,can cause severe cardiotoxicity. OBJECTIVE:To perform a heart safety re-evaluation by observing the effects of antitumor drugs on isolated heart electrocardiograph,cardiac action potential and associated ion channels and cytotoxicity. METHODS:Extracorporeal cardiac perfusion was given to the isolated rabbit heart using Langendorff perfusion:Sunitinib(0.3,3,10 μmol/L),Crizotinib(0.3,1,3 μmol/L),and Doxorubicin(1,30 μmol/L)were perfused sequentially for 120 minutes to record electrocardiograph and left ventricular pressure.A blank control group was set for comparison.Manual patch clamp was used to record the effects of Crizotinib,Sunitinib,Doxorubicin on hERG,Cav1.2,Nav1.5 channel currents and action potential in human induced pluripotent stem cell derived cardiomyocytes.Adenosine triphosphate level in human induced pluripotent stem cell derived cardiomyocytes was detected by CellTiter-Glo luminescent cell viability assay. RESULTS AND CONCLUSION:Isolated rabbit heart using Langendorff perfusion:Compared with the blank ontrol group,Sunitinib and Crizotinib at≥3 μmol/L decreased heart rate(P<0.01)and prolonged QT/QTc interval(P<0.01),and reduced left ventricular pressure to different extents.Manual patch clamp recording:Compared with the blank control group,Sunitinib and Crizotinib at 3 μmol/L inhibited the activities of hERG,Nav1.5 and Cav1.2 channels and significantly prolonged the duration of action potential(P<0.01).According to the analysis of the test article,the difference between the labeled concentration and the measured concentration of the recovered solution was not significant.Cell viability assays:Compared with the blank control group,adenosine triphosphate content in human induced pluripotent stem cell derived cardiomyocytes significantly decreased after treatment with Sunitinib(IC50=4.64 μmol/L),Doxorubicin(IC50=4.21 μmol/L)and Crizotinib(IC50=2.87 μmol/L),indicating that cell viability significantly decreased(P<0.01).To conclude,this study successfully established an early cardiac safety evaluation method for antitumor drugs,which provides good support and help for the subsequent development of antitumor drugs.
