OBJECTIVE: To prepare indapamide sustained release capsules and investigate its release in vitro. METHODS: Blank pellets, hydrated magnesium silicate, hypromellose cellulose, triethyl citrate, Eudragit RL 100, Eudragit RS 100, Eudragit LS 55, etc, were used as materials to prepare indapamide sustained release capsules The content was determined by HPLC, and the effects of different solid weight gains of sustained release material on release of indapamide from capsules were evaluated by in vitro release test. RESULTS: The release in vitro of the sustained release pills met the requirements when the solid weight gain of the materials was 5.0% to 5.3%. CONCLUSION: The optimum preparation technique of indapamide sustained release capsules is established, which has guiding significance for the practical manufacture.

Objective To investigate the relationship of hypothyroxinemia to gestational diabetes mellitus during early pregnancy. Methods A total of 11 365 cases of women with early singleton pregnancies were collected from Chongqing Health Center for Women and Children. The screening of thyroid function was performed. The postload glucose concentrations and the risks of gestational diabetes mellitus in hypothyroxinemia were investigated. The relationship of thyroid hormones to postload glucose concentration was evaluated. The association between thyroid peroxidase antibodies(TPOAb) and gestational diabetes mellitus was analyzed. Results Early pregnancy women with hypothyroxinemia had a higher postload glucose concentration. Comparing to normal pregnancy women, the prevalence of gestational diabetes mellitus in hypothyroxinemia was higher(42.2%vs18.4%, P<0.05). Free thyroxine(FT4) was significantly negatively correlated with postload glucose concentration. FT4 was associated with gestational diabetes mellitus. The risk of gestational diabetes mellitus was greater when FT4 was lower. The prevalence of gestational diabetes mellitus in early pregnancy women with TPOAb levels ≥500 IU/ml was significantly higher than that in normal pregnancy women (31.2%vs 18.4%, P<0.05). Conclusion Early pregnancy women with hypothyroxinemia and TPOAb≥500 IU/ml were related to the risk of gestational diabetes mellitus. Lower FT4 was the risk factor of gestational diabetes mellitus.

Objective To investigate the features of the mixed epithelial and stromal tumor of kidney (MESTK) and cystic nephroma (CN) on multi-detector CT (MDCT), in order to improve the accuracy of preoperative diagnosis and differential diagnosis. Methods Based on blind-retrospective review, CT features of 6 MESTK patients and 6 CN patients were reviewed retrospectively, and compared with pathological results postoperatively. Results All the lesions were single regular multi-cystic masses. Five patients with MESTK were diagnosed as Bosniak Ⅲ and 1 as Bosniak Ⅳ. In contrast, 6 of CN were all diagnosed as Bosniak Ⅱ. Six MESTK lesions were all consisted of irregular mixture of solid and cystic components, and the average thickness of septa was 0.93 cm. Six CN lesions were entirely cystic masses without solid components, and the average thickness of septa was 0.23 cm. After contrast enhancement, the solid components of MESTK and the cystic wall of CN presented mild-to-moderate delayed enhancement. Conclusion Some signs on multi-detector CT can help to differentiate MESTK and CN, especially the presence of solid components of the tumor.

Ricin is a plant-derived ribosome-inhibitor which can be easily purified in large quantities from castor beans. It is a potent irreversible inhibitor of protein synthesis. The mode of intoxication could be inhalation, ingestion, intravenous injection.Ricin has been classified as a schedule 1 threat agent by the Chemical Weapons Convention.A fast and sensitive method for the detection of this threat agent is an important tool for preventing or dealing with the consequences of intoxication. An ideal method should be highly sensitve, highly selective, and well capable of identifying ricin in a short assay time. Several methods have been established for ricin detection. This review summaries the development of detection methods for ricin in recent years.

Objective To reclassify the patients with former diagnosis of vulvar intraepithelial neoplasia (VIN)Ⅱ, VIN Ⅱ - Ⅲ, VIN Ⅲ, Bowen disease and Bowenoid papulosis according to the 2004 modified terminology of International Society for the Study of Vulvar Disease (ISSVD) and evaluate the effects of surgical treatment. Methods According to the 2004 modified terminology of ISSVD, a retrospective review of the histological slides of 19 cases, who diagnosed as VIN Ⅱ , VIN Ⅱ - Ⅲ, VIN Ⅲ,Bowen disease and Bowenoid papulosis treated in Peking Union Medical College Hospital from 1999 to 2006,were performed to reclassify by pathologist and the clinicopathologic data were also analyze& Results According to the 2004 modified terminology of ISSVD, all 19 cases were reclassified as usual type VIN and were belonged to the subtype of warty type except one, the only Bowen disease patient whose one of focuses belonged to the subtype of basaloid type and the other focus belonged to the subtype of warty type. The primary cure rate of extended local excision was 89% (17/19). There were two cases recurred during follow-up period, and were cured by second surgical treatment and the focuses of recurrence site were still diagnosed as warty type VIN. Conclusions The patients with former diagnosis of VIN Ⅱ , VIN Ⅱ-Ⅲ, VINⅢ, Bowen disease and Bowenoid papulosis are basically reclassified as usual type VIN (warty type) according to the 2004 modified terminology of ISSVD. The results showed that the diagnosis of VIN could be simplified by new terminology. The surgical excision is the good choice and combined cytologic and human papillomavims (HPV) test during follow up is also important for patients of VIN.

Objective To study the molecular genetic mechanism of a patient with 17? hydroxylase (CYP17) deficiency. Methods Genomic DNA were abstracted from the blood of the patient, her parents and healthy control. The 8 exons of CYP17 gene were amplified, using 5 pairs of designed primers, with polymerase chain reaction (PCR), and the 8 exons were sequenced by the dideoxy terminator method to determined the mutation sites. The corresponding exons of the parents of the patients were also amplified and sequenced to determine the zygosity of the patient and the source of the gene variances. Results The analysis revealed that the patient (46, XY) was a compound heterozygote carrying two different inherited mutations on CYP17 gene, one from mother containing a point mutation Arg 96 (C G G)→ Gln(C A G) and the other from father containing a nine base deletion (CACTCTTTC) at amino acid position 487～489 (Asp Ser Phe) near the carboxyl terminus of P450c17. Conclusion The CYP17 gene of the patient with 17? hydroxylase deficiency is a compound heterozygous mutation. The mutation changes the amino acid sequence of P450c17 enzyme, which in turn affected the enzymatic activity. Arg 96 is essential in P450c17 enzyme activity. Deletion of Asp 487 Ser 488 Phe 489 in exon 8 may be a prevalent mutation causing P450c17 deficiency in Southeast Asia.

OBJECTIVE: To investigate the clinical effect of nasal packing of pulmicort respules combined withnasopore after endoscopic sinus surgery. METHOD: A total of 30 CRSwNP and CRSsNP patients with bilateral functional endoscopic sinus surgery and finished following up visit were randomly choosed, conventionally select the left nasal cavity as the experimental group, the right nasal cavity as the control group. Experimental group to pack the nasal cavity with pulmicort respules union nasopore after surgery and control group to pack the nasal cavity with only nasopore after surgery. The differences were observed in patients with subjective symptoms and recovery of mucosa of operative cavity between the two groups after two weeks, one month and three months. RESULT: (1) The postoperative VAS symptoms score about nasal obstruction, nasal secretion, headache, dizziness and distending pain after two weeks,one month and three months in the experiment group were significantly better than those in the control group(P<0.05). (2) The postoperative Lund-Kennedy endoscopic mucosa morphology score after two weeks, one month and three months in the experiment group were significantly better than those in the control group(P<0.05); (3) After three months, the experiment group had 28 cases with clinic symptoms cured(93. 3%), Total effective rate was 96. 6%; The control group had 22 cases with clinic symptoms cured (73. 3%), total effective rate was 93. 3%. The cure rate of the experiment group was significantly higher than the control group(P<0.05), but there was no statistic difference between the two groups in the total effective rate (P>0.05). CONCLUSION The application of nasal packing of pulmicort respules combined with nasopore after functional endoscopic sinus surgery can effectively relieve postoperative uncomfortable symptoms, promote recovery of mucosa of perative cavity, which deserves clinical promotion.
