1.Effect of a risk of malignancy index in preoperative diagnosis of ovarian cancer

Objective To evaluate the ability of a risk of malignancy index (RMI), based on a serum CA125 level, ultrasound findings and menopausal status,to discriminate a benign from a malignant pelvic mass. Methods One hundred and forty women with a pelvic mass, 30 years or older,admitted between January 1998 and June 1999, were studied. The sensitivity,specificity and positive predictive value of serum CA125 level, ultrasound findings and the menopausal status separately and combined into the RMI,to diagnose ovarian cancer. Results RMI was more accurate than any individual criterion in diagnosing cancer. Using a RMI cut-off level of 200 to indicate malignancy, the RMI derived from this data set gave a sensitivity of 87.3%, specificity of 84.4%, and positive predictive value of 82.1%. Conclusions RMI is able to correctly discriminate between malignant and benign pelvic mass. It can be introduced easily into clinical practice to facilitate the selection of patients for primary surgery.
2.ESTABLISHMENT OF TWO CELL LINES SECRETING MONOCLONAL ANTIBODIES AGAINST PATHOGENIC FREE-LIVING AMOEBA IN SHANGHAI

Taking Naegleria australiensis, a species of pathogenic free-living amoeba (isolated in Shanghai in 1986), as antigen, two cell lines which provided potentially permanent source of monoclonal antibodies were established by lymphocytic hybridoma technique. The results of identification showed that: (1) the two cell lines could secret two different kinds of McAbs; (2) both of the McAbs were IgG (by gel diffusion); (3) McAbs produced in BALB/c mice were at high concentrations. One of them had a titer of ≥1 : 8 192 and the other≥l : 1 024 (by enzyme linked immunosorbent assay). A decline of the titers after purification by salting-out method was shown. One of the purified McAbs had a titer of≥1: 2 024 and the other≥l : 128.We have adopted two ways of recovering cryopreserved cells : ordinary recovering and "direct" recovering. The latter way was more practical because it could reduce the cycle of antibody production, and avoid contamination and chromosome variation. Experiments with different doses of cells revealed that, if the latter way was used, the optimal dose was 3 to 5 ? 106 cells per mouse.
3.A study on anti-inflammatory and analgesic effects of alkaloids of Toddalia asiatica

OBJECTIVE: To study the pharmacological activities and toxicity of the crude alkaloids of Toddalia asiatica and to provide pharmacological data for the further development of this herbal medicine. METHODS: We observed the anti-inflammatory effects of the crude alkaloids of Toddalia asiatica, using xylol and agra to induce the turgidness and sodium carboxymethyl cellulose (CMC-Na) to induce leucocyte strolling in the rats. The analgesic effects were observed by body-distortion methods. The effects of alkaloids of Toddalia asiatica on hepatic function were observed by testing the contents of alanine aminotransferase (ALT) and aspartate aminotransferase (AST) in serum and calculating the liver index. The LD 50 and 95% creditability were calculated with developed Karber Method. RESULTS: The administration of alkaloids of Toddalia asiatica had the function of inhibiting the auricle swelling caused by xylol and joint swelling caused by agar and leucocyte migration caused by CMC-Na, decreasing the body-distortion of the rats. After two weeks administration, the contents of ALT and AST showed that there was no obvious difference between administered group and control group. The LD50 of the crude alkaloids of Toddalia asiatica was 1.622 g/kg and the 95% creditability was 1.29-2.03 g/kg. CONCLUSION: Toddalia asiatica has anti-inflammatory and analgesic effects, and there is no injury to the liver after long-term administration in rats.
4.Epidemiological characteristics of inhalational allergen in children with allergic rhinitis in Tianjin

OBJECTIVE To analyze the results of the inhalational allergen detection in children with allergic rhinitis in Tianjin, and to understand the common allergens and their distribution.METHODS Mediwiss (Screen Allergy) was used to determine the specificity of allergen specific IgE in serum. According to sex, age, season, the distribution of allergens was described.RESULTS The most common inhalant allergens in Tianjin were mixed fungus(52.35%), followed by Dermatophagoides pteronyssinus(35.88%), dog dander(24.31%), cat dander(15.10%). The positive single allergen(59.03%) was most common, followed by positive double allergens(25.88%); The positive rate of the boy with inhalation allergen was higher than that of the girl; The positive rate of the inhalation allergen in the infant stage was significantly lower than that in the preschool, the school age, and the adolescent; In summer, the inhaled allergens positive rate was significantly higher than that in winter and spring, above differences were statistically significant(P<0.05). CONCLUSION Mixed fungus, Dermatophagoides pteronyssinus, dog and cat dander are the most common inhalant allergens in children with AR in Tianjin area. The positive rate of allergens has certain regularity in sex, age and season.
5.Disucussion on the Training Mode of Medical Management Graduates Based on the Combination of School and Enterprise

OBJECTIVE:To explore the training method for the postgraduates majoring in medicine management,and provide reference for the teaching effect of medical management. METHODS:Based on the existing problems in the training mode of post-graduates majoring in medical management,the foundation,feasibility implementation process of the coalition of college and enter-prise were analyzed. RESULTS & CONCLUSIONS:The training mode of coalition of college and enterprise can really make teach-ers,students and enterprises to achieve a win-win situation;at the same time,through the feasibility analysis of the training mode, the performance evaluation method of multi-level evaluation system and fuzzy comprehensive evaluation are put forward to finally achieve the exploration of training management mode of undergraduates from the perspective of operational level. But in the owner-ship of intellectual property rights,technology security,personal safety of graduate and other aspects still exist some problems and need further improvement.
6.CT Value in Examining Intestinal Tumor Obstruction

Objective To analyze the CT features of intestinal tumor obstruction, and explore its CT value.Methods CT manifestation and clinical materials of intestinal tumor obstruction proved by surgical findings or endoscopy biopsy in 35 cases were analyzed restrospectively, and compared with the results of surgery-pathology and endoscopy. Results CT findings were consistence with the results of surgery-pathology and endoscopy in 33 of 35 cases. The CT diagnostic accuracy of intestinal obstruction was 100%. And the accuracy of the cause was 94%, including colon carcinoma in 25, lyphoma in 5, gastrointestinal stromal tumor in 2, and lipoma in 3.Conclusion CT has unique advantage in examining intestinal tumor obstruction, not only for definiting the existence of the obstruction, but also locating the site of obstruction diagnosing the cause and chosing the appropriate treatment.
7.Effects of dihydroartiminisin on proliferation and phosphorylation of mitogen-activated protein kinase in epithelial ovarian cancer cell lines

Objective To determine the effect of dihydroartiminisin on the proliferation and phosphorylation of mitogen-activated protein kinase (MAPK) in SKOV3 and OVCAR3 ovarian cancer cell lines.Methods Methyl thiazolyl tetrazolium assay was performed to evaluate the anti-proliferative effect of dihydroartiminisin in SKOV3 and OVCAR3 cells,and Western blot was used to determine its effect on phosphorylation level of MAPK,including extra-cell regulated kinase (ERK)1/2 and p38 protein kinase,in the two cell lines.Results Dihydroartiminisin inhibited the proliferation of ovarian cancer cells in vitro,with a mean of 50% inhibition concentration (IC50) at 72 h of (9.0 ±1.4) μmol/L for SKOV3 and (5.5 ±1.2)μmol/L for OVCAR3 respectively. Compared to cells without dihydroartiminisin treatment,phosphorylation level of ERK 1/2 in SKOV3 and OVCAR3 cells treated with dihydroartiminisin decreased by 64.2% and 75.3% respectively (P<0.05),while phosphorylation of p38 protein kinase in SKOV3 and OVCAR3 only decreased by 8.5% and 6.4% respectively (P >0.05).Conclusion Dihydroartiminisin can inhibit the proliferation of ovarian cancer cell in vitro, probably through down-regulation of the phosphorylation of ERK 1/2 in ovarian cancer cells.
8.The results and drug susceptibility of respiratory secretion culture of children with trachea bronchial foreign bodies.

OBJECTIVE:
To analyze the distribution and drug resistance of the pathogenic bacteria in respiratory secretion in children with trachea bronchial foreign bodies so as to assist physicians in clinical prescription.
METHOD:
Sputum specimens of 622 children with trachea bronchial foreign bodies were collected,and the drug susceptibility test was peformed.
RESULT:
Pathogenic bacteria were detected in 124(19. 94%) of 622 sputum specimens. Most detected gram-negtive bacilli were highly sensitive to amikacin, ciprofloxacin, ceftriaxone, cefepime and ceftazidime, no strains were resistant to imipenem and meropenem; 42 strains were gram-positive bacilli. The former were highly sensitive to levofloxacin and chloramphenico,the latter were highly sensitive to ciprofloxacin, moxifloxacin and linezolid, no strains were resistant to rifampicin and vancomycin.
CONCLUSION
The frequent pathogenic bacteria in respiratory secretion in children with trachea bronchial foreign bodies include gram-negtive bacilli such as enterobacter cloacae, klebsiella pneumonia, escherichia coli, acinetobacter baumannii, serratia marcescens, and gram-positive bacilli such as streptococcus pneumonia,staphylococcus aureus. The detected gram-negtive bacilli were sensitive to imipenem and meropenem;the detected gram-positive bacilli were sensitive to rifampicin and vancomycin.

9.Quality of life and sexual function of cervical cancer patients following radical hysterectomy and vaginal extension

Objective To investigate the quality of life and sexual function of cervical cancer patients following radical hysterectomy (RH) and vaginal extension.Methods Case-control and questionnaire-based method was employed in this study.Thirty-one patients of early-stage (Ⅰ b1-Ⅰ b2) cervical cancer who had undergone vaginal extension following classic RH in Peking Union Medical College Hospital from December 2008 to September 2012 were included in study group,while 28 patients with matching factors and RH only during the same period were allocated to control group.There was no significant difference between two groups in terms of clinical and demographic variables including age at diagnosis,tumor stage and follow-up time (P>0.05).Patients were assessed retrospectively by validated selfreported questionnaires the European Organization for Research and Treatment of Cancer Cervix Cancer Module Questionnaire (EORTC QLQ-CX24) mainly for quality of life and sexual function for cervical cancer patients; the Sexual Function and Vaginal Changes Questionnaire (SVQ) further investigates sexual function and vaginal changes of patients with gynecologic malignancy at least 6 months after treatment.Results Vaginal length acquired by pelvic examination by gynecologic oncologists during follow-up visits was (10.0±1.3) cm and (5.9± 1.0) cm in study group and control group respectively (P=0.000).Sixty-eight percent (21/31) of cases in study group and 64% (18/28) of cases in control group had resumed sexual activity at the time of interview,and the time interval between treatment and regular sexual activity was mean 6 months (range 3-20 months) and mean 5 months (range 1-12 months) in study and control group respectively,in which there was not statistical significance (P>0.05).No difference was observed regarding pelvic floor symptoms (P>0.05) while difficulty emptying bladder,incomplete emptying and constipation were most commonly reported.Both group presented with hypoactive sexual desire disorder [88% (52/59)],orgasm dysfunction [72%(28/39)] and low enjoyment or relaxation after sex [51%(20/39)],which was not statistically significant (P>0.05).Reduced vagina size and shorter vagina was more prominent in control group (12/18) than that in study group [19% (4/21)] with statistical significance (P<0.05),while no difference in sexual desire,vaginal lubrication,dyspareunia and sexual enjoyment (P>0.05).Conclusions Patients with peritoneovaginoplasty following RH had much longer vagina and less self-perceived short vagina.Vaginal extension following RH does not worsen the pelvic floor symptoms.
