Objective:To create a convenient, quick and personalized upgrade application (APP) based on current Pocket Shengjing hospital mobile medical APP.Methods: To establish the best suitable mobile terminal APP to patient by using smartphone and mobile internet technique.Result: A series of new function, such as appointed register at any time before diagnosis, anticipated waiting time during diagnosis, enquiry of detection after diagnosis, different kind of payment during diagnosis, healthy information net among family members and effective control for broken appointment of patient, were achieved by the upgrade mobile medical APP. It also can optimize the process of diagnosis, reduce waiting time, increase outpatient service effective and improve satisfaction of patient for outpatient serviceConclusion: The upgrade mobile medical APP is a more practical and convenience software, and it can improve medical treatment process and provide convenient condition for medical service based on the perspective of patient.
2.Effect of a risk of malignancy index in preoperative diagnosis of ovarian cancer

Objective To evaluate the ability of a risk of malignancy index (RMI), based on a serum CA125 level, ultrasound findings and menopausal status,to discriminate a benign from a malignant pelvic mass. Methods One hundred and forty women with a pelvic mass, 30 years or older,admitted between January 1998 and June 1999, were studied. The sensitivity,specificity and positive predictive value of serum CA125 level, ultrasound findings and the menopausal status separately and combined into the RMI,to diagnose ovarian cancer. Results RMI was more accurate than any individual criterion in diagnosing cancer. Using a RMI cut-off level of 200 to indicate malignancy, the RMI derived from this data set gave a sensitivity of 87.3%, specificity of 84.4%, and positive predictive value of 82.1%. Conclusions RMI is able to correctly discriminate between malignant and benign pelvic mass. It can be introduced easily into clinical practice to facilitate the selection of patients for primary surgery.
3.Hysterectomy in obstetric problems

Objective To compare and analyse the elective and emergency hysterectomies in obstetric practice. Meth odsA retrospective study was performed to compare elective versus emergency cesarean hysterectomies in PUMC Hos pital from January 1969 to December 1998.ResultsObstetric hysterectomies were performed on 36 patients over 30 years out of 33207 deliveries. 26 emergency cesarean hysterectomies and 10 elective hysterectomies had been done. Pla cental disorder was the main indication of emergency hysterectomy. The perinatal mortality of newborn was 179.5‰. ConclusionObstetric hysterectomy has been proved to be one of the most useful life- saving procedure for patients with postpartum hemorrhage, especially at the time of cesarean section, particularly for patients with a history of benign or malignant gynecologic tumor or irregular vaginal bleeding before pregnancy.

5.External application of Chinese medicine on acupoints in prevention and cure of bronchial asthma in children

Objective To observe the therapeutic of Xiaochuanling ointment applied on acupoints combined with internal use of Yupingfeng pellets in the prevention and cure of bronchial asthma in children.Methods 80 children diagnosed of bronchial asthma were randomly recruited into a control group and a treatment group, 40 patients in each group. Patients in treatment group were treated with Xiaochuanling ointment applied on acupoints combined with internal use of Yupingfeng pellets. No treatment was used in the control group in relieving period, except observation and follow-up visit were performed. Observe the frequency,degree of asthma before and after treatment in both groups, along with the level variation of IgA, IgG, IgE and Eosnophils (EOS). Results After treatment, the average disease grade in treatment group was 1.82S with serum IgA (1.32±0.56) g/L, IgG (12.48± 1.85) g/L, IgE (257.06± 194.04) g/L, EOS (128.56±76.86) ×106/L and frequency of asthma (7.22 ±2.01). Compared with the control group and the treatment group before the treatment, there were statistical differences (P<0.01, P<0.05). Conclusion The effect of external application of Chinese medicine on acupoints in prevention and cure of bronchial asthma in children is especially remarkable.
6.Noninvasive positive pressure ventilation for patients with chronic obstructive pulmonary disease after upper abdominal and thoracic surgery

Objective To compare the efficacy of postoperative noninvasive positive pressure ventilation ( NPPV ) plus standard medical therapy ( SMT) with SMT alone in patients with moderate to severe chronic obstructive pulmonary disease ( COPD )Methods Twenty-four patients, after upper abdominal or thoracic surgery, who suffered from COPD and moderate to severe respiratory insufficiency, were randomly allocated to receiving SMT alone (oxygen, aminophylline infusion, nebulized beta-2 agonists and anticholinergics, antibiotics administration and chest physiotherapy; control group ,n=12) or NPPV in addition to SMT(NPPV group ,n=12) NPPV was intermittently given with an air-cushioned face mask under the continuous positive airway pressure of 3 cmH 2O and FiO2 of 35%Results There were not significant differences in baselines between both groups 8 patients in control group ( 667%) and all patients in NPPV group recovered with initial therapy with statistically significant difference (P
7.The very severe sensorineural deafness patients caused by rubella virus infection: two cases report.

To explore the audiological features in children who were sever sensorineural hearing loss infected with rubella virus. There were two cases of rubella virus infection in children who were deaf, they conducted the distortion product otoacoustic emission, ABR and auditory steady-state evoked response (ASSR) examination, then analyzed the results comprehensively. Two patients' mothers were prompted to have infected rubella virus during the early three months pregnant period by history and laboratory tests. The two patients were not detected deafness gene mutation. Audiology results implied the two patients were very severe binaural sensorineural deafness, so they were recommended to equipped with hearing aids and cochlear implant surgery. Early pregnancy women infected with rubella virus can cause very severe offspring sensorineural deafness. The crowd whose mother were suspected to infect with rubella virus in early pregnancy, that should be tracked and detected hearing in order to achieve early detection, early intervention and early treatment.

8.Post-stroke dysphagia treated with acupoint injection combined with neural electrical stimulation.

OBJECTIVETo observe the effect differences on post-stroke dysphagia among acupoint injection combined with neural electrical stimulation, acupoint injection, neural electrical stimulation, and swallowing training respectively, so eligible intervention can be applied to this kind of disease.
METHODSOne hundred and eight-three patients of post-stroke dysphagia were randomized into a comprehensive treatment group (42 cases), an acupoint injection group (44 cases), a neural electrical stimulation group (49 cases) and a swallow training group (48 cases) and were treated with the comprehensive therapy of acupoint injection and neural electrical stimulation, acupoint injection, neural electrical stimulation and swallowing training separately. The treatments for 10 days made one session. There were 3 days at the interval among treatment sessions and 3 sessions were required totally. The cases in those treatment groups were blankly controlled with the other 47 patients of post-stroke dysphagia. All the patients received basic rehabilitation treatment. The modified water swallowing test was conducted to assess the efficacy before treatment, 10 days after treatment and 30 days after treatment in each group separately. The clinical efficacy, score of water swallowing test and improvement in water swallow test were compared among the groups.
RESULTSAfter 10-day treatment, the differences in efficacy and score of water swallow test were not significant in each group (all P > 0.05). After 30-day treatment, the effective rate (94.29%, 33/35) in the comprehensive treatment group was apparently better than 68.75% (22/32) in the acupoint injection group, 80.00% (32/40) in the neural electrical stimulation group, 67.50% (27/40) in the swallowing training group and 42.86% (12/28) in the blank group separately. The score in water swallow test in the comprehensive treatment group was lower than that in each of the other groups (1.37 ± 0.60 vs 2.03 ± 1.00, 1.90 ± 0.90, 2.20 ± 0.72, 2.71 ± 0.90, all P < 0.05). The differences in the effective rate and score in water swallow test were not significant among the acupoint injection group, neural electrical stimulation group and swallowing training group (all P > 0.05), which indicated that the improvement in swallowing function in the comprehensive treatment group was significantly superior to the other groups (all P < 0.05).
CONCLUSIONThe comprehensive therapy of acupoint injection and neural electrical stimulation achieves the much better efficacy on post-stroke dysphagia.

9.Early discovery and intervention study in autism spectrum disorders children with dysaudia

OBJECTIVE To explore the early intervention effect in autism spectrum disorders children with dysaudia. METHODS Three cases of child with language barrier were diagnosed with dysaudia by acoustic impedance test, distortion otoacoustic emission (DPOAE) and short sound auditory brainstem response test (ABR). They were diagnosed with autism pectrum disorders through further test. They were matched with hearing aid, and took speech rehabilitation training, followed-up one year to observe the language communication effect. RESULTS After one year of treatment with hearing aid, both CAP and SIR score increased. CONCLUSION Autism spectrum disorders children with dysaudia can get better language exchange by early intervention with hearing aid and speech rehabilitation training.
10.Efficacy of Voice Therapy Combined with Drug Therapy in the Patients with Vocal Nodules

Objective To study the efficacy of voice therapy combined with Jinsangsanjie pill in patients with vocal nodule .Methods A total of 146 patients with vocal nodes were randomly divided into three groups :45 cases in group A(single Jinsangsanjie pill therapy) ,47 cases in group B(single voice therapy) and 54 cases in group C(voice therapy combined with Jinsangsanjie pill therapy) ,30 healthy adults were as a normal control group .The treatment lasted 1 month .The results were evaluated by voice handicap index ,voice acoustic analysis and electronic laryngo‐scope which were collected before and after 1 month treatments .Results The VHI ,jitter ,shimmer and NNE of all patients were reduced while the MPT was increased after the treatment .The differences were significant (P<0 .05) .VHI 3 .64 ± 2 .12 ,jitter 0 .30% ± 0 .08% ,shimmer 1 .41% ± 0 .31% ,NNE -16 .83 ± 5 .84 dB of group C were significantly lower while MPT 18 .87 ± 3 .38 s and cure rate(93 .5% ) were significantly higher than those of in groupA(7.39±2.56,0.38% ±0.12% ,1.78% ±0.41% ,-13.38±4.42dB,16.38±3.11s,80.5% )andgroupB (23.6±12.5,0.48% ±0.18% ,1.98% ±0.42% ,-9.42±3.82dB,14.98±3.28s,52.4% )(P< 0.05).The VHI ,,jitter shimmer ,NNE of group A were significantly lower .MPT and cure rate were significantly higher than those of in group B .There were no statistical differences in each vocal indicators between group C and the normal control group (P>0 .05) .Conclusion The voice therapy combined with Jinsangsanjie pill in the treatment of vocal nodules is more efficiently ,and can improve patient’s pronunciation features .
