Objective To investigate the impact of carvedilol on plasma cytokines in patients with chronic congestive heart failure(CHF) and its efficacy analysis.Methods 72 hospitalized patients with CHF were randomly divided into observation group and control group.The two groups were given standard therapy after admission.The observation group were given addition of carvedilol with an initial dose of 3.125mg,2 times daily,gradually increasing amount until the maintenance dose of 25mg,2 times daily for 12 weeks.Results After 12 weeks of treatment,plasma TNF-α,IL-1 and IL-6 in both groups were significantly decreased than before (t =3.02,3.17,2.97,2.26,2.25,2.21,all P ＜ 0.05) and the decline in the observation group was more significant than those in the control group(t =2.32,2.35,2.29,all P ＜ 0.05);The total clinical efficiency in the observation group(94.44％) was significantly higher(77.78％)(x2 =4.18,P ＜ 0.05),and there were no significant adverse drug reactions during treatment in both groups.During 1-year follow-up after treatment,re-hospitalization rate in the observation group (61.11％) was significantly lower than that in the control group (36.11％) (x2 =4.50,P ＜ 0.05),but there was no statistically significant difference in mortality between two groups (x2 =0.26,P ＞ 0.05).Conclusion Carvedilol is a safe and effective drug for the treatment of CHF,which can lower plasma cells factor TNF-α,IL-1 and IL-6 levels,reduce patient re-hospitalization rates,and help to improve the patients' prognosis.

According to the patients to nursing job satisfaction questionaire by a third party, through telephone follow-up forms, carry out the satisfac-tion survey. Combined with their own advantages of TCM medical volunteers, to carry out the diet instruction, psy-chological counseling, massage, warm room service, telephone return services such as ward, score of life quality of tumor patients on admission and after the volunteer service. Results:Ward accompanying rate dropping, increasing patient satisfaction with nursing service, improving the degree of identification of volunteer service, tumor patients and improving the quality of life, and is conducive to building a perfect medical students social practice platform. Conclusion:Volunteer services improve the quality of nursing service, cultivating a correct outlook on life, values and morality of medical volunteers, strengthening medical volunteer service consciousness, achieving win-win for patients, departments, volunteers.

Objective To observe any therapeutic effect of pulsed electromagnetic fields (PEMFs) on bone loss in spinal cord injury (SCI) patients.Methods Fifty-five patients with SCI were divided into two groups randomly.The twenty-six patients in the control group (group B) were given only routine rehabilitation treatment; the twenty-six patients in the treatment group (group A) received PEMF therapy in addition.Results After 12 weeks of treatment,the average bone mineral density (BMD) of the proximal femur (including total,neck,Wards,inter,troch) in group A was significantly higher than in group B.The levels of bone-gamma-carboxyglutamic acid containing protein (BGP) and 1,25 (OH)2D3 in group A increased significantly,while they decreased in group B.Urine-pyridinium/crealinine (U-Pyd/Cr) levels in group A decreased significantly,while in group B they were higher than before.There were statistically significant differences between the two groups.Conclusion PEMF treatment can effectively retard bone loss in SCI patients.It has good preventive and curative effects on osteoporosis after SCI.

Objective To establish a RP-HPLC method for simultaneous quantitative determination of 4 anthraquinones in Shugan Quzhi capsules. Methods A RP-HPLC method was used with a Diamonsil C18 column (5 ?m,4.6 mm?150 mm). The mobile phase was composed of methanol and 0.1% H3PO4 water solution (77∶23,v/v). The wavelength of UV detector was 254 nm and column temperature was 30 ℃. Flow rate was 1.0 mL/min and 20 ?L was injected every time. Results The linear ranges of rhein,emodin,chrysophanol and physcion were 0.083 2～2.08,0.100 8～2.52,0.291 2～7.28 and 0.088～2.20 ?g/mL,respectively. Their average recoveries were 97.3%,96.9%,96.5% and 95.9%,respectively. Conclusion This method was sensitive,repeatable and suitable to determine the contents of 4 anthraquinones in Shugan Quzhi capsules.

Objeact:To study the stress distribution of angled dowe l crown in anterior maxillary teeth. Method:A 3-dimensio nal finite element model (3-DFEM) of a maxillary central incisor with a dowel, a core and the supporting tissues was established.The stress in various angled d owel designs with a 150 N force applied along the long axis of the crown in a v ertical load on the incisal edge was mesured on the 3-DFEM and analyzed with AN SYS sorft ware.Results:The maximum stress was found in la bical cervical zone of the cortical bone around the teeth,the compressive stress in labical and tensile stress in palatal respectively. Conclusion: In anterior maxilla with angled dowel crown maximum stress is in the cervical zone of cortical bone,and the magnitude of stress increases with the i ncreace of the angulation of dowel crown.

Fermentation inhibitors are toxic to cells, which is one of the bottlenecks for lignocellulose bio-refinery process. How to remove those inhibitors serves a key role in the bioconversion of lignocellulose. This article reviews the sources and the types of the inhibitors, especially the updated removal strategies including physical methods, chemical methods, biological methods and inhibitor-tolerant strain construction strategies. Based on these, we introduce a new bio-refinery model named "fractional conversion", which reduces the production of inhibitors at pretreatment stage, and a novel in situ detoxification method named "fermentation promoter exploitation technology". This review could provide new research ideas on the removal of fermentation inhibitors.

Objective To observe influence of salmeterol xinafoate and fluticasone propionate powder applied in asthmatic exacerbated period on relieving efficacy of a fast-acting beta2-agonist.Methods Eighty-nine children with asthma in exacerbated period were randomly assigned into two groups.Thirty-nine cases in treatment group inhaled salmeterol xinafoate and fluticasone propionate powder, compared with 50 cases in control group treated with budesonide aerosol. Objective:To investigate the methods and nutritional effects of endoscopic jejunal ac cess for enteral feeding.Methods:106 cases placed jejunum feeding tube were randomly divided into the observation group and the control group,54 cases in the observation group and 52 cases in the control group.43 patients underwent nasogastric enteral nutrition were as the nasogastric tube group.The traditional endoscopic jejunal access method was used in control group,and the modified endoscopic jejunal access method was used in observation group.The operation time,the success rate of indwelling,complications and nutritional indexes were compared among these three groups.Results:The operative time of the observation group and nasogastric tube group was significantly shorter than that of the control group (P ＜ 0.05).The success rate of indwelling was significantly lower in the control group (P ＜ 0.05).There was no significant difference in the incidence of postoperative complications among these three groups (P ＞ 0.05).After intubation and nutritional treatment in all the groups,hemoglobin,plasma albumin and serum albumin levels were significantly improved (P ＜ 0.05).There were no significant differences in nutritional indexes between the observation group and the control group (P ＞ 0.05).The levels of hemoglobin,plasma albumin and plasma prealbumin in nasogastric tube group were significantly lower than those in the observation group and the control group (P ＜ 0.05).Conclusion:Modified endoscopic jejunal access for enteral feeding can effectively shorten the intubation operation time and improve success rate and nutritional indexes.

Objective To investigate the allele polymorphism and characteristic of HPA-1-17 genotyping of Li ethnic on Hainan island,and to evaluate the HPA mismatched ratio between donor and recipient in randomly chosen cases of blood transfusion,in order to investigate the platelet transfusion refractoriness in Li population.Methods Genotyping tests of A-1—17 allele of the blood sample from 180 Lis were conducted with PCR-SSP.Results A total of 180 blood samples were genotyped successfully in HPA genetic system,among which 0.9972 were HPA-2a,0.0028 were HPA-2b,0.4889 were HPA-3a,0.5111 were HPA-3b,0.9667 were HPA-5a,0.0333were HPA-5b,0.9972 were HPA-6a,00028 were HPA-6b,0.4250 were HPA-15a,0.05750 were HPA-15b,and they all showed polymorphism. But polymorphism was not detected out in other allelic genes,such as HPA-1,-4,-7—14,-16,and -17. The bb homozygous were detected in two alleles,among which 0.2834 was HPA-3 and 0.3667 was HPA-15,but it wasn't detected in the others systems. In the randomly chosen cases of blood transfusion,the mismatch rate of HPA-3 ,HPA-15 and HPA-5 was 37.49%,36.93% and 6.23%,respectively.Conclusion This research revealed the distribution of HPA gene in Li population on Hainan island,and found the allelic frequency and thier characteristics. According to our findings,HPA-2,HPA-3,HPA-5 and HPA-15 genes between donor and recipient should be matched in order to avoid platelet transfusion refractoriness.

OBJECTIVE:To provide reference for the construction and management of hospital clinical drug trial institution. METHODS: The practice of our hospital in the construction and management of hospital clinical drug trial institution in accordance with the Good Clinical Practice and the Trial Provisions for Drug Clinical Trial Institution Qualification was summarized. RESULTS: Hospital attached great importance by taking effective measures,strengthening the construction of software and hardware and emphasizing reforming was the key to pass the qualification confirmation,and the key points in the management are to tighten control on the clinical trial process and establish complete management regulations. CONCLUSION: The construction and management of hospital clinical drug trial institution is conducive to the improvement of clinical trial level and it serves as a scientific,accurate and reliable basis for the evaluation and marketing approval of new drugs.
