1.Rehabilitation for Thallium Poisoning: One Case Report

ObjectiveTo investigate the clinical manifestations, rehabilitation evaluation and treatment for Thallium poisoning. MethodsA patient with Thallium poisoning was reviewed. ResultsGastroenteritis, peripheral polyneuropathy and alopecia were the typical symptoms of Thallium poisoning. After the shortwave therapy, medium frequency electrotherapy and physical therapy, the index of sensory, extremities perimeter and muscle strength, sitting-stand-walking, ADL, and emotions were apparently improved. ConclusionComprehensive rehabilitation with shortwave therapy, medium frequency electrotherapy and physical therapy might be effective treatments for the patients with Thallium poisoning.
2.Preparation of quercetin-arginine complex

Object To prepare the water soluble quercetin arginine complex (QAC) and widen the administration path of quercetin (QUE). Methods Definite QUE and L arginine were refluxed in alcohol to prepare QAC. The QAC structure was identified by micellar paper chromatography, UV spectrometry, IR spectrometry, and X ray diffraction. Results QAC was prepared from QUE and L arginine in molar ratio 1∶1. The inhibitory activity of QAC that existed stably in room temperature on cancer cell growth was as strong as that of QUE, and the solubility of QAC in water was remarkably enhanced. Conclusion The above preparation method is simple and available, and it is suitable to improve the bioavailability.

Objective To evaluate the value of percutaneous hyperthermia-chemotherapy (PHC)under CT guided in treating original and secondary hepatic and pulmonary malignant tumor.Methods Percutaneous hyperthermia and chemotherapy under CT guided was performed for 21 patient with original and secondary hepatic and pulmonary malignant tumor.Chemical drugs against tumors were warmed to 55~60℃ and injected into the tumors.Injected volume was according to:V=4/3 ?(r+0.5 cm) 3.Observation depends upon attenuation changes of CT scanning and biochemical index(AFP)The therapeutic effect was classified into Ⅰ~Ⅴgrade.Results The period of observation was 36 monthes,In 20 cases,survival period was 8~22 monthes,average survival period was 16 monthes.A patient had treated with PHC and transcatheter arterial embolization and was alive for 28 monthes.Total effective rate was 95.2%.Conclusion PHC under CT guidence is an effective method in treating hepatic and pulmonary malignant tumors.especially for unresected tumors.Cooperating transcatheter arterial embolization(TAE)can raise curative effect.
5.Genotyping of hepatitis E virus by PCR combining with single restriction endonuclease analysis.

OBJECTIVETo develop a simple method for genotyping of hepatitis E virus (HEV) and to investigate HEV genotype distribution in Nanjing area.
METHODSTwenty-seven full HEV sequences currently-available in GenBank were analyzed with MegAlign and MapDraw programs of DNA STAR software. Degenerate primers were designed and applied to amplify a fragment in HEV ORF1 region. HEV genotypes were determined by the size of the PCR products and by single restriction endonuclease analysis.
RESULTSThe PCR products of HEV genotype 1 and 2 were 275 bp and 269 bp in size. Distinctively, the PCR products of genotype 3 and 4 were 317 bp and 314 bp in size. Moreover, the PCR products of genotype 1 could be digested by Nae 1, but the products of genotype 2 could not. Distinctively, the PCR products of HEV genotype 3 could be digested by Not 1, but the products of genotype 4 could not. Six HEV reference strains standing for different HEV genotypes were clustered into their own types as predicted. Within 43 HEV IgM-positive clinical specimens collected in Nanjing, 19 were HEV PCR-positive and identified as genotype 4.
CONCLUSIONA simple method of PCR combined with single restriction endonuclease analysis is developed for HEV genotyping. This assay allows rapid identification of a large number of HEV isolates directly from clinical specimens. Among patients with hepatitis E in Nanjing, most were infected with HEV genotype 4.

6.Interruption failure of heptitis B virus vaccination in mother-to-infant transmission and heptitis B virus genotypes and preC/BCP mutations.

OBJECTIVETo investigate the association of heptitis B virus (HBV) genotypes and precore(PreC)/basal core promoter(BCP) mutation with interruption failure of HBV vaccination in mother-to-infant transmission.
METHODSA total number of 208 serum samples were collected from infants and mothers,including 16 infants who had become HBsAg-positive despite a complete and timely course of immunization and another 88 infants successfully protected from mother-to infant HBV transmission. HBV genotypes were determined by type-specific primers PCR method. PreC/BCP mutations were detected by direct sequencing of PCR products, and Clustal W 1.8 software was applied to analyzing the sequences.
RESULTSOf 16 mothers who were having vaccine failure infants, 15 (93.8%) were HBeAg positive and infected with genotype C (15/15, 100%). Among 88 mothers of having children being protected by vaccine, 51 (58.0%) were HBeAg positive, with 45.1% (23/51) of genotype C. The proportion of genotype C in HBeAg mothers of infants with vaccine failure, was significantly higher than that of mothers with vaccine protected infants (chi2 = 14.3, P = 0.003). However, the frequencies of T1762/A1764 mutations had no significant differences between genotype C HBeAg positive mothers with vaccine failure or protected infants (33.3% and 13.3%, respectively, P = 0.4). No A1896 mutation was found in these two groups.
CONCLUSIONHBV genotype C might contribute to the immune failure of HBV vaccination in mother-to-infant transmission, while PreC/BCP mutation might not have correlation with it.

7.A clinical comparison of two strategies for minimal invasive treatment of secondary common bile duct stone.

OBJECTIVETo compare the clinical efficiency of two strategy in the treatment of secondary common bile duct stones: laparoscopic cholecystectomy with transcytotic common bile duct exploration (LC + LTCBDE), or laparoscopic cholecystectomy after endoscopic sphincterotomy (EST + LC).
METHODSAccording to the screening standard, 88 patients with secondary bile duct stones (43 cases in LC + LTCBDE group, 45 cases in EST + LC group) who were treated from June 2005 to November 2007 were analyzed retrospectively. The achievement ratio, complications, average hospital stay and cost between two groups were compared. And all patients were followed up.
RESULTSThere were no significant differences on achievement ratio and complications between two groups (P > 0.05). The LC + LTCBDE group has significantly shorter hospital stay and lower cost (P < 0.05). There were no recurrence in 1 year.
CONCLUSIONSLTCBDE is a promising strategy which has more advantages in the minimal invasive treatment of secondary common bile duct stones.

8.Establishment of Acute Immunological Liver Injury Wistar Rat Model Induced by Concanavalin A

Objective To explore the dosage and injection method of concanavalin A(Con A) for inducing Wistar rats into the acute hepatic injury model. Methods (1)According to the dosage of Con A, 42 Wistar rats were randomly divided into groups A, B, C, D, E, N, 7 rats in each group. Group N was given tail intravenous injection of normal saline as normal control group. Groups A, B, C, D, E were given intravenous injection of 4, 8, 16, 30, 40 mg/kg of Con A respectively. At the 8th hour after modeling, the levels of alanine transaminase(ALT), aspartate aminotransferase(AST), albumin(ALB), interleukin(IL)-2 , IL-10, interferon (IFN)-γ, and tumor necrosis factor(TNF)-αwere detected. And HE staining was used to observe the pathological feature of hepatic tissue. (2)According to the injection method of Con A, 21 Wistar rats were randomly divided into normal control group, intraperitoneal injection group and tail intravenous injection group, 7 rats in each group. The dosage of Con A for the rats in intraperitoneal injection group and tail intravenous injection group was 16 mg/kg. At the 8th hour after modeling, the levels of serum ALT, AST, and ALB were determined. Results The number of abnormal deaths in various dose Con A groups at the end of each experiment was 0 in groups A, B, C, and 2 in group D, and 7 in group E. A small amount of spotty necrosis, inflammatory cell infiltration, and hepatic lobule with almost integrity of structure were found in groups A, B, while obvious bridging-like necrosis was seen in groups C, D. Serum ALT, AST, and ALB levels in intraperitoneal injection group had no statistically significant difference as compared with the normal control group. Conclusion Tail intravenous injection of 16 mg/kg of Con A can be used to induce an acute immunological liver injury rat model successfully.
9.Effect of leflunomide combined with methotrexate in the treatment of skin lesions caused by psoriatic arthritis

Objective To study the clinical effect of leflunomide combined with methotrexate in the treatment of skin lesions caused by psoriatic arthritis .Methods The clinical data of 90 patients with skin lesions caused by psoriatic arthritis were collected ,and they were divided into control group and observation group by simple random method,45 cases in each group.The control group was treated with methotrexate ,and the observation group was given leflunomide combined with methotrexate .Before treatment and after treatment for one month ,two months ,three months ,the skin lesions , psoriatic arthritis symptoms improvement , clinical effects and adverse reactions were observed in the two groups.Results After treatment for one month,two months,three months,the DLQL index scores in the observation group were (8.12 ±2.02)points,(6.55 ±1.47)points,(3.68 ±1.21)points,respectively,which were significantly better than those in the control group (t=2.54,3.92,4.44,P=0.01,0.01,0.01).After treatment for one month,two months,three months,the PASI index scores in the observation group were (7.57 ±3.22)points, (4.61 ±1.86)points,(3.11 ±2.17)points,respectively,which were better than those in the control group (t=3.42, 7.81,4.07,P =0.01,0.01,0.01).The total effective rate of the observation group was 88.89%,which was significantly higher than 68.89%of the control group (χ2 =5.40,P=0.02).No serious adverse reactions occurred in both two groups,and the difference was not statistically significant (11.11% vs.13.33%,χ2 =0.10,P=0.75). Conclusion Compared with methotrexate alone , leflunomide combined with methotrexate in the treatment of skin lesions caused by psoriatic arthritis has obvious effect , and can significantly improve the symptoms of patients with skin and arthritis symptoms ,and it is more conducive to relieve patients'pain.
