4.Preparation and characterization of 7-ethyl-10-hydroxycamptothecin-loaded hypomicron of amphiphilic block copolymer.
Acta Pharmaceutica Sinica 2007;42(4):440-444
Methoxypolyethylene glycol-poly lactic acid (PELA) was synthesized by ring-opening copolymerization of lactide in the presence of mPEG and its structure was characterized by 1H NMR. The novel hypomicrons were prepared by solution-casting method using PELA block copolymer as a matrix and 7-ethyl-10-hydroxycamptothecin (SN-38) as an antitumor agent. The morphology, size and size distribution, drug loading, entrapment efficiency, and release characteristics in vitro of the SN-38 loaded hypomicrons were studied. The results showed that the obtained hypomicrons showed spherical shape with the core-shell structure, the sizes are in the range of 157-238 nm, and the drug loading content varied from 1.35%-3.58% depending on the copolymer composition and the SN-38 fed amount. The in vitro release behavior in phosphate-buffered saline, pH 7.4, exhibited a sustaining release manner and was affected by the copolymer composition. The drug-loaded amount and entrapment efficiency decreased with increasing the molecular weight of the copolymer. With the increasing of the SN-38 fed amount, the drug-loaded amount and the size of hypomicrons increased, the entrapment efficiency decreased. The SN-38 hypomicrons increased the solubility of SN-38 in water and were valuable for the development of the novel dosage form of SN-38.

Journal of Clinical Otorhinolaryngology Head and Neck Surgery 2007;21(22):1015-1017
OBJECTIVE:
To analyze the changing features of mandarin-tone recognition in lower prelingual children with cochlear implant after mapping.
METHOD:
Twenty-nine children with CI were registered in this test, who were divided into two groups according to the age received the operation. They were group A whose ages were 3 to 4.5 years old, and group B whose ages were 5.0 to 6.5 years old. The time after first mapping was between 1.5 and -2.0 years. The test only included close-set phonetic recognition which was mainly used to evaluate Mandarin-tone recognition. The Phonetic Recognition List was used as the test material.
RESULT:
The results showed that the percentages of correct recognition were same-single-syllable tones average (63.00+/-16.75)%; bi-syllable tones average (75. 60+/-11.18)%; single-character words average (72.38+/-11.39)% in A group children and respectively, (49.46+/-13.91)%; (64.71+/-9.64)%; (55.71+/-8.59)% in B group children. The recognition scores exceeded chance level in all results and they were better in A group. Statistical analysis(t test) showed significant difference between two groups.
CONCLUSION
The age is one of the most influence factors about Mandarin-tone recognition after implanting in pre-lingual children with CI. It is another important factor to influence studying Chinese after operation in the children.

6.Expression of Twist1 and Twist2 in hypopharyngeal carcinoma and its relationship with clinicopathological features
Journal of Chinese Physician 2021;23(8):1219-1223
Objective:To investigate the relationship between the expression of Twist1 and Twist2 protein and with clinicopathological features in hypopharyngeal carcinoma. Methods:The clinical and pathological data of 58 patients with hypopharyngeal carcinoma who underwent surgery in Liaoyang Central Hospital from May 2014 to September 2019 were retrospectively analyzed. The general demographic and pathological data of the patients were collected, and the expression of Twist1 and Twist2 protein in the hypopharyngeal cancer tissues and adjacent tissues were detected by immunohistochemistry. The patients were followed up to record their survival. The relationship between protein expression of Twist1 and Twist2 and clinical pathological stages and prognosis was analyzed. Results:Immunohistochemistry showed that the positive expression rates of Twist1 and Twist2 protein in 58 patients with hypopharyngeal cancer were significantly higher than those in adjacent tissues ( P<0.05). The positive expression rates of Twist1 and Twist2 in patients with T stage T 3+ T 4 and clinical stage Ⅲ+ Ⅳ were significantly higher than those in patients with T 1+ T 2 and Ⅰ+ Ⅱ ( P<0.05). The positive expression rates of Twist1 and Twist2 in patients with lymph node metastasis were significantly higher than those without lymph node metastasis, and the positive expression rates of Twist1 and Twist2 in low differentiation were significantly higher than those in high differentiation ( P<0.05). 52 patients were followed up postoperatively, the follow-up rate was 89.67%, among which 28 patients recurred (53.85%), and the 1-, 3-, and 5-year survival rates were 80.77%(42/52), 53.85%(28/52), and 21.15%(11/52), respectively. The survival rate of patients with positive expression of Twist1 and Twist2 protein was significantly lower than that of patients with negative expression ( P<0.05). Multiple analysis showed that recurrence, tumor stage, positive expression of Twist1 and Twist2 protein were independent risk factors for survival ( P<0.05). Conclusions:The expression of Twist1 and Twist2 protein in hypopharyngeal carcinoma is stronger than that in normal tissues. The expression of Twist1 and Twist2 protein is related to T stage, clinical stage, pathological tissue type and lymph node metastasis. Meanwhile, the high expression of Twist1 and Twist2 proteins is an independent factor affecting the prognosis of patients. They may be potential indicators to judge the prognosis of patients with hypopharyngeal cancer.

OBJECTIVETo investigate the feasibility of transaxillary dual-plane breast augmentation with endoscope assistant.
METHODSThe submuscular pockets were separated through a 4 cm transverse incision in the axilla. The pectoralis major muscle were split from the costal margin with the help of a 10 mm, 30 degree endoscope and endoscopic diathermy scissors. Soft cohesive gel microtextured anatomic style silicone implants ranging in size from 185 to 315 g were placed through the axillary incision. The vacuum drainage was used for 3 approximately 5 days.
RESULTS79 patients underwent the dual-plane breast augmentation. Postoperative analgesia requirements were reduced because of muscle strength releasing. The follow-up period was 4 to 6 months with satisfactory results. All the patients had aesthetically natural appearance, with the nipple at the most projected part of the breast. No rippling, lateral displacement, double-bubble deformity, or muscle contraction-associated deformities were seen. There was no complications such as capsular contracture, hematoma, hypertrophic scar, and infection.
CONCLUSIONSDual-plane breast augmentation using textured silicon gel implant can be completed with the endoscope assistance through the axillary incision. The technique combined retromammary and partial retropectoral pocket locations to optimize the benefits of each pocket location while avoiding the drawbacks of extra incision on the breast.

Chinese Journal of Plastic Surgery 2005;21(4):278-280
OBJECTIVETo find anatomic basis for clinically modifying technique of harvesting superior and inferior gluteal artery perforator flap, in order to avoid muscle lossing in conventional superior and inferior myocutaneous flaps, keep the advantage such as large rich supplied volume soft tissue.
METHODS5 cases 10 sides adult cadaver were used to study the numbers, position, Course of superior and inferior gluteal artery perforators. The position of perforators was located by ultrasound Doppler in 6 cases and 12 sides in patient's superior and inferior gluteal area.
RESULTSSuperior and inferior gluteal artery originated from internal iliac artery. Several main perforators of large caliber were found in the paraischia and central portions of the gluteal muscle, its number was 10 - 15. The length of the vessels varies from 3 to 8 cm and their diameter from 1 - 1.5 mm. These significant perforators pass through the muscle itself and the fascial portion of the muscle to the overlying skin on the gluteal region. The dorsal branches of nervorum lumbalium perforate the deep fascia just above the iliac crest, lateral to the posterior superior iliac spine. If a nerve branch with a substantial diameter crosses the incision line, the nerve can be harvested within the flap. This nerve can be anastomosed to the anterior ramus of the lateral branch of the 4th intercostals nerve. In adult female, 3 - 5 perforators were located by ultrasound Doppler. They distributed in the triangle area among posterior superior iliac crest, the great trochanter and the coccyx.
CONCLUSIONSThe area and diameter of perforators of superior gluteal artery were relatively confirmed. It's possible to harvest the perforator flap without any muscle. It has the advantage of conventional myocutaneous flap with out of its disadvantages. It's easy to detect those perforator by ultrasound Doppler clinically. The nerve can be harvested and anastomosed simultaneously. Because the inferior gluteal area is a weight loading area, we suggested to use superior gluteal artery perforator flap. This flap can be transferred pedicled to treat sacral pressure sores or to be transferred freely for the breast reconstruction.
