1.Ventilation administration during fiberoptic bronchoscopy in neonates

Objective To assess ventilation administration during fiberoptic bronchoscopy (FB) in neonates. Methods Twenty-three neonates divided into two group (A group 12 neonates, B group 11 neonates) received FB. All were given pressure support ventilation (PSV)by a Y-like facility which connected to fiberoptic bronchoscope suction hole. In A group,after the tip of fiberoptic bronchoscope arrived at the carina, PSV was administrated. In B group, PSV was administrated in the entire process during FB, SpO2 and electrocardio were monitoring. Artery blood samples for blood gas analysis were obtained at four stages of just before FB,with the tip of the bronchoscope at the supralarynx,just before withdrawing bronchoscope out off trachea and within 20-30 minutes after FB. The arterial blood oxygen tension (PaO2), arterial blood carbon dioxide tension (PaCO2) and SpO2 just before FB served as baseline. The same indexes of other three stages were compared with the baseline. Results All 23 neonates were studied completely. When the tip of fiberoptic bronchoscope advanced from nostril to the supralarynx, SpO2, PaO2 and PaCO2 in two groups were similar to the baseline. In A group, when the tip below the glottis, cyanosis occurred, and SpO2 decreased significantly ( P<0. 01 ) in 11 cases (92%) by 25% ; When tip at the carina, after PSV, cyanosis disappeared, and SpO2 returned to the baseline level, PaO2 keep on the baseline just before withdrawing the bronchoscope out of the trachea. SpO2 ,PaO2 in all B group neonates keep on the baseline during FB. After the tip below the glottis,PaCO2 in all neonates of the two groups increased significantly ( P<0. 01 ), but returned to baseline within 20-30 minutes after FB. Conclusion FB can cause significant hypoxemia and hypercapnia in neonates. PSV through fiberoptic bronchoscope can be considered a safe and beneficial ventilation technique for maintaining oxygenation during FB.
2.Analysis of Perioperative Application of Antibiotics in GynecologicalⅡandⅢIncision Surgery of Our Hospital before and after Special Rectification

OBJECTIVE:To provide reference for perioperative rational application of antibiotics in gynecologicalⅡandⅢinci-sion surgery. METHODS:The perioperative patients of typeⅡandⅢincision surgery were collected from our hospital during Jul.-Dec. 2013-Jul.-Dec.2014,and then divided into pre-rectification group and post-rectification group. The prophylactic application of antibiotics in 2 groups were analyzed statistically. RESULTS:After rectification,rational rate of prophylactic medication in-creased from 5.38% to 89.69%(P<0.01);rational rate of prophylactic medication time increased from 11.83% to 91.75%(P<0.01);prophylactic medication duration decreased from (12 ± 2.1)d to(5.5 ± 1.9)d(P<0.01);after rectification,both total drug cost and drug cost per capita decreased significantly (P<0.01);postoperative infection rate showed no significant change before and after rectification (P>0.05). After rectification,usage and dosage of β-lactam antibiotics were in line with pharmacokinetics, Intravenous drip was adjusted from once a day to twice a day,and its rational rate increased significantly compared to before rectifi-cation(P<0.05). CONCLUSIONS:The antibiotics special rectification activities significantly promote the rationality of periopera-tive prophylactic application of antibiotics in gynaecologyⅡandⅢincision surgery of our hospital,reduce adverse drug reactions and resistant strains due to irrational use of drugs,and decrease the drug cost.
3.Study of follicular helper T lymphocytes related cytokines and receptors in children with systemic lupus erythematosus

Objective To study the change of the plasma concentrations of follicular helper T lymphocytes (TFH cells) related cytokines and receptors in children with systemic lupus erythematosus (SLE) .Methods The plasma concentrations of TFH-cells-related cytokine IL-21 and CXCL13 ,and cell surface expression of TFH-related receptor CXCR5 and IL-21R on CD19+B cells in 22 SLE children and 20 sex-and age matched control subjects were measured by enzyme-linked immunosorbent assay and flow cytome-try ,respectively .Results Plasma CXCL13 and IL-21 concentrations were significantly higher in SLE children than controls (all P<0 .01) .Cell surface expression of CXCR5 on B cells was significantly lower in SLE children than controls (P<0 .05) .However ,the cell surface expression of IL-21R had no significant changes between SLE children and controls(P>0 .05) .Conclusion The elevat-ed production of CXCL13 and IL-21 may be associated with the function of TFH for the immunopathogenesis in SLE children .
4.CFTR gene mutation in patients with congenital bilateral absence of vas deferens

Objective To study the frequency of cystic fibrosis transmembrane conductance regulator(CFTR)mutations in patients with congenital bilateral absence of vas deferens(CBAVD).Methods Eighty-five CBAVD patients were collected from May 2007 to May 2009.The diagnosis of CBAVD included azoospermia,normal of 4 sex hormone items,absence of seminal vesicle,normal volume of testicular and epididymis dilated siltation.And 85 normal fertile men served as controls.Genomic DNA was isolated from peripheral blood.The mutations of CFTR exons 10,11 were detected by PCR-single strand conformation polymorphism,and direct sequencing was performed on 85 cases of CBAVD and the control males.Results Of the 85 CBAVD,10 cases(11.8%)exhibited an abnormal CFTR gene mutation,with 4 cases I556V,2 cases M469V,and 1 case of E527N,A F508,L558S,S485C.No mutations were detected in 85 controls.There was a significant difference between the 2 groups(x2 =8.606,P =0.003).Conclusions CBAVD might be caused by the CFTR mutations.The frequencies and the spectrum of CFTR mutations might be different from those Caucasian population in the west country.
5.Factor analysis model for studying the influence of psychological stress on the endocrine network of reproduction axis.

In order to explore the influence of psychological stress on the reproductive axis, we started with the original data of biology from animal experiments, then we conducted the factor analysis of 6 important bio-regulatory factors and established the Factor Analysis Model for the endocrine network of reproduction axis under psychological stress. Based on the biological data obtained from animal experiments, it is found that, compared with the relationship seen in the factor analysis model in normal condition, the relationship among the biological factors is more complicated. The key factor, GnRH, has lost its strong regulating action on the subordinate factors, especially on E2; on the other hand, beta-EP has very strong regulating action on E2, and E2 is controlled mainly by beta-EP.
