1.Effectiveness of massage-presspoint methodin treating oligogalactia on women after delivery 5 months

Background: Feeding baby by mother\u2019s breast milk play an important role in primary health care. Objectives: To assess the effectiveness of improving secretion of mother\u2019s milk by massage-presspoint method through several clinical indicators. Subjects and method: 33 mothers suffering oligogalactia after 5 months of delivery were applied the method of massage-presspoint. The technique was performed on Giap Tich points from D1-TL3, one time per day with 30 minutes of each time and treatment period of 20 days. The opened clinical and comparing pre-post treatment methods were used in the study. Results: 50% of good results, 36,67% of better and 13,33% of failure.Conclusion: The method of massage \ufffd?presspoint can improve the secretion of mother\u2019sbreast milk, which were evidenced through some clinical indicators measuring on mothers and babies feeding mother\u2019s milk.

2.Effect of electroacupuncture at Shaoze (SI 1) on breast milk volume and composition in postpartum hypogalactia.

OBJECTIVE:
To observe the effect of electroacupuncture at Shaoze (SI 1) on breast milk volume and nutrient composition in postpartum hypogalactia.
METHODS:
A total of 62 women with postpartum hypogalactia were randomized into an observation group (31 cases, 3 cases dropped off) and a control group (31 cases, 3 cases dropped off). Electroacupuncture was exerted at Shaoze (SI 1) in the observation group and at Shangyang (LI 1) in the control group, 10 Hz in frequency, 30 min each time, once a day. The treatment for 5 times as one course and 3 courses of treatment were required totally. Separately, before treatment and at the end of 1 course and 3 courses of treatment, the breast milk volume was measured and the composition was analyzed. The breast milk volume, nutrient composition (lactose, fat and protein), breast milk energy and density were observed in the two groups. After treatment, the clinical effect was compared between the two groups.
RESULTS:
In the observation group, after 1 course of treatment, the breast milk volume and the values of lactose and protein in the breast milk were increased as compared with those before treatment, the breast milk energy and density were increased as compared with those before treatment (all <0.05). After 3 courses of treatment, the breast milk volume and the values of nutrient composition (lactose, fat and protein) were improved as compared with those before treatment, and the breast milk energy and density were increased obviously as compared with those before treatment in the observation group (all <0.05). In the control group, after 1 course and 3 courses of treatment, only breast milk volume was increased obviously as compared with that before treatment (both <0.05). After 1 course of treatment, the nutrient composition (lactose and protein), breast milk energy and density in the observation group were higher than those in the control group (all <0.05). After 3 courses of treatment, the levels of breast milk volume, nutrient composition (lactose, fat and protein), breast milk energy and density in the observation group were all higher than those in the control group (all <0.05). The effective rate was 92.9% (26/28) in the observation group, which was higher than 17.9% (5/28) in the control group (<0.05).
CONCLUSION
Electroacupuncture at Shaozhe (SI 1) increases breast milk volume in patients with postpartum hypogalactia and promotes the production of the nutrient composition in breast milk, as well as the breast milk energy and density.

OBJECTIVETo study on clinical role of acupuncture at Tanzhong (CV 17) for treatment of postpartum hypolactation and to provide clinical basis for indications of acupoints.
METHODSThis was a single blind randomized controlled multi-center study by Beijing obstetrical and gynecological hospital, Beijing Mother and Child health institute and Haidian Mother and Child health institute, 276 cases of postpartum hypolactation were randomly divided into an acupuncture group and a Chinese drug group. The acupuncture group were treated with acupuncture at Tanzhong (CV 17) and the Chinese drug group with traditional drug Tongre Decoction. Degree of breast engorge, lactating volume, prolactin, neonate body weight, artificial feeding frequency and volume, urination frequency and crying time of neonate were recorded for comparison of therapeutic effects.
RESULTSAfter treatment, degree of breast engorge, lactating volume, neonate body weight, artificial feeding frequency and volume, urination frequency and crying time of neonate were significantly improved in the two groups, with no significant difference between the two groups.
CONCLUSIONAcupuncture at Tanzhong (CV 17) can effectively promote lactation with a same therapeutic effect as the traditional Chinese drug Tongru Decocnon.

5.Research on Tuina treatment for postpartum hypolactation.

In this paper, the TCM typing of postpartum hypolactation is introduced and the manipulation and duration of Tuina treatment for postpartum hypolactation are summarized, and the effectiveness index used in Tuina treatment is proposed. It is held that the Tuina therapy will be widely used for treatment of the postpartum hypolactation, and the convenient and effective Tuina manipulation will promote the development of postpartum nursing and will be used in gynecology department and enlarge the therapeutic range of the Tuina therapy.

OBJECTIVES:To determine the prevalence of breastfeeding-related skin problems as well as the knowledge, attitude and practices on breastfeeding among mothers in Barangay Batasan Hills, Quezon City.
METHODOLOGY: This is a cross-sectional study conducted at the Superhealth Center in Barangay Batasan Hills, Quezon City, Metro Manila, Philippines. Included in the study were women of reproductive age who had breastfed at least on child in the past or who were breastfeeding at the time of the survey. An exploratory phase was conducted to determine the extent of the problem. Data gathered from this phase helped in the formulation of a questionnaire. A cross sectional survey was then conducted to determine the common reasons for breastfeeding, the average number of children being breastfed by the participants, the frequency and duration of breastfeeding per child, the hygienic practices of breastfeeding women, the prevalence of breastfeeding-related skin conditions, health seeking behavior of those who experienced the skin conditions, and the common reasons for discontinuation of breastfeeding.
RESULTS: Ninety-four women participated in the survey. The most common reason for breastfeeding given was, "breast milk is good for the health of the baby" (59.6%). Most of the women learned about breastfeeding from their mothers (41.5%). Most of the participants were able to breastfeed one child (43.6%). The average time spent for breastfeeding one child was three hours per day. Most of the mothers used damp cloth/cotton balls (38.3%) or tepid water (34.0%) to prepare their nipple prior to breastfeeding. Majority of them (72.3%) did not experience any breastfeeding-related skin problems. Among the 27.7%, who experienced skin problems, nipple cracks or fissures (15.0%), breast engorgement (10.6%), and nipple pain (4.2%) were the top three reported skin problems. None of the respondents discontinued breastfeeding due to breastfeeding-related skin problems. A minority of interviewed women (7.0%) discontinued breastfeeding because they were unable to express milk.
CONCLUSION: The proportion of women with breastfeeding-related skin problems in Barangay Batasan Hills, Quezon City was 27.7%. More than half of those who experienced such problems still continued to breastfeed without any intervention. There was a low rate of discontinuation of breastfeeding (7.4%) among the participants of this study. This finding may be due to the prevalent support of the respondents' mothers and relatives as well as the active promotion of breastfeeding by the local health units and centers in the barangay.

7.Multicentral randomized controlled studies on acupuncture at Shaoze (SI 1) for treatment of postpartum hypolactation.

OBJECTIVETo explore effect of acupuncture at Shaoze (SI 1) for treatment of postpartum hypolactation.
METHODSMulticentral randomized controlled and single blind clinical trial was adopted, and 276 cases were divided into a treatment group and a control group, 138 cases in each group. The treatment group were treated with electroacupuncture (EA) at Shaoze (SI 1) and the control group with EA at Shangyang (LI 1). After treatment of 2 courses, the therapeutic effects and changes of cumulative score of TCM symptoms, mammary filling degree, lactation amount, prolactin level were evaluated and investigated.
RESULTSThe cured and markedly effective rate was 97.8% in the treatment group and 24.3% in the control group with a significant difference between the two groups (P < 0.05). The treatment group in improvement of the cumulative score of TCM symptoms and the mammary filling degree, and increasing the lactation amount and the prolactin level were better than the control group (P < 0.01).
CONCLUSIONAcupuncture at Shaoze (SI 1) has obvious therapeutic effect on hypolactation.

8.Effects of Maidang Rutong granule on l-dopa-induced galactozemia in rats.

OBJECTIVETo study galactagogue effect of Maidang Rutong granule on the lactation rats.
METHODThe experiments were designed to observe the efficiency of Maidang Rutong granule on lactescence, serum prolactin, and morphology of mammary gland with rat galactozemia model established by injecting l-dopa.
RESULTMaidang Rutong granule showed significant enhancement for lactescence and the offspring's body weight. It could antagonize the decrease of serum prolactin and the atrophy of mammary gland induced by l-dopa.
CONCLUSIONMaidang Rutong granule exhibited significant galactagogue effect on the l-dopa-induced galactozemia in rats.

OBJECTIVETo investigate and analyze the nutritional status of infants, preschool and primary school students, pregnant and lactating women in Wenchuan earthquake disaster area after 3 months.
METHODSIn August 2008, the nutritional and health status information of special population were collected and evaluated using the questionnaires, anthropometric and hemoglobin concentration measurements from four settlements of villages and towns in Mao and Mianzhu Country of Sichuan and Kang Country and Wudu District in Gansu provinces. A total of 236 infants aged below 36 months, 48 preschool children, 368 primary students, 32 pregnant women and 72 lactating women were investigated. Principal investigator indexes included the low body weight, growth retardation, anemia prevalence, two-week prevalence of diarrhea and respiratory disease, food intake and nutrition-related diseases, the percentage of patients morbidity of 36 months infants, preschool and primary school students; the prevalence of anemia, the prevalence of nutrition-related diseases of pregnant and lactating women.
RESULTSThe stunting prevalence was 14.6% (34/236) and the anemia prevalence was up to 40.1% (61/236) among infants younger than 36 months. Besides, the percentages of infants and young children suffered from the respiratory-infected disease and diarrhea within recent two weeks were 40.4% (95/236) and 30.2% (71/236) respectively. The percentage of low body weight of preschool children was 14.6% (7/48) and the growth retardation and anemia prevalence was 14.6% (7/48) and 39.6% (19/48), respectively. Among primary students, 6.3% (23/368) showed growth retardation and 12.2% (45/368) were anemia. The prevalence of anemia status of pregnant women and lactating mothers were 53.9% (17/32) and 24.4% (18/72) respectively. The main food composition of 45.8% (33/72) lactating women were grain and vegetables, 29 (40.3%), 32 (44.4%), 28 (38.9%) and 53 (73.6%) lactating women did not have animal originated (including meat, aquatic, livestock and poultry products) food, eggs, beans and their products, milk and dairy products, respectively.
CONCLUSIONSpecial population lived in disaster area have suffered different degree of malnutrition. The main dietary pattern was vegetable food. The consumption of meat, eggs, milk and milk products was relatively insufficient.

10.Clinical observation on the treatment of post-cesarean hypogalactia by auricular points sticking-pressing.

OBJECTIVETo explore the effect of auricular points sticking-pressing (APSP) in treating post-cesarean hypogalactia (PCH).
METHODSA randomized, controlled, single-blinded clinical trial on 116 patients with PCH was carried out. They were equally assigned to the treatment group and the control group. The treatment group received APSP, with the pellets pressed for 4 times daily, while the control group was only asked to do lactation to meet infant demand. The therapeutic efficacy and the changes in scores of traditional Chinese medicine (TCM) syndrome, volume of milk secretion, supplementary feeding and serum level of prolactin (PRL) in the two groups were estimated and compared after the patients had been treated for 5 days.
RESULTSThe cured and markedly effective rate in the treatment group was 89.7%, which was significantly higher than that in the control group (27.6%, P<0.05), 95% CI (0.1543, 0.2527). The improvement of TCM syndrome, elevation of milking volume, decrease of the supplementary feeding and increase of PRL level revealed in the treatment group were all superior to those in the control group, showing statistical significance (P<0.01).
CONCLUSIONAPSP shows an apparent efficacy in treating PCH and is worthy of application in clinical practice.
