1.Advance of glitazones on hypoglycemic effect

Chemically Glitazones belong to Thi-azolidinediones. Glitazones enhance the sensitivity of target tissues to insulin and reduce insulin resistance. They, celled insulin sensitizers, are a new class of oral hypoglycemics. These agents can't reduce the blood glucose values either in normal animal or I type diabetic animal, but do have good effects on spontaneous diabetic animal and artificial insulin resistance animal. The research on insulin resistance needs euglycemic clamp for determination and the main index of assessing euglycemicclamp is glucose infusion rate in stable state. So far, the hypoglycemic mechanism of glitazones has been believed to be associated with PPARs(Peroxi-some Proliferator-Activated Receptors ). Gli-taziones act on PPAR Y, play an indirect part in insulin signal transduction and enhance the action of insulin. However, the precise mechanism still remains unknown.

3.Bone morphogenetic protein-4 compounded with platelet-rich plasma promotes bone healing

BACKGROUND:Studies have shown that bone morphogenetic proteins play a key role in skeletal development. Platelet-rich plasma alone in animal or clinical trials cannot significantly promote bone graft healing.
OBJECTIVE:To investigate the osteogenesis effect of bone morphogenetic protein-4 compounded with platelet-rich plasma the bone defect area.
METHODTwenty-four New Zealand white rabbits were selected to establish maxil ary bone defect models, and then were randomly divided into four groups, six rats in each group. Group A was blank control group;Group B wasβ-tricalcium phosphate (0.1 g)+Bio-gide group;group C wasβ-tricalcium phosphate (0.1 g)+Bio-gide+platelet-rich plasma (1 mL) group;and group D wasβ-tricalcium phosphate (0.1 g)+Bio-gide+platelet-rich plasma (1 mL)+bone morphogenetic protein-4 (5μg). Gross observation, histological observation and imaging analysis were performed for analysis of new bone formation at weeks 4, 8, 12 after modeling.
RESULTS AND CONCLUSION:After 4 weeks, group D had more new bone and vessels formed in the bone defect area than the group B (P<0.01);however, the amount of new bone was higher in the group B than the group A (P<0.01). After 4, 8, 12 weeks, the amount of new bone in the bone defect area was higher in the group D than the groups B and C (P<0.01), and lowest in the group A (P<0.01). In theβ-tricalcium phosphate scaffold, platelet-rich plasma combined with bone morphogenetic protein can significantly promote the healing of bone defects.

5.Expression and clinical significance of insulin-like growth factor binding protein 5 in gastric adenocarcinoma

Objective: To investigate the expression and prognostic significance of insulin-like growth factor binding protein 5 (IGFBP5) in gastric adenocarcinoma (GAC). Methods:IGFBP5 expression in tissue samples from 236 GAC patients was analyzed us-ing immunohistochemical methods. These patients had undergone surgical resection between 20003 and 2006 in Sun Yat-Sen Universi-ty Cancer Center. The relationship between IGFBP5 expression and clinicopathological factors in the 236 GAC patients was analyzed using Pearson correlation analysis. The significance of IGFBP5 in predicting the survival status of these patients was analyzed using Ka-plan-Meier survival analysis and Cox's proportional hazards regression model. Results:Immunohistochemical staining data indicated that IGFBP5 expression was significantly decreased in 159 of the total GAC cases (67.4%). Of the 62 cases with well-and moderately differentiated GAC, 31 (50%) exhibited reduced IGFBP5 expression. Of the 174 cases with poorly differentiated GAC, 128 showed re-duced IGFBP5 expression. Reduced IGFBP5 expression was also observed in female patients and in patients with tumors over 5 cm in size or with poorly differentiated tumors (P<0.05). The reduced expression of IGFBP5 was common in the tumors that were staged as T3+4a/b andⅢ/Ⅳ(P<0.05). Kaplan-Meier survival analysis revealed that the reduced expression of IGFBP5 was associated with poor prognosis in GAC patients (P<0.001). Cox regression analysis identified IGFBP5 expression as an independent prognostic factor for the overall survival of these cancer patients (HR=1.897, P=0.029). Conclusion: IGFBP5 expression is reduced in GAC tissues, and IGFBP5 independently predicts an unfavorable prognosis in GAC patients.

Tissue was obtained from 14 aborted human fetuses, ranging from 13-32 weeks of gestation (wg). The crown-rump length (CR) ranged from 8.3-33 cm. Frontal sections of the specimens were prosessed for SEM and observation were focused on the areas adjacent to the middle part of the calcarine fissure.At 13 wg (CR 8.3 cm), the visual cortex (area 17) was composed of five zones: viz., the ventricular zone, the subventricular zone, the intermediate zone, the cortical plate and the marginal zone. These five zones showed a series of transformations with increasing age. 1) The ventricular zone became progressively thinner, mitotic activity of the ventrieular cells decreased progressively and finally the ventricular ceils differentiated into a single layer of ependymal cells. 2) The subventricular zone and the inter mediated zone were replaced by fiber bundles of white matter. 3) The cortical plate increased in width, exhibited the greatest growth rate, and became differentiated. At 21 wg (CR 20cm), the lower part of the cortical plate first gave rise to laminae VI and V. At 23 wg (CR 22cm), lamina Ⅳ was established in the middle part of cortical plate. At 26 wg (CR 25cm), laminae Ⅲ and Ⅱ could be identified in the upper part of cortical plate. 4) The marginal zone transformed into lamina Ⅰ at its original site.
7.Effects of Intensive Induction Chemotherapy on Consolidation Radiotherapy in Young Children with High-risk Hodgkin Lymphoma

OBJECTIVE:To investigate the safety of induction chemotherapy and the implementation method of consolidation radiotherapy in young children with high-risk Hodgkin lymphoma(HL),by evaluating the effects of intensive induction chemother-apy on consolidation radiotherapy. METHODS:Six pediatric patients with high-risk HL received low-dose involved field radiation therapy (IFRT) via volumetric modulated arc therapy (VMAT) following 6 cycles of intensive chemotherapy [Ara-C/VP16 (cyto-sine arabinoside+etoposide),ABVE-PC(adriamycin+bleomycin+leurocristine+etoposide+metacortandracin+cyclophosphamide)and CHOP(cyclophosphamide+leurocristine+adriamycin+ prednisone)in turn]. Therapeutic efficacy and toxic effects were evaluated af-ter treatment. RESULTS:Intensive induction chemotherapy and consolidation radiotherapy were acceptable by young children,and mild acute or subacute toxic reaction were observed. Intensive induction chemotherapy didn’t affect toxic effects of consolidation ra-diotherapy. In this regimen,the cumulative doses of bleomycin and adriamycin were 20 mg/m2 and 270 mg/m2,respectively. VMAT optimized plan to ensure that the dose that involved organs received was safe. In the patient with mediastional radiation therapy,the mean lung and heart doses were 525.6 cGy and 503.9 cGy,respectively. CONCLUSIONS:It is safe to give high-risk HL young children 6 cycles of intensive induction chemotherapy(Ara-C/VP16,ABVE-PC and CHOP in turn)before radiotherapy. It is feasi-ble to conduct IFRT with 18-20 Gy and an additional 20-25 Gy boost,1.5-1.8 Gy/fraction. VMAT deserves to be advised.
8.Effect of clinical nursing pathway on allergic rhinitis in patients with subcutaneous immunotherapy

Objective To explore the effect of clinical nursing pathway(CNP)on allergic rhinitis in patients with subcutaneous immunotherapy.Methods One hundred and twenty patients with allergic rhinitis treated with subcutaneous immunotherapy were equally randomized into the observation group and the control group.The former group were managed with clinical nursing pathway and the latter received only routine nursing care.The curative effect of subcutaneous immunotherapy was compared between the two groups including local adverse reactions,treatment compliance,patients' satisfaction and quality of life.Results The scores by symptom assessment at time points of half a year,years 1,2 and 3 showed the symptoms at different time points between the groups were significantly different by the comparison between time and the interactive effect of main effect(F=678.24,P<0.01),and the symptoms in the two groups were both alleviated with the extension of treatment due to the compared time effect at F=47.16, P<0.01.The scores on the symptoms in the observation group were significantly lower than those of the control group at different time points(P<0.01).The experiment group was better than the control group in terms of local adverse reactions,treatment compliance, quality of life and patient's satisfaction(P<0.01)Conclusion Clinical nursing pathway can improve patient's compliance,improve immunotherapy,reduce adverse reactions and improve the quality of life of patients as well as patients' satisfaction.
9.Chronic inhibition of cilazapril on pulmonary vascular and myocardial cell proliferation in hypoxic rats

group A by turns. (3) The concentrations of plasma endothelin-1 (ET-1) and angiotensin converting enzyme (ACE) were significantly higher in group B than that in group A. However, the ET-1 and ACE were significantly lower in group C than those in group B. (4) The ET-1 and ACE had a significant positive correlation with R/L+S, mPAP and PI, respectively. The multivariate linear regression analysis revealed that ET-1 and ACE were major factor affecting PI. CONCLUSION: The pulmonary vascular and myocardial structural remodeling are one of the pathogenesis accompanied with excessive cell proliferation in hypoxic pulmonary hypertension (PH). Cilazapril effectively prevents and treats the hypoxic PH by inhibiting cell proliferation and structural remodeling of pulmonary circulation, as induced by ET-1 and ACE.
