1.Effect of hand intrinsic muscle exercise on the postoperative recovery of fine movement in children with hand injury

Objective To study the effect of hand intrinsic muscle exercise on postoperative recovery of fine movement in children with hand injury.Methods From January 2009 to December 2011,64 children with hand injury were randomly divided to exercise group and control group equally.Hand intrinsic muscle exercise and rehabilitative care were conducted in the exercise group while traditional rehabilitative therapies in control group.After 3-month therapy,MMDT(Minnesota Manual Dexterity Test)and PPT(Purdue Pegboard Test)were conducted to evaluate the functional recovery of hands.Result The results of MMDT and PPT were obviously better in the exercise group than in the control group(P<0.01).Conclusion Hand intrinsic muscle exercise may be effectively in promoting the recovery of fine movement in children with hand injury.
2.Allelic polymorphism of human leukocyte antigen-DRB1~* 04 in Chinese Han populations

Objective To investigate the allelic polymorphism of Human Leukocyte Antigen(HLA)-DRB1*04 in Southern and Northern Chinese Han populations.Method A total of 104 and 168 samples of HLA-DRB1*04 alleles from Southern and Northern Chinese Han populations respectively were genotyped at high-resolution by PCR-SBT,and then the DRB1*0406/0449 ambiguous allele pairs were identified via high-resolution PCR-SSP.The allelic distribution of DRB1*04 in Southern and Northern Chinese Han populations were compared with other different populations by chi-square test.Results All 56 ambiguous allelic pairs of DRB1*0406/0449 were identified as DRB1*0406.A total of 5 alleles were observed in Southern Chinese Han population and the most frequent alleles were DRB1*0405(47.32%),DRB1*0406(24.24%)and DRB1*0403(16.35%).There were 10 alleles tested in Northern Chinese Han population,and the major alleles were DRB1*0405(40.48%),DRB1*0406(18.45%) and DRB1*0403(17.26%).The overall distribution of DRB1*04 alleles in Southern and Northern Chinese Han differed significantly from that of the Caucasian,African American,Hispanics,and Asian and Pacific Islanders,but not from the Korean.Conclusions The overall distribution of HLA-DRB1*04 alleles differed significantly between Southern and Northern Chinese Han populations,but the major alleles DRB1*0405,DRB1*0406 and DRB1*0403 showed the same distribute characteristic.The distribution of DRB1*04 alleles in different populations has their own characteristics.
4.Effects of Recombinant Human Growth Hormone on Hypoalbuminemia in Cirrhotic Rats after Partial Hepatectomy

Objective To investigate the effects of rhGH on hypoalbuminemia in cirrhotic rats after partial hepatectomy. Methods Thirty rats were randomly divided into normal control group ( n =6), liver cirrhosis group (LC group, n =6), liver cirrhosis and hepatectomy group (LCH group, n =6), PN (parenteral nutrition) group ( n =6, given PN after hepatectomy) and rhGH+PN group ( n =6,given rhGH and PN after hepatectomy). Liver function and blood glucose were measured. The expression of ALB mRNA was detected by RT PCR. Liver Ki67 immunohistochemistry was studied. Results Compared with PN group, serum ALP was lower; serum ALB and blood glucose were higher in rhGH+PN group. The expression of hepatic ALB mRNA was higher, and hepatic Ki67 labeling index was higher as well in this group. Conclusion rhGH can improve hypoalbuminemia after partial hepatectomy in cirrhotic rats with partial hepatectomy.
5.Lichtenstein tension-free hernioplasty repair in adult patients with inguinal incarcerated hernia

Objective To investigate the preoperative management and the clinical effeciency of Lichtenstein tension-free hernioplasty in adult patients with inguinal incarcerated hernia.Methods Clinical data of 86 patients with inguinal incarcerated hernia were analyzed retrospectively.Hernia was repaired with Lichtenstein tension free after reposition.Results There were 59 male patients and 27 female patients with median age of (63 ± 18) years.There were 8 patients with liver cirrhosis.The operation was performed successfully in all patients.Segmental bowel resection with end-to-end anastomosis was performed in 38 emergency cases.Operative time was 20-120 min,with an average time of 54 min.The postoperative hospitalization was 5-17 d,with an average of 8 d.There were 7 cases of skin ecchymosis at the scrotum,there were no intestinal perforation,hepatic encephalopathy and upper gastrointestinal hemorrhage after operation.In early series of 24 cases without drainage tube left in place,there were 10 cases of fat liquefaction,10 cases of hydrops of hernial sac,6 cases of seroma and 3 cases of wound infection after operation.After 12 to 48 months of follow-up,there was no mortality after 2 years,no hernia recurrence.Conclusions Tension free repair in the treatment of incarcerated inguinal hernia is safe and feasible.

7.Impact stilbene glycosides and PNS Concomitantly on AD model PC12 cell injury

Objective To investigate stilbene glycosides(TSG) and PNS concomitantly on PC12 cell survival rate of Alzheimer's disease.Methods The nerve cells that were seeded on the two culture plates were cultured for 1 day after the removal of primary culture fluid.In addition to the blank group, the model group and drug compatibility group were added 5 μl Aβ25-35 perpore to induced PC12 cell damage.To established AD cell damage model after exposure to the circumstances for 24 hours.Uniform design and factorial design were used respectively.After 1 d, using MTT method in ELISA analyzer measured the OD value of each pore, and calculating the survival rate of cells.Results The uniform design results showed that the cell survival rate was significantly linear with TSG and PNS (P<0.05).From the equation, The higher the dose, the higher the cell survival rate.In this experimental condition, TSG and PNS respectively 50 mg/L, 200 mmol/L achieved the highest cell survival rate.2×2 factorial design experiments showed that, compared with the model group, the cell survival rate of TSG-PNS group (74.46% ± 2.06% vs.65.42% ± 1.42%) increased (P<0.05), but there was no interaction between the two groups (P=0.053).This showed that the combination of the two drugs in this dose has a protective effect on AD damage.Conclusion The compatibility of total saponins of two stilbene glucoside and three seven combination has the synergistic effect of anti AD damage.
8.Re-endothelialization after placement of drug-eluting stents in patients with CHD

Objective To investigate the characteristics of coronary vessel re-endothelialization after placement of drug-eluting stents (DES), and to provide clinical evidence for the double anti-platelet treatment. Methods Optical coherence tomography (OCT) was performed in 43 patients in 1 year after DES implantation. Characteristics of re-endothelialization and percentage of neointimal coverage of stent struts were evaluated by OCT. Results The rate of stent struts intimal coverage was 90.70%, and the remain was lack of endothelial coverage; The ratio of neointimal thickness (NIT) between 0-99, 100-199 and above 200 microns was 19.92%, 37.55% and 42.53%, respectively. The rate of neointimal coverage was higher and the degree of neointimal hy-perplasia was more extensive in patients with DM and in patients with ACS than those of patients without DM and of patients with stable angina pectoris. Conclusion One year after stent placement, most of the stent struts were covered with neointima and few struts obtained poor coverage of endothelial. DM and ACS may be impact factors for the progress of re-endothelialization after DES placement.
9.Hemodialysis membrane:its biocompatibility and application in severe pancreatitis

BACKGROUND: Continuous blood purification can remove cytokines and inflammatory mediators, maintain homeostasis and prevent the occurrence of multiple organ dysfunction syndrome in patients with severe pancreatitis, which has become the main therapy for severe pancreatitis. Since the hemodialysis technology began to be applied clinical y, the biological and physicochemical properties of hemodialysis membrane materials have been studied. A variety of hemodialysis membranes have been developed in order to improve the biocompatibility and anticoagulant effect in vitro. OBJECTIVE: To investigate the application effect of hemodialysis membranes on severe pancreatitis. METHODS: Ten Wistar rats were selected to make rat models of severe pancreatitis and then were randomized into two groups (n=5 per group): homophone membrane group and polysulfone membrane group. Hemodialysis- related biochemical parameters were detected in the two groups. RESULTS AND CONCLUSION: Compared with the homophone membrane, ultrafiltration coefficient, creatinine clearance, blood urea nitrogen clearance, phosphorus clearance, number of circulating endothelial cel s, and levels of plasma nitric oxide and asymmetric dimethylarginine were significantly lower in the polysulfone membrane group (P < 0.05). Vitamin B12 clearance and amount of pre-congestion increased significantly in the polysulfone membrane group as compared with the homophone membrane (P < 0.05). These findings indicate that the polysulfone membrane for hemodialysis has good biocompatibility, and keeps a stable environment in vivo for severe pancreatitis patients.
10.Development of the fiscal input model for public hospitals and calculation of subsidy standard

Objective To explore a scientific and reasonable fiscal input mechanism for public hospitals, in order to fully leverage the policy guidance and efficiency of such funding.Methods With literature review, expert consultation and demonstration, a basic subsidy model for public hospitals was established.According to the past operation data of 4 public hospitals in Baoshan district of Shanghai, the study figured out specific subsidy standards.Results The basic subsidy for public hospitals should be determined according to the number of approved beds, the number of outpatients and emergency visits, hospital bed days, surgeries, key services, and the quality and efficiency of work.In Baoshan district, the standard reference value of subsidy for each approved bed, each outpatient and emergency visit, each bed-day, each surgical operation is 42 096 yuan, 27.9 yuan, 104.9 yuan and 244 yuan respectively.The standard value of subsidy is 100 yuan per bed for critically ill inpatients.For patients under clinical pathway management, the subsidy is 300 yuan per case, and for hospital maternal care, it is 150 yuan per person.Conclusions The basic subsidy model for public hospitals has overcome the shortcomings of fiscal input based on hospital scale or hospital workload, and established an incentive mechanism to promote the implementation of key services.These measures can improve the operation mechanism of public hospitals and encourage them to play their role of public welfare as designed.
