Objective:To observe effects of Yangxin Tongmai Tablet on correlative indexes of insulin resistance syndrome.Methods:67 cases of insulin resistance syndrome were randomly divided into treatment group(Yangxin Tongmai Tablet group,n=33)and control group(dimethyldiguanide group,n=34).They were treated for 8 weeks.Changes of blood glucose(FPG),insulin(FINS),insulin sensitive index(ISI),insulin resistance index(IRI),islet ? cell function index(HBCI),serum total cholesterol(TC),serum triglyceride(TG),high density lipoprotein-cholesterol(HDL-C),low density lipoprotein-cholesterol(LDL-C),systolic pressure (SBP),diastolic pressure(DBP)and body mass index(BMI)before and after treatment were observed.Results:After treatment, FPG,FINS,IRI,TC,TG,LDL-C,SBP,DBP,BMI and clinical symptom scores decreased,and ISI and HDL-C increased significantly in two groups(P
2.Efficacy of Maxing Shigan Decoction combined with Western medicine for pneumonia in children: a systematic review and meta-analysis.

Objective: To systematically evaluate the clinical effects of Maxing Shigan Decoction (MXSGD), a compound traditional Chinese herbal medicine, combined with Western medicine on pneumonia in children. Methods: In this study, the relative trials published from 1994 to 2008 were searched in Chongqing Weipu Database, Chinese Journal Full-text Database, Wanfang database, Chinese Biomedical Literature Database and other electronic database by using the method of Cochrane systematic review. At the same time the information from related journals, professional data and network were hand-searched. The methodological quality of the included trials was assessed by two evaluators, and homogeneous evaluation by meta-analysis was performed. Statistical analysis of clinical data was performed by using RevMan 4.2.7 software provided by the Cochrane Collaboration. Results: A total of 146 reports were found, while only eight randomized controlled trials met the inclusion criteria. The methodology quality of the reports included in the study was evaluated by the Jadad scale, and the specific random method, allocation concealment, blinding and intention-to-treat analysis were not described in all of the eight trial reports. As MXSGD combined with Western medicine group (treatment group) was compared with Western medicine group (control group), the meta-analysis indicated that the odds ratio for the total effective rate was 4.06, and the 95% confidence interval was from 2.63 to 6.27. MXSGD combined with Western medicine was good at increasing the total effective rate as compared with Western medicine, and the difference was statistically significant (P<0.000 01). Conclusion: MXSGD combined with Western medicine can improve clinical symptoms and increase the total effective rate of the patients with pneumonia in children. However, its clinical effects should be further identified by high quality, multicenter and randomized controlled trials with large-scale design.
3.Selection of implants and repair in posterolateral fractures of the tibial plateau

BACKGROUND:Tibial plateau fractures involving the posterolateral articular surface present a great challenge due to its complex anatomy.The posterolateral corner complex and the proximity of the common peroneal nerve restrict both the exposure of the joint surface and the ability to distract across the joint using a varus force.OBJECTIVE:To analyze the anatomy,morphology and classifications of the posterolateral fractures of the tibial plateau,and compare different surgical approaches and fixation materials.METHODS:A computer-based research of PubMed and CNKI databases was performed for the relative literatures published from 1990 to 2016 using the keywords of tibial plateau,posterolateral fragment,fixation in English and Chinese,respectively.The anatomy,morphology and typing of posterolateral fractures of the tibial plateau were analyzed and different surgical approaches and fixation materials were compared.RESULTS AND CONCLUSION:Posterolateral fractures of the tibial plateau should be treated in accordance with the intra-articular fracture principle,such as repairing joint surface to normal morphology,restoring the lower limb alignment,and maintaining joint mobility painlessly.Fractures are clarified according to Schatzker,AO,and three-column classification systems,but still need to be improved.To choose a proper surgical approach mainly depends on the fracture pattern.Biomechanics tests suggest that posterolateral locked plate exhibits excellent biomechanics characters,and can avoid the reduction loss caused by the shear force of fracture fragments.

Objective: To investigate the biological phenotype and cell cycle distribution characteristics of nasal and/or nasopharyngeal epithelia of TgN(p53mt-LMP1)/HT mice.Methods: The biological phenotype changes(mainly the incidence of precancerous lesion) in nasal and nasopharyngeal epithelial tissues of the generation G4 of TgN(p53mt-LMP1)/HT mice aged 5,11,15 and 18 months were determined by H-E staining,and the cell cycle characteristics of nasal and nasopharyngeal epithelia detected by flow cytometry(FCM).Results: The incidences of precancerous lesion in the nasal and nasopharyngeal epithelia were 0,50%,60% and 75% respectively in the 5,11,15 and 18 mos groups of the positively expressed transgenic mice,and 0 in the four age groups of the negatively expressed ones.Compared with the negatively expressed transgenic mice,the number of nasal or nasopharyngeal epithelial cells was markedly decreased in the G0/G1 phase,but obviously increased in the S and G2/M phases,with the proliferation index(PI) significantly enhanced(P

Background and purpose:The development of nasopharyngeal carcinoma in rat and transformation of embryo nasophryngeal epithelia induced by N,N'-dinitrosopiperazine (DNP) were interfered by Yiqijiedu formular medicine via inhibiting telomerase. We investigated the impact of Yiqijiedu formula on precancerous lesion of nasal and nasopharyngeal epithelia in TgN (p53mt-LMP1) / HT mice induced by DNP and on its expression level of c-jun and p16 genes. Methods:The fourth generation TgN (p53mt-LMP1) / HT mice aged 5 months were randomly divided into 2 groups:(1)TgN(p53mt-LMP1)/HT cancerous lesion-inducing group (group A) was treated by normal saline with 15ml/kg, and Chinese traditional medicine treating group (group B) was treated by Yiqijiedu formular drug with 12.91 g/(kg?d). C57BL/6J wild-type mice were chosen as negative control group (group C) treated by normal saline with 15ml/kg. Epithelial tissue samples were taken from nasal cavity and nasopharyngnx of mice respectively for pathohistological evaluation by H-E staining procedures, and expression level of c-Jun and p16 in these tissue sampleswere determined by immunohistochemical methods and western blot. 6.Observation of curative effect of avulsion fracture of anterolateral ligament of knee joint

Objective To explore the clinical effect of periarticular knee fractures combined with anterolateral ligament (ALL) avulsion fracture.Methods From June 2014 to November 2015,13 patients with ALL avulsion fracture in periarticular knee fractures were fixed by the star plate,cannulated or screws suture anchor,including 9 male and 3 female with the mean age of 39.5 years (range from 28 to 51 years).6 patients were diagnosed distal femoral fracture,4 tibial plateau fracture,and 3 tibial intercondylar eminence avulsion fracture which all combined with ALL avulsion fracture.Postoperatively,all patients were treated with the adjustable knee brace for 3 months.Results All patients were followed up for 4-20 months,the mean follow-up time was 13.5 months.The average surgical time was 130 min (range from 90 to 210 min).In all 13 patients,ALL was found and identified.Bone union was obtained in all patients,and the bone union time was 11.4 weeks (10-12 weeks).7 cases were fixed with star plate,5 cases were fixed with hollow screw,and 1 case was fixed with anchor.All follow-up patients underwent anteroposterior and lateral X-rays.Pain,swelling and functional recovery of the knee joint were observed.Patients were assessed postoperatively with International Knee Documentation Committee (IKDC) score,Lysholm score,and Tegner activity scale to evaluate the clinical effect.At the last follow-up,all patients had full range of motion of the knee joint which from 0° to approximate 120°.The Lysholm,objective IKDC were 86.5 and 84.6 respectively.Among the 13 patients evaluated,9 patients were graded A,and 4 patients were graded B by subjective IKDC.The Tegner activity scale at the last follow-up was 6.7.Incision of poor healing in 1 case,and got healed after 32 d cleaning dressing.Conclusion This study confirmed the presence of the ALL.The fixation of ALL avulsion fracture in periarticular knee fractures can be an effective procedure at a minimum follow-up of 13.5 months.

8.Effects of allogeneicversus autologous serum on the proliferation of bone marrow stromal stem cells

BACKGROUND:Fetal bovine serum as nutritional support is often used in the traditional cel culture. Consequently, a host of potential problems such as the spread of disease and immunological reactions exist. To find a suitable fetal bovine serum substitute and to establish a culture system of human bone marrow stromal stem cels in vitro which has been standardized, safe and efficient has just started. OBJECTIVE:To investigate the effects of different serums on proliferation of bone marrow stromal stem celsin vitro. METHODS:Bone marrow stromal stem cels were obtained from adult bone marrow, which were cultured in DMEM containing 10% AB serum, 10% autologous serum, or 10% fetal bovine serum. Cels at passage 3 were used in this study. RESULTS AND CONCLUSION:The cel confluence in the AB serum group was earlier than that in the fetal bovine serum group and autologous serum group. Human bone marrow stromal stem cels maintained the phenotypes of bone marrow stem cels in three serums detected by flow cytometry. AB serum group showed the highest fluorescence intensity and the most efficiency of cel proliferation which examined by the AlamarBlue assay. Apoptosis rate was < 5% in al the three groups, and cels grew wel in these serums. Alkaline phosphatase, calcium nodules and oil red O staining showed that the cels maintained the osteogenesis and adipogenesis capacity in the three groups. AB serum was found to have a better effect on proliferation capability of cels than fetal bovine serum and autologous serum. Taken together, AB serum is expected to be a substitute of fetal bovine serum to build anin vitro culture system of adult bone marrow stromal stem cels that accord with the clinical requirements of bone tissue engineering.
9.Acupoints heat-sensitive moxibustion in the application of traditional Chinese surgery.

Under the guidance of meridian theory, the acupoints heat-sensitive moxibustion is a treatment method which applies moxa stick to perform mild moxibustion at heat-sensitive acupoints, which can arouse the meridian sensation transmission and promote the movement of meridian qi; consequently, the qi can be extended to the diseases. For its many advantages, such as no direct contact on skin, no injuries, no pains, fewer side effects, easy operating and moderate cost, the acupoints heat-sensitive moxibustion is widely accepted in dermatology, male urology disease, rectum and anus diseases and breast diseases. The application and research status of the acupoints heat-sensitive moxibustion in traditional Chinese surgery in recent years is reviewed, and several problems and suggestions in its clinical application and research are proposed, aiming to provide clinical basis for its further development and clinical application in traditional Chinese surgery.

10.Effects of shoutai pills on expression of Th1/Th2 cytokine in maternal-fetal interface and pregnancy outcome.

OBJECTIVETo evaluate its mechanism of inducing the maternal-fetal immune tolerance by studying the effects of Shoutai pills on the expression of Th1/Th2 cytokine and pregnancy in maternal-fetal interface of mice with recurrent spontaneous abortion (RSA).
METHODThe normal pregnancy and RSA model were respectively induced with CBA/J x BALB/c and CBA/J x DBA/2. The mice with RSA were randomly divided into model group and low, middle and high dose groups of Shoutai pills. The mice were killed in 14 days after administration and embryo resorption rate was counted and their decidual and placental tissues were co-cultured to detect the expressions of IL-4, IL-10, IFN-gamma and TNF-alpha with ELISA.
RESULTThe embryo resorption rate of the model group was significantly higher than the normal pregnancy, middle and high dose groups of Shoutai pills could decreased the embryo resorption rate of the mice with RSA (P < 0.05). All the doses in 3 groups of Shoutai pills could decreased the expression of IFN-gamma and TNF-alpha (P < 0. 05) and there was no obvious difference between normal pregnancy group and all groups of Shoutai pills. Middle and high doses of Shoutai pills could increased the expression of IL-4 and IL-10 (P < 0.05) and there was no obvious differences between normal pregnancy and high dose group of Shoutai pills.
CONCLUSIONThe mechanism about Shoutai pills can change Th1 /Th2 cytokine towards Th2 bias, which induced the maternal-fetal immune tolerance.
