1.Clinical Features of Type 2 Diabetes Mellitus Patients with Polycystic Ovary Syndrome

Objective To provide an evidence to type 2 diabetes with PCOS patients,the study compared the clinical features between T2DM,AGT,IR and Non-IR patients.Methods A analysis on 30 T2DM,69 impaired glucose tolerance,87 IR and 31 Non-IR patients of PCOS was performed.Results WHR of the former three groups was significantly higher than the non-IR group(P < 0.05).The systolic pressure and progress of T2DM patients were significantly higher than IR and Non-IR groups(P < 0.05).In T2DM,AGT and IR groups,SHBG level was significantly lower than Non-IR group.In PCOS + T2DM group,FPG,2h blood glucose,HOMA-IR and LDL were significantly higher than the three latter groups(P < 0.05).In PCOS + T2DM group,LN (HOMA-β) was significantly lower than AGT and IR groups,2h blood insulin was significantly lower than AGT group (P < 0.05).PCOS A GT group,FPG,2h blood glucose,2h blood insulin were significantly higher than IR and non-IR.In PCOS + T2DM and AGT groups,TG,TC/HDL,TG/HDL,LDL/HDL,APOB/APOA was significantly higher than IR group and non-IR group (P < 0.05),whlile TG/HDL,LDL/HDL,APOB/APOA of IR group was significantly higher than non-IR group(P < 0.05).Conclusion T2DM patients with PCOS have long progress,with β-cell sensitivity decreased and had higher lipid level than AGT,IR patients.
2.Influence of acupoint-injection on TXB2 and 6-keto-PGF1a in patients with pseudobulbar palsy:a randomized controlled trial

Objective:To prove the feasibility and validity ofXing Nao Jing acupoint-injection (XNJ-AI) at Fengchi (GB 20) for pseudobulbar palsy caused by ischemic stroke (PBP-IS).
Methods:An assessor-blinded, two-parallel-group, randomized controlled trial was conducted, and the patients with PBP-IS were recruited and randomly divided into two groups. Patients in the control group received oral aspirin (100 mg per day for 2 weeks). In addition to oral aspirin; patients in the treatment group received XNJ-AI at Fengchi (GB 20), once a day, for two weeks. The primary outcome was assessed by the water-swallowing test (WST). Thromboxane B2 (TXB2) and 6-keto-prostaglandin F1a (6-keto-PGF1a) in plasma were measured before and after the treatment.
Results:In the treatment group, the percentage of swallowing function no less than grade 3 before and after the treatment was 32% and 88%, respectively; in the control group, it was 28% and 76% before and after the treatment, respectively; the difference after the treatment between the two groups was statistically significant (P<0.05). There were statistical differences between pre- and post-treatment levels of plasma TXB2and 6-keto-PGF1αin the two groups (bothP<0.05). After the treatment, there were significant differences in the levels of plasma TXB2 and 6-keto-PGF1α between the two groups (bothP<0.05).
Conclusion:XNJ-AI at Fengchi (GB 20) can improve the patients’ swallowing function and balance the levels of TXB2 and 6-keto-PGF1α in plasma.
3.Determination of Serum Concentrations of Phenbarbital,Phenytoin Sodium and Carbamazepine by MEKC

OBJECTIVE:To determine serum concentrations of phenbarbital(PB),phenytoin sodium(PHT),carbamazepine(CBZ)by micellar electrokinetic capillary chromatograph(MEKC).METHODS:The determination was performed on elastic quartz column(37 cm?50 ?m)with barbital as internal standard.The buffer solution consisted of 10 mmol?L-1 phosphate sodium buffer-30 mmol?L-1 SDS sodium(micellar phase)-15% methanol at a constant voltage of 18 kV.The detection wavelength was set at 210 nm.RESULTS:The linear concentration ranges of PB and PHT were 2.5~80 ?g?mL-1,with detective concentration of 0.1 ?g?mL-1;the linear range and detective concentration of CBZ were 1~32 ?g?mL-1 and 0.2 ?g?mL-1,respectively.The average recoveries of the PB,PHT,and CBZ were 99.90%,99.91%,and 99.42%,respectively.CONCLUSION:The method is rapid,simple,accurate,sensitive and reproducible,and suitable for clinical monitoring of drug level and pharmaceutical study.
4.Study on correlation of ambulatory arterial stiffness index and pulse wave velocity

Mective To study the correlation of ambulatory arterial stiffness index(AASI)and pule wave velocity(PWV),explore the evaluation of AASI on arterial stiffness and analyze the influential factors of AASI.Method One hundred patients were selected,brachial-ankle pulse wave velocity (baPWV)of all patients wag measured,simultaneously 24-hour ambulatory blood pressure Wag examined, calculated AASI.AASI and baPWV were analyzed with correlation and regression analysis.Results AASI Wagpositively and signifieandy correlatedwithPWV(r=0.516,P<0.01)and age(r=0.417,P<0.01). AASIWag negatively correlatedwithbody height(r=-0.223,P<0.05).Themultiplefactor stepwise regres-sion analysis showed that the regression of AASl with PWV were great significant.Conclusions AASI is positively and significantly correlated with PWV.It is a novel meagure ofartdrial stiffness.
5.A STUDY ON THE RELATIONSHIP BETWEEN GASTRIC MOTILITY AND PLASMA MOTILIN IN THE PATIENTS WITH HEPATOCIRRHOSIS

Objective:We clarify the relationship between dysfunction of gastric motility and the plasma motilin,in order to study the pathogenesis,diagnosis and therapy of the hepatocirrhosis with dyspepsia.Methods:The liquid gastric emptying (GE),electrogastrograpgy(EGG) and plasma motilin were examined in 51 patients with hepatocirrhosis.Results:The dysfunction of gastric motility is associated with the liver function,gastric rhythm and upper gastrointestinal symptom.The plasma motilin concentration increased significantly in patients with hepatocirrhosis as compared with that of controls (P<0.01).The sorbefacient prokinetic drug (prepulsid) can improve the function of gastric power,but the lever of motilin no change.Conclusion:The patients with hepatocirrhosis have the dysfunction of gastric motility,and gastric emptying delayed,and there is a relation between GE and the liver function.We evaluate that this disorder is associated with the low sensitivity about stomach to the plasma molitin in this case.We can judge the degree of the liver damage,gastric emptying delayed.gastric rhythm by testing the gastric motilin.Drug can deal it.
6.Application of polypropylene repair material in treating inguinal hernia

OBJECTIVE:To summarizethe application of polypropylene repair material in treating inguinal hernia.METHODS:Papers regarding polypropylene repair material in treating inguinal hernia published in VIP data between January 1998 and October 2009 were researched using key words of "polypropylene,inguinal hernia,and complication".Inclusion criteria Repairing inguinal hernia using polypropylene materials.Experimental results concerning related complications.Exclusion criteria:repetitive study and relatively old literature.A total of 17 documents were included in this study.RESULTS:Organic polymeric materials were clinical utilized in repairing inguinal hernia at the end of 1950s.The polypropylene patch was knitted by slngle fiber or multi-strands fibers of polypropylene,and exhibited single layer network structures,which is preferred material for repairing abdominal-wall defect.Compared with other unabsorble materials,the polypropylene patch had following advantage:obviously stimulating fibroplasias; easy permeated by fibrous tissues and embedded into tissues; maintaining highly tensile strength following implantation; and relative low cost.Few material-related complications occurred following repairing,mainly presented with infection,serofiuid swelling,ankylenteron,intestinal leakage,material displacement,as well as material shrinkage.CONCLUSION:Polypropylene material is an ideal repair material due to good histocompatibility,few rejections,and anti-infection capability.It leads to easy operation,small trauma,fast recovery and few complications in repairing defects.
7.NOK Interacts With Akt and Enhances Its Activation

NOK is a newly identified receptor protein-tyrosine kinase (PRTK) molecule that can promote tumorigenesis and metastasis. Previous data showed that NOK could activate the phosphatidylinositol 3'-kinase (PI3K) pathway in stable BaF3 cells. But how does NOK activate PI3K in cells remains unknown. It was showed that NOK physically interacted with the PI3K downstream effector Akt and enhanced its activation in human embryo kidney 293T (HEK293T) cells. Deletion mapping indicated that protein kinase B (Akt) was able to directly contact with the kinase domain of NOK. Inactivating the Akt kinase domain significantly reduced the intermolecular interaction between NOK and Akt, while constitutively active mutant of Akt apparently had a stronger interaction with NOK. NOK did not have an additive effect on insulin-mediated Akt activation. Overall, the results indicate that NOK might complex with Akt and directly activate PI3K/Akt signaling pathway.
8.Combined interventional therapy for malignant vena cava obstruction

Objective To evaluate the effectiveness of combined treatment with thrombolysis,PTA,and endovascular stent placement for malignant obstruction of vena cava.Methods The study included 29 patients with malignant obstruction of vena cava(superior,8 patients;inferior,21 patients).The patients underwent treatment of PTA and endovascular stent placement(for patients with complete vena cava obstruction or patients with incompletely expanding stent),or endovascular stent placement(for patients with incomplete vena cava obstruction),or endovascular stenting combined with thrombolysis(for patients with thrombosis in vena cava),respectively.Results A total of 29 stents was implanted,with a success rate of stent placement of 100%.Symptoms of SVC obstruction completely disappeared in the 8 patients.Scores of patients' IVC syndrome were declined from 4~5(median,4.2) preoperatively to 0~2(median,0.3) postoperatively,with statistically significant difference(Z=-55.245,P=0.000).Follow-up checkups for 1~20 months(mean,6.2 months) in the 29 patients found patent vena cava.There were no serious complications except for vena cava laceration in 1 patient and stent displacing to the right atrium on the third day after therapy in 1 patient.Conclusions The combined treatment of thrombolysis,PTA,and endovascular stenting is safe and effective in the treatment of vena cava obstruction.
9.Application of data envelopment analysis in assessment of nursing efficiency of a general hospital

Objective To study the effect of data envelopment analysis evaluation on nursing efficiency in general hospitals.Method The input indexes including the number of enrolled nurse,indirect nursing time,average annual nursing consumables and average annual continuing education time and the output indexes including hospitalized patient number and year-round nursing income of 10 nursing units in our hospital during October 2012 to October 2013 were collected and analyzed with C2R,C2GS2 model and C2R/ C2GS2 ratio from Data Envelopment Analysis.Result About 5 nursing units were effective in terms of overall efficiency,8 ones in terms of unit technical efficiency and 5 one in terms of nursing unite scale efficiency.Conclusions We need improve nursing efficiency in our hospital.Nursing managers should pay attention to the results and improve working process aiming at the practical situations.
10.Endogenous cystathionine-gamma-lyase/hydrogen sulfide pathway regulates apoptosis of HepG2 cells.

This study is to investigate the role of endogenous CSE/H2S in regulating apoptosis of HepG2 cells. MTT and Trypan blue assay were performed to determine the effect of CSE inhibitor PAG and CSE siRNA on proliferation of HepG2. Production of H2S from HepG2 cells was assessed spectrophotometrically using N, N-dimethyl-p-phenylenediamine-dihydrochloride. Cells apoptosis was detected by means of double staining of Hoechst 33342 and PI with Array Scan V(TI)HCS600 High-Contents. Dihydroethidine (DHE) and 2', 7'-dichlorodihydrofluorescein diacetate (DCFH-DA) assay was used to determine intracellular superoxide anion and ROS level. Reduced glutathione (GSH) was determined by OxiSelect Total Glutathione Assay Kit. Recombinant plasmid pcDNA 3.1/myc-His(-)-CSE was constructed and transfected into 293T cells to rescue the ROS and GSH level to further investigate the effect of CSE/H2S on ROS and GSH. Western blotting was performed to test the effect of CSE siRNA on expression of activated caspase 3 and p-AKT and Nrf2 protein. The results showed that PAG and CSE siRNA could significantly decrease the production of H2S in HepG2 cells and inhibit the proliferation of HepG2 cells at a dose-dependent and time-dependent manner, respectively. PAG and CSE siRNA could promote the cell apoptosis of HepG2 cells. Moreover, PAG and CSE siRNA induced increased ROS generation and depletion of the critical antioxidant GSH and recombinant plasmid pcDNA 3.1/myc-His(-)-CSE rescued the level of ROS and GSH. Meanwhile, CSE siRNA increased the expression of activated caspase 3, but CSE siRNA did not affect the expression of p-AKT and Nrf2. These results suggested that the CSE/H2S pathway was involved in suppression of HepG2 cell growth and promoted apoptosis of HepG2 cells in an oxidative stress-dependent manner.
