1.Construction of Detenial Sigmoid Neobladder after Laparoscopic Radical Cystectomy:Report of 71 Cases

Objective To report our experience on construction of detenial sigmoid neobladder after laparoscopic radical cystectomy in 71 cases.Methods From August 2002 to May 2006,a total of 71 patients with invasive bladder carcinoma underwent construction of detenial sigmoid neobladder after laparoscopic radical cystectomy in our hospital.After the bladder was excised by laparoscopy,a 5-to 7-cm incision was made on the abdomen to remove the resected tissues,and then a 15-to 20-cm sigmoid colon was resected,the two colic bands opposite to the mesentery and the circular muscle and seromuscular layers between them were removed to construct a detenial sigmoid neobladder.Afterwards,the neobladder was anastomosed with the posterior urethra.Laparoscopic anastomosis was performed in 26 of the cases.Results The operation time was 240-390 min totally in the 71 cases.Laparoscopic radical cystectomy was finished in 80-270 min(mean,180 min),and the open surgery was completed in 160-240 min(mean 140 min).Oral intake was started at day 4-8 postoperation,ureteral stents were removed at week 3-4,and urethral catheter was removed at week 4.Three months after the operation,the mean intra-bladder pressure was 22.0 cm H2O(17-38 cm H2O),average bladder reservoir was 340 ml(200-410 ml),volume of residual urine was less than 25 ml in all the cases(
2.Application of large piece of mid -thick head skin for repair of wound in childrens′functional location

Objective To investigate the clinical effect of large piece of mid -thick head skin applicated in repairing of wound in childrens′functional location.Methods 24 children with functional parts burn and scar con-tracture were selected.Using electric machine,the large piece of mid -thick head skin(thickness ≥0.25mm)was taken,then transplanted it to functional location directly or punching,and dressed or suture package fixed it,1 week after the skin transplanting,anti -scarring therapy and functional exercise were carried out.Control group:selected the thighs,back,abdomen as donor sites;The scar of operation area of the two groups were evaluated and analyed after 3 months by the Vancouver Scar Scale Rating.Results In the study group,the functional parts burn and scar con-tracture deformity was primary wound repairing,by the early anti -scarring therapy,the function and appearance was satisfied with repairing,during follow -up,there were varying degrees of repair site scar,but functional activity was not significantly affected;There were no scar hyperplasia of the head blood supply area,no bald hair deformity,compared with the control group after 3 months:surgical color (t =0.48,P >0.05),surgical vascular distribution (t =0.47, P >0.05),scar thickness (t =0.07,P >0.05)and softness (t =0.42,P >0.05),four indicators had no significant statistical difference.Conclusion The large piece of mid -thick head skin transplanting applied in children functional parts and scar contracture deformity restorative treatment,combined with early anti -scarring therapy and functional exercise,function and appearance can achieve satisfactory clinical results,but also for further post -repair function and appearance of the skin to provide more options source and surgical methods.
3.Effects of curcumin on the expression of heat shock protein 70 in the hippocampus and spatial learning ability in APPswe/PS1△9 transgenic mice

Objective To investigate the effect of curcumin on the expression of heat shock protein 70 (HSP70) in the hippocampus and spatial learning ability in APPswe/PS1△9 transgenic mice,and provide a new theoretical basis for the treatment of Alzheimer' s disease (AD).Methods Mice were divided into wild-type (WT) group,APP/PS1 transgenic group,curcumin-treated group,quercetin combined with curcumin-treated group.The 5-month-old mice in curcumin-treated group received 150 mg · kg-1 · d-1 curcumin by intraperitoneal injection for four consecutive weeks.In addition to curcumin,the mice in quercetin combined with curcumin-treated group also received quercetin 50 mg· kg-1 · d-1 intraperitoneally for four consecutive weeks.The expression of HSP70 in the hippocampus of APP/PS1 mice was detected by Western blot.Learning and memory ability of APP/ PS1 mice were measured by Morris water maze test.The latency of finding the platforms and the number of crossing the platforms were observed.Results The results of Western blot analyses showed that the expression of HSP70 in the hippocampus of APP/PS1 transgenic mice decreased significantly compared with that of WT mice (OD:APPswe/PS1△9:0.31±0.14;WT:1.14±0.51;P<0.01).The expression of HSP70 in curcumin-treated group significantly increased compared with that of APP/PS1 transgenic group (OD:0.91±0.20 in curcumin-treated group;P<0.05).The results of water maze test showed that compared with WT group,in APP/PS1 group,the latency of finding the platforms was significantly longer (APPswe/PS 1 △9:(82.3 ± 6.8) s;WT:(19.5±4.4) s),and the number of crossing tbe platforms were significantly decreased (APPswe/PS1 △9:(2.7± 1.1);WT:(5.2± 2.1)).In curcumin-treated group,the latency of finding the platform ((47.7±7.6) s) was significantly shorter and the number of crossing platform ((4.5±1.9) s) was more than those in APP/ PS1 group.However,in quercetin combined with curcumin-treated group,the latency of finding the platform was significantly longer ((69.7±9.1) s) and the number of crossing platform (3.2±1.6) was decreased compared with those in curcumin-treated group.Conclusion Curcumin can promote the expression of HSP70 in the hippocampus of APP/PS1 transgenic mice,thus improve spatial learning ability of APP/PS1 transgenic mice.These results provide evidences that curcumin may have potential therapeutic value for the treatment of AD.
4.Clinical Effect of Entecavir Combined with Pegylated Interferons in Patients with Hepatitis B

Objective: To investigate the clinical effect of entecavir combined with pegylated interferons in the patients with hepatitis B.Methods: Totally 72 patients with hepatitis B were randomly divided into the experimental group and the control group with 36 ones in each.The patients in the experimental group were treated with entecavir combined with pegylated interferons, and those in the control group were treated with entecavir.The clinical indicators in the two groups were detected and compared after the 24-week, 48-week and 6-month treatment.The adverse drug reactions in the two groups were recorded and compared.Results: After the 24-week treatment, the differences in the clinical indicators between the groups were not statistically significant (P >0.05).After the 48-week and 6-month treatment, the clinical indicators in the experimental group were obviously higher than those in the control group, and the differences were statistically significant (P <0.05).The difference in the adverse drug reactions in the two groups was not statistically significant (P >0.05).Conclusion: Compared with entecavir, pegylated interferon combined with entecavir in the treatment of hepatitis B shows better clinical effect and security, which is more worthy of clinical promotion.
5.Association between HbA1C and plasma lipid profiles in newly diagnosed diabetic patients

[Summary] In newly diagnosed diabetic patients, fasting plasma glucose, HbA1C , and plasma lipid profiles were measured to analyze the association between HbA1C and plasma lipid profiles. HbA1C might affect plasma lipid profiles in newly diagnosed diabetic patients. Higher HbA1C was associated with the worse plasma lipid profiles and more severe insulin resistance.

7.A novel peptides, like nerve growth factor, inducing pheochromocytoma PC12 cell differentiation

Objective: To isolate the key domain of a novel polypeptide fragment from NGN-β that functions like intact NGF molecule.Methods: NGF-β had been treated with cyanogen bromide and trypsin skillfully. The peptide fragment with the activity inducing PC12 pheochromocytoma cells differentiation was isolated and purified by Sephadex G-50 gel filtration chromatography , DE-52 celluloseion exchange chromatography and C-18 reversed-phase column HPLC after NGF-β cleavaged by CNBr at 9th met then by trypsin at Arg or Lyscleavaged by CNBr at 9th met then by trypsin at Arg or Lys. Amino acid sequencing of this novel peptide fragment was performed by Automatic Amino Acid Analyser and Amino Acid Sequencer. Results: The functional fragment from cleavaged NGF might induce differentiation of PC12 cells. The fragment was consisting of two linear polypeptides . One of them was 16 peptide, GEFSVCDSVSVWVGDK , and other was 14 peptide, HWNSYCTTTHTFVK, linked by a disulphide bridge corresponding to residues 10~25 and 75~88, respectively, of the amino acid sequence of nerve growth factor, the result of biological activity assay in PC12 cells showed that the optimum concentration of this peptide were 0.001~0.1 μg*L-1. Conclusion: A novel peptide inducing differentiation of PC12 cell line of pheochromocytoma cells was obtained in the study. It′s isolation and purification successfully will underlie synthesis or expression of hyperactive neurotrophic small molecular substance although the relationship between the configuration and functions is not clearly.
8.The flow density of macular and optic disc and area of foveal avascular zone in severe nonproliferative diabetic retinopathy

Objective:To observe the flow density (FD) of macular and optic disc and area of foveal avascular zone (FAZ) in severe nonproliferative diabetic retinopathy (S-NPDR).Methods:A prospective cross-sectional study. From October 2019 to April 2020, 31 eyes of 25 S-NPDR patients (S-NPDR group) who were diagnosed in the ophthalmological examination of Jiangsu Province Hospital and 30 eyes of 30 age- and sex-matched healthy volunteers (control group) were included in this study. Optical coherence tomography angiography (OCTA) was used to scan the macular area of 6 mm×6 mm and optic disc of 4.5 mm×4.5 mm. The software automatically divides it into three concentric circles centered on the macular fovea, which were foveal area with a diameter of 1 mm, parafoveal area of 1 to 3 mm, and foveal peripheral area of 3 to 6 mm. The area around the optic disc was divided into 8 areas: nasal upper, nasal lower, inferior nasal, inferior temporal, temporal lower, temporal upper, superior temporal and superior nasal. The FD of the optic disc, the superficial capillary layer (SCP) and deep capillary layer (DCP) of the retina and FAZ area were measured. The FD and FAZ area were compared between the two groups by independent sample t test. The correlation between FAZ area and FD was analyzed by Pearson correlation. Results:In parafoveal and perifoveal area, compared with the control group, the FD of SCP ( t=6.470, 5.220; P<0.001) and DCP ( t=7.270, 7.370; P<0.001) decreased in S-NPDR group. In foveal area, there was statistically significant difference in the FD of DCP between the two groups ( t=2.250, P=0.030), while the difference in FD of SCP between the two groups was not statistically significant ( t=0.000, P=0.900). The FAZ area in S-NPDR group was larger than that in control group, and the difference was statistically significant ( t=2.390, P=0.030). The FD in the S-NPDR group was lower than that in the control group except the superior nasal, the difference was statistically significant ( t=7.520, 5.000, 4.870, 3.120, 2.360, 2.120, 5.410, 5.560, 2.640; P<0.05). Pearson correlation analysis showed that the FAZ area of S-NPDR was negatively correlated with FD of SCP ( r=-0.513, P=0.004), and had no correlation with FD of DCP ( r=0.034, P=0.859). Conclusion:The overall FD in macular area and optic disc of patients with S-NPDR decreased and the FAZ area enlarged.
9.Age-, period- and cohort-specific incidence of sexually transmitted diseases in Yantian District from 2008 to 2022

Objective:
To investigate the incidence trend of sexually transmitted diseases (STDs) in Yantian District, Shenzhen City, Guangdong Province from 2008 to 2022, and the effects of age, period, and cohort on STDs incidence, so as to provide the basis for formulating control measures for STDs.
Methods:
Data of reported STDs cases (syphilis, gonorrhea, genital Chlamydia trachomatis infections, condyloma acuminatum and genital herpes) among population aged 15 to 64 years in Yantian District from 2008 to 2022 were collected through the Chinese Disease Control and Prevention Information System. The temporal and population distribution of STDs were descriptively analyzed. Trends in incidence of STDs were analyzed using average annual percent change (AAPC). Impacts of age, period and cohort on the incidence of STDs were analyzed by an age-period-cohort model.
Results:
A total of 6 156 cases of STDs aged 15 to 64 years were reported from 2008 to 2022, and the average annual incidence was 229.92/105. The overall incidence showed an upward trend (AAPC=2.409%, P<0.05), and an upward trend among females (AAPC=5.846%, P<0.05), but the trend was not statistically significant among males (AAPC=0.193%, P>0.05). The incidence of STDs showed an increasing trend followed by a decreasing trend with the increase of age. The incidence trend among males was consistent with the overall population, reaching its peak in the age group of 30 to 34 years (371.04/105), while the incidence among females showed an increasing trend with the increase of age, peaking in the age group of 60 to 64 years (442.15/105). Compared with 2013-2017, the incidence risk of STDs showed an increasing trend in 2018-2022 (RR=1.349, 95%CI: 1.217-1.495). Compared with the birth cohort of 1975-1979, the incidence risk of STDs in 1980-2005 birth cohort showed an increasing trend, with the highest incidence risk in 2000-2004 birth cohort (RR=4.178, 95%CI: 2.717-6.423).
Conclusions
The overall incidence of STDs among residents aged 15 to 64 years in Yantian District from 2008 to 2022 appeared a tendency towards a rise. Age, period and cohort all had impacts on the incidence, with a greater effect among females than males.
10.Relationship between character of head-chest lead electrocardiogram and coronary lesions in elderly patients with abnormal Q waves in inferior lead

Objective To evaluate the ability of further discriminating diagnosis of the headchest lead electrocardiogram (HCECG) in elderly patients with abnormal Q waves in routine lead electrocardiogram (RLECG) in inferior lead. Methods The 55 male patients, aged 65-88 years,with abnormal Q waves in both lead Ⅲ and aVF were selected and divided into two groups: myocardial infarction (MI) group and non-MI group, according to if the patient had a history of acute MI. All the patients accepted examination of coronary computed tomographic angiography (CTA) and ultrasound cardiogram, those with Wolff-Parkinson-White syndrome and myocardial hypertrophy were excluded.The 30 individuals of control group had no abnormal Q waves in lead Ⅱ , Ⅲ and aVF. HCECG and RLECG were recorded simultaneously in respective groups and occurrence rate of Q waves in correlative leads Ⅱ, Ⅲ, aVF and HL3, H0, HR3 were matched and compared, sensitivity and specificity were compared in respective leads. Results There were serious stenosis in 22 patients (100%) in MI group, and there were mild stenosis in 10 (30. 3%) and moderate stenosis in 23 patients (69.7%) in non-MI group. There was no significant difference between HCECG and RLECG in occurrence rate of Q waves in MI group (P> 0. 05 ). Non-MI group left anterior axillary line, Ⅱ ,Ⅲ, right anterior axillary line, near the anterior midline, aVF without Q wave and exclusion of old MI diagnostic specificity were 100%, 97.0%(32/33), 97.0% (32/33), 15.2% (5/33), 100%, 39.4%( 13/33)respectively. Conclusions Pseudo-changes are rarely found in HCECG and there is a higher degree of conformity in HCECG with coronary lesions, therefore HCECG may be used to discriminate whether the inferior abnormal Q waves occurred in RLECG are truly abnormal or not.
