1.Anesthesia selection for senile total hip arthroplasty

Total hip arthroplasty is common joint replacement method in senile patients,which is characterized by large wound,large amount of bleeding,and great stimulation.Anesthesia methods during total hip arthroplasty include epidural anesthesia,epidural and lumbar anesthesia,and general anesthesia.The anesthesia method is dependent on patient condition.Appropriate anesthesia based on disease condition can reduce anesthesia risk.The key of anesthesia is to control anesthesia plane,supplement blood and monitor toxic reaction during cement application.Because senile patients are always complicated by respiratory,circulatory and endocrine system diseases,comprehensive treatment before surgery to improve body symptoms can increase anesthesia and surgery tolerance and better guarantee patient safety during surgery.
2.THE CLINICAL APPLICATION OF ANESTHESIA FOR DENTAL PHOBIA IN CHILDREN'S O-RAL TREATMENT SEVOFLURANE INHALATION

Objective To explore dental phobia in children 's oral treatment sevoflurane inhalation anesthesia clinical results.Methods 100 cases of dental phobia children in our hospital between January 2012 to January 2014 were observed, which were divided to the school -age group and preschool group , 50 patients, respectively.They were all treated with heptafluorobutyric alkyl inhalation anesthesia , and the anesthesia effects were compared .Results The adverse effects, oxygen saturation and mean arterial blood pressure changes and induction of anesthesia in chil -dren awake have statistically significant difference (p <0.05).Conclusion This study suggest that sevoflurane in -halation anesthesia is an effective dental phobia children 's oral therapy, and has a high application value .
3.Risk factors of myasthenic crisis after thymectomy: a meta analysis

Objective To evaluate the risk factors of myasthenic crisis after thymectomy for myasthenia gravis.Methods Meta-analysis was performed to screen risk factors of myasthenic crisis after thymectomy for myasthenia gravis based on 21 associated articles published and carried out using Stata12.0.Results Twenty one studies were identified, including 3 092 patients and 698 patients developed myasthenic crisis after thymectomy.Complicated with thymoma(OR =3.93, 95% CI 2.37-6.50, P =0.000), Ossermen classification (OR =0.12, 95 % CI 0.09-0.17 , P =0.000) , history of preoperative myasthenic crisis (OR =5.36, 95 % CI 3.82-7.51, P =0.000) , use steroid hormonea (OR =1.75,95 % CI 1.26-2.43, P =0.001) , postopertive lung infection(OR =4.27,95% CI 1.63-11.18, P =0.003) , operation methods(OR =0.34,95% CI 0.21-0.56, P =0.000) , the dose of preoperative anticholinesterase drags (OR =0.35, 95 % CI 0.18-0.66, P =0.000) ,pulmonary function(OR =0.18, 95% CI 0.06-0.57, P =0.004), preoperative serum level of anti-acetylcholine receptor antibody(OR =0.06, 95% CI0.03-0.13, P =0.000)were associated with myasthenic crisis after thymectomy.Gender, course of disease were not associated with myasthenic crisis after thymectomy.Conclusion With thymoma, Ossermen classification, history of preoperative myasthenic crisis, use steroid hormonea, operation methods, the high dose of preoperative anticholinesterase drugs, poor pulmonary function, postopertive lung infection, high preoperative serum level of anti-acetylcholine receptor antibody are risk factors affecting myasthenic crisis after thymectomy.
4.Significance of treadmill exercise test combined with risk factors for the diagnosis of coronary heart disease

Objective To evaluate the diagnostic value of treadmill exercise test for coronary heart disease. Methods The results of coronary angiography, exercise treadmill test, and the main risk factors of 447 patients suspected of coronary heart disease were analyzed. Results Treadmill exercise test for coronary artery stenosis resulted in 90 3% sensitivity, 19 6% specificity, 65 1% positive predictive value, and 55 0% negative predictive value. Compared with patients with single vessel disease, lower, earlier, and longer lasting ST segment depression and less exercise burden for the maximum heart rate were found in those with multiple vessel disease. A total of 72 5% patients with coronary artery lesion (including coronary stenosis and coronary tunica intima lesion) had more than 2 risk factors. Conclusion Treadmill exercise test is a good non invasive method for screening of coronary heart disease: (1) negative result of treadmill exercise test can be used to exclude coronary heart disease, but positive result of treadmill exercise test plus more than 2 risk factors can predict the likelihood of coronary heart disease; (2) the treadmill exercise test can also reflect the degree and the range of coronary artery lesion.
5.Maladjustment and Attitude towards Help of Middle School Students

Objective:To study the relationship between and maladjustment of middle school students and their attitude towards help-seeking.Methods:276 middle school students completed questionnaire of maladjustment and attitude towards help-seeking.Results:There exists grade difference on the problem of school life and the stigma tolerance, and there was also gender difference on the physical and mental development problem, the reliance on the counselor and the integral for help. The experience of help seeking had no influence on maladjustment and attitude toward help seeking.Conclusion:The maladjustment of middle school students is not so severe and the attitude towards help is comparatively positive; the more serious the maladjustment is, the more negative attitude toward help seeking.
6.Comparison of the clinical value of XELOX and FOLFOX6 in the treatment of elderly patients with colorec-tal cancer

Objective To compare the clinical value of XELOX and FOLFOX6 in the treatment of elderly patients with colorectal cancer. Methods Clinical data of 68 elderly patients(aged>60 years)with colorectal cancer were retrospectively analyzed.The patients were divided into XELOX group(n =32)and FOLFOX6 group(n =36) according to the chemotherapy regimen.The response evaluation criteria in solid tumors,progression free survival, 2 -year mortality,chemotherapy related adverse reactions and health related quality of life in the two groups were compared.Results Compared with the FOLFOX6 group,the neurotoxicity incidence in the XELOX group was signifi-cantly lower (3.13% vs.22.22%,χ2 =5.380,P =0.020).The neutrophils reduced incidence was significantly lower(9.38% vs.30.56%,χ2 =4.649,P =0.039).The hand foot syndrome complications significantly increased (34.38% vs.8.33%,χ2 =7.027,P =0.008).There was no significant difference in the incidence of nausea,vomi-ting,diarrhea,oral mucosal ulcer,thrombocytopenia and elevation of transaminase in the two groups(χ2 =0.001, 0.000,0.008,2.102,1.071,1.851,all P >0.05).The quality of health related quality of life was significantly higher in the XELOX group than in the FOLFOX6 group at 6 months after the operation[(67.94 ±12.49)points vs. (57.39 ±13.59)points,t =3.198,P =0.015].There were no significant differences in the curative effect grade at 6 months after the operation,2 -year mortality rate and progression free survival time between the two groups(χ2 =0.949,0.008,t =0.573,all P >0.05).Conclusion Although XELOX increases the incidence of hand foot syn-drome in elderly patients with colorectal cancer,but it significantly reduces the incidence of neurotoxicity and neutro-phil,and improves the quality of life of patients after surgery.
7.Improvement in preventing the displacement of tissue expander implanted between the panniculus carnosus and deep fasia in rabbits

Objective To investigate the method to prevent the displacement of tissue expander implanted between the panniculus carnosus and deep fasia in rabbits .Method 40 rabbits were divided into two groups randomly: the experimental group and the control group .Through an incision on the back , a pocket was made by dissecting between the rabbit panniculus carnosus and deep fasia .The panniculus carnosus at the pocket boundary were sutured with the deep fascia inside the pocket by interrupted 3-0 sutures.A 50 mL kidney-shaped tissue expander was implanted .The expander was inflated with 10 mL saline after the wound was sutured .7 days after implantation , black tattoos were made on the skin around the expander to mark the site of expander .Two months after implantation , the expanders in the experimental group were inflated with 10 mL saline weekly to a total of 140 mL.7 days after implantation the expanders of control group were inflated with 10 mL saline weekly to a total of 140 mL.The incidence of expander displacement , dehiscence of incision , reversal of injection pot and death were recorded .Results There was no expander displacement in the 19 rabbits of experimental group .All the expanders were located within the tattoo area .There was no expander displacement in 11 rabbits of the control group .4 expanders of the control group moved from the back to belly .The difference between them was statistically significant ( P <0.05 ) .The incidence of dehiscence of incision in the experimental group was not significantly lower than that in the control group ( P >0.05 ) .Meanwhile there was no significant difference in the incidences of reversal of injection pot , infections and deaths ( P >0.05 for all ) .Conclusions Sutures between the panniculus carnosus and the deep fascia at the pocket boundary asisted by delayed inflating can prevent the displacement of tissue expander implanted between the panniculus carnosus and deep fasia in rabbits , thus to obtain effective expansion .
8.Advances in Reproductive and Developmental Toxicity of Dibutyl Phthalate

Dibutyl phthalate (DBP) has been found as a ubiquitous environmental pollutant with reproductive and developmental toxicity. The characteristics of reproductive and developmental toxicity of DBP, the mechanisms of the toxicity and effects on human health were reviewed in this paper according to the toxic effects on pregnant animals exposed to DBP, which might provide reference for the further study on toxicity of DBP and preventive measures.
9.Advance in treatment of hyperuricemia by Chinese medicine based on uric acid transporterome.

With the development of the quality of life, the morbidity of hyperuricemia is increasing year by year. At the same time, it appears that this disease attacks the young people currently. As the study of pathogenesis of hyperuricemia advanced, a series of uric acid transporters were found during this process. Meanwhile, the definition of transporterome was proposed. They were divided into three groups according to the functions: reabsorption proteins, excretion proteins and skeleton proteins. At moment, the drugs for hyperuricmia mainly include uric acid composition inhibitors and uric acid excretion promoters. Since the excretion of uric acid plays a leading role during the process of attack of hyperurecimia, it makes sense to explore Chinese medicines with clear mechanism targeting the transporterome. Therefore, this paper would focus on transporterome and summarize the mechanisms of Chinese medicines in treating hyperuricemia.
