1.Review of medical surge capacity for catastrophic events

Following an introduction to the concept of medical surge capacity against catastrophic events,the paper made a systematic examination of the studies on the definition and classification of the capacity,surge capacity assessment,and surge capacity planning from the aspects of management science.On this basis,the authors call for medical surge capacity optimization and resource allocation study,in light of the guideline of multi-disciplinary study,development and evolution principles of such events,as well as the operation management thinking and mathematical modeling approach.These efforts will enable medical institutions in their emergency means against such events.
2.The clinical effects of single-agent Xeloda (Capecitabine) and related risk factors in treatment of patients with advanced colorectal cancer

Objective To explore the clinical effects of single-agent Xeloda (Capecitabine) therapy and the related risk factors in patients with advanced colorectal cancer. Method Seventy-eight patients with advanced colorectal cancer were treated with oral Xeloda, 1250 mg/m 2 twice daily, on days 1-14 every 21 days. At least 2 cycles were administered. The short-term clinical effects were evaluated, and the related risk factors were tested by Logistic regression analysis. Results The overall response rate was 32.05%with 5 cases complete response (CR), 20 cases partial response (PR), 31 cases stable disease (SD), 22 cases progress disease (PD). The Logistic regression analysis showed that the age (OR=1.52, 95%CI 1.015~2.319), fast blood glucose (OR=1.30, 95%CI 1.483~3.677), albumin (OR=1.98, 95%CI 1.526~2.572), ALT (OR=2.37, 95%CI 1.621~3.509) and AST (OR=2.21, 95%CI 1.526~2.572) were independent risk factors for inefficient treatment. Conclusion The single-agent Xeloda (Capecitabine) is an efficacious treatment for the patients with advanced colorectal cancer. However, the inefficient rate is also high and it relates to a variety of factors. We should comprehensively evaluate the patients to improve the short-term clinical effects.
3.R/D on Polymeric Products Used for Cataract Operation. Study on Moulds of Nonrigid Flats Shafting

The tri-plate horizontal mould with nonrigid surgical sponge inserts was developed to establish an industrial system for shaping surgical sponge which is used for cataract operation. The conception on an insertion mechanism of nonrigid flat inserts was firstly suggested by us. The prototype horizontal moulds with tri plate have been designed and manufactured. The experiment results of hundred thousand of times injection shaping have proved that the conception of the design is rational. The structure of shafting mould for nonrigid flats has been confirmed. Present article discussed the design and working mechanism of the insertion in the horizontal injection mould.
4.Research Progress in Extraction Technology, Structural Characteristics and Pharmacological Activities of Hippophae rhamnoides Polysaccharides

Hippophae rhamnoides is a traditional Chinese medicine with homology of medicine and food, which has the effects of relieving cough and resolving phlegm, strengthening the stomach and digestion, and promoting blood circulation and resolving blood stasis. H. rhamnoides contains not only flavonoids, phenols, proteins, vitamins, and amino acids but also abundant polysaccharides. In order to explore the functional value and current research status of H. rhamnoides polysaccharides, this study systematically summarized the extraction process, structural characteristics, pharmacological effects, and mechanism of action of H. rhamnoides polysaccharides by reviewing Chinese and foreign literature. The results showed that H. rhamnoides polysaccharides have anti-tumor, hypoglycemic, antioxidant, immune regulation, anti-inflammatory, and anti-hyperlipidemia functions and could improve intestinal microbiota. There were various extraction processes for polysaccharides, including hot water extraction, microwave extraction, ultrasonic extraction, enzymatic extraction, flash extraction, ultrasonic-microwave synergistic extraction, emerald hot water extraction, enzymatic-ultrasonic synergistic extraction, etc. Based on comprehensive analysis, hot water extraction is suitable for industrial development and application. However, multiple homogeneous polysaccharides have been isolated and purified from H. rhamnoides polysaccharides, but their efficacy, structure, and structure-activity relationship still need to be further explored and studied. This study can provide a reference for the research and development of H. rhamnoides polysaccharides.
5.Preparation of rmOP-1 and its protective effect on renal ischemia reperfusion injury

Objective To investigate the protective effect of recombinant mouse OP 1 (rmOP 1) on renal ischemia reperfusion injury. Methods The full length open reading frame of mouse OP 1 was cloned by RT PCR. The rmOP 1 was prepared and its osteogenesis was assayed in vitro. Thirty Wistar rat models of renal ischemia reperfusion were randomly divided into 3 groups: preventive group, therapeutic group and control group. The former two groups were administered with rmOP 1 (250 ?g/kg) via the superior mesenteric vein at different time points. The renal function and structure were assessed by serum BUN, Cr, histological findings and the modified Miller's scoring. Results rmOP 1 showed osteoinductive activities in vitro. The level of serum BUN, Cr and Miller's scoring were higher in control group than the others ( P 0.05 ). Histological finding showed that rmOP 1 minimized the cell necrosis. Conclusion rmOP 1 has an protective effect on renal function and structure.
6.Central role of GRP78 in growth of gastric carcinoma cells

AIM:To explore the effect of glucose-regulated protein 78 (GRP78) on the gastric carcinogenesis. METHODS:GPR78 expression patterns were examined in 34 specimens from gastric carcinoma patients using the immu-nohistochemistry (IHC) assay, and in 10 specimens using Western blotting analysis .In addition, the expression of GPR78 and cyclin D1 was detected in human gastric cancer cell lines SGC 7901 and SGC7901-H78 (overexpressing GRP78) by Western blotting.RESULTS:By IHC assay, GRP78 was found to be highly expressed in the cytoplasm of gastric carcino-mas as compared with the adjacent non-malignant tissues and corresponding normal tissues .GRP78 expression was positive-ly correlated with gender and histological differentiation (P<0.05), but not with age, tumor stage and lymph node metas-tasis (P>0.05).Furthermore, we found that with the increased expression of GRP 78 in SGC7901-H78 cells, the expres-sion of cyclin D1 was also elevated .CONCLUSION:GRP78 might be a key player to be involved in the growth of gastric cancer.
7.Method and Complications of Direct Fetal Blood Sampling

Objective To study the indications of percutaneous ultrasound-guided direct fetal blood sampling; the methods of fetal blood assessment; and the relationship between the different puncture techniques and the complications. Methods Direct fetal blood sampling were performed on 90 pregnant women whose gestational age were from 16 to 36 weeks for medical indications under the guide of ultrasound. None stress test were performed before and after the puncture. Ulrtrasonographies were done at 2 hours and 24 hours after the procedure. Results There were no severe maternal or fetal complications such as fetal loss and placental abruption. The incidence of bradycardia was 35.6%(32/90), all recovered within 60 seconds; the incidence of bradycardia happened within 4 hours after the puncture was 2.25(2/90). There was a positive relationship among the incidence of bradycardia, the volume of blood taken and the place of punctures,r=0.27 and 0.36 respectively. There was one case with placental changing happened after 2 hours of the puncture, the placenta changing incidence was 1.1%. No emergency cesarean section was done due to the complications of the procedure. All the umbilical cord were normal when the babies were born. Conclusions Direct fetal blood sampling is a safe technique for both fetus and the mother under the guide of real time ultrasound. The data come from fetal blood analysis give reliable informations about chromosome, congenital infections, the status of fetal metabolism, endocrine system and hematologic system directly, which make this technique unreplacable for intrauterine diagnosis.
8.The expression characteristic of T regulatory cell, T help cell 17 and the correlation to disease progression in chronic hepatitis B patients undergoing antivirus treatment

Objective To explore the expression characteristic of T regulatory cell (Treg)/T help cell (Th)17 and the correlation to disease progression in chronic hepatitis B (CHB) patients undergoing antivirus treatment,and to explore their roles in pathogenesis of CHB.Methods A total of 53 patients with CHB (CHB group) and 21 healthy controls (healthy control group) were selected,CHB patients included mild 18 cases,moderate 16 cases and severe 19 cases.The expression of Treg and Th17 were detected by flow cytometry and compared.The levels of alanine aminotransferase (ALT),total bilirubin (TBIL),cholinesterase,albumin were measured by automatic biochemical machine and for correlation analysis.Results The levels ofTh17,Treg,Treg/Th17 were (2.13 ± 0.65)%,(2.99 ± 0.68)% and (6.07 ± 1.18)%,(5.14 ± 0.96)% and 2.86 ± 0.67,1.73 ± 0.45 in CHB group before and after treatment for 24 weeks and (3.59 ± 0.75)%,(4.02 ± 0.77)%,1.04 ± 0.34 in healthy control group.There were significant differences (F =14.78,10.12,17.19; P < 0.01).The level of Th17 in mild,moderate and severe CHB was gradually decreased,but there was no significant difference (F =1.10,P =0.337).The level of Treg in mild,moderate and severe CHB patients was gradually increased,but there was no significant difference (F =0.54,P =0.585).The level of Treg/Th17 was gradually increased with aggravation (2.58 ± 0.59,2.76 ± 0.63,3.21 ± 0.71),and there was significant difference (F =3.15,P < 0.01),the level of Treg/Th17 in severe CHB patients was significantly higher than that in mild,moderate CHB patients (P <0.05).The level of Treg/Th17 had significantly positive correlation with ALT,TBIL (r =0.272,P=0.000; r =0.226,P=0.000).Conclusion Treg/Th17 balance not only relates with the pathogenesis of CHB,but also with the related immune inflammatory of liver tissue.
9.Clinical Significance of HBV Molecular Genotypes and Quantitative Serology Detection in Patients with Chronic Hepatitis B

Objective To characterize the molecular genotypes and quantitative serological biomarker, and to explore their clinical significance in patients with chronic hepatitis B ( CHB) . Methods A total of 210 CHB patients were enrolled and HBV DNA loading, HBV molecular genotypes, quantitative serological biomarker, and ALT were measured. Patients were grouped according to the natural phases of HBV infection:HBeAg (+) immune tolerance, immune clearance, HBeAg (-) low replicative and reactivation phase. The correlation was analyzed between molecular genotypes and serological biomarkers, HBsAg and other biomarkers. Results Subgenotypes B2 with high level of HBV DNA and C2 with middle level of HBV DNA were found to be most prevalent. The positive rates were 59. 0%(B2) and 25. 2%(C2). Patients genotype with B showing more potent viral activity. Subgenotype B2 was sinificantly corelated with HB-sAg, HbeAg, and ALT levels(P < 0. 01), but there was no statistical difference for subgenotype C2. There was statistical difference be-tween positive rates of subgenotypes(P>0. 05), but HBsAg level was found to be significantly correlated with HBV DNA, HBeAg, ALT levels during some phases of CHB. Conclusion Molecular genotyping and quantitative serological biomarkers detection might be helpful for earlier prediction of the long-term disease outcomes in patients with CHB.
10.Clinical Controlled Trial of Lumbar Intervertebral Disc Herniation Treated by Leverage Replacement Manipulation

[Objective]Observe the clinical effect of leverage replacement manipulation on lumbar intervertebral disc herniation.[Method]299 hospitalized patients were divided into two groups.153 cases were in trial group and 146 patients were in controlled group.Trial group accepted constant traction and leverage replacement manipulation.While the controlled group accepted the same traction and muscular massage.[Result]In trial group,90 patients were cured,57 improved and 6 no improvement.The total effective rate was 96.08%;in controlled group,21 were cured,75 cases improved,and 50 no improvement.The total effective rate was 65.75%.The effect of trial group was better than the controlled group(P
