1.Surgical strategies for type Ⅱ a hepatolithiasis

The clinical data of 143 patients with type Ⅱ a hepatolithiasis with normal function of the sphincter of Oddi were analyzed.The patients were divided into two groups at random.The data of the 2 groups of patients were compared on the operating time,hospitalization,rates of complication,residual stone rates and recurrence rates.The A group patients underwent laparoscopic video choledochoscopic hepaticocholangiolithotomy,T-tube drainage and choledochoscopic lithotomy.The B group patients underwent laparoscopic video hepatectomy and choledochoscopic lithotomy.For most of the patients in the A group,the treatment had the advantages of less trauma,less complications and quicker recovery.There was no significant differences in residual stone rates and recurrence rates between the two groups.As it was difficult to remove stones in the caudate lobes of Ⅱ a type patients in A group due to the sharp angle between the hepatic duct and bile ducts of the caudate lobe,individualized programs should be adopted according to the location of stones on individual patients.
2.Predictive effect of rs-fMRI data in acute phase on memory function of chronic phase in ischemic stroke patients

Objectives:To investigate the predictive effect of regional homogeneity (ReHo) from resting-state functional magnetic resonance imaging (rs-fMRI) in acute phase on memory function of chronic phage in ischemic stroke patients and the effects of residual learning (REL) on the predictive performance of machine learning models.Methods:From June 2019 to June 2021, rs-fMRI data of one-week after stroke (acute phase) were collected from 35 first-time ischemic stroke patients, and their memory scores were assessed by the Rey auditory verbal learning test (RAVLT) at 6 months after stroke (chronic phase). Using ReHo from rs-fMRI data in acute phase of ischemic stroke patients, the support vector regression (SVR) and the REL-based SVR (REL-SVR) were constructed to predict the patients’ memory scores at 6 months after stroke, and the performance of the two models was compared using Pearson correlation coefficient.Results:Based on the ReHo from acute phase, the correlation coefficient between the predicted values and the true scores from the SVR model was r=0.524, P=0.001, while the correlation coefficient obtained by the REL-SVR model was r=0.671, P<0.001. Brain regions with relatively higher weights such as Temporal_Pole_Mid_R (weight value: 1.03), Temporal_Mid_R(weight value: 1.03), Temporal_Inf_R (weight value: 1.03), Occipital_Mid_R (weight value: 0.57), Frontal_Mid_L (weight value: 0.32), Frontal_Sup_Medial_L (weight value: 0.53), SupraMarginal_L (weight value: 1.54), Calcarine_L (weight value: 0.65), Lingual_L (weight value: 0.58), Cuneus_L (weight value: 0.65), Precuneus_L (weight value: 0.83), cerebellum(weight value>1.0) made larger contributions to the prediction model. Conclusions:ReHo in the acute-phase can effectively predict memory in the chronic phase of ischemic stroke patients. Furthermore, REL can improve the performance of the traditional SVR model and achieve higher predictive accuracy.
3.Application of rigid curved laryngoscope in the examination of hypopharynx and the treatment of early hypopharyngeal cancer

Objective:To evaluate the efficacy of curved suspension laryngoscope assistant transoral surgery (CLATOS) in the examination of hypopharynx, and diagnosis and surgery of early hypopharyngeal carcinoma.Methods:Thirty-four patients who underwent detailed examination for lower part of hypopharynx and surgery for early hypopharyngeal carcinoma with CLATOS technique in the Second Affiliated Hospital of Harbin Medical University from January 2019 to January 2020 were analyzed retrospectively. The age ranged from 42 to 74 years old including 28 males and 6 females. Among them, 25 patients complaining of foreign body sensation in the pharynx for more than half a year with a poor exposure of the lower pharynx in the examination with flexible laryngoscope in the outpatient department were admitted to the hospital for the further examination with rigid curved laryngoscopy and 9 patients with stage T1-2 hypopharyngeal squamous cell carcinoma were operated with CLATOS technique. Preoperative, intraoperative and postoperative data were analized.Results:One case of squamous cell carcinoma in esophageal entrance (T1N0M0) and one case of carcinoma in pyriform sinus (T1N0M0) with cervical esophageal carcinoma (T1N0M0) were found in the 25 patients with foreign body sensation in the pharynx. En bloc resection of cancer was obtained in 9 patients with stage T1-2 hypopharyngeal carcinomas and 2 of them underwent tracheotomy. One patient with T1 retrocricoid carcinoma was found to have a carcinoma in situ at the lower part of posterior hypopharyngeal wall in the examination during follow-up, which was resected simultaneously. Postoperatively this patient developed a stenosis in the esophageal entrance, which was dilated twice until swallowing normally. Two patients of T2N0M0 and one of T2N1M0 underwent bilateral neck lymph node dissection just after the removal of primary tumors. During follow-up, none of the 9 patients showed tumor recurrence and complications such as dysphonia and dyspnea.Conclusion:CLATOS technique can provide a promising way in the examination of hypopharynx and the detection of early lesions of hypopharynx and esophageal entrance, and this technique has also the advantages such as full visual angle and easy operation for the resection of early hypopharyngeal carcinoma. The quality of life of patients can be improved while the tumor is removed en bloc with this technique.
4.Optimization of the extraction technology of the leaves of Dimocarpus longan by Box-Behnken response surface methodology combined with multi-index comprehensive score

OBJECTI VE To optimize the extraction technology of the leaves of Dimocarpus longan according to flavonoids and phenolic acids. METHODS The contents of gallic acid ,protocatechuic acid ,ethyl gallate ,quercetin,luteolin and kaempferol in the leaves of D. longan were determined by HPLC. Based on single factor test ,with the ethanol volume fraction ,solid-liquid ratio and extraction time as factors ,using comprehensive scores of the contents of above six components as indexes ,the extraction technology of the leaves of D. longan was optimized by Box-Behnken response surface methodology. RESULTS The optimal extraction technology included ethanol volume fraction of 100%,solid-liquid ratio of l ∶ 7(g/mL),extraction time of 90 min, extraction temperature of 80 ℃. After 3 times of validation tests ,the average comprehensive score was 97.54(RSD=0.33%,n= 3),relative error of which with predicted score (99.05)was 1.55%. CONCLUSIONS Box-Behnken response surface methodology combined with multi-index comprehensive score can be used for the extraction technology of the leaves of D. longan ,and the optimized extraction technology is stable and feasible.
5.Anti-inflammatory effect and mechanism of the ethanol extract and the drug-containing serum of Zhuang medicine Stahlianthus involucratus on LPS-induced RAW264.7 cell inflammation

OBJECTIVE To study the anti-inflammatory effect and mechanism of the ethanol extract and the drug-containing serum of Zhuang medicine Stahlianthus involucratus on lipopolysaccharide (LPS)-induced RAW264.7 cell inflammation. METHODS The drug-containing serum or blank serum was obtained by intragastrical administration of ethanol extract of S. involucratus (75.35 g/kg) or purified water. Using RAW264.7 cells as objects, RAW264.7 cells were divided into normal control group, LPS group (1 μg/mL), S. involucratus ethanol extract high-dose, medium-dose and low-dose groups (50, 25, 12.5 μg/mL), 4% or 15% blank serum groups, 4% or 15% blank serum+LPS groups, 4% or 15% drug-containing serum groups, 4% or 15% drug-containing serum+LPS groups. After culturing for 24 h, cell viability, the contents of nitric oxide tumor necrosis factor α (TNF-α), interleukinand IL-6 as well as mRNA expressions of Toll-like eceptor 4 (TLR4) and nuclear factor κB (NF- κB) and protein expressions of nitric oxide synthase (NOS) and cyclooxygenase 2 (COX-2) were all detected in each group. 0771-4953513。E-mail:zhuhuagx@163.com RESULTS After culturing for 24 h, there was no statisticalsignificance in the difference of cell viability. Compared with normal control group, the contents of NO, TNF-α, IL-1β and IL-6, mRNA expressions of TLR4 and NF-κB, and protein expressions of NOS and COX-2 were increased significantly in LPS group (P<0.05). Compared with 4% or 15% blank serum groups, the levels of above indexes were increased significantly in 4% or 15% blank serum+LPS groups (P<0.05). Compared with LPS group, the levels of above indexes were decreased significantly in S. involucratus ethanol extract groups (P<0.05). Compared with 4% or 15% blank serum+LPS groups, the levels of above indexes were decreased significantly in 4% or 15% drug-containing serum+LPS groups (P<0.05). CONCLUSIONS The ethanol extract and the drug-containing serum of S. involucratus can significantly alleviate LPS-induced inflammatory reaction, the mechanism of which may be associated with inhibiting the activity of TLR4/NF-κB signaling pathway, down-regulating the protein expressions of COX-2 and NOS, and reducing the release of inflammatory factors.
6.Study on Quality Standard of the Leaves of Litchi Chinensis Sonn.

OBJECTIVE To establish the quality standard of the leaves of Litchi chinensis Sonn.. METHODS The identification of medicinal properties, microscopic characteristics, TLC and content determination method of the leaves of Litchi chinensis Sonn. was conducted. The moisture, total ash, acid insoluble ash and extract of the leaves of Litchi chinensis Sonn. were determined according to Chinese Pharmacopoeia(Volume IV), 2020. RESULTS The leaves of Litchi chinensis Sonn. under forest exhibited specific properties in characteristics, microscopic features and TLC results. The moisture content of the medicinal materials was 2.66%-6.42%, the total ash content was 2.96%-4.52%, the acid insoluble ash content was 0.17%-0.94%, the water soluble extract was 15.38%-19.87%, and the alcohol soluble extract was 24.94%-30.33%. The eight components of gallic acid, protocatechin, catechin, epicatechin, rutin, astragalin, quercetin and kaempferol had a good linear relationship in 0.003 3-0.052 0 mg·mL-1, 0.006 9-0.109 6 mg·mL-1, 0.013 0-0.208 0 mg·mL-1, 0.057 5-0.920 0 mg·mL-1, 0.013 4-0.213 6 mg·mL-1, 0.013 2-0.211 2 mg·mL-1, 0.002 9-0.045 6 mg·mL-1, 0.003 6-0.056 8 mg·mL-1(r>0.999). The average sample recovery rate was 97.43%-102.97% and the RSD was 1.70%-2.90%. CONCLUSION The established quality standard of the leaves of Litchi chinensis Sonn. medicinal material has good specificity, accuracy and reproducibility, which can be used for quality control research of the leaves of Litchi chinensis Sonn..
7.Preliminary study on metabolites derived from the ethanol extract from the leaves of Dimocarpus longan in rats in vivo

OBJECTIVE To study the metabolites derived from the ethanol extract from the leaves of Dimocarpus longan preliminarily in rats in vivo ,and to provide reference for elucidating the possible metabolic mechanism of the leaves of D. longan in lowering blood glucose . METHODS Ultra high performance liquid chromatography quadrupole time -of-flight mass spectrometry (UPLC-Q-TOF-MS/MS) was adopted by taking ethanol extract of D. longan leaves,the feces and urine of rats at 0-72 h and 0-48 h after intragastric administration of 33.8 g/kg ethanol extract of D. longan leaves(by extract ),the feces and urine of rats at the corresponding time after intragastric administration of normal saline (blank control ) as samples . The accurate relative molecular weight ,formula and fragment information of the compounds were collected , and the compounds were speculated and i dentified by matching with the database and spectrum library of the instrument ,and comparing with the reference substance and relevant literature . RESULTS A total of eight compounds were identified in urine and feces of rats ,including 2 prototype components and 6 metabolites. Three compounds (including two prototype components as quercetin ,luteolin and one metabolite as luteolin or kaempferol) in feces of rats were identified ;five compounds (all metabolites ) in urine of rats were identified ,involving metabolites of quercetin ,luteolin or kaempferol . Metabolites mainly included the products of methylation ,glucuronidation and oxidation. CONCLUSIONS After intragastric administration ,the ethanol extract from the leaves of D. longan is mainly metabolized in rats through methylation ,glucuronidation and other pathways . The identified compounds are mostly metabolites of quercetin and luteolin .
