1.Progress on Diagnosis and Treatment of Ulna Coronoid Process Fractures

[Summary] Ulna coronoid process fractures are not uncommon in clinical practice,but its treatment is difficult.Regan-Morrey type Ⅰ and Ⅱ fracture has no significant effect on the stability of the elbow joint,thus conservative treatment can be used.For some type Ⅱ fractures combined with elbow posterior dislocation,surgical treatment is required because of the potential danger of recurrent dislocation after reduction.Ulna coronoid process fractures often accompany other tissue injury,therefore manipulative reduction and external fixation with a cast or brace should be given as soon as possible.Once severe swelling complicated with bone fascia compartment syndrome,early decompression should be performed.The surgery can be carried out via medial approach,or in combination with lateral approach when lateral collateral ligament injury exists.Internal fixation materials include miniature titanium plate,micro screws,absorbable screws,hollow screw,anchor nail,external fixator and coronoid process of prosthetic replacement.
2.The reliability and validity of the Chinese version of the intolerance of uncertainty scale

Objective To analyze the reliability and validity of the Chinese version of the intolerance of uncertainty scale (IUS).Methods 878 college students were investigated,then the data were analysis by SPSS13.0 and Amos17.0.Results The project analysis and exploratory factor analyzed showed that,the Chinese version of the intolerance of uncertainty scale consisted of 24 projects and four dimensions,which could account for over 50.355% of tolal varance.The confirmatory factor analysis further revealed the structure of four-dimensional scale (x2 =660.645,x2/df =2.686,NFI =0.806,NNFI = 0.817,CFI =0.837,RMSEA =0.071).The score of the Chinese version of the intolerance of uncertainty scale was significantly correlated with all criterion,and the correlation coefficients to criterion ranged from-0.288 to-0.481,so the scale had the better criterion validity.The Cronbach's a coefficient of the Chinese version of the intolerance of uncertainty scale was 0.889,and the test-retest stability coefficient was 0.687.Conclusion The Chinese version of the intolerance of uncertainty scale has good psychometric quality,and can be used in Chinese college students.
3.A Simple Method for Dispelling Motion Artifacts in MRI Examination of Brain

Objective To explore a method that restrain and dispel motion artifacts of MRI examination for better image quality and higher working efficiency.Methods The sponge package of CT tube was used to design and make sponge cushions in types of 2 cm?7.5 cm?22 cm,3 cm?7.5 cm?22 cm and 4 cm?7.5 cm?22 cm,which were used to MRI examination by quadrature coil of brain.Results The method of"adding cushion" was adopted to examine brain of MRI in 2 638 cases.Among them,2 633 cases were successfully scanned,which accounts for 99.81%;5 cases failed,accounts for 0.19%.For the failed cases,4 cases were successfully scanned in the second time by using medical bandage in cooperation,while 1 case failed.Conclusion The method of"adding cushion" in routine MRI examination of brain can restrain and dispel motion artifacts,improving image quality and work efficiency.It is an easy,reliable,ideal method for restraining and dispelling motion artifacts in brain MRI examination.
4.Risk factors for multidrug-resistant organism pulmonary infection in elderly patients

Objective To evaluate risk factors for multidrug-resistant organisms(MDRO)pulmonary infection in elderly patients in a hospital, and provide scientific basis for prevention and control of MDRO infection. Methods 188 elderly patients who developed pulmonary infection between August 2010 and December 2012 were divided into MDR group (n=95)and non-MDR group (n=93)according to sputum bacterial culture results,clini-cal data of two groups of patients were analyzed.Results 102 strains of MDROs were isolated from patients with MDRO pulmonary infection,the top three were Acinetobacter baumannii (31 .37%),Pseudomonas aeruginosa (25.49%),and methicillin-resistant Staphylococcus aureus (15.69%).Univariate analysis showed that 7 risk fac-tors for MDRO pulmonary infection in elderly patients were history of cerebrovascular disease or dementia,hy-poproteinemia,dysphagia,bedridden,history of ICU hospitalization,recent application of 3,4-generation cephalo-sporins,and invasive procedures.Multivariate logistic regression analysis showed that hypoproteinemia (OR,6.02 [95%CI ,1 .50-24.18]),invasive procedures (OR,6.55 [95%CI,1.69-25.44]),and bedridden (OR,6.03 [95%CI, 1.92-18.91])were independent risk factors for MDRO pulmonary infection in elderly patients.Conclusion Multiple factors contribute to MDRO pulmonary infection in elderly patients,comprehensive prevention and control measures should be taken against the risk factors,so as to prevent and control MRDO pulmonary infection in elderly patients.
5.Comparison and Analysis on Foreign and Chinese Instructions of Felodipine

OBJECTIVE:To provide reference for the standardization of the instructions of felodipine.METHODS:The instructions of felodipine in China were compared with the national regulation as well as foreign instructions of felodipine.The problems of Chinese instructions of felodipine were analyzed to put forward revision measures.RESULTS & CONCLUSIONS:The problems of Chinese instructions of felodipine are items shortage or missing,lacking of warnings for drug safety.It is suggested that related administration departments and enterprises should revise and improve the instructions of felodipine.
6.Compositional differences in different processing technology of radix paeoniae alba and different origin of red peony

Objective To determine and compare the contents of oxypaeoniflorin and paeoniflorin of Paeoniae Radix,and to improve the quality standard of Radix Paeonia.Methods The contents of oxypaeoniflorin and paeoniflorin in different kind of White and Red Peony Root were successfully analyzed by HPLC.Results The content of oxypaeoniflorin in Red Peony Root is more than White Peony Root at 0.8361% ;The content of paeoniflorin in White Peony Root is more than Red Peony Root at 0.2157%.The proportion of paeoniflorin and oxypaeoniflorin in White Peony Root is more than Red Peony Root.Conclusion With HPLC technology,quantitative analysis of active constituents in Paeoniae Radix was performed accurately.
7.Efficacy of neoadjuvant chemotherapy on breast cancer and dynamic changes of circulating tumor cells

Objective: This study aims to investigate the neoadjuvant chemotherapy effect of TC and TEC by detecting the change of circulating tumor cells (CTC) in peripheral blood before and after every neoadjuvant cycle of breast cancer patients. Meth-ods:A total of 96 locally advanced breast cancer patients were selected in our hospital from January 2010 to December 2012. These pa-tients were divided into TC (docetaxel and cyclophosphamide) group and TEC (docetaxel, epirubicin and cyclophosphamide) group. Ve-nous blood (5 mL) was extracted from 96 patients prior to neoadjuvant chemotherapy, 48 h after the first, second, third, and fourth cy-cles of neoadjuvant chemotherapy. The CTC values of these two groups of patients were detected by flow cytometry, and their chemo-therapy effect was compared. Results: No significant difference was observed for age, menopausal status, ER, PR, C-erbB-2, tumor size, and clinical stage between the two groups of patients before chemotherapy and for CTC between the two groups before chemother-apy. Before neoadjuvant chemotherapy, the CTC values were correlated with tumor size. The CTC values for patients with tumor size>5 cm were higher than those with tumor size<5 cm. After neoadjuvant chemotherapy, the CTC values of the two groups of patients de-creased (P<0.05), and the CTC values of the TEC group were lower than those of the TC group (P<0.05). Conclusion:After TC or TEC neoadjuvant chemotherapy, the tumors of patients were smaller, and the CTC values were lower than those prior to neoadjuvant chemotherapy. The efficacy of neoadjuvant chemotherapy can be evaluated by monitoring the values of circulating tumor cells in pe-ripheral blood changes.

9.Preliminary clinical experience of multi-slice spiral CT in the diagnosis of small lung adenocarcinoma

Objective To investigate the value of multi-slice spiral CT in the diagnosis of small lung adenocarcinomas. Methods Fourteen patients underwent thin multi-slice spiral CT scan. Images were post-processed and 2 kinds of postprocessing images including multiple planar reconstruction (MPR) and minimum intensity projection (MinIP) were performed. The image characteristics of the tumors assessed by multi-slice spiral CT were correlated with the clinical/pathological manifestation. Results In 14 cases of small lung adenocarcinoma, tumor density had two forms. Four of five cases with localized bronchioloalveolar carcinoma (LBAC) showed total or partial ground-glass opacity, and 9 adenocarcinomas showed solid attenuation. 3 cases with LBAC showed partial ground-glass opacity mixed with airway dilatation. MPR in 14 patients clearly showed interlobar pleura and image characteristics of the tumors. MinIP in 10 cases were able to demonstrate 5~6 scale bronchial divisions and presented air bronchogram of tumors in a vivid way. Conclusion Thin multi-slice spiral CT scan and reconstruction images can play an important role and are the valuable method in the diagnosis of small lung adenocarcinomas.
10.Comparison of the effect of salt bag compression and arterial compressor haemostat after cerebral angiography

ObjectiveTo compare the effect of salt bag compression and arterial compressor haemostat for patients after cerebral angiography.Methods Four hundred and ninety-five patients undergoing cerebral angiography were divided into two groups according to the methods of arterial compression:salt bag compression group (243 cases) and arterial compressor haemostat group (252 cases) in which 131 patients used the YM-GU-1229 arterial compressor haemostat and 121 patients used the NZ-1 arterial compressor haemostat.The associated complications within 3 days were recorded.ResultsThe incidence of complications in salt bag compression group was 3.70%(9/243),in arterial compressor haemostat group was 9.92% (25/252),and there was significant difference between the two groups(P < 0.01 ).The incidence of complications in YM-GU-1229 arterial compressor haemostat was 12.21% ( 16/131 ),in NZ-1 arterial compressor haemostat was 7.44%(9/121 ),and there was no significant difference between the two arterial compressor haemostats (P > 0.05).ConclusionThe incidence of complications of arterial compressor haemostat is higher than that of salt bag compression.
