1.THE PRELIMINARY OBSERVATION OF THE EFFECT OF HIGH PROTEIN DIET ON THE NUTRITIONAL STATUS OF PERITONEAL DIALYSIS PATIENTS

Objective To observe the effect of adjusting high dietary profein intake on nutritional status of peritoneal dialysis (PD) patients. Method Thirty three PD patients whose daily protein intake (DPI) was over 1.2g/(kg? d) were included in study from July, 2005 to November, 2006. A recipe was given by a professional dietitian for every patient to reduce DPI meanwhile maintaining sufficient daily energy intake (DEI). DPI, DEI, serum albumin (ALB), lean body mass(LBM) and subjective global assessment(SGA) and the prevalence of hyperphosphatemia, hyperkalemia and metabolic acidosis were evaluated before and after 3 and 6 mo adjustment. Results At the end of observation, 28 patients were enrolled in the study. Their mean age was 61.37?12.25 yrs, 11 males and 17 females with dialysis period 8.8 mon (1~66 mo). DPI and DEI were significantly decreased during 6 mo (P=0.000). Nobody complained about gastrointestinal symptoms, and the quality and satisfaction of life during the study were not significantly different. 2.Analysis on Medication Principles of Formulas in Treating Insomnia from Spleen and Stomach Based on Literatures

Objective To discuss the medication principles of diagnosis and treatment for insomnia from spleen and stomach; To provide references for clinical treatment for insomnia.Methods Relevant articles about diagnosis and treatment forinsomnia from spleen and stomach were searched in CNKI and Wanfang Data through computers, the data were input into Traditional Chinese Medicine Inheritance Supporting System (V2.5) and then association rules, apriori arithmetic, improved mutual information method, and complex system entropy cluster methods were used to analyze the medication principles of diagnosis and treatment for insomnia from spleen and stomach. Results Through screening, totally 87 prescriptions were included, involving 165 herbal medicine. Data mining results showed that there were 30 high-frequency herbal medicine, 19 high-frequency medicinal combinations, and 4 medicinal association rules. Evolution led to 13 medicinal combinations of 2 herbal medicine and 3 new prescriptions. Conclusion The main method of diagnosis and treatment for insomnia from the spleen and stomach is regulating qi activity of spleen and stomach. The features of common medicine are invigorating spleen to reduce phlegm, clearing stomach and calming the adverse-rising energy, and tonifying qi and soothing nerves, which can offer references for clinical treatment for insomnia.
3.Clinical analysis of 38 cases solid pseudo-papillary tumor of pancreas

Objective To clarify the clinical features of solid pseudo-papillary tumor of pancreas. Methods Clinical data of 38 patients with solid pseudo-papillary tumor of pancreas were analyzed retrospectively. Results The clinical manifestations included a palpable abdominal mass, abdominal pain and discomfort. Tumor occured in 1 case younger than 10 years old, 11 cases between 11 years old and 20 years old, 7 cases between 21 years old and 30 years old, 9 cases between 31 years old and 40 years old, 5 cases between 41 years old and 50 years old and 1 case older than 50 years. Immunohistochemistry staining showed CgA( + ) in 6 cases,Vim( + )in 29 cases,AAT( + ) in 29 cases,Syn( + ) in 21cases,CD10( + ) in 34 cases,CD56( + ) in 37 cases, Ki-67 ( + ) in 36 cases and PR( + ) in 38 cases. Conclusion Solid pseudo-papillary tumor of pancreas is closely associated with gonadal hormone and reproductive system.
4.Relevant factors analysis of the cranial cerebrospinal fluid volume

Objective To study the relationship between the cerebrospinal fluid (CSF) volume and intracranial measurement line in normal Chinese by useing CT data and computer technology.Methods By CT scan,to measure CSF volume and third ventricle maximum width,suprasellar cistern minimus width,lateral ventricle width in hypothalamic layer,caudate nucleus pitch and other index by age and gender in 178 normal adults.Results There were significant difference between male and female CSF volume in the groups of 14-20 years(23 cases),31-40 years(33 cases),41-50 years(32 cases) and > 60 years (13 cases)(P< 0.05 or < 0.01).The CSF volume between male and female had significant difference (t =3.302,P =0.001).The CSF volume and third ventricle maximum width had significant correlation (r =0.685).Conclusion The third ventricle maximum width is the most closely with CSF volume.

On the basis of physical and chemical properties and spectra analysis,they were identified as 2'-de sacetoxyaustrospicatine(Ⅰ). taxol(Ⅱ),cephalomannine (Ⅲ), 2-deacetoxyl taxinine J (Ⅳ),taxinine B(Ⅴ) and 10-deacetyl-7-0-?-Dxylosyl-taxol (Ⅵ).
6.Expressions of human fibrinogen-like protein 2 in peripheral and cerebrospinal fluid white blood cells in patients with cryptococcal meningitis

Objective The purpose of this study was to explore the expression of human fibrinogen-like protein 2 (hFgl2) in patients with cryptococcal meningitis (CM).Methods The expression of hFgl2 in the peripheral blood leukocytes from 20 CM cases and 20 healthy individuals,and the expression of hFgl2 in cerebrospinal fluid (CSF) leukocytes in 11 pairs of CM patients and matched controls was detected using immunohistochemical methods.Medical image analysis system (MIAS) was used to analyze the expression of hFgl2 gray values,which were negatively correlated to the expression of hFgl2 protein in both peripheral blood and CSF.Results The average gray values for hFgl2 protein in peripheral monocytes of CM patients and healthy controls were 114.49± 11.82 and 137.56± 10.64,respectively,and those in peripheral neutrophils were 111.71 ± 8.62 and 143.56±12.57,respectively.Compared with the healthy control group,expressions of hFgl2 in peripheral blood monocytes and neutrophils were both significantly increased in CM patients (t =21.21,t=27.19; P<0.01).The average gray values for hFgl2 protein in CSF monocytes from CM patients and negative controls were 115.44±8.67 and 149.25±8.57,respectively,and those in CSFneutrophils were 111.71±8.62 and 143.56 ± 12.57,respectively.The expressions of hFgl2 in CSFmonocytes and neutrophils were significantly increased in CM patients (t =29.95,t=28.87; P<0.01),while the expression intensity of hFgl2 was similar when peripheral blood monocytes and neutrophils compared with their CSF counterparts (t=0.924,t=0.607; both P>0.05).Conclusions hFgl2 can be expressed in both peripheral and CSF monocytes and neutrophils in CM patients.

Objective To observe effects of senior neurogenic hypertension treated with microvascular decompression(MVD) Methods All 692 cases of senior cranial nerve disease with MVD were retrospectively analyze, 236 cases of preoperative neurogenic hypertension(prosopalgia 179 cases,facial spasm 48 cases, glossopharyngeal neuralgia 9cases)were treated with MVD Results In 236 cases,145 Patients had cure results(61 4%),36cases obvious effectiveness(15 3%),18 cases effectiveness(9 7%),32 cases ineffectiveness (13 6%) Two hundred and eight cases have been flowed up for 74 months average,133 cases had cure prognosis,30 cases obvious effectiveness,18 cases effectiveness,27 cases ineffectiveness Conclusion It is pathogenesis of neurogenic hypertension that oppressing cranial nerve roots and bulb by abnormal vascular tab leads to long term aching stimulation and emotional stress MVD is an effective treatment of neurogenic hypertension
8.Transit time flow measurement in coronary surgery

Objective: To review the experience of using the transit time flow measurement(TTFM) in coronary artery bypass grafting(CABG). Methods: From Sept.2001 to May, 2002,90 cases underwent CABG. Among them, 40 cases with off-pump beating heart (OPCAB) and 50 with conventional coronary bypass grafting. All graft patency were verified intraoperatinlly with TTFM. The flow value of left internal mammary artery (LIMA) in 30 cases was also detected by the same way before grafting to left anterior descending (LAD). Results: There was no operative death and severe complications. The mean flow value of LIMA in 30 cases was (42.9?33.0)ml/min. The actual blood flow value, measured simultaneously by exsanguination was (37.4?28.8)ml/min. Excellent correlation was demonstrated. The mean pulsatility index value (PI) was 1.00?0.04. The flow curve showed both diastolic and systolic filling. The mean flow value of LIMA to LAD, saphenous vein graft (SVG) or radial artery (RA) to circumflex coronary artery (LCX) or diagonal (D), SVG or RA to right coronary artery (RCA) or post anterior artery (PDA) were (29.9?9.5)ml/min, (33.7?17.5)ml/min and (31.5?19.2)ml/min. The PI values were 2.47?0.88, 4.00?1.90 and 2.60?1.30, respectively. The typical flow curve was shown in all LIMA to LAD, and SVG (or RA) to LCX (or D). Blood flow filling was mainly during diastole with minimal systolic peaks during the isovolumetric ventricular contraction. SVG or RA to RCA presents a particular flow pattern characterized by a dual filling. 6 grafts were revised basing on unsatisfied flow curves, PI, and the mean flow or all of them. Significant technical error such as conduit kinking, stenosis of distal anastomosis was found. Conclusion: The patency of graft in CABG can be verified in intraoperatively by use of TTFM. The decision of checking or revising a graft can be made basing on parameters acquired from the TTFM device. To correctly analyze the parameters warrants good clinical results.

10.Speckle Tracking Imaging Technique in Detection of Coronary Artery Disease in Patients with Hypertension

Purpose To explore the effect of speckle tracking imaging (STI) in early detection of coronary artery disease (CAD) in patients with hypertension (HT),and to intervene early to alleviate or delay the progression of CAD.This would provide theoretical evidence in controlling the development and improving the prognosis of CAD.Materials and Methods From January 2013 to April 2015,a total of 166 cases of hospitalized patients from the First Affiliated Hospital of Sun Yat-sen and Jiangmen Central Hospital were retrospectively analyzed,including HT group (n=42),CAD group (n=35),HT+CAD group (n=49) and control group (n=40).Conventional measurement and STI were used to analyze the conventional values and the 17 section peak strain value of the left ventricle.Results The total mean strain,average peak strain of lesion segment and minimum peak strain in CAD group and HT+CAD group were significantly lower (P<0.05) compared to the control group.ROC analyses showed that the AUC for total mean strain,average peak strain of lesion segment and minimum peak strain value were 0.71,0.76,and 0.70,respectively.Using a combined cut-off of ≥-18% of average peak strain of lesion segment,≥-19% of total mean strain and ≥-6% of minimum peak strain value,the performance of CHD diagnosis had a sensitivity of 91.3% and a specificity of 83.5%.Conclusion Speckle tracking automatic function imaging can be used to quantitatively analyze abnormal left ventricular segmental wall movement.It improves the detection in hypertension patients with coronary artery disease.
