1.Effects of gastric mucosa washing with povidone-iodine solution on bacteremia after endoscopic ultrasonography-guide fine needle aspiration

Objective To study the occurrence of bacteremia and whether washing gastric mucosa with povidone-iodine could reduce its occurrence after endoscopic ultrasonography (EUS)-guided fine needle aspiration (FNA). Methods A total of 20 healthy adult dogs were randomly assigned into experimental group and control group, with 10 in each. The animals in experimental group underwent gastric mucosa washing with 10 ml 0. 5% povidone-iodine solution before FNA, while those in control group with 10ml normal saline. Blood samples were collected for culture before EUS, between EUS and FNA, and 5, 15 and 30 minutes after FNA. Results There were 3 cases of positive bacteremia in experimental group and l in control group,which were not significantly different ( P = 0. 582). Conclusion Pre-washing gastric mucosa with 0. 5% povidone-iodine solution cannot reduce bacterial infection after EUS-guided FNA.
2.Using Bard 3DMax mesh without fixation in laparoscopic total extraperitoneal inguinal hernioplasty

Objective To summarize local experience and skills of using Bard 3DMax mesh with-out fixation in lapamscopic total extraperitoneal inguinal hemioplasty(TEP) to treat patients with inguinal hernia.Methed From August 2006 to December 2007,38 patients suffered in inguinal hernia underwent TEP under general anesthesia,with using Bard 3DMax mesh without ixation.Results Thirty-eight cases had been performed 42 sides TEP.The operating time was 25 to 90 min.The mean operating duration was (35±13) min for unilateral hernia and (58±22) min for bilateral ones.The patients did not need analgesics after operation.The patients stayed time after operation was 1-4 days with the mean of (2.5±1.5) days.Hematnrrms occurred in 2 cases.There was no chronic inguinal pain,foreign body sensation cases and no recurrence after (13.0±5.5) months follow-up.Conclusions Using Bard 3DMax mesh without fixation in lapamscopic TEP can be performed safely and reliably.It also helps to shorten the operating time and reduces the complication rates of the operation.The procedure has advantages of faster postoperative recover,less pain,lower complications and recurrence rate.

4.A PreIiminary Study of the CIinicaI Significance of Word Recognition Scores in EvaIuating the Curative Effects of Sudden Hearing Loss

Objective To preliminarily explore the clinical significance of word recognition scores in evalua_ting the curative effects of sudden hearing loss .Methods Pure tone averages (PTA) and word recognition scores (WRS) in 11 cases of sudden hearing loss were obtained on the pre-treatment day and the post -treatment day , and the results were compared .ResuIts The average PTA of 11 cases was 62 .27 ± 15 .36 dB HL on the pre-treat_ment day ,and 60 .23 ± 14 .32 dB HL on the post -treatment day .The average WRS in 11 cases was 42 .91% ± 30 .83% on the pre-treatment day ,and 72 .73% ± 26 .72% on the post-treatment day .There were no significant differences in PTA and the intensity of the sound for testing WRS before and after treatment(P> 0 .05) ,and the difference of the WRS between the pre -treatment and the post -treatment day was statistically significant(P<0 .05) .ConcIusion The WRS could be used in evaluating the curative effect of sudden hearing loss ,which is a sup_plement for PTA .
5.Investigation of risk factors of acute kidney injury after off-pump coronary artery bypass grafting and 3 years' follow-up

Objective:To investigate the incidence rate and risk factors of acute kidney injury (AKI) after off-pump coronary artery bypass grafting (CABG),and to compare the effects of AKI on complications after operation and major adverse cardiovascular and cerebrovascular events (MACCE) after 3 years' follow-up.Methods:In the study,299 consecutive patients who underwent scheduled off-pump CABG from January 2010 to March 2012 were included.The patients were divided into AKI group with AKI and control group without AKI after operation.The data during perioperative stage were compared,and multivariable Logistic regression modeling was used to identify the risk factors of AKI.The complications were compared after surgery and the patients were followed up for 3 years to observe the difference of MACCE between the two groups.Results:AKI occurred in 37.1% patients (111/299).The elevated serum creatinine levels (Wald =9.276,P =0.002,95 % CI 1.006-1.028),chronic obstructive pulmonary disease(COPD) (Wald =3.469,P =0.063,95% CI 0.950-7.630),decreased left ventricular ejection fraction (LVEF) (Wald =4.414,P =0.036,95 % CI 0.965-0.999),and implantation of intraaortic balloon pump (IABP) before or after operation(Wald =6.745,P =0.009,95% CI 1.336-7.925)were risk factors of AKI in multivariable logistic regression modeling.More complications occurred in AKI group,such as the duration of mechanic ventilation,the time of ICU and the length of stay post operation,reintubation,pulmonary infection,stroke,hemorrhage of digestive tract,the volume of blood transfusion (plasma and red blood cell) and renal replacement therapy(P < 0.05).The difference of mortality rate had no statistical significance between the AKI group and the control group,but two patients died in the AKI group.The difference of MACCE between the two groups had no statistical significance after 3 years' follow-up either.Conclusion:The incidence of AKI was high (37.1%) after off-pump CABG.The elevated serum creatinine levels,COPD,decreased LVEF,and implantation IABP before or after operation were independent risk factors of AKI in multivariable Logistic regression modeling.More complications occurred in AKI group during perioperative period,but the difference of MACCE between the two groups after 3 years' follow-up had no statistical significance.
6.Endoscopic ultrasonography guided celiac plexus neurolysis for relieving abdominal pain in advanced pancreatic carcinoma

To evaluate the effect of endoscopic ultrasonography guided celiac plexus neurolysis (EUS CPN) on pain associated with pancreatic carcinoma. Methods EUS CPN was performed to relieve pain in 16 patients with advanced pancreatic carcinoma. By EUS, we used fine needle to puncture at the region of celiac ganglion and inject alcohol. The pain tensions were followed up under visual analogue scale rule at 2 days and weekly thereafter. Results All patients were performed EUS CPN successfully. No serious complications happened. No satisfactory pain control after the first 2 days of EUS CPN.Whereas it was relieved obviously after 7 days and the effects maintain a rather long term. Conclusion EUS CPN is a safe and effective method for releiving the pain in pancreatic carcinoma.
7.The change of the serum myeloperoxidase and lipoxin A4 in patients with coronary heart disease

Objective To investigate the changes and significance of the serum myeloperoxidase (MPO) and lipoxin (LXA4) in patients with coronary heart disease (CHD). Methods From December 2010 to February 2011,143 patients with CHD (CHD group) and 44 patients without CHD (control group) were selected. The serum high sensitive C-reactive protein (hs-CRP), MPO and LXA4 levels were assessed, and the ratio of MPO and LXA4 (M/L) was calculated. These indexes were compared between 2 groups. The influencing factors of MPO, LXA4 and M/L levels were analyzed by multifactor Logistic regression analysis. Results The serum levels of hs-CRP, LXA4, MPO, and M/L in CHD group were significantly higher than those in control group: 3.25 mg/L vs. 0.99 mg/L, 229.88 ng/L vs. 178.63 ng/L, 422.58 U/L vs. 186.85 U/L and (1.78 ± 0.52) U/ng vs. (1.02 ± 0.17) U/ng, and there were statistical differences (P<0.05). Multifactor Logistic regression analysis result showed: LXA4 level, unstable angina, acute myocardial infarction and severe elevation of white blood cell count were the impact factors for the elevation of MPO (P<0.01 or<0.05);there were no correlations between MPO level and stable angina, mild and midrange elevation of white blood cell count (P>0.05). MPO was the impact factor for the increase of LXA4 (P<0.01), and stable angina was the impact factor for the decline of LXA4 (P =0.019);there were no correlations between LXA4 level and unstable angina, acute myocardial infarction and other factors (P>0.05). Stable angina, unstable angina and acute myocardial infarction were the impact factors for the elevation of M/L (P<0.01); there were no correlations between M/L and other factors (P>0.05). Conclusions The inflammatory response in patients with CHD is accompanied by the presence of inflammation and resolution imbalance.
8.Ammonia Emission from Concrete Admixtures Sold in Lanzhou

0.05). The average amount of ammonia emission was 0.012%, 0.024% and 0.081% at pH values of 9, 11 and 13 respectively. Ammonia emission increased with the increasing pH values, especially for those admixtures containing carboxylate. Conclusion Ammonia emission from concrete admixtures sold in Lanzhou exceed the standard in different degree.Higher pH value may accelerate the ammonia emission from concrete admixtures and cause higher concentration of ammonia in indoor air.
9.An early continuous experimental study on magnetic resonance diffusion-weighted image of focal cerebral ischemia and reperfusion in rats.

The chronological and spatial rules of changes during focal cerebral ischemia and reperfusion in different brain regions with magnetic resonance diffusion-weighted imaging (DWI) in a model of occlusion of middle cerebral artery (MCAO) and the development of cytotoxic edema in acute phase were explored. Fifteen healthy S-D rats with MCA occluded by thread-emboli were randomly divided into three groups. 15 min after the operation, the serial imaging was scanned on DWI for the three groups. The relative mean signal intensity (RMSI) of the frontal lobe, parietal lobe, lateral cauda-putamen, medial cauda-putamen and the volume of regions of hyperintense signal on DWI were calculated. After the last DWI scanning, T2 WI was performed for the three groups. After 15 min ischemia, the rats was presented hyperintense signals on DWI. The regions of hyperintense signal were enlarged with prolonging ischemia time. The regions of hyperintense signal were back to normal after 60 min reperfusion with a small part remaining to show hyperintense signal. The RMSIs of parietal lobe and lateral cauda-putamen were higher than that of the frontal lobe and medial cauda-putamen both in ischemia phase and recanalization phase. The three groups were normal on T2 WI imaging. DWI had good sensitivity to acute cerebral ischemia, which was used to study the chronological and spatial rules of development of early cell edema in ischemia regions.

10. Effect of chronic cerebral hypoperfusion on expression of occludin and claudin-2 in rat ileum mucosa

BACKGROUND: Previous studies have found that chronic cerebral hypoperfusion can cause changes in the intestinal mucosal barrier in rats, and Occludin and Claudin are important components of the intestinal mucosal barrier.OBJECTIVE: To explore the effect of chronic cerebral hypoperfusion on the ileum mucosal barrier. METHODS: Twenty male Sprague-Dawley rats were randomly divided into sham operation (sham) group and chronic cerebral hypoperfusion model group. Animal model of chronic cerebral hypoperfusion was built by permanent bilateral common carotid artery ligation. The bilateral common carotid arteries of the sham group were isolated without ligation. The animals were killed at 4 weeks after operation, and their ileums were isolated for morphological observation and pathological scoring using hematoxylin-eosin staining. Apoptosis in the ileum cells was detected using TUNEL. Western blot was used to detect the expression of Claudin-2 and Occludin. immunohistochemical staining was used to detect the expression of Occludin. The study protocol was approved by the Animal Experiment Ethics Committee of General Hospital of Western Theater Command. RESULTS AND CONCLUSION: Compared with the sham group, hematoxylin-eosin staining showed that the ileum in the model group did not develop obvious injury with no higher pathological scores (P > 0.05). Western blot results indicated that compared with the sham group, the expression of claudin-2 was increased in the ileum tissue of the rats with chronic cerebral hypoperfusion, while the expression of Occludin was decreased (P < 0.05). TUNEL fluorescence staining showed that the apoptotic rate in the model group was significantly increased compared with the sham group (P < 0.05). Immunohistochemical results showed that the expression of Occludin in the ileum tissue of the chronic cerebral hypoperfusion group was significantly reduced (P < 0.05). These findings indicate that chronic cerebral hypoperfusion may damage the intestinal mucosal barrier by downregulating Occludin expression and upregulating Claudin-2 expression.
