1.The effects ofShenkang bolt on the IL-10 and IL-6 of the patients with continous ambulatory peritoneal dialysis

Objective To observe the effects ofShenkang bolt on the IL-10 and IL-6 of patients with continous ambulatory peritoneal dialysis (CAPD).MethodsA total of 60 patients with CAPD were divided into the control group (n=30) andShenkang bolt observation group (n=30). The control group was treated with convertional treatment of CAPD, low salt, low fat, low phosphorus and high quality diet. The observation group was treated withShenkang bolt based on control group treatment. The clinic effect was detected after treatment for 8 weeks. The serum creatinine was determinated by basic picric acid method, and blood urea nitrogen was determinated by urease test. The clinic and biochemical indicator of Scr, BUN, 24h urine volume, Kt/Vurea, RRF, and ultrafiltration volume were compared between the two groups. The IL-10 and IL-6 were detected by ELISA analysis.Results After treatment, IL-10 (19.56 ± 4.38μg/mlvs. 8.98 ± 2.05μg/ml,t=4.392,P<0.01) was significantly higher in observation group than that in control group, and IL-6 (21.82 ± 3.57μg/mlvs. 49.66 ± 5.26μg/ml,t=5.264,P<0.01) was significantly lower in observation group than that in control group. The RRF (7.86 ± 2.12vs. 5.31 ± 1.62;t=2.436, P=0.046) and ultrafiltration volume (421.37 ± 81.61 ml/dvs. 321.23 ± 71.94 ml/d;t=2.617, P=0.038) was significantly higher in observation group than those in control group. ConclusionShenkang bolt could help patients with CAPD balancethe immune, suppress inflammation and improve the RRF and ultrafiltration volume.
2.pAdKDR-tk and pAdCMV suicide gene system cytotoxicity and specificity in vitro

Objective In order to identify cytotoxicity and specificity of the suicide gene systems which vascular endothelial growth factor receptor 2(KDR) and CMV were used as promoters respectively. Methods Two cell lines, HUVEC and CNE-2, one expresses KDR highly,another doesn't, were used as target cells for identifying the cytotoxicity and specificity of recombinant adenoviral vectors with herpes simplex virus thymidine kinase (HSV-tk) gene expression driven by KDR promoter and CMV promoter respectively (pAdKDR-tk and pAdCMV-tk) in Vitro by adding GCV. Results HUVEC and CNE-2 showed similarly sensitivity to GCV when they were transfected by pAdCMV-tk. However, They shown different sensitivity to GCV when they were transfected by pAdKDR-tk. When HUVEC was transfected in high MOI (MOI=100) and concentration of GCV was 50 ?g/ml, The cell growth rate decreased to (23.12?4.90)% .On the contrary, CNE-2 cells growth rate remained as high as (70.46 ?3.27)%. Conclusion Two kinds of cell line CNE-2 and HUVEC can be killed by pAdCMV-tk suicide gene system, but pAdKDR-tk suicide gene system only can kill HUVEC cell line. The result implied that pAdKDR-tk could kill the cells that express KDR specifitly.
3.The lethal factor of acute obstructive suppurative cholangitis

Objective To summarize and investigate the lethal factor of acute obstructive suppurative cholangitis (AOSC).Methods The clinical data of 56 patients with AOSC were retrospectively analyzed.Results Six cases died,5 cases with acidosis,5 cases with thrombocytopenia and 5 cases with temperature change obviously,4 cases with heart,lung and kidney disease or diabetes,5 cases with operation and operation time ≥ 150 min,5 cases with from onset to treatment time ≥72 h.Eighteen cases of elderly patients ≥70 years old,4 cases died.The patients whose age≥70 years,temperature ≥39 ℃ or < 36 ℃,combined with acidosis,platelet counts ≤6.0 × 1012/L,with heart,lung,kidney diease or diabetes,time of anesthesia and operation ≥ 150 min and from onset to treatment time ≥72 h had higher death rate (P < 0.05).Conclusion Age,obvious temperature abnormalities,significantly platelet decrease,with heart,lung,kidney diease or diabetes,acidosis,long time of anesthesia and operation and from onset to treatment time ≥ 72 h are the lethal factor of AOSC.

5.Hospital Acquired Bacterial Infection in Intensive Care Unit (ICU):A Clinical Investigation

OBJECTIVE To study the bacterial infections in ICU,pathogens distribution and their drug-resistance characteristics,so as to offer a reference to the prevention and control of hospital infection.METHODS Specimens from ICU from Jan 2003 to Dec 2004 were underwent a process of germ culture,identification and drug-sensitivity test.Germ culture-positive cases were confirmed by Diagnostic Standards of Hospital Infections.RESULTS Ninety three pathogens were identified from ICU in two years.Of all cases,63.44% were Gram-negative,30.1% were Gram-positive,and 9.68% were epiphyte.Among them,19.35% were Pseudomonas aeruginosa and it was the main one of Gram-negatives;9.68% were Staphylococcus aureus and it composed the most of Gram-positives;6.45% were epiphyte.Of all 93 pathogens,60 gained from respiratory tract,and 23 gained from urinary tract.CONCLUSIONS To emphasize on the strictly control of ICU,hospital infections and high risk disease,and to adopt certain preventive measures are the ways to control and decrease hospital infections.
6.Effect of Bushen Tiaojing Recipe Extract on Sexual Hormone Levels of Premature Ovarian Failure Patients with Kidney Deficiency Syndrome

Objective To investigate the clinical efficacy of Bushen Tiaojing Recipe Extract(an extract preparation of Chinese herbal medicine with the actions of tonifying kidney and regulating menstruation) for the treatment of kidney deficiency type of premature ovarian failure (POF),and to observe the effect of the extract formula on sexual hormone levels of POF patients.Methods A total of 75 POF patients were randomly divided into Chinese medicine (CM) decoction group,CM extract group and western medicine (WM) group,25 cases in each group.CM decoction group was treated with the decoction of Bushen Tiaojing Recipe (a decoction recipe for tonifying kidney and regulating menstruation),CM extract group was treated with Bushen Tiaojing Recipe Extract,and WM group was treated with hormone replacement therapy(HRT).Twenty-eight days constituted one treatment course,and the treatment for the 3 groups lasted 3 continuous courses.After treatment,the patients were followed up for 3 months.Clinical efficacy of the 3 groups was evaluated,and clinical symptom scores,the improvement of menstruation and the changes ofsexual hormones ofestrogen(E2),follicle-stimulatinghormone(FSH) and luteinizing hormone (LH) were observed before and after treatment.Results (1) Two cases of each group dropped out,and a total of 69 patients completed the study.(2) The total effective rate of CM decoction group and CM extract group was 73.91%,78.26% respectively,higher than 52.17% of WM group (P < 0.05).(3)After treatment,clinical symptoms of CM decoction group and CM extract group were obviously improved (P < 0.05 or P < 0.01 compared with those before treatment),while WM group only had an effect on improving the abnormal menstruation (P < 0.01 compared with that before treatment).CM decoction group and CM extract group had better effect on improving the clinical symptoms than WM group (P < 0.05 or P < 0.01).(4) CM decoction group and CM extract group had higher menstrual improvement rate,shorter menstrual-improvement time and lower recurrence rate than WM group (P <0.05).(5) CM decoction group and CM extract group had better effect on increasing serum E2 level and decreasing FSH and LH levels than WM group (P < 0.05 or P < 0.01).(6) The adverse reaction incidence of CM decoction group and CM extract group was all 4.35%,lower than 13.04% of WM group (P < 0.05).Conclusion Bushen Tiaojing Recipe Extract has high drug concentration,and its effect is stable,mild and lasting,having the advantages of convenience for carrying and medication.The extract exerts certain clinical efficacy for the treatment of POF,and is effective on alleviating clinical symptoms,recovering normal menstruation,regulating serum sexual hormone levels,with lower abnormal menstruation relapse rate and less adverse reaction.
7.Clinical outcomes of modified 360° fusion for lumbar spondylolisthesis

Objective To analyze the clinical results of 360° fusion for spondylolisthesis.Methods Data of 76 patients from March 2005 to November 2012,including 26 males and 50 females with a mean age of 54.3 years were retrospectively analyzed.All patients had undergone modified 360° fusion.The clinical outcomes were evaluated by the Japanese Orthopaedic Association scores (JOA) and visual analogue scale (VAS).The fusion status and loss of correction were assessed by CT and plain radiographs.Results All patients had been successfully followed up for at least one year.Post-operative reduction rate was 96.8%,with a mean loss rate of 1.1% at six months and 0.8% at one year follow-up separately,but the difference was not significant.Postoperative percentage of slip,lumbar lordosis,intervertebral disc height and slip angle had significant differences,compared with those of the pre-operative,but the difference between one week post-operative,six months follow-up and one year follow-up were not significant.The fusion rate was 76.3% at six months follow-up and 98.7% at one year follow-up,and there were significant differences.Both JOA and VAS score at six months and one year follow-up indicated significant differences in contrast to that of preoperative.Dural injury was found in one patient,and subsidence was found in one patient.One received a second operation at 3 months after the surgery due to screw loose.Conclusions Modified 360° fusion shows great clinical outcome and fusion rate,which can be a dominant procedure for treating spondylolisthesis.
8.Experiment study of three dimensional navigation assisted spinal surgery using structured light scanning

Objective To introduce a new spinal surgery navigation system based on structured light scanning(structured light navigation system,SLNS),and to compare accuracy of pedicle screw placement using SLNS,CT based navigation system and free hand technique.Methods Thirty-two calf vertebral pedicles,40 and 32 were placed with pedicle screws using SLNS,CT navigation system,and free hand technique,respectively.The pedicle breakage ratio,transverse section angle (TSA),sagittal section angle (SSA),screw offset deviation of each screw were detected according to CT images.Results The pedicle breakage ratio of pedicle screws in SLNS group,CT navigation group and free hand group were 6%(2./32),5%(2/40),25%(8/32),respectively.The difference between each of the two navigation groups and free hand group were statistically significant regarding frequency of screw misplacement.The difference between the two navigation systems was not obvious.The average SSA error and TSA error in SLNS group were 2.58°±2.74° and 4.26°±5.20°.For CT navigation group,they were 2.95°±2.61° and 3.13°±-2.75°.There was no statistical difference between the two navigation methods for the SSA or TSA error.There was no statistical difference in offset deviation among SLNS group,CT navigation group and free hand group.Conclusion The SLNS is a new and practical navigation system,which has similar accuracy with CT navigation system.
9. Nursing of a case of male patients with periodic psychosis

Objective:
To summarize the nursing methods of one male patient with periodic psychosis.
Methods:
The nursing methods of a male patient with periodic psychosis were summarized and analyzed.
Results:
According to the change rule of the disease, different nursing measures were taken, no adverse nursing events occurred during the hospitalization, which improved the treatment compliance and treatment effect.
Conclusions
According to the condition characteristics of periodic psychosis, nursing by stages was put forward, which provided the basis for definite diagnosis and promoted the patients' condition recovery.
10.Effects of Spleen-warming and Kidney-tonifying Therapy on TLR4 Signaling Pathway of Cerebral Infarction Sequelae Patients

Objective To observe the effect of spleen-warming and kidney-tonifying therapy on Toll-like receptor 4 (TLR4) signaling pathway of cerebral infarction sequelae patients. Methods A total of 60 cerebral infarction sequelae patients with spleen and kidney(yang) deficiency were divided into control group and treatment group, 30 cases in each group. The two groups were given function rehabilitation training following by the theory of modern rehabilitation medicine, and additionally the treatment group was treated with spleen-warming and kidney-tonifying herbs orally. The treatment period for the two groups averaged 3 months. Before and after treatment, the scores of traditional Chinese medicine (TCM) symptoms and signs, peripheral blood TLR4 mRNA expression level, and the inflammation-related factors in the two groups were observed, and the observation parameters were compared with 30 healthy volunteers. Meanwhile, the changes of modern rehabilitation evaluation indexes were also evaluated before and after treatment. Results (1) After treatment, the Fugl-Meyer scores for motor function and Modified Barthel Index(MBI) scores for activities of daily living in the treatment group were obviously increased(P < 0.05 or P < 0.01 compared with those before treatment); the MBI scores in the control group was much increased(P < 0.05 compared with those before treatment), but the Fugl -Meyer scores only showed an increasing trend(P > 0.05). (2) After treatment, the predominant symptoms and signs of hemiplegia, facial distortion and dysphasia in the two groups were much improved(P < 0.01 compared with those before treatment), and the improvement in the treatment group was superior to that in the control group(P < 0.05). The control group had no significant effect on relieving the symptoms and signs of dizziness and blurred vision, numbness of limbs, spontaneous sweating, shortness of breath and weakness, and darkish tongue(P > 0.05 compared with those before treatment); the treatment group had obvious effect on the above symptoms and signs, and the effect was superior to that in the control group(P < 0.05 or P < 0.01).(3) For the inflammation-related factors, tumor necrosis factor alpha(TNF-α) level in the treatment group was much decreased and even arrived to the normal level(P > 0.05), and TNF-α level in the control group was decreased but did not arrive to the normal level(P < 0.05); levels of interleukin(IL)-1β, IL-6 and TLR4 mRNA in the two groups were decreased but did not arrive to the normal(P < 0.01), and the decrease in the treatment group was superior to that in the control group(P < 0.05 or P < 0.01). (4) During the treatment period, no obvious adverse effect was found in the two groups. Conclusion Oral use of spleen -warming and kidney –tonifying Chinese medicine combined with modern rehabilitation therapy is effective for the treatment of the sequelae of cerebral infarction patients by relieving the clinical TCM symptoms, and one of the therapeutic mechanisms is probably related with the TLR4 signaling pathway through regulating the expression of inflammatory factors.
